Chromogenic In Situ Hybridisation Test for Breast Cancer Patients with Equivocal IHC Results - a Study from Iran

  • Background: HER2/neu overexpression on cell membranes of breast cancer cells is due to HER2/neu gene amplification and it is important to identify potential candidates for anti HER2 therapy with trastuzumab. IHC, FISH and CISH are standard FDA approved assays currently used to determine HER2 status in routine practice. The aim of this study was to determine HER2 gene amplification, using the CISH method in breast carcinoma samples which had IHC +2 reactions. Materials and Methods: This study was conducted from 2008-2010 using 334 consecutive breast carcinoma samples referred from local laboratories to Mehr Hospital. CISH assays were performed for all cases, and IHC tests were also done for determining efficacy and accuracy of local labs. HER2 status in local IHC tests was compared with central IHC and CISH results. Results: Of 334 breast cancer patients, 16 were negative for HER2 IHC (0, +1), 201 cases were equivocal (+2), and 31 positive (+3). Of 334 referral cases, 88 were CISH positive (26.3%) and 246 were CISH negative (73.7%). Of 201 IHC +2 cases, HER2 gene amplification was observed in 42 cases (kappa: 0.42). A 29.9% concordance was found between local IHC and central IHC. Sensitivity and specificity of local IHC were 90% and 53.8%, respectively. Conclusions: Low accuracy of IHC results in local labs was associated with the following factors: using former FDA-approved criteria for HER2 interpretation, utilizing non-validated kits, and lack of any quality assurance program. Therefore, following the new 2014 ASCO/CAP guideline and comprehensive quality assurance should be implemented to ensure accuracy of HER2 testing.

횡문근융해증이 합병된 중증 저나트륨혈증 환자에서 발생한 삼투성 탈수초 증후군 1예 (A Case of Osmotic Demyelination Syndrome in a Patient with Severe Hyponatremia Complicated by Rhabdomyolysis)

  • Hyponatremia, the most common electrolyte disorder, has been rarely reported as causing rhabdomyolysis. Osmotic demyelination syndrome (ODS), a demyelinating disease of the central pons and/or other areas of the brain, is infrequently reported as associated with rapid correction of hyponatremia. This paper reports a case of ODS after correction of severe hyponatremia complicated by rhabdomyolysis. A 47-year-old female with a history of chronic alcoholism presented herself at the hospital with altered consciousness after three days of nausea and vomiting. She was on a thiazide diuretic for essential hypertension. Her blood tests upon her hospital admission showed hyponatremia ($Na^+$ 98 mEq/L), hypokalemia ($K^+$ 3.0 mEq/L), and elevation of her serum creatine phosphokinase (3,370 IU/L) with an increase in her serum myoglobin level 11,267 ng/mL). She was treated with intravenous fluid therapy that included isotonic and hypertonic salines along with potassium chloride. She became more alert, and her neurological condition gradually improved after the first five days of her therapy. On the ninth day after her admission, she developed progressive quadiaresis associated with dysarthria, dysphagia, and dystonia despite the resolution of her hyponatremia. Magnetic resonance imaging of her brain on 16th day revealed symmetrical areas of signal hyperintensity in her central pons, basal ganglia, and precentral gyrus in T2-weighted images, which are consistent with ODS. Her neurological symptoms steadily improved after six weeks with only supportive treatment and rehabilitation.

The Characterization of Anti-HER-2/neu Monoclonal Antibody using Different in vivo Imaging Techniques

  • Recently, specific antibodies have been used extensively to diagnose and treat various diseases. It is essential to assess the efficacy and specificity of antibodies, especially the in vivo environment. Anti-HER-2/neu mAb was evaluated as a possible transporting agent for radioimmunotherapy. The monoclonal antibody was successfully radio-labeled with $^{131}I$. In vitro binding assays were performed to confirm its targeting ability using another radio-iodine, $^{125}I$. Binding percentage of $^{125}I$ labeled anti-HER-2/neu mAb in HER-2/neu expressing CT-26 cells was found to be 4.5%, whereas the binding percentage of $^{125}I$ labeled anti-HER-2/neu mAb in wild-type CT-26 was only 0.45%. In vivo images were obtained and analyzed through $\gamma$-camera and an optical fluorescent modality, IVIS-200. $\gamma$-camera images showed that $^{131}I$ labeled anti-HER-2/neu mAb accumulated in HER-2/neu CT-26 tumors. Optical imaging based on near infrared fluorescence labeled anti-HER-2/neu mAb showed higher fluorescence intensities in HER-2/neu CT-26 tumors than in wild-type CT-26 tumors. Anti-HER-2/neu mAb was found to specifically bind to its receptor expressing tumor. Our study demonstrates that in vivo imaging technique is a useful method for the evaluation of an antibody's therapeutic and diagnostic potentials.

