Prevalence of Dry Eye Disease Its Affecting Factors by Using OSDI Questionnaire (OSDI 설문지를 이용한 안구건조증 유병률과 관련요인)

  • This study consisted of descriptive research to determine the prevalence of dry eye disease using the Ocular Surface Disease Index (OSDI) questionnaire targeting workers in hospitals to understand the differences between general and health related characteristics depending on whether people have dry eye disease, and to determine the influencing factors of the disease. The subjects were 502 (Male: 51, Female: 451) general hospital employees in G-city. The data were collected from October 1 to October 31, 2015. Logistic regression was used to analyze the data. The results showed that hospital workers' prevalence of dry eye disease was 76.1%. Factors influencing DED in this population included currently having or adjusting medication (OR: 4.73), among those who felt their eyes dry, those who visited clinics and received medical treatment (artificial tears) (OR: 2.37), those who felt eye dryness (OR: 2.23), and not eating regularly (OR: 0.55). Moreover, 54.0% of those who had dry eye disease reported serious conditions. Accordingly, education to manage the risk factors of the disease and improve lifestyle is needed to prevent and manage dry eye disease.

An Investigation into Oral Medicine Inpatients by Systemic Disease (전신질환으로 입원한 환자의 구강내과질환 발현실태)

  • Purpose : To investigate the actual conditions of diagnosis and treatment of oral medicine inpatient with systemic disease. Methods : A total of 54 oral medicine subjects, inpatient due to systemic disease for diagnosis and treatment of oral disease was requested to answer the medical history and dental treatment record. Results : The ratio of gender is composed of male 44% and female 56%, the distribution of age is the order of the 50-59 group 37%, the 60-69 group 26%, the 40-49 group 22%. Systemic disease is composed of Endocrine, nutritional and metabolic diseases 36%, Diseases of the circulatory system 36%, Diseases of the nervous system 10%. Chief complain of oral disease is composed of routine check for oral health 26%, craniomandibular disorders 18%, soft tissue problem 18%. Oral disease is composed of Diseases of salivary glands 32%, Gingivitis and periodontal diseases 23%, Dentofacial anomalies 16% Conclusion : These findings indicate that oral medicine inpatient due to the systemic disease is significantly correlated to the oral disease. The patients of oral disease interrelationship between inpatient and outpatient of systemic disease should be validated by future research.

Performance Evaluation of Aprons according to Lead Equivalent and Form Types (방사선 방어용 앞치마의 납당량, 형태에 따른 성능 평가)

  • The apron is one of the essential protectors to reduce the exposure dose of radiological technologists. This study is to provide a guideline for purchasing the aprons with excellent performance and to help reducing the exposure dose by measuring the shielding ration and uniformity of aprons according to lead equivalent and form types. The shielding ratio of aprons were measured by using radiation generator and dosimeter. Exposure conditions were 81 kVp, 25 mAs, source to image receptor distance (SID) 100 cm and field of view (FOV) $17^{{\prime}{\prime}}{\times}17^{{\prime}{\prime}}$. Exposure areas for front type and around type aprons were divided into 9 areas and for 2 pieces type aprons were divided into 3 areas of top and 4 areas of skirt. The uniformity of aprons were measured by using fluoroscopy and Image J. The 4 regions of interest (ROI) were set into acquired images and measured uniformity by measuring the standard deviation of pixel intensity in ROIs. In continuous shielding ration measurement of aprons according to exposure area, there was not statistical significance (P>0.05). In ANOVA test of aprons, there was statistical significance (P<0.01). In the results of sheilding ratio, although the aprons had equal lead equivalent, there were difference in shielding ratio from 83.59% to 98.15%. In the results of uniformity, the front type aprons with equal lead equivalent indicated the similar uniformity. However, the around type and 2 pieces type apron with equal lead equivalent indicated the different uniformity each other, from 1.8 to 22.2. If the performance evaluation in this study were conducted regularly before and after purchase the aprons, the exposure does to patients and radiological technologists could be reduced.

