Buried bumper syndrome is a rare but potentially severe complication of percutaneous endoscopic gastrostomy tube insertion. Though this complication is uncommon, it may lead to pressure necrosis, bleeding, perforation, peritonitis, sepsis, or death. Each case of buried bumper syndrome is unique in terms of patient comorbidities and anatomic positioning of the buried bumper. For this reason, many approaches have been described in the management of buried bumper syndrome. In this case report, we describe the case of an adolescent Caucasian female who developed buried bumper syndrome three years after undergoing percutaneous endoscopic gastrostomy insertion. We review diagnosis and management of buried bumper syndrome and describe a novel technique for bumper removal in which we use a guide wire in combination with external traction to maintain a patent gastrostomy lumen while removing the internal percutaneous endoscopic gastrostomy bumper.

Objective: This study introduces exposure concentrations of time-weighted average standard (TWA) evaluation and short-time exposure standard (STEL) evaluation for trichloroethylene in the cleaning process. Methods: Trichloroethylene measurement was conducted according to the KOSHA Guide (A-24-2019) method. It was carried out twice. Results: As a result of the first measurement, TWA concentration exceeded 4 times the exposure standard and STEL concentration exceeded 16 times, but the inaccuracy and breakthrough of the collection time could not be considered, so the second measurement was conducted. The second measurement result was lower than the first measurement result, but exceeded the exposure standards (TWA, STEL). Conclusions: We were able to confirm that the exposure level of workers in the cleaning process using trichloroethylene exceeded the exposure standard. And it is also considered necessary to grasp the approximate concentration using a detector tube in the preliminary survey.
The thallium-doped sodium iodide [NaI(Tl)] scintillation detector is preferred as a gamma spectrometer in many fields because of its general advantages. A silicon photomultiplier (SiPM) has recently been developed and its application area has been expanded as an alternative to photomultiplier tubes (PMTs). It has merits such as a low operating voltage, compact size, cheap production cost, and magnetic resonance compatibility. In this study, an array of SiPMs is used to develop an NaI(Tl) gamma spectrometer. To maintain detection efficiency, a commercial NaI(Tl) $2^{\prime}{\times}2^{\prime}$ scintillator is used, and a light guide is used for the transport and collection of generated photons from the scintillator to the SiPMs without loss. The test light guides were fabricated with polymethyl methacrylate and reflective materials. The gamma spectrometer systems were set up and included light guides. Through a series of measurements, the characteristics of the light guides and the proposed gamma spectrometer were evaluated. Simulation of the light collection was accomplished using the DETECT 97 code (A. Levin, E. Hoskinson, and C. Moison, University of Michigan, USA) to analyze the measurement results. The system, which included SiPMs and the light guide, achieved 14.11% full width at half maximum energy resolution at 662 keV.

