A Convergence Study on the Current Managerial Status and Policy of Regional Public Medical Centers : Focusing on Jeollabuk-do Province's cases (지방의료원의 운영 현황과 정책에 관한 융합적 연구 : 전라북도 사례를 중심으로)

  • The study selected two regional public medical centers(G, N) in Jeollabuk-do to recognize problems and provide solutions for regional medical centers. The medical centers were analyzed, focusing on availability of medical personnel, budget and management thereof, medical treatment performance, public project performance, and patient satisfaction. The results showed that the medical centers were understaffed with doctors compared to private hospitals or national university hospitals and suffering financial debt. The number of patient with medical benefit was increasing in the medical center G, whereas it was decreasing in the medical center N. Patient satisfaction survey showed that the medical center G was scoring similar to the national average, whereas the medical center N was scoring slightly less than the average. Policies needed for development and efficient management of Regional public medical centers are clarification of Regional medical centers identity, modernization of medical equipment and facilities, recruitment of competent medical personnels, specialization of hospitals and establishment of stronger referral system. The regional medical centers should secure financial supports for public service from regional and national governments, and establish revenues from funerary service, leasing service, and national projects participation.

  • Objectives: To assess knowledge of HPV and attitudes towards HPV vaccination among the general female population, government officials, and healthcare providers in China to assist the development of an effective national HPV vaccination program. Methods: A cross-sectional epidemiologic survey was conducted across 21 urban and rural sites in China using a short questionnaire. 763 government officials, 760 healthcare providers, and 11,681 women aged 15-59 years were included in the final analysis. Data were analyzed using standard descriptive statistics and logistic regression. Results: Knowledge of HPV among the general female population was low; only 24% had heard of HPV. Less than 20% of healthcare providers recognized sexually na$\ddot{i}$ve women as the most appropriate population for HPV vaccination. There was high acceptance of the HPV vaccine for all categories of respondents. Only 6% of women were willing to pay more than US $300 for the vaccine. Conclusions: Aggressive education is necessary to increase knowledge of HPV and its vaccine. Further proof of vaccine safety and efficacy and government subsidies combined with increased awareness could facilitate development and implementation of HPV vaccination in China.

Evaluation of the Effectiveness of Surveillance on Improving the Detection of Healthcare Associated Infections (의료관련감염에서 감시 개선을 위한 평가)

  • The development of reliable and objective definitions as well as automated processes for the detection of health care-associated infections (HAIs) is crucial; however, transformation to an automated surveillance system remains a challenge. Early outbreak identification usually requires clinicians who can recognize abnormal events as well as ongoing disease surveillance to determine the baseline rate of cases. The system screens the laboratory information system (LIS) data daily to detect candidates for health care-associated bloodstream infection (HABSI) according to well-defined detection rules. The system detects and reserves professional autonomy by requiring further confirmation. In addition, web-based HABSI surveillance and classification systems use discrete data elements obtained from the LIS, and the LIS-provided data correlates strongly with the conventional infection-control personnel surveillance system. The system was timely, acceptable, useful, and sensitive according to the prevention guidelines. The surveillance system is useful because it can help health care professionals better understand when and where the transmission of a wide range of potential pathogens may be occurring in a hospital. A national plan is needed to strengthen the main structures in HAI prevention, Healthcare Associated Prevention and Control Committee (HAIPCC), sterilization service (SS), microbiology laboratories, and hand hygiene resources, considering their impact on HAI prevention.

Future Tasks of the Law Forcing CCTV Installation in Operating Rooms (수술실 내 CCTV 설치 의무화 법안의 향후 과제)

  • On September 24, 2021, the new provisions(Article 38-2 of the Medical Service Act) mandatory CCTV installation in operating rooms where the unconscious patient is operating such as general anesthesia. The revised medical law aims to effectively prevent illegal activities that may occur in the operating rooms and to promote appropriate resolution to medical dispute. According to the law, medical institutions operating unconscious patients, such as general anesthesia, must install CCTVs in the operating rooms by September 25, 2023, and film surgical scenes only at the request of patients and their guardians, regardless of the consent of the medical personnel. The bill delegated the legislative device to minimize infringement of fundamental rights to subordinate statutes without stipulating it in the law.(Article 38-2(10)) The most realistic policy plan to minimize the infringement of the fundamental rights of patients is to prepare specific regulations. Therefore, this study examines the legislative background and main contents of the amended CCTV installation bill, and suggests issues to be reviewed when preparing subordinate statutes by analyzing major issues. It was reviewed based on compliance with the principle of minimizing infringement of fundamental rights of information subjects in the operating rooms. The information subjects of CCTV are health professionals and patients. Suggesting issues should be considered when preparing subordinate statutes so that the purpose of the CCTV installation law can be achieved while minimizing infringement of right of self-determination of personal information, personality rights, and human rights. It is hoped that this paper will be referred when discussing subordinate statutes and regulations to contribute minimizing infringement of fundamental rights.

