Reliability of Stool Antigen Tests: Investigation of the Diagnostic Value of a New Immunochromatographic Helicobacter pylori Approach in Dyspeptic Patients

  • Background: A diagnosis of H. pylori infection can be made by invasive or non-invasive methods. Several noninvasive diagnostic tests based on the detection of H. pylori stool antigen (HpSA) have been developed. The Genx H. pylori stool antigen card test is a new rapid, non-invasive test that is based on monoclonal immunochromatographic assay. The aim of this study was to determine its sensitivity, specificity, and diagnostic accuracy for diagnosing H. pylori infection in adult patients. Materials and Methods: A total of 162 patients were included in the study. A gastric biopsy was collected for histopathology and rapid urease testing. Stool specimens for HpSA testing were also collected. Patients were considered H. pylori positive if two invasive tests (histological and rapid urease tests) were positive. Results: Using the reference test, 50.6% of the samples were positive for H. pylori infection. The Genx H. pylori antigen test was positive in 19.7% of patients. The sensitivity, specificity, positive predictive value, negative predictive value, and diagnostic accuracy of the Genx H. pylori antigen test were 51.6%, 96.0%, 88.8%, 76.1%, and 79.0%, respectively. Conclusions: The Genx H. pylori stool antigen card test is a new non-invasive method that is fast and simple to perform but provides less reliable results.

  • Identification of escherchia coli K1 polysaccharide antigen isolated from urine specimens of urinary tract infections in children were performed from of 1992 to 1993 in Kyoto, Japan. The serotypes of E. coli were categorized that O1:H7, O2:H6, O2:H7, O16:H6, O18:H7, O18:H ̄, and O135:H44 among 14 strains isolated from urine specimens of urinary tract infections in children by the serological test. And, one strain (O18:H ̄, isolation rate: 7.1%) of E. coli K1 polysaccharide antigen among 14 strains were isolated from urine specimens of urinary tract infections in children by the bacteriophage test.

  • 본 실험은 H-Y 항원 유전자 크로닝을 위한 기초연구로서 H-Y 항원의 특성을 규명하기 위하여 친화성 크로마토그래피에 의하여 H-Y 항원을 분리·정제하였다. 정소 추출액을 항체가 결합된 column에 결합시킨 뒤 10% acetic acid로 용출시켰다. 용출된 분획을 모아 농축한 후 HPLC와 SDS-PAGE를 실시하여 H-Y 항원의 분자량은 약 6,7000달톤 임을 알 수 있었으며 isoelectric focusing에 의하여 등전점(pI)은 5.0인 것으로 측정되었다. H-Y 항원에 대한 단일클론항체와 표지항원으로는 H-Y 항원-ABEI(aminobutylethyl isoluminol)를 사용하여 H-Y 항원 정량을 위한 화학발광면역분석법을 개발하였다. 항원항체 반응후 빛의 측정은 NaOH 존재하에서 microperoxidase/H2O2를 이용한 산화반응으로 실시하여 10초간 측정한 빛의 양을 적분하였다. H-Y 항원의 농도와 빛의 양과는 역비례하였으며 감도는 11.8ng/tube 정도이었다.

  • Antibody based biosensors are very selective and ultra-sensitive. Antigen-antibody reactions have been used in immunoassays. In this research, a biosensor which uses antigen-antibody reaction was developed to measure and detect rat IgG. Because the antigen-antibody reaction is a physical bounding between antigen and antibody, there are several ways to measure an antigen-antibody reaction. Among the methods, impedance analysis has short measuring time and possibilities of analyzing various properties of the reaction using frequency analysis. Rat IgG could be detected with developed biosensor and impedance analyzer. The biosensor showed good repeatability and availability of detecting concentration changes of rat IgG.

