  • Purpose: The purpose of this study was to evaluate the use of OraQuick tests in previously reported articles. Methods: The literature was searched using Pubmed Medline with keywords, such as "OraQuick" or "rapid HIV test". Articles that included the specificity and sensitivity of this device were reviewed. Results: A total of 11 journal articles including 3 domestic articles were reviewed. The sensitivity of the OraQuick Test was reported to be 97.8 to 100% and its specificity was 98.8 to 100%. Conclusion: The results indicated that the simple OraQuick assay has proven to be accurate and it can be used to detect patients with HIV and to prevent the spread of HIV on test screens.

An Investigation into the Effects of the Work Environment / Occupation on Hiv Related Stigma: A Case of Service Staff in Grahamstown: Eastern Cape, South Africa

  • Purpose - This paper investigates whether one's occupation influences one's attitude toward people with HIV/AIDS and suggests ways to counter HIV-related stigma in different work settings. Two samples comprising workers in different environments and occupations with contrasting personalities were chosen. Thus, security guards (authoritarian types) and catering workers (social types) were included. Research design, data, and methodology - The sample comprised246 service staff from the Rhodes University Catering Division and the Hi-Tec Security company, both in Grahamstown, South Africa, a small Eastern Cape province town. All employees at these organizations during the survey were eligible to participate. Results -The security sample displays significantly higher personal stigma scores than the catering sample, according to the Visser personal stigma scale (mean scores of 4.01 and 1.37, respectively; t=10.30, df=244, p=0.00). Similar results were found using Visser subscales. Conclusions - This study shows that occupation is a strong predictor of HIV-related stigma in the workplace, suggesting that workplace settings, by attracting particular personalities and influencing workers, may shape attitudes towards those who are HIV positive.

  • This study investigates the population model of the spread of HIV/AIDS which the infection is generated by an infectious individual in a population of susceptible. A mathematical model is presented for the transmission dynamics of HIV infection within the communities of homosexual males. The pattern on the epidemic character of HIV, the causative agent of AIDS, was analysed by the mathematical model of AIDS system which is derived according to the ecological relationship between five epidemilogic states of individuals. The computer simulation was performed using real data and the following conclusions are drawn on the basis of the simulations. 1. The model structure and the algorithm described n the thesis is good. 2. In proportion to increase Ro, the population of AIDS patient increases and the time of its widespread reaches earlier. 3. The AIDS patients will be maximum between 7 and 21 years after an attack of AIDS and widespread between 10 and 20 years. 4. Considering the properties of the incubation periods, the maximum number of infected person is increased, and the attack rate is decreased.

Factors affecting stigma communication towards people living with HIV/AIDS (HIV 감염인 및 에이즈 환자에 대한 낙인 의사소통 영향요인)

  • Purpose: This study aims to identify factors influencing the stigma communication concerning HIV (Human Immunodeficiency Virus)/AIDS (Acquired Immune Deficiency Syndrome) patients between health and medical college students. Methods: In this descriptive study, the participants were 158 college students from a certain university. Data were collected from September 18 to September 30, 2018. Self-reported questionnaires were used to measure AIDS-related knowledge, homosexual awareness, fear of HIV/AIDS patients and stigma communication. The collected data were analyzed with descriptive statistics, an independent t-test, one way analysis of variance, Pearson's correlation and a stepwise multiple regression analysis using the SPSS 23.0 program. Results: The factors influencing the stigma communication between health and medical college students were a fear of HIV/AIDS patients (β=.47, p<.001) and homosexual awareness (β=-.15, p=.040), which together explained 28.0% of the variance in the model. Conclusion: The study findings suggest that an education program for health and medical college students should be developed. Additionally, when designing the education program, it is recommended that the contents for inducing attitude changes should include affective areas as well as knowledge-oriented contents. The study suggests that it is necessary to provide a systematic education program to decrease the stigma for HIV/AIDS patients before health care providers enter clinical practice.

  • 현재 HIV(human immunodeficiency virus) 감염에 대하여 보다 많은 효과적인 약물치료법이 가능하다. Highly active antiretroviral therapy (HAART)로 언급되는 이 치료법은 항 HIV치료제의 다양한 병용법으로 구성된다. 그러나, 최근에 이렇게 치료된 환자들에게 중요한 독성들(toxicities)로서 빈번하게 상당한 지질이상과 혈중당의 항상성 조절장애와 연관된 몸의 지방 분포 비정상으로서 나타나는, 광범위한 대사성 합병증(metabolic complications)이 출현해왔다. 이러한 합병증의 관리는 표준적인 치료 중재(interventions)와 연관하여 지질과 당 대사와 관련된 항 HIV치료제의 특성 있는 효과를 이해하면서, 항 HIV 약물들을 조절하는 것을 포함한다. 본 증례는 항HIV 약물요법과정에서 나타난 상당한 지질 이상, 매우 높은 LDL 수치와 높은 TG수치에 따르는 후속 약물요법을 보여주며, 개별화된 항 HIV 약물요법을 수행하면서, 대사성 합병증에 관련된 수치의 검사와 주기적인 약물치료과정의 모니터링을 권하여 HIV에 감염된 환자들의 효과적인 치료를 향상시키기 위한 것이다.