The Role and Focus Areas of Medical Technologists in the Field of Diagnostic Tests in the COVID-19 Era (COVID-19 시대 임상병리사의 역할 및 영역)

  • This study attempted to provide the basic data for developing a system to identify the role of medical technologists and ensure an efficient response for quick and accurate diagnostic tests in the COVID-19 era. The research method involved using focus group interviews for a survey and analysis of 15 medical institutions. Eleven sample collection institutions, 10.4 medical technologists, 2.1 minutes of collection time, 5.4 hours of test time, 9,670 tests, 6.2 member test workforce size, and 7 screening center operating institutions were surveyed. The results of the focus group interview analysis revealed that there were no standardized guidelines covering working hours, area, and environment to protect sample collectors and testers in relation to the COVID-19 tests. Also, legal protection measures were insufficient in the event of accidental infections and there were no personnel regulations related to COVID-19. In addition, the professional training of sample collectors and molecular diagnostic testers was required for reliable COVID-19 testing. In conclusion, it is necessary to provide professional education through special test short-term training institutions to cope with emergency infectious diseases such as COVID-19. Legal systems should be put in place to protect the workforce and ensure stability.

Birth Registration Rate and Accuracy of Reported Birth Date in Rural Area (농촌지역의 법정-기간내 출생신고율과 신고된 생년월일의 정확도)

  • To measure the birth registration rate and the validity of birth registration data in rural area, all of the 4,014 married women under 49 years of age who had not been sterilized in Gunwee county of Kyungpook province were followed by Myun health workers for 2 years from April 1, 1985 to March 31, 1987 and 766 births were detected. All of the birth registration records of Myun offices were reviewed on September 30, 1987 and 944 births which occurred within the above mentioned period were found. Actual birth date obtained by follow-up study were compared with the birth date on registration card. Among 766 births detected by follow-up study,576 births(75.2%) which were reported within 6 months after birth were ascertained on the official registration records and 96 births(12.5%) were not found on the records although mother stated that the birth was registered. The registration rate within legal due date was 61.3% among 576 births detected by follow-up study and also ascertained on the official records. The registration rate within legal due date was lower in mothers under 20 years of age and above 35 years and in mothers who had only primary education. It was decreased as the birth order increased. The registration rate was higher in births occurred from October to March than births occurred from April to September. All of the births of 7 neonatal deaths were not reported. The registered birth date was consistent with the actual birth date in 78.0%. Birth date on record was earlier than the actual birth date in 6.8% and later in 15.3%. The consistency rate was lower in mothers above 35 years of age(54.5%), and in infants of 4th birth order and above(56.3%). The rate was increased as the maternal education level increased. The rate of boys was higher than that of girls. A higher percentage(17.4%) of infants born in March was registered with earlier date than the actual birth date and most of these registered birth dates were lunar calendar date. This might be related with the age for entering the primary school. The study findings revealed that the birth registration rate within legal due date and accuracy of report have been increased in recent years, but the infant mortality rate derived from the birth registration seems to be very inaccurate. It is suggested to let the medical personnel who delivered the baby report the birth by mail directly to the current address of parent while infants delivered at home without professional attendant may comply with the present registration system.

Influential Job-Related Characteristics for the Job Satisfaction and Turnover Possibility of Dental Hygienists (치과위생사의 직무만족과 이직가능성에 영향을 미치는 직무관련특성 요인)

  • The purpose of this study was to examine the job-related characteristics of dental hygienists affecting their job satisfaction and turnover possibility. The subjects in this study were 297 dental hygienists who worked in dental hospitals and clinics in the metropolitan area. The dental hygienists who worked for three to less than five years, who were unmarried and who didn't benefit from any incentive systems expressed a greater satisfaction, and those who were married, whose workplaces were established jointly with other dental institutions in the form of a network and whose workplaces offered incentives were more likely to change their employment. As for influential factors for job satisfaction and turnover possibility, the respondents whose workplaces were established jointly with other dental institutions were less satisfied, and a larger number of patients and better communication led to higher satisfaction. Those who worked in general hospitals were more likely to change their employment, and better welfare benefits, more appropriate length of working hours and a larger number of patients led to lower turnover responsibility. The findings of the study ascertained that working environments were one of the factors to impact on job satisfaction and turnover possibility, and both of hospitals and dental hygienists should make an effort to improve working environments.

