The usefulness of soluble transferrin receptor in the diagnosis and treatment of iron deficiency anemia in children

  • Purpose: Soluble transferrin receptor (sTfR) is a truncated extracellular form of the membrane transferrin receptor produced by proteolysis. Concentrations of serum sTfR are related to iron status and erythropoiesis in the body. We investigated whether serum sTfR levels can aid in diagnosis and treatment of iron deficiency anemia (IDA) in children. Methods: Ninety-eight patients with IDA were enrolled and were classified according to age at diagnosis. Group 1 comprised 78 children, aged 6-59 months, and group 2 comprised 20 adolescents, aged 12-16 years. Results: In group 1, patients' serum sTfR levels correlated negatively with mean corpuscular volume; hemoglobin (Hb), ferritin, and serum iron levels; and transferrin saturation and positively with total iron binding capacity (TIBC) and red cell distribution width. In group 2, patients' serum sTfR levels did not correlate with ferritin levels and TIBC, but had a significant relationship with other iron indices. Hb and serum sTfR levels had a significant inverse relationship in both groups; however, in group 1, there was no correlation between Hb and serum ferritin levels. In 30 patients of group 1, serum sTfR levels were significantly decreased with an increase in Hb levels after iron supplementation for 1 month. Conclusion: Serum sTfR levels significantly correlated with other diagnostic iron parameters of IDA and inversely correlated with an increase in Hb levels following iron supplementation. Therefore, serum sTfR levels can be a useful marker for the diagnosis and treatment of IDA in children.

Comprehensive Laboratory Analysis of Korean Acute Alcoholic Intoxication Patients Reveals the Need for a National Hepatitis B Virus Vaccination Program in Korea

  • Background: Acute alcoholic intoxication patients (AAIP) are a common public health problem. The aim of this study was to perform a comprehensive laboratory analysis for these patients to investigate the co-morbid medical problem. Methods: We retrospectively reviewed laboratory findings of AAIP who were transferred to the emergency department (ED) from January 2017 to June 2017. Results: A total of 160 male patients were enrolled. Sixteen patients (16/160, 10.0%) and three patients (3/160, 1.9%) had macrocytic anemia and microcytic anemia, respectively. A total of 33 patients (33/160, 20.6%) showed thrombocytopenia ($<150{\times}10^9/L$). Twelve patients (12/159, 7.5%) showed low serum albumin level (<3.5 g/dL). Three patients (3/160, 1.9%) had chronic kidney disease stages 3-4 based on estimated glomerular filtration rate. Six patients (6/27, 22.2%) had high hemoglobin A1c (HbA1c) level (>7.0%). Positive rates of hepatitis B surface antigen and antiHBs antibody (anti-HBs Ab) were 3.5% (5/141) and 49.0% (68/141), respectively. Conclusion: Patients with AAIP who were transferred to ED had various laboratory abnormalities (anemia, thrombocytopenia, high HbA1c). They had low positive rate of anti-HBs Ab. This might be a public health problem, suggesting the need of hepatitis B virus vaccination program for AAIP. Our data suggest the need of further nationwide studies.

Association of Hemoglobin A1c with Visceral Fat Measured by Computed Tomography in Nondiabetic Adults (당뇨병이 없는 농촌지역의 건강한 성인 남녀에서 당화혈색소와 내장지방과의 관계)

  • Objective: A prediabetes hemoglobin A1c (HbA1c) level of 5.7%-6.4% is considered a risk factor for diabetes mellitus and cardiovascular disease (CVD) in the USA. In this study, we assessed the HbA1c and visceral fat levels as CVD risk factors in health check-up examinees who were not yet diagnosed with diabetes. Methods: Totally, 507 study subject were categorized as per criteria of the American Diabetes Association, depending on whether the HbA1c level was ${\geq}5.7%$ or <5.7%. Lipid levels, blood pressure, BMI (kg/$m^2$), total abdominal, and visceral fat levels were measured by computed tomography. Results: The mean of HbA1c in the male group was larger than the mean in the female group and their values were, respectively, $6.03{\pm}0.82%$ and $5.88{\pm}0.72%$(p<0.05). Only the mean values of age and visceral fat area were different between $HbA1c{\geq}5.7%$ and <5.7% in both male and female group(p<0.05). Visceral fat levels were significantly associated with HbA1c in the group of HbA1c ${\geq}5.7%$ (odds ratio=1.005, 95% CI 1.002~1.008). Conclusions: Visceral fat levels were significantly higher and correlated with the group which HbA1c level is ${\geq}5.7%$. This finding suggests that subjects who have high levels of HbA1c should be carefully monitored during prediabetes and should have chance to have health education programs.