  • Background: Helicobacter pylori (H.pylori) is one of the most important causes of dyspepsia and gastric cancer and diagnosis can be made by invasive or non-invasive methods. The Atlas Helicobacter pylori antigen test is a new rapid non-invasive method which is simple to conduct. The aim of this study was to determine its sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV) and accuracy. Materials and Methods: This prospective study was conducted between July 2012 and December 2013. Stool samples of 59 dyspeptic patients who underwent upper endoscopy were evaluated for H. pylori stool antigen. Results: From the 59 patients who participated in this study, there were 36 (61%) males and 23 (39%) females. H. pylori was diagnosed in 24 (40.7%) gastric biopsies, 22 (91.7 %) of these being positive for the Atlas H. pylori antigen test. The sensitivity, specificity, PPV, NPV and accuracy were 91.7%, 100%, 100%, 94.6% and 96.6% respectively. Conclusions: The Atlas H. pylori antigen test is a new non-invasive method which is simple to perform and avails reliable results in a few minutes. Thus it can be the best option for the diagnosis of H. pylori infection due to its high sensitivity and specificity.

Latex 응집반응을 이용한 동물의 톡소플라즈마병 진단액 개발에 관한 연구 (Development of antigen for the microplate latex agglutination test on toxoplasmosis in animals)

  • This study was conducted to develop a sensitized latex-antigen for serodiagnosis of toxoplasmosis in animals. Tachyzoites of T gondii(RH-strain) harvested from mouse peritoneal cavity were purified through the filtraton of polycarbonate membrane(pore size, $3.0{{\mu}m}$, Costar Co.) and disrupted by ultrasonicator. The tachyzoite suspension was ultracentrifuged for 30 min at $60,000{\times}g(4{^{\circ}C})$ and the supernatant was used as a water-lysate antigen. Polystyrene latex particles of $0.8{{\mu}m}$ in diameter(Sigma) were used for the preparation of sensitized latex-antigen suspension. The several parameters including the preparation conditions, incubation buffer. serum dilution buffer and stability of agglutination reactions were evaluated and the results obtained were summarized as follows : 1. The antigen consisting of a water-lysate of T gondii tachyzoites was adsorbed onto polystyrene latex particles of $0.8{{\mu}m}$ in diameter by adding a latex suspension to an equal volume of diluted antigen solution and by incubating the mixture at $37{^{\circ}C}$ under different conditions. 2. The optimum incubation buffer used for the antigen sensitization was 0.1M Tris-HCl buffer(pH 8.0). 3. The optimum serum dilution buffer used for the latex agglutination test was 0.1M Tris-HCl-NaCl buffer(pH 7.4) containing 300 mM NaCl. But 0.1M Tris-HCl-NaCl buffer(pH 7.4) containing 300-600 mM NaCl, 0.5% BSA and 0.01% Tween-20 improved the agglutination pattems and cleared the background of microplate well without the effects on L.A titer. 4. The time required for antigen sensitization was 40 and 60 min in incubation buffer(pH 8.0) at $37{^{\circ}C}$. But the optimun time for antigen sensitization was min at $37{^{\circ}C}$. 5. The optimun quantity of antigen absorbed on latex particles for proper agglutination was the range of 20 to $32{\mu}g$ of latex particles. 6. The optimun concentration of the latex-antigen suspension for the proper agglutination reaction was determined as 0.2%(w/v). 7. The specificity, rapidity and simplicity of the latex-particle agglutination test suggested that it might be adaptable to large scale serum screening.

Helicobacter pylori로부터 유래된 항원의 항원성에 관한 연구 (Immunotoxicity Study of Separated Antigen from Helicobacter pylori.)