Edith Head의 작품을 통해 본 영화의상에 관한 연구 (A Study on Cinema Costumes of Edith Head s Work)

  • Throughout her life time, Edith Head produced cinema costumes for over one thousand movies. She was the leading cinema costume designer in Hollywood. Since the Academy Awards added the area of costume design to be recognized [during 33 years of her work(1948∼1981)], she was nominated 35 times an awarded eight times. However, there were no significant studies done on her work or accomplishments although she was acknowledged as an ingenue and creative designer who elicited artistic effects by reflect character's images through their costumes. The purpose of the study was focused on revealing Edith Head's life and work as her creations had a great impact in he field of cinema costumes. It was also to emphasize the forceful affect of cinema costumes on fashion and to disclose the powerful influence of cinema costume designers. The method of the study includes reviews of literature, movies for which she created costumes, her own possessions, and fashion magazines as well as her works that were nominated and chosen for the Academy Awards. Edith head's emphasis in costume design was based on a blend of both simplicity and sophistication avoiding excessive exaggeration and transformation in her work. Her work was also realistic and contemporary. She developed a dramatic ambience through an innovative design while preserving the actor's individual character and uniqueness. Through this accept her her work was highly renowned as an art form as it was gaining popularity. She was also a dominant figure who influenced trends in the areas of fashion magazines, uniforms, and public clothing. In the review of her cinema costumes, it was revealed that the styles of cinema costumes can be as quickly adapted by the public as movies are viewed because the cinema costumes are highly projected through a visual channel. Subsequently, a follow up study on the affect of cinema costumes on fashion would be beneficial.

Vivienne Westwood in Context and Englishness in Her Work

  • A representative British designer, Vivienne Westwood's work. world from radical punk style to aristocratic historical dress is explored in context in terms of Englishness. National identity opens up into the process of mobilization of collective sentiment in the national context, unlike nationalism, and Englishness signifies the idea or emotion of England in contrast with Britishness, the political constructor influenced by geographical aspects. There is no doubt that Vivienne Westwood is central to ideas about creativity and originality in English design on subculture. However, in evaluating a designer and her work we should consider the entire context surrounding her from a broader view, rather than arguing only her own ingenuity. In this article, through reconsidering her originality in the historical reference as well as the resistant punk style in aspect of fluid national identity, I show a case of a constituted Englishness, forged by Vivienne Westwood as a cultural creator of national identity. Vivienne Westwood's case hints the complexities of national culture, which constantly shifts, translating her understandings of history and culture into fashion in her contemporary insight and glamorous ways.

"한진인천"호 침몰종의 원인에 관한 이론적인 연구 (A Theoretical Study on the Causes of the Sinking Disaster of M/V Hanjin-Inchon)

  • The disasters of wet bound vessels have been more frequent than those of east bound ones on the sea routes of the North Pacific Ocean in winter season. M/V Hanjin-Inchon was also west bound in her missing voyage. The container vessel of 17, 676 gross tons, M/V Hanjin-Inchon owned by Hanjin Shipping Co.Ltd in Seoul Korea left seattle in west coast of U.S.A for Pusan , Korea on the 5th Feb., 1987 and sailed along the exact courses recommended by Ocean Routes until she reported her position and speed as 49-30N, 158-00E and 8 knots to her head office in Seoul by this ship's time 2200 hours on the 13th Feb., 1987. The above message turned out to be last message from her because she had been missing since then leaving no message but only two life boats of her name, three containers cases, large scale of oil slicks and the corpse of her 3rd mate drifting on the sea near the position reported by her last message.

HER2 induces expression of leptin in human breast epithelial cells

  • A close association between the obesity hormone leptin and breast cancer progression has been suggested. The present study investigated the molecular mechanism for enhanced leptin expression in breast cancer cells and its functional significance in breast cancer aggressiveness. We examined whether leptin expression level is affected by the oncoprotein human epidermal growth factor receptor2 (HER2), which is overexpressed in ~30% of breast tumors. Here, we report, for the first time, that HER2 induces transcriptional activation of leptin in MCF10A human breast epithelial cells. We also showed that p38 mitogen-activated protein kinase signaling was involved in leptin expression induced by HER2. We showed a crucial role of leptin in the invasiveness of HER2-MCF10A cells using an siRNA molecule targeting leptin. Taken together, the results indicate a molecular link between HER2 and leptin, providing supporting evidence that leptin represents a target for breast cancer therapy.

