Medical Expenditure Attributable to Overweight and Obesity in Adults with Hypertension, Diabetes and Dyslipidemia : Evidence from Korea National Health and Nutrition Examination Survey Data and Korea National Health Corporation Data (성인의 고혈압, 당뇨병, 이상지질혈증으로 인한 총 진료비 중 과체중 및 비만의 기여분 : 국민건강영양조사자료와 국민건강보험공단 자료를 중심으로)

  • Objectives: This study was conducted to estimate medical expenditure attributable to overweight and obesity in adults with hypertension, diabetes and dyslipidemia using Korea National Health and Nutrition Examination survey data and Korea National Health Corporation data. Methods: The medical expenditure of hypertension, diabetes and dyslipidemia related to overweight and obesity were composed of inpatient care costs, outpatient care costs and medication costs. The population attributable risk (PAR) of overweight and obesity was calculated from national representative data of Korea such as the National Health Insurance Corporation cohort data and 2005 Korea National Health and Nutrition Examination survey data. Results: The medical expenditure attributable to overweight and obesity of hypertension were 456 billion won (men : 215 billion won, women : 241 billion won). Those of diabetes were 282 billion won (men : 148 billion won, women : 135 billion won), and of dyslipidemia were 17 billion won (men : 9 billion won, women : 8 billion won). Consequently, these costs corresponded to 33.3% of total medical expenditure due to hypertension, diabetes and dyslipidemia. Conclusions: We found a substantial medical expenditure due to overweight and obesity of hypertension, diabetes and dyslipidemia were very high. In order to reduce these costs, effective national strategies for prevention and management of overweight and obesity should be established and implemented.

Comparative Analysis on Health Care Expenditure and Its Improvement Effect for Social Welfare: An Application of Principal Component Analysis (보건의료비의 사회후생 증대효과 비교분석: 주성분분석을 중심으로)

  • Health care expenditure might be understood as a core source of social welfare. It is mainly due to the fact that the health care expenditure is closely related to the level of social welfare. And, therefore, the paper is basically designed to investigate the effect of social welfare improvement resulted from an increasing in per capita, private, public, and total health care expenditures during 1995-2009 periods. Empirical evidence reveals that the improvement of social welfare in middle income group reveals the highest improvement in social welfare, and the other income groups maintain a quite similar pattern in improvement of social welfare. As far as category of health care expenditure concerns, the effect of social welfare improvement by per capita health care expenditure is in general lower in all case. And, private sector achieves the highest improvement of social welfare in comparing with public and total health care expenditures. To this end, it could be tentatively concluded that the private health care expenditure has to be substituted for the public sector to induce market-oriented operational system. Accordingly, it would be better to build an institutional basis more elaborately in setting up the market system.

Trend and Prediction of Urban Family Expenditure for Health Care (도시 가계의료비 지출의 추이와 예측)

  • The increase of health care expenditures is an important problem in the almost countries. Also, suppression of the health care expenditures is an important problem in the health field of Korea since the national health insurance for total people in 1989. Thus, it is very important to grasp the change of the health care expenditures of family and proportions of the health care expenditures to total expenditures of family, because they are the basis of national health care expenditures in Korea. While the health care expenditures of urban family were increased during 1980-1993 by 12.8% annually, the total expenditures of urban family were increased by 14.8% annually. Consequently, the proportions of health care expenditures to total expenditures were decreased from 5.98% to 4.76%. The proportions of health care expenditure for 3 years to come were predicted to 4.75% in 1994, 4.67% in 1995, and 4.63% in 1996 by the time-series analysis. That is, it was predicted that they would be decreasing slowly. The product elasticity of health care expenditure was less than 1 in the multiple regression analysis. so the health care is normal good rather than superior good. Therefore, it seems that the household economy is able to bear the expense pursuing the improvement of quality of health care by actualizing the medical insurance fee.

Relationship between Work Unstability and Personal Medical Expenditure Ratio (고용불안정과 가구소득 대비 개인 의료비 지출 비중의 연관성)

  • Purpose: The purpose of this study is to identify relationship between work unstability and personal medical expenditure ratio focusing on wage workers' contract period. Method: This study analyzed 2015 yearly data beta version of Korea Health Panel, co-managed by Korea Institute for Health and Social Affairs and National Insurance Corporation for data analysis. When executing linear regression, Household income was applied with equivalized income, and the proportion of personal medical expenditure was naturally logged to perform linear regression and the demographic and socioeconomic factors were taken into account. The demographic and socio-economic factors were also considered. Findings: As a result of reviewing the used factors, it was found that the more unstable work status, the higher personal medical expenditure ratio. This result corresponds to 'The Theory of Fundamental Causes' by Link & Phelan. Conclusion : It indicates that policy efforts should be made to improve the working environment and health level of socially unstable workers.