Effect of Nutrition Education on Diabetic Management in Diabetic Patients (영양교육이 당뇨병환자의 식사요법에 대한 지식과 실천 및 인지도, 혈당관리에 미치는 영향)

  • The purpose of this study was to investigate the effect of nutritional education on the knowledge and the practice & Perception of diet therapy, and on the level of FBG and HBAlc. The subjects were selected among the patients treated in the Inha University Hospital from June of 1996 until April of 1998. Educated group consisted of 20 patients who visited the diet-counseling department as a part of diabetic treatment. Non-educated group consisted of 20 patients who visited the diet-counseling department ignoring doctors prescription purpose. The knowledge and the practice & perception of diet therapy drinking & smoking habits, regular exercise, and general characteristics including family history were analyzed by questionnaires. FBG and HbAlc were measured before and 4 months after the treatment. The results were as follows : Both groups were composed of 12(60%) male and 8(40%) female respectively. The mean age of subjects was 51.4 years in educated group and 51.9 tears in non-educated group. The duration and %IBW were 5.9 years and 109.8 % in educated group and 5.5 years and 111% non-educated group respectively. There were no significant differences in drinking & smoking habits and regular exercise between groups. The diet therapy knowledge score was significantly higher in educated group(p<0.001, 5.8$\pm$2.3/10.0 vs 3.4$\pm$2.0/10.0). The diet therapy practice & perception score was also significantly higher in educated group(p<0.001, 5.6$\pm$2.3/10.0 vs 2.8$\pm$2.2/10.0). Levels of FBG and HbAlc in both group were reduced at 4 month after the treatment(-74.2 mg/dl and -1.4% in educated group vs -58.7 mg/dl and -2.0% in non-educated group). However, the reduced amounts of them were not significantly different. Therefore, nutritional education seems to be very important in improving the knowledge and the practice & perception of the diet therapy. However, it appears no direct influence in reducing FBG and HbAlc levels. Further studies on diabetic management by using more efficient, consecutive and intensive nutritional education methods are needed. (Korean J Nutrition 34(1):69-78, 2001)

  • Comparing the amount of Hb, the nutriture of the deaf-mutes are better than that of the amentias and the nursery school children. The former, however, shows less condition than that of the boys and girls in the junior physical education school (Hb : 12.7) and the normal family children (Hb : 10.8). In the group of six to eleven years old that is a class a elementary school, the physical condition such as stature, chest circumference, and weight of the deaf -mute is respectively almost the same degree, which is above standard in our country. On the contrary the degree of the amentias and the nursery school children has an inferiority approaching to the standard. In the group of twelve to fifteen years old that is a class of junior high school, the nursery school children have the lowest physical condition and the boys and girls of the physical training school the highest.   • Purpose : The lipid profile is a major predictive index for cardiovascular disease, but there have only been a few studies conducted on the relationship between lipid ratio and cardiovascular disease risk factors in the Korean population. To address this research gap, we investigated the association between three lipid ratios and cardiovascular disease risk factors among Korean adults. Methods : This study used data from the seventh Korea National Health and Nutrition Examination Survey conducted in 2016. Lipid ratios included triglyceride (TG)/high density lipoprotein cholesterol (HDL), total cholesterol (TC)/HDL, and low density lipoprotein cholesterol (LDL)/HDL. Cardiovascular risk factors included in this research were: systolic blood pressure $(SBP){\geq}140$, diastolic blood pressure $(DBP){\geq}90$, fasting blood sugar $(FBS){\geq}126$, $HbA1c{\geq}6.5$, body mass index $(BMI){\geq}25$, waist circumference (WC) $men{\geq}90$, $women{\geq}85$, and metabolic syndrome (MetS). A complex samples logistic regression test was performed to analyze the association between lipid ratios and cardiovascular disease risk factors. Results : 1) TG/HDL ratio had statistically significant relationships with DBP, FBS, HbA1c, BMI, WC and MetS. 2) TC/HDL ratio was correlated to SBP, DBP, FBS, BMI, WC, and MetS. 3) LDL/HDL ratio had association with BMI, WC, and MetS. Conclusion : We identified significant association between lipid ratios and cardiovascular disease risk factors. The three lipid ratios were particularly strongly associated with BMI, WC, and MetS.