  • 본 연구에서는 H. pylori의 감염을 예방하고 치료보조제로 사용할 목적으로 포유동물을 통한 피동면역용 항체를 생산하고자 하였다. 따라서 anti-H. pylori 항체를 함유한 면역우유 생산용 백신개발에 기초자료를 얻고자 H. pylori의 면역성과 면역독성에 관한 실험을 수행하였다. 백신을 반복해서 투여했을 때 야기될 수 있는 알레르기 및 과민반응을 예측하고, 페니실린 쇼크와 같은 심각한 부작용 및 독성유발 가능성을 검색하기 위하여 백신의 면역독성을 평가하였다. 전신성 anaphylaxis 쇼크반응의 유무를 평가하기 위해 각 군당 5마리의 guinea pig에 감작투여한 후 최종감작 1주와 2주째에 귀정맥(ear vein)으로 야기항원을 투여하였다. anaphylaxis 쇼크 반응 시험에서 5마리 중 1마리가 양성반응을 나타낸 경우 의약품의 안전성 평가라는 관점에서는 양성으로 판정한다[11]. H. pylori의 WC항원에 대한 1차 및 2차 야기항원 투여 후 전신성 anaphylaxis 쇼크반응에 대한 관찰 결과는 다음과 같다. H. pylori의 WC에 대한 anaphylaxis 쇼크반응은 WC (H) $60\;{\mu}g/100\;{\mu}l$의 항원농도에서 1차, 2차 야기항원 투여 모두 경증의 증상을 나타내었고, WC (L) $20\;{\mu}g/100\;{\mu}l$의 항원농도에서는 아무런 anaphylaxis 쇼크 증상이 관찰되지 않았다. 그리고 crude urease에 대한 anaphylaxis 쇼크반응은 항원농도가 $20\;{\mu}g/100\;{\mu}l$의 urease (L)와 $60\;{\mu}g/100\;{\mu}l$의 urease (H) 모두에서 아무런 증상도 관찰되지 않았다. Guinea pig-rat를 이용한 PCA 시험에서는 WC (H), WC (L), urease (H), urease (L) 투여군 모두에서 양성반응이 나타나지 않았다. 피부감작성 시험에서는 항원농도에 따라 각각 $80\;{\mu}g/100\;{\mu}l$, $40\;{\mu}g/100\;{\mu}l$, $20\;{\mu}g/100\;{\mu}l$, $20\;{\mu}g/100\;{\mu}l$ 일 때 피부이상 증상 즉, 피부 트러블이 발생하지 않는 최고의 항원농도는 $40\;{\mu}g/100\;{\mu}l$인 것으로 관찰되었다. 결론적으로 항원성 시험에서 H. pylori로부터 분리된 urease 항원이 WC 항원보다 면역독성 측면에서 좀 더 안전할 것으로 조사되었다.

Clinical characteristics of 2009 pandemic influenza A (H1N1) infection in children and the performance of rapid antigen test

  • Purpose: In autumn 2009, the swine-origin influenza A (H1N1) virus spread throughout South Korea. The aims of this study were to determine the clinical characteristics of children infected by the 2009 H1N1 influenza A virus, and to compare the rapid antigen and realtime polymerase chain reaction (PCR) tests. Methods: We conducted a retrospective review of patients ${\geq}18$ years of age who presented to Soonchunhyang University Hospital in Seoul with respiratory symptoms, including fever, between September 2009 and January 2010. A real-time PCR test was used to definitively diagnose 2009 H1N1 influenza A infection. Medical records of confirmed cases were reviewed for sex, age, and the time of infection. The decision to perform rapid antigen testing was not influenced by clinical conditions, but by individual factors such as economic conditions. Its sensitivity and specificity were evaluated compared to real-time PCR test results. Results: In total, 934 patients tested positive for H1N1 by real-time PCR. The highest number of patients (48.9%) was diagnosed in November. Most patients (48.2%) were aged between 6 and 10 years. Compared with the H1N1 real-time PCR test results, the rapid antigen test showed 22% sensitivity and 83% specificity. Seventy-eight patients were hospitalized for H1N1 influenza A virus infection, and fever was the most common symptom (97.4%). Conclusion: For diagnosis of 2009 H1N1 influenza A virus infection, the rapid antigen test was inferior to the real-time PCR test in both sensitivity and specificity. This outcome suggests that the rapid antigen test is inappropriate for screening.

H2-M3의 세포 표면 발현이 NK 세포의 활성에 미치는 영향 분석 (The Cell Surface Expression of H2-M3 Does Not Directly Effect on the Killing Activity of NK Cell)

  • H2-M3 (M3) is a unique antigen presenting molecule which provides N-formylated peptide to certain type of T cells. Previous observation indicated that NK cell activity is significantly diminished during listerial infection in $H2-M3^{-/-}$ mice. To explore the possibility that M3 expression directly effect on NK cell activity, we measured NK cell activity with or without stimulation of N-formylated peptide on antigen presenting cells. Results indicated that the expression of M3 is not directly influence on NK cell activity. Further study will be focused on the indirect effect of M3 on regulating NK cell activity.
