Squeak Noise of Ceramic-on-ceramic Hip Joint using FEM (FEM을 이용한 세라믹-세라믹 고관절 마찰소음의 불안정성 해석에 관한 연구)

  • This paper describes the FEM analysis for squeak problem of the ceramic-on-ceramic hip joint system. The onset of hip squeak is estimated by the positive real parts of the eigenvalues in the hip joint system. From the complex eigenvalue analysis, the unstable frequencies and the corresponding mode shapes are determined at the certain severe friction coefficients. It is found that some bending and torsion modes of the femoral stem can be unstable due to the mode-coupling mechanism. It also shows that the magnitude of the friction coefficient plays a key role on the occurrence of hip squeak.

  • The Purpose of this study was to seek determinant factors through analysis of 65 snatch skill kinematic factors of Athletics participated in 2001 Asian weightlifting competetion. The conclusion were as follows ; 1. In order to enhance snatch skill, when barbell move on knee position, One should be flex knee joint to 105-110 degree, and In pull motion, One should be move powerful extension of knee and hip joint. 2. In last pull motion, One try to make more lock out motion than extra extention motion of hip joint 3. In order to enhance snatch skill, It is inportant that elevate barbell highly by last pull motion through powerful knee extention, poweful hip flextion and One should be make lock out motion fast in the same time. 4. In order to enhance snatch skill, anterior-posterior movement width of shoulder joint should be small. 5. In order to enhance snatch skill, Hip joint should be move vertically on start and lock out phase, but In pull phase, extension motion of hip joint shoulde be performed more largely and powerfully.

Effect of Sagittal Pelvic Tilt on Kinematic Changes of Hip and Knee Joint During Sit-to-Stand (일어서기 동작 시 시상면 골반 기울임이 엉덩관절과 무릎관절의 운동형상학에 미치는 영향)

  • Although there have been various studies related to the body's movement from a sitting to a standing position (sit-to-stand task), there is limited information on the kinematic changes on the frontal and transverse planes. The purpose of this study was to ascertain how pelvic tilt affects kinematic changes in the frontal and transverse planes in the hip and knee joints during a sit-to-stand task. For this study, 33 healthy participants (13 female) were recruited. Each participant rose from a sitting to a standing posture at his or her preferred speed for each of three different pelvic tilt trials (anterior, posterior, and neutral), and the measured angles were analyzed using a 3-D motion analysis system. A one-way repeated measure analysis of variance was performed with Bonferroni's post hoc test. In addition, an independent t-test was carried out to determine the sex differences in hip and knee joint kinematic changes during the sit-to-stand tasks. The results were as follows: 1) The hip and knee joint angle in the frontal and transverse planes showed a significant difference between the different pelvic tilt postures during sitting in the pre-buttock lift-off phase (pre-LO) (p<.05). Compared to the posterior pelvic tilt posture, the anterior pelvic tilt posture involved significantly greater hip joint adduction and internal rotation, knee joint adduction, and reduced internal rotation of the knee joint. 2) Sex differences were found with significant differences for males in the initial and maximal angles in the frontal plane of the hip and knee joint (p<.05). Females had a significantly smaller initial abduction angle of the hip joint and a significantly greater maximal angle of the hip adduction joint. These results suggest that selecting a sit-to-stand exercise for pelvic tilt posture should be considered to control abnormal movement in the lower extremities.

Total Hip Replacement for Treatment of Chronic Coxofemoral Joint Dislocation in 7 Dogs

  • Total Hip Replacement in a patient with chronic coxofemoral joint dislocation is a challenging problem because chronic coxofemoral joint dislocation causes severe morphological changes. These challenging factors make surgery more difficult and increase the possibility of postoperative complications including cup implant dislocation and prosthetic joint luxation. All patients were diagnosed having at least 2 months of coxofemoral joint dislocation. On physical examination, crepitus was noted in the coxofemoral joint. Radiographs revealed hip joint dislocation with mild to severe degenerative changes. Total hip replacement was planned for all patients. Dorsal acetabular rim deficiency in 3 cases was augmented using a locking plate and polymethylmethacrylate bone cement. All cases have shown difficulty in prosthetic joint reduction. Pectineus and rectus femoris muscle origin were released in all cases. At 12 months follow up, all patients showed satisfactory ambulation. The issues on perioperative prosthetic joint reduction and luxation due to chronic coxofemoral joint luxation with muscle contracture and dorsal acetabular rim deficiency can be resolved by muscle releasing and dorsal acetabular rim augmentation.