An Analysis of the Effect of Employer's Safety Management Commitment on Occupational Accident - Focusing on the Mediating Effect of Safety Climate and Safety Expenditure - (사업주의 안전경영의지가 산업재해에 미치는 영향 분석 - 안전분위기와 안전지출의 매개효과를 중심으로 -)

  • We need a new approach to reduce serious occupational accidents. This needs a commitment to secure employer's safety, characterized by a culture that prevents occupational accidents and appropriates safety expenditure. This study analyzed the factors that relate how a safety management commitment to safety climate in the workplace and safety expenditure affects the generation of occupational accidents. The summary of the result is as follows. First, both safety climate and safety expenditure have direct negative effects on the generation of occupational accidents (-0.136 [p = 0.010] and -0.100 [p = 0.008] respectively). Second, the safety management commitment has no significant direct effect towards generating occupational accidents (p = 0.105). Third, the safety management commitment has positive effects toward both building a safety climate and increasing safety expenditure (0.664 [p = 0.000] and 0.178 [p = 0.000] respectively). The safety management commitment however, has negative effects on generating occupational accidents (-0.090 [p = 0.004] through safety climate and -0.018 [p = 0.004] through safety expenditure). Hence, the safety management commitment has positive effects on decreasing occupational accidents through safety climate and safety expenditure (-0.108 [p = 0.004]). In conclusion, the safety management commitment by employers reduces occupational accidents through its impact on safety climate and safety expenditure in work places. Therefore, in order to reduce accidents, the employer should focus on creating a safety climate and investing in safety expenditure in the work place.

An Analysis of the Consumption Expenditure Structure for Leisure and Recreational Service in Urban Households (도시 가계의 여가오락서비스 소비지출구조 및 영향요인분석)

  • The purpose of this study was to examine the consumption expenditure structure for leisure & recreational service in urban households. For these purpose, the data collected 175 households in Pusan metropolitan city. Statistics employed for the analys were frequencies, means, one -way ANDVA, and multiple regression analysis. The major results of this study were as follows; Average monthly expenditure for leisure, recreational service was 190,342 won. And their expenditure for leisure & recreational service share was 12.89 % of total expenditure. Those expenditure with high income elasticity were hobby & culture education, and journey. And those with low income elasticity were play & inspection, and health & sports. In leisure & recreational service expenditure, the variables which influence were job and degree of household head, and income.

  • A multiple regression analysis using ordinary least square (OLS) is frequently used for the projection of health expenditure as well as for the identification of factors affecting health care costs. Data for the analysis often have mixed characteristics of time series and cross section. Parameters as a result of OLS estimation, in this case, are no longer the best linear unbiased estimators (BLUE) because the data do not satisfy basic assumptions of regression analysis. The study theoretically examined statistical problems induced when OLS estimation was applied with the time series cross section data. Then both the OLS regression and time series cross section regression (TSCS regression) were applied to the same empirical da. Finally, the difference in parameters between the two estimations were explained through residual analysis.

The Determinants of National Health Expenditure: A Decision Tree Analysis (국민의료비 결정요인 및 영향력 분석)

  • This paper draws the determinants of National Health Expenditures(min) and collectivizes OECD countries which are positioned by same conditions using the decision tree analysis. Major findings are summarized as follows. We find that the power of influence of income level on NHE has been 58.35% in 1985, 65.37% in 1990, 66.90% in 1995, and 66.47% in 1997. The power of influence of public share in NHE has been on the increase during that period: 19.50% in 1985, 19.91% in 1990, 22.81% in 1995 and 26.88% in 1997. The two factors(income level, public share) tells for the most part of NHE: 77.85% in 1985, 85.28% in 1990, 89.71% in 1995, 93.35% in 1997. Our results support the hypothesis that NHE could be explained mostly by the income level and show that public share is negatively correlated with the growth of NHE.

Consumption Quintile Effect on Per Capita Consumption Expenditure of Middle and Older Elderly Households (중고령자 가구의 개인 소비지출 결정에 미치는 소비분위 효과)

  • This study analyzed per capita expenditure (food expenses, housing expenses, health care costs, and cultural & entertainment expenses) by the consumption quintile for middle and older elderly households in addition to personal characteristics, household characteristics and economic factors affecting it. A sample collected from the 6th KLoSHA in 2016, was 2,983 households. First, among each per capita expenditure, the largest expenditure was food expenses, followed by housing expenses, health care costs and cultural & entertainment expenses. Compared with the first quintile of personal consumption expenditure, the largest increase in the fifth quintile was food expenses, followed by cultural & entertainment expenses, housing expenses, and health care costs. Second, compared to the fifth quintile of per capita food expenses, all other quintile had negative effects, and only the first quintile showed a negative effect compared to the fifth quintile of per capita housing expenses. The first, the second, and third quintile had a negative effect compared to the fifth quintile of per capita health care costs. Compared with the fifth quintile of per capita cultural & entertainment expenses, only the third quintile showed a negative effect. Third, in all quintile of per capita food expenses, the most influential variable from the first quintile to the third quintile was marital status, while in the fourth and fifth quintile included household income. In all quintile of per capita health care costs, health status was the most influential variable from the first quintile to the fourth quintile, and residence was in the fifth quintile.

Estimation Model of Energy Expenditure of Working in a Clean Room for Manufacturing Embedded Needles by Ergonomic Programs (인간공학 프로그램에 의한 매선 제작 청정실작업의 에너지소모량 예측 모델)

  • The purpose of this study is to estimate the energy expenditure of working in a clean room for manufacturing embedded needles by ergonomic programs. Embedding needle is one of medical devices and it should be manufactured in a clean room. 3D static strength prediction program was used to analyze the slow movements during embedding needle manufacturing in a clean room. Also the energy expenditure prediction program was used to estimate energy expenditure rates for materials handling tasks to help assure worker safety and health in clean room. The energy expenditures of the tasks were calculated using prediction equations derived from empirical data. The energy expenditure rate of 3.09 kcal/min in a clean room didn't exceed the 3.5 kcal/min action limit guideline for an average 8-hour day set by the National Institute for Occupational Safety and Health (NIOSH). Energy consumption was calculated on the same working conditions as EEPP program, using an average body weight of female 20 years old to 59 years who would be the candidates of the real workers.