Fine Needle Aspiration Cytology of the Plasmablastic Lymphoma in Human Immunodeficiency Virus (HIV) Negative Patient - A Case Report - (HIV 음성 환자에서 형질모세포종의 세침흡인 세포소견 - 1예 보고 -)

  • Plasmablastic lymphoma (PBL) is a recently described aggressive B-cell neoplasm, which usually manifests as a localized disease of the oral mucosa in individuals infected with human immunodeficiency virus (HIV). Recently we encountered a case of plasmablastic lymphoma manifesting in the left maxillary sinus and cervical lymph node of a previously healthy HIV-negative man, 48 years of age. we conducted a fine-needle aspiration smear of the cervical lymph node, and this was found to be highly cellular with numerous large cells exhibiting eccentrically positioned nuclei, prominent nucleoli, and moderate quantities of basophilic cytoplasm. A biopsy of the mass in the maxillary sinus evidenced diffuse growth of similar plasmablastic cells. These tumor cells were negative for the leukocyte common antigens, CD20, CD3, CD30, and EMA. However, the cells tested positive for CD79a and CD138/syndecan-1. The tumor cells also exhibited L-light-chain restriction. The Ki-67 proliferation index was measured at almost 100%. The patient was diagnosed with plasmablastic lymphoma. After three cycles of combination chemotherapy and radiotherapy, the patient went into complete remission, and currently remains in this state.

Factors Associated With Long-term Retention in Antiretroviral Therapy Among People Living With HIV: Evidence From a Tertiary Hospital in Jakarta, Indonesia

  • Objectives: This study investigated factors associated with the retention of people living with human immunodeficiency virus (HIV) on antiretroviral therapy (ART) during the first 3 years of treatment. Methods: A retrospective study using electronic health records was conducted at a tertiary hospital in Jakarta, Indonesia. Adult HIV-positive patients who started ART from 2010 until 2020 were included. A binary logistic regression model was used to identify factors associated with ART retention in the first 3 years. Results: In total, 535 respondents were included in the analysis. The ART retention rates for the first, second, and third years were 83.7%, 79.1%, and 77.2%, respectively. The multivariate analysis revealed a negative association between CD4 count when starting ART and retention. Patients with CD4 counts >200 cells/mL were 0.65 times less likely to have good retention than those with CD4 counts ≤200 cells/mL. The year of starting ART was also significantly associated with retention. Patients who started ART in 2010-2013 or 2014-2016 were less likely to have good retention than those who started ART in 2017-2020, with adjusted odds ratios of 0.52 and 0.40, respectively. Patients who received efavirenz-based therapy were 1.69 times more likely to have good retention than those who received nevirapine (95% confidence interval, 1.05 to 2.72). Conclusions: Our study revealed a decline in ART retention in the third year. The CD4 count, year of enrollment, and an efavirenz-based regimen were significantly associated with retention. Patient engagement has long been a priority in HIV programs, with interventions being implemented to address this issue.

  • Mathematical models for describing the Human Immunodeficiency Virus(HIV) infection can be devised to better understand how the HIV causes Acquired Immune Deficiency Syndrome(AIDS). The HIV models can then be used to find clues to curing AIDS from a control theoretical point of view. Some models take Cytotoxic T Lymphocytes(CTL) response to HIV infection into account, and others consider mutants against the drugs. However, to the best of our knowledge, there has been no model developed, which describes CTL response and mutant HIV together. Hence we propose a unified model to consider both of these. On the basis of the resulting model, we also present a Model Predictive Control(MPC) scheme to find an optimal treatment strategy. The optimization is performed under the assumption that the Structured Treatment Interruption(STI) policy is employed.

The Palliative Care and Hospice for the People Living with HIV (HIV 감염인을 위한 완화의료와 호스피스)

  • According to the advance of antiretroviral regimen and the early treatment strategy, people living with human immunodeficiency virus (PLWH) are achieving the goal of virologic suppression and immune restoration. Most of them no more die of acquired immunodeficiency syndrome (AIDS) defining illnesses, and become older with chronic comorbidities such as cardiovascular, metabolic, hepatic, renal and neurological diseases. However some PLWH still visit hospitals as late presenters with very low CD4+ T cell counts, so that they suffer AIDS defining illnesses to die or experience severe neurological complications resulting in disabilities. Early palliative interventions are needed on the various symptoms of PLWH. Thus far chronic pains such as distal symmetric sensory polyneuropathies have been underevaluated. Active pain-relieving interventions are important to them. Recently we define end of life condition of human immunodeficiency virus (HIV) or eligibility to hospice care after adjusting current status of HIV treatment. Hospice teams should pay attention to the specific medical conditions, psychological needs, and social circumstances of PLWH. With just standard precautions as common infection control measures, general hospice cares can be provided to them like to other hospices subjects. For giving PLWH opportunities to have the end of life with value and dignity, hospice multidisciplinary team should intervene them early and aggressively. Now we need more clinical experiences and institutional improvements.

Functional Nucleotides of U5 LTR Determining Substrate Specificity of Prototype Foamy Virus Integrase

  • In order to study functional nucleotides in prototype foamy virus (PFV) DNA on specific recognition by PFV integrase (IN), we designed chimeric U5 long terminal repeat (LTR) DNA substrates by exchanging comparative sequences between human immunodeficiency virus type-1 (HIV-1) and PFV U5 LTRs, and investigated the 3'-end processing reactivity using HIV-1 and PFV INs, respectively. HIV-1 IN recognized the nucleotides present in the fifth and sixth positions at the 3'-end of the substrates more specifically than any other nucleotides in the viral DNA. However, PFV IN recognized the eighth and ninth nucleotides as distinctively as the fifth and sixth nucleotides in the reactions. In addition, none of the nucleotides present in the twelfth, sixteenth, seventeenth, eighteenth, nineteenth, and twentieth positions were not differentially recognized by HIV-1 and PFV INs, respectively. Therefore, our results suggest that the functional nucleotides that are specifically recognized by its own IN in the PFV U5 LTR are different from those in the HIV-1 U5 LTR in aspects of the positions and nucleotide sequences. Furthermore, it is proposed that the functional nucleotides related to the specific recognition by retroviral INs are present inside ten nucleotides from the 3'-end of the U5 LTR.