부산 402호의 조종 성능에 관한 연구 (Maneuverabilities of the M . S . Pusan 402)

  • The maneuverabilities of the M. S. PUSAN 402 are studied, based on maneuvering indices and the data obtained from her Z test. The results obtained are summarized as follows: 1. The maneuvering indices K' and T' of the M. S. PUSAN 402 are 1. 490, 1.030 at 10$^{\circ}$ Z test and 2.644, 1.153 at 20$^{\circ}$ Z test and 3.382. 1. 027 at 30$^{\circ}$ Z test respectively. The above calculated values K', T' showed that her maneuverabilities are more increased when the rudder is used to large angle than to smaIl angle. 2. As her maneuvering indices K' and T' at 10$^{\circ}$ Z test are higher than the standard maneuvering indices of fishing boats, her turning ability was found to be higher but her obeying ability lower. 3. When the M. S. PUSAN 402 took a turn at her 10$^{\circ}$ test, running distance was about 8.4 times her own length and didn't exceed the standard maneuvering distance, 5 to 11 times ship's own length, therefore she was considered to have good maneuverabilities syntheticaIly.

  • In the history of fashion, few designers have interpreted the background of the time more accurately and energetically than did Elsa Schiaparelli. She understood the new role of women and believed clothes should suit one's life style. Schiaparelli begun with sportswear, later included suits and dresses. She produced them of great elegance and extreme chic. Simplicity of line was the key to her distinctive and elegant silhouette. Even her simplest designs had elegance. Her concept of clothes was architectual : the more the plane of the body were respected, the more the garment acquired vitality. Schiaparelli combined her knowledge, timing, and sense of daring in the presentation of her designs, colors, fabrics, and embroideries. For V, the garment was not only the medium for the couturier's craft but also the place for artistic expression. Her self-conscious equation of designer's objectives with that of artist is at the heart of her work. Schiaparelli's work is an expression of desire, not merely of design.

인간 유방암 세포 이식마우스에서 EGFR/HER2 복합 Tyrosine Kinase 억제제인 GW572016에 의한 방사선증진효과 (Radiation Response Modulation of GW572016 (EGFR/HER2 Dual Tyrosine Kinase Inhibitor) in Human Breast Cancer Xenografts)

  • 목적: EGFR, HER2 과발현 인간 유방암 세포를 이용한 종양이식 마우스에서 EGFR/HER2 복합 Tyrosine Kinase 억제제인 GW572016이 방사선반응성에 미치는 영향을 알아보고 종양조직의 EGFR/HER2수용체 억제효과 및 EGFR down stream signal pathway 단백인 ERK 1/2, PI3k/Akt 억제효과를 알아보고자 하였다. 대상 및 방법: SUM 102와 SUM 149 EGFR 과발현 세포와 SUM 185, SUM 225 HER2 과발현 세포를 우측 옆구리 피하에 접종하여 종양이식마우스를 만들었다. 이식마우스는 2군으로 나누어 한 군은 GW572016에 의한 EGFR/HER2 수용체 억제와 down stream signal 단백의 활성 변화를 Immunoprecipitation과 Western blot의 방법을 사용하여 관찰하였고 다른 한군은 GW572016에 의한 방사선감수성 변화를 알아보기 위해 1) 대조군, 2) GW572016 단독군, 3) 방사선단독군, 4) GW572016+방사선병용투여군으로 나누어 종양성장을 비교 관찰하였다. GW572016에 의해서 SUM 149, SUM 185이식종양에서 EGFR및 HER2 수용체의 활성이 억제되었으며 특히 SUM 185, HER2 과발현 이식종양에서는 ERK 1/2 down stream 단백의 활성도 억제되었다 SUM 225 HEH2 과발현 이식종양에서는 이전의 in vitro실험에서와 달리 GW572016에 의해 HER2수용체의 활성변화가 없었으나 ERK 1/2, Akt의 활성은 모두 억제되었다. GW572016에 의해 SUM 149과 SUM 185에서 종양성장억제효과가 관찰되었고 특히 SUM 149에서는 GW572016과 방사선치료병용군에서 종양성장억제효과가 좀더 뚜렷하여 방사선감수성을 증가시키는 것으로 생각되었다. 결 론: GW572016은 EGFR 혹은 HER2 과발현 유방암세포에서 EGFR/HER2 수용체 억제와 down stream signal 단백의 활성을 억제시켰으며 SUM 149에서는 방사선감수성을 증가시키는 것으로 생각된다. 향후 EGFR을 표적으로 하는 억제제치료에서 EGFR 수용체억제뿐 아니라 down stream 단백의 활성억제 여부가 방사선 감수성 및 저항성의 극복과 관련이 있으리라는 근거를 설명할 수 있으며 향후 좀더 깊이 있는 연구가 필요하다.