Septic arthritis of the hip joint caused by Klebsiella pneumoniae: a case report

  • Klebsiella pneumoniae is an uncommon cause of septic arthritis in adults. However, late detection can cause serious complications, including joint destruction and immobility. The purpose of this study was to report a case of successfully treated septic arthritis of the hip joint (SAHJ) caused by K. pneumoniae. A 49-year-old female patient presented to our hospital with fever and progressive severe pain in the right hip area. Although there was no abnormality on plain radiographs, ultrasonography revealed diffuse swelling of the right hip joint. Under ultrasonography guidance, the hip joint fluid was aspirated, and Gram staining and culturing were performed. The patient's pain was significantly reduced after the joint aspiration. The Gram staining and culturing revealed gram-negative bacilli, which were subsequently identified as K. pneumoniae. According to the results, systemic intravenous antibiotic (ceftriaxone) was administered without complications, and the patient was discharged on oral antibiotic (ciprofloxacin). Clinical cases of septic arthritis of the knee or sacroiliac joint have been occasionally reported in adults, but cases of SAHJ are rare. Moreover, K. pneumonia-induced SAHJ has not been reported to date. Therefore, we report this very rare case and its successful treatment.

Clinical Characteristics of Hip Joint Rotations and Knee Adduction Moment through 3D Gait Analysis (3차원 보행분석을 통한 무릎 모음 모멘트와 고관절 내외회전의 임상적 특성)

  • Purpose : The purpose of this study was to verify the relationships among the knee adduction moment, hip rotation range, strength of hip rotators, and Foot Posture Index of healthy young adults. Method : Thirty-two healthy adults(24 male, 8 females) participated in this study. Subjects performed 5 walking trials to evaluate the knee adduction moments using a three-dimensional motion analysis system. Hip rotation ranges and hip rotator strengths were measured using a standard goniometer and a handheld dynamometer, respectively. The mean of three trials of clinical tests was used for data analysis. Results : The first peak knee adduction moment was significantly correlated with the hip rotation ranges and hip rotator strengths (P<.05). The second peak knee adduction moment was showed significant correlations with hip external rotation and rotation ratio. There were no correlations between Foot Posture Index and all knee adduction moments (P>.05). Conclusion : This study suggests that imbalances of the range of motion and strength of the internal and external rotation of the hip joint can affect knee adduction moments. The impact may exacerbate musculoskeletal disorders such as osteoarthritis of the knee. Therefore, further studies should be conducted to evaluate the effects of clinical interventions to correct these imbalances on the reduction of the knee adduction moments in patients with knee osteoarthritis.

Concurrent Validity and Clinical Usefulness of Universal Plastic Goniometer for Hip Internal and External Rotation Range Measurement (고관절 내외회전 가동범위 검사에 대한 범용플라스틱 측각기의 동시타당도와 임상적 유용성)

  • PURPOSE: The aim of this study was to evaluate the concurrent validity and clinical usefulness of the universal plastic goniometer to measure the range of motion of the internal and external rotation of the hip joint using the three dimensional motion analysis which can analyze the joints and segment movements in the most objective and quantitative method. METHODS: Clinical and kinematic data were collected from thirty individuals using a universal plastic goniometer and a ten camera motion analysis system. Passive hip rotation range was obtained three trials for left and right hip joints using two measure methods simultaneously. RESULTS: There were significant differences between all matching measures of the two measures of internal and external rotation of the hip joint (p<.05). The relationship between the two tests for all measurements of the internal and external rotation of the hip was statistically significant with correlation coefficient form r=.87 to .96. (p<.01). CONCLUSION: Clinical measurement of the internal and external rotation of the hip using a universal plastic goniometer is effective to assess the hip condition. However, application of universal plastic goniometer requires careful attention in more accurate evaluation and research verification of the internal and external rotation of hip joint.