  • From 1 January 2020, the limit for Sulphur in fuel oil used on board ships operating outside designated emission control areas will be reduced to 0.5 %. This regulation by international maritime organization (IMO) is able to significantly reduce the amount of Sulphur oxides (SOx) discharging from ships and should have environmental advantages and health for all over the world. To meet the regulation, in these days, wet scrubber system is being actively developed. However, this process leads to make washing wastewater. In this study, we evaluated ion exchange resin system in accordance with scrubber wastewater discharge regulation by IMO. Theoretical wastewater used as feed solution of lab scale water treatment systems. The results revealed that nitrate ion was removed selectively in spite of high TDS wash wastewater solution depending on ion exchange resin property. Moreover, it was possible to improve efficiency of the system by optimizing operating conditions.

Psychosomatic Integrative Care for Psychosocial Distress of Patients With Breast Cancer (유방암 환자의 정신사회적 디스트레스에 대한 정신신체의학적 통합치료)

  • Breast cancer is the most prevalent oncological disease among women. Various psychosocial distress is common at the diagnosis, treatment, and posttreatment phase of breast cancer. For the treatment of breast cancer, not only medical treatment but also psychosomatic integrative care will be needed. Patients with breast cancer may lead to increased vulnerability to stress, adjustment disorder, anxiety disorder, and depressive disorder, and these psychiatric diseases and conditions are associated with recurrence or exacerbation of breast cancer. Psychosocial treatment of anxiety and depression could increase the quality of life of patients and decrease the recurrence and progression of breast cancer. In this article, we reviewed 5 clinical breast cancer survivorship guidelines focused on psychosomatic integrative care including psychosocial treatment and alternative treatment for psychosocial distress. Because 5 treatment guidelines were using various definitions of evidence, we confirmed evidence of various psychosocial treatments for patients with breast cancer based on the definition of evidence by the US Preventive Service Task Force (USPSTF) guideline. We also reviewed the effect size of psychosocial treatment for anxiety, depression, mood, and quality of life in patients with breast cancer. This article discusses the barrier to the delivery of psychosomatic integrative care and suggests integrative care planning for breast cancer. Multi-disciplinary teams, patient's needs assessment, information technology support, patient and caregiver engagement, planned periodic monitoring of psychosocial distress by a psychosomatic specialist or consultation-liaison psychiatrist are recommended as key features of a psychosomatic integrated care plan.

A Liability for Damage caused by Drug (의약품 부작용과 손해배상)

  • The use of drugs that reflect the experiences and achievements of modern science has given human being the benefits of treating diseases and improving health conditions. However, in addition to the benefits of those, medicines have inherently inevitable adverse reactions. Many countries are taking measures such as market entry regulations or post-marketing surveillance to minimize damage caused by drug side effects, but the occurrence of side effects cannot be eliminated. Although the damage is force majeure, in some cases, the doctor who prescribed the drug or the pharmacist who administered the drug may have to compensate for the damage. The liability depends on whether the side effects were known in advance, the type of medicine, etc. On the other hand, in some cases, drug manufacturer may have to take liability for the side effect itself. As it is not easy for victims to be compensated for damages in those cases, many countries, including Korea, are setting to protect victims through the Product Liability Act. Drugs are also one of the product, so liability set by the Product Liability Act may apply. Even before the enactment and enforcement of the Product Liability Act, damage caused by drug has occurred. To resolve them, precedents have developed case law, which have many similarities with the Product Liability Act, but also have differences. Damage caused by drug manufactured prior to the enforcement of the Product Liability Act may occur in the future. In this context, the legal principles of the case laws will remain valid and be applied. This is an important reason to review the case law of precedents.