Effects of Araliaceae Water Extracts on Blood Glucose Level and Biochemical Parameters in Diabetic Rats (두릅나무과 열수추출물이 당뇨흰쥐의 혈당과 혈액 성분에 미치는 영향)

  • This study was conducted to investigate the effects of Aralia elata, Acanthopanacis cortex and Ulmus davidiana water extracts on blood hemoglobin, $HbA_{1c}$, levels and biomarkers in the streptozotocin (STZ)-induced diabetic rats. Male Wistar rats divided into normal and diabetic groups. The diabetic groups subdivided into the control group (DM) and Araliaceae water extracts supplemented groups: Aralia elata (AE), Acanthopanacis cortex (AC) and Ulmus davidiana (UD). The extracts were supplemented in diet base on 11.42 g of raw Araliaceae/kg diet for 7 weeks. The diabetes was induced by injecting 572 (55 mg/kg B.W., i.p.) once 2 weeks before sacrifying. Relative weights of liver were significantly lowered in the DM group compared to the normal group, whereas those of kidney and heart were significantly increased in the DM group. Supplementation of the Araliaceae water extracts improved reduced liver weights in STZ-induced diabetic rats. Blood glucose level was significantly higher in the DM group than in the normal group, whereas insulin contents were significantly lowered in the DM groups. However, these parameters were normalized in the An, AC and UD supplemented groups, respectively. Blood hemoglobin and $HbA_{1c}$ levels were significantly higher in the DM group than in the normal group. When all of Araliaceae water extracts were supplemented to the diabetic rats lowered hemoglobin and $HbA_{1c}$ levels. Red blood cell, white blood cell and Lymphocyte were significantly higher in the DM group than in the normal group. The supplementation of Araliaceae family water extracts significantly lowered these parameters compared to the DM group. MCV, MCH contents were declined in the DM group, while the supplementation of Aralia elata, Acanthopanacis cortex and Ulmus davidiana water extracts elevated of these contents in STZ-induced diabetic rats. Accordingly, these results indicate that Aralia elata, Aeanthopanacis corex and Ulmus davidiana water extracts would seem to improve the blood biomarkers in STZ-induced diabetic rats.

An Antibacterial Lectin from Lampteromyces japonicus (화경버섯의 항세균성 렉틴)

  • A lectin was isolated from the fruiting bodies of Lampteromyces japonicus by preparative PAGE and named LJAP (Lampteromyces japonicus antibacterial protein). LJAP was a polymeric protein of more than one hundred kDa consisting of 17-kDa subunits. The amino acid analysis revealed a high content of serine, glycine, and acidic amino acids. LJAP has an excellent antibacterial activity for Escherichia coli, JM 109, K 12, HB 101, and JW 380. By the inhibition assay of the antibacterial activity, a glycoprotein, asialofetuin was confirmed as the best inhibitor. This is the first lectin isolated and characterized its antibacterial and agglutination activities from the family Lampteromyces.

A Case Report of Add-On Traditional Korean Medical Treatments in a Patient with Uncontrolled Type 2 Diabetes Mellitus with Conventional Oral Hypoglycemic Agents (기존의 경구용 혈당강하제로 혈당조절이 불량한 제2형 당뇨병 환자에 대한 한양방 병용 치료 1례)

  • This study presents a case of add-on traditional Korean medical treatments in a 77-year-old male patient with uncontrolled type 2 diabetes mellitus with conventional oral hypoglycemic agents. Chief complaints were uncontrolled glucose level and weight loss. The patient was treated with electroacupuncture (on ST36, CV4, and CV12), herbal medication, and lifestyle advice. The conventional oral hypoglycemic medications were maintained. After 12 weeks, the glycated hemoglobin A1c (HbA1c) decreased from 11.0% to 8.4%. There were significant abnormalities in the liver function and renal function tests, but no adverse event was reported. Further studies are needed to confirm the findings of this case.

The Changes of Appearance Formability of Hanji Blended Fabrics after Fusing (한지사 혼용 직물의 접착심 접착 후 외관 형성능의 변화)

  • In order to examine the changes in the appearance properties and the post-adhesion appearance properties of Hanji yarn blended fabrics : 100% Hanji yarn fabric, two kinds of cotton / Hanji yarn blended fabrics and 100% cotton fabric, were selected and fused with three kinds of interlinings. After fusing, changes of standardized KES values were examined. 1. W/T, B/W of Hanji yarn blended fabrics was higher than that cotton fabric. WC/W, 2HB/W, 2HB/B, and 2HG/G values of Hanji yarn blended fabrics are lower than cotton fabric. This means that the Hanji yarn was mixed, shape retention, wrinkle recovery was improved, and the drape property was lowered. 2. After fusing, W/T, shape retention, wrinkle recovery of Hanji yarn blended fabrics increased, and WC/W values of Hanji yarn blended fabrics decreased. The wrinkle recovery property of Hanji yarn blended fabrics were improved; however, the 2HG/G value of Hanji yarn fabric increased due to fusing, and the wrinkle recovery property of Hanji yarn fabric decreased. 3. In the selection of adhesive core, I1 adhesive core is excellent in terms of shape stability and wrinkle recovery; however, an I3 adhesive core is recommended for drape and silhouette formation. When the fabric of the adhesive core was PET, it was found to penetrate better between the fabrics during adhesion than the case of cotton fabrics.