Identifications of Reflex Muscle Activities and Joint Moments Triggered by Electrical Stimulation to Sole of the Foot during Lokomat Treadmill Walking

  • The aim of this study was to investigate the characteristics of the flexion withdrawal reflex modulated during Lokomat treadmill walking in people with spinal cord injury. The influence of the limb position and movement were tested in 5 subjects with chronic spinal cord injury. EMG activities from tibialis anterior and moments of the hip joint elicited by the foot stimulation were examined during Lokomat treadmill walking. To trigger the flexion withdrawal reflex during Lokomat treadmill walking, a train of 10 stimulus pulses was applied at the skin of the medial arch. The TA EMG activity was modulated during gait phase and the largest TA reflex was obtained after heel-off and initial swing phase. During swing phase, TA EMG was 40.9% greater for the extended hip position (phase 6), compared with flexed hip position (phase 8). The measured reflex moment of the hip joint was also modulated during gait phase. In order to characterize the neural contribution of flexion reflex at the hip joint, we compared estimated moments consisted of the static and dynamic components with measured moment of the hip joint. The mean static gains of reflex hip moments for swing and stance phase are -0.1, -0.8, respectively. The mean dynamic gains of reflex hip moments are 0.25 for swing, 0.75 for stance phase. From this study, we postulate that the joint moment and muscle response of flexion withdrawal reflex have the phase-dependent modulation and linear relationship with hip angle and angular velocity for swing phase during Lokomat treadmill walking.

Analysis on lower extremity joint moment during a developpe devant (Developpe devant 수행시 하지 관절 모멘트 분석)

  • The purpose of this study was to analyze the joint moment on lower extremity during a developpe devant. Data were collected by Kwon3D, KwonGRF program. Two professional modem female dancers were participated in this experiment. Subjects performed a developpe devant in meddle heights. On the axes of X, Y, Z, it was shown that the maximum joint moment was occurred in hip joint. The moments are plotted during developpe devant. The ankle muscles generate a plantar flexion moment and the knee muscles generate a flexion moment and The hip muscles generate a extension moment. So these muscles of joint muscles were known to play a key role in keeping the body balance while doing developpe devant. In addition adduction moment occurred at hip, knee, an ankle in the order of amount, we could assume from this data that him out motion started from the hip joint. There was small active turn out possible below the hip joint. A small amount of extra turn out could be obtained when standing because of flexion between the foot and floor, which could be used to give a passive external rotation force to the whole leg and this could produce a rotation between the knee and foot. This passive external rotation could produce very damaging results. Therefore, lower extremity joint muscles such as hip, knee, and ankle muscle should be trained to keep the body balance and prevent injury during developpe devant performance. And for the safe and perfect turn ort performance, hip joint abduction, the most important external rotating muscle for him out is needed to train and full stretching should be done in advance.

A Clinical Report on the Case of Bilateral Sacroilitis with Synovitis in Rt. Hip Joint Caused by Ankylosing Spondylitis, treated by Conservative Korean Medical Treatment Including Hip Joint MST(Motion Style Treatment) (강직성 척추염으로 인한 천장관절염 및 고관절 윤활막염 환자를 대상으로 시행한 고관절 MST 및 한방 보존적 치료 치험 1례)

  • Objectives : The Purpose of this study is to investigate the clinical application of conservative Korean treatment including hip joint MST for Bilateral Sacroillitis with Synovitis in Rt. Hip Joint Caused by Ankylosing Spondylitis. Methods : Patient is hospitalized at Dept. of Korean Internal Medicine, Jaseng Korean Medicine Hospital, diagnosed as Bilateral Sacroilitis with Synovitis in Rt. Hip Joint Caused by Ankylosing Spondylitis, and treated by herbal medicine, acupuncture, moxibustion, and hip joint MST. This study was measured by NRS(Numeric Rating Scale), SLRT(Straight leg raising test), Patrick test, and Blood test(ESR, CRP). Result : After conservative treatment, the patient's pain was controlled and NRS score was decreased. SLRT score also improved. Patrick test unchanged. ESR, CRP score was decreased. Conclusion : As seen in this one case, conservative Korean treatment including hip joint MST has a positive effect to control pain with Bilateral Sacroillitis with Synovitis in Rt. Hip Joint Caused by Ankylosing Spondylitis.