The Effects of Smartphone Use on Structured Social Network Types among Retired Older Adults in South Korea (스마트폰 이용이 은퇴 노인의 구조적 사회관계망에 미치는 영향)

  • The purpose of this study is to examine the impacts of smartphone use on structured social network types in the Korean elderly population. Data was derived from the 2014 survey of living conditions and welfare needs of Korean older persons collected by Korea Institute for Health and Social Affairs. A total of 4,180 participants were selected for the Propensity Score Matching (PSM) analysis. Based on propensity score estimates, the 491 smartphone users (treatment group) and 491 featurephone users (control group) were matched. Ordinary Least Squares (OLS) regression analysis was conducted to examine the relationship between smartphone use and structural social network types. The results showed that among retired older adults, people using smartphone had significantly better structured social networks than those using featurephone even after controlling for covariates. Smartphone users had the higher levels of social contact and social activity. These findings suggested empirical evidence that using smartphone positively affects structured social networks, which might be used as the basis for designing intervention programs to promote social networks and social engagement of retired older adults.

Effects of Cordyceps militaris supplementation on the immune response and upper respiratory infection in healthy adults: a randomized, double-blind, placebo-controlled study (건강한 성인을 대상으로 면역증강 및 상기도 감염 예방에 대한 동충하초주정추출물의 유효성 평가 : 이중맹검 플라세보 대조 연구)

  • Purpose: Upper respiratory tract infections are major causes of the common cold throughout the world. Cordyceps militaris (C. militaris) is a well-known functional food for its anti-fatigue and immunomodulating activities. On the other hand, there are no reports on the protective effect against upper respiratory tract infections (URI). This study was a 12 week randomized, double-blind, and placebo-controlled trial in healthy volunteers. Methods: A total of 100 subjects 20 ~ 70 years of age with a history of at least two colds in the year were enrolled in the study. The participants were required to record any adverse events and rate any cold-related incidents in a diary during the investigation period. The efficacy end point was the symptoms and incidence of URI, and changes in cytokines, IgA and natural killer (NK) cell activity. Results: The Cordyceps militaris group over 12 weeks showed no significant impact on the incidence and symptomatology of URI compared to the placebo group. On the other hand, the experimental group showed significantly higher NK cell activity (p = 0.047) and IgA level (p = 0.035) compared to the placebo group. The NK-cell activity and IgA level were increased significantly by Cordyceps militaris over 12 weeks. Conclusion: The results suggest the possible beneficial immunomodulating effects, but the protective effects on URI could not be demonstrated under these conditions. Additional research will be needed to determine the efficacy and mechanisms of Cordyceps militaris function.

Mitigation effects of red Platycodon grandiflorum extract on lipopolysaccharide-induced inflammation in splenocytes isolated from mice (홍도라지 추출물이 마우스에서 분리한 비장세포에서 lipopolysaccharide로 유도된 염증에 미치는 영향)

  • Purpose: Platycodon grandiflorum (PG) is known to have effective antimicrobial and anticancer activity. The main bioactive components of PG are saponins, and these could contribute to anti-inflammatory activity. However, little is known about the anti-inflammatory effect of PG. In this study, we aim to assess the anti-inflammatory response to Red PG Extract (RPGE) in splenocytes under ex vivo conditions. Methods: The cell viability of isolated splenocytes taken from mice was analyzed by performing a Cell Counting Kit-8 assay. The productions of nitric oxide (NO) and cytokines (specifically interleukin-6 (IL-6) and interleukin-10 (IL-10)) were measured utilizing Griess reagent and ELISA, respectively. Results: We found that co-treatment with RPGE and Lipopolysaccharide (LPS) decreased isolated splenocyte proliferation as compared with that of the LPS-stimulated control. We also observed that RPGE markedly suppressed NO synthesis and IL-6 production that was induced by LPS. There were no significant differences of IL-10 production between co-treatment with RPGE plus LPS and treatment with LPS alone. Conclusion: When taken together, our data has shown that RPGE mitigates LPS-induced inflammation in splenocytes isolated from mice. Further research is surely needed to confirm the anti-inflammation effects of RPGE in an in vivo model.