The comparative analysis on the distribution of cancers (암환자 분포의 비교 분석)

  • This paper is based on the records of researching the patients with cancer in the Chunnam National University Hospital from January 1990 to December 1999 and referred to the Korea Cancer Center Hospital's annual report. The results are as the following : 1. Along the total O.P.D 5,680,284, the patients of therapeutic radiology are 201,367 in $3.55\%$ 2. Among the 12,404 cancer patients, stomach cancer patients are $2,048(16.51\%),\;liver\;cancer\;patients\;are\;1,490(12.01\%),\;lung\;cancer\;patients\;are\;1,418(11.43\%)\;cervix\;and\;uterus\;cancer\;patients\;are\;1,102(8.88\%)$ and sexual ratio of male to female appeared 1.38 : 1 Therefore, male cancer patients are more than female patients. 3. The age distribution of cancer was $2,791(22.50\%)\;by\;55{\sim}64\;age,\;2,582(20.82\%)\;by\;45{\sim}54\;age,\;and\;2,530(20.40\%)\;by\;65{\sim}74\;age.\;Therefore,\;the\;60\%\;of\;all\;cancer\;patients\;were\;45{\sim}75$ ages. 4. Among the 9,815 therapeutic radiology patients, $1,755(17.89\%)\;are\;in\;the\;uterus\;system,\;1,534(15.63\%)\;are\;in\;the\;head\;and\;neck,\;1,519(15.48\%)$ are in the lung. Therefore, the proportion of three kinds of therapeutic radiology patients is about $50\%$. 5. The occupational distributions of 12,404 cancer patients$(90{\sim}99\;year)\;are\;2,795(22.53\%)\;by\;orderly\;farmers,\;2,763(22.28\%)\;by\;house\;wives,\;586(4.71\%)\;by\;businessmen,\;and\;391(3.15\%)$ by public service personnel. Industrial, miners, forest men, soldiers didn't nearly appear. 6. The number of patients of cancer diagnosis was $9,431(76.03\%)\;by\;the\;biopsy\;of\;primary\;region,\;and\;2,201(17.74\%)$ by clinical examination Therefore, those two kinds of methods took a propertion of about $94\%.\;The\;number\;of\;deceases\;on\;departments\;was\;153(32.42\%)\;in\;PD,\;133(28.18\%)\;in\;GI,\;63(13.35\%)$ in GS. Therefore, those three kinds of deceases took a proportion of about $74\%$.

  • More than 70% of the accidents that occur on offshore installations stem from hydrocarbon explosions and fires, which, because they involve blast effects and heat, are extremely hazardous and have serious consequences in terms of human health, structural safety, and the surrounding environment. Blast barriers are integral structures in a typical offshore topside module to protect personnel and safety critical equipment by preventing the escalation of events caused by hydrocarbon explosions. Many researchers have shown the adequacy of the simple design tool commonly used by the offshore industry for the analysis and design of blast walls. However, limited information is available for corrugated blast wall design with explosion impact response characteristics. Therefore, this paper presents a parametric study on the explosion impact response characteristics of an offshore installation's stainless steel corrugated blast wall. This paperalso investigates and recommends design parameters for the structural design of a corrugated blast wall based on a nonlinear structural analysis of experiential results.

Factors Influencing Nursing Students' Social Distancing for COVID-19 Prevention (간호대학생의 COVID-19 감염 예방을 위한 사회적 거리두기 영향요인)

  • Purpose: This study aims to understand the effect of attitudes toward COVID-19, subjective norms, and intentions toward social distancing as a preventative measure for COVID-19 in nursing university students, and to provide basic data for COVID-19 prevention. Methods: A cross-sectional study design was used in this study. The participants included 169 nursing students at a university in G city. Data were collected from 22 May 2020 to 1 June 2020. A self-report questionnaire was used. Data were analyzed using descriptive statistics, t-test, ANOVA, Pearson correlation coefficients, and multiple regression analyses with the IBM/SPSS 26.0 for Window Program. Results: Intention was the most influential factor on social distancing (β=.29, p=.002), followed by the number of breakfasts per week (β=.18, p=.005), school grade (β=-.16, p<.001), and smoking status (β=-.13, p=.046). These variables explained 35.0% of factors influencing social distancing (F=12.24, p<.001). Conclusion: The results of this study are meaningful and present a direction for nursing students and future medical personnel to effectively perform social distancing. It is necessary to educate nursing students so that they can practice proper health care habits and intentions for social distancing.