A confocal microscope was developed utilizing a scanning sample stage based on a home-built double-compound flexure guide. A scanning sample stage with nano-scale resolution consisted of a double leaf spring based flexure, a displacement amplifying lever, a Piezo-electric Transducer(PZT) actuator and capacitance sensors. The performance of the two-axis stage was analyzed using a commercial finite element method program prior to the implementation. A single line laser was employed as the light source along with the Photo Multiplier Tube(PMT) that served as the detector. The performance of the developed confocal microscope was evaluated with a mouse ear skin imaging test. The designed scanning stage enabled us to build the confocal microscope without the two optical scanning mirror modules that are essential in the conventional laser scanning confocal microscope. The elimination of the scanning mirror modules makes the optical design of the confocal microscope simpler and more compact than the conventional system.
In domestic nondestructive testing(NDT) field, there have recently been radiation exposure accidents due to a disregard for confirmation of the position of radioisotope during the test. In order to prevent these kinds of accidents, a scintillating film has been developed. The scintillating film that can convert gamma-ray to visible light has a function of the position detection of radioisotope in a opaque guide tube of an NDT apparatus. The aim of this study is to enhance the visibility performance of the scintillating film and find out the best configuration of the scintillating film. In order to find appropriate materials for the scintillating film, various inorganic scintillating materials were evaluated in this work. An absolute luminance of the scintillating films was measured by luminance meter for evaluation of visibility performance. Ir-192 gamma projector was used for NDT apparatus. The experiment shows that the scintillating film with reflective layer was the more effective performance for visibility. The higher mixing ratio of scintillating material to binding material, the higher luminance was measured. $Gd_2O_2S(Tb)$ inorganic powder as the scintillating materials had the best performance for visibility of the scintillating film. The developed scintillating film helps to ensure safer environment to the operators.
A new effective methodology for optimizing the enrichment of low-enriched zones as well as gadolinia fuel ($UO_2/Gd_2O_3$) rod designs in PLUS7 fuel assemblies was developed to minimize the maximum peak power in the core and to maximize the cycle lifetime. An automated link code was developed to integrate the genetic algorithm (GA) and the core design code package of ALPHA/PHOENIX-P/ANC and to generate and evaluate the candidates to be optimized efficiently through the integrated code package. This study introduces an optimization technique for the optimization of gadolinia fuel rod designs in order to effectively reduce the peak powers for a few hot assemblies simultaneously during the cycle. Coupled with the gadolinia optimization, the optimum enrichments were determined using the same automated code package. Applying this technique to the reference core of Ulchin Unit 4 Cycle 11, the gadolinia fuel rods in each hot assembly were optimized to different numbers and positions from their original designs, and the maximum peak power was decreased by 2.5%, while the independent optimization technique showed a decrease of 1.6% for the same fuel assembly. The lower enrichments at the fuel rods adjacent to the corner gap (CG), guide tube (GT), and instrumentation tube (IT) were optimized from the current 4.1, 4.1, 4.1 w/o to 4.65, 4.2, 4.2 w/o. The increase in the cycle lifetime achieved through this methodology was 5 effective full-power days (EFPD) on an ideal equilibrium cycle basis while keeping the peak power as low as 2.3% compared with the original design.
This study was conducted to assess need for education on the infection control by examining the knowledge on infection control in home care of the home care trainees who had completed an infection control course. Using 64 items questionnaire. home care trainees of eight home care education institutions who took classes related with infection control. The level of knowledge evaluated subjectively was 4.24point (full mark: 5 point). The correct answer rates for handwashing and gloving. cleaning patients' rooms. preventing sharp injury, preventing exposure to potential infectious agents, home care bag technique were 84.5% 86.4% 70.7% 65.3% 76.2% representatively. For the knowledge on the infection control principles to prevent catheter related infection, indwelling catheter related infection, tracheal tube related pneumonia. L-tube related infection, oxygen therapy related infection. and wound infection were 62.8% 27.8% 39.1% 87.8% 76.5% 80.5% representatively. The correct answer rates varied depending upon the educational institutes (p< = .0001), educational levels (p= .001), workplaces (p<.0001), and the experience of infection control education (p= .001). Considering these results. a standardized curriculum must be developed and implemented after analyzing the existing infection control curricula of the home care education institutes. and guide books for infection control in home care should be developed and distributed. In addition. continuous infection control education be provided to the home care nurses through reeducation so they can acquire new knowledge needed for carrying out infection control activities effectively.

A metrological atomic force microscope (M-AFM) was developed fur the length measurements of nanometer range, through the modification of a commercial AFM. To eliminate nonlinearity and crosstalk of the PZT tube scanner of the commercial AFM, a two-axis flexure hinge scanner employing built-in capacitive sensors is used for X-Y motion instead of PZT tube scanner. Then two-dimensional displacement of the scanner is measured using two-axis heterodyne laser interferometer to ensure the meter-traceability. Through the measurements of several specimens, we could verify the elimination of nonlinearity and crosstalk. The uncertainty of length measurements was estimated according to the Guide to the Expression of Uncertainty in Measurement. Among several sources of uncertainty, the primary one is the drift of laser interferometer output, which occurs mainly from the variation of refractive index of air and the thermal stability. The Abbe error, which is proportional to the measured length, is another primary uncertainty source coming from the parasitic motion of the scanner. The expanded uncertainty (k =2) of length measurements using the M-AFM is √(4.26)$^2$+(2.84${\times}$10$^{-4}$${\times}$L)$^2$(nm), where f is the measured length in nm. We also measured the pitch of one-dimensional grating and compared the results with those obtained by optical diffractometry. The relative difference between these results is less than 0.01 %.

An electromagnetic flowmeter(EMF) was developed and its characteristics were compared with a commercial EMF. The developed EMF was designed as the 100 mm nominal diameter. A signal processing circuit was also developed for generating the magnetic field and converting the flow signal to flowrate and flow quantity. In order to obtain a more stable and reliable flow signal, the double magnetizing frequency was adopted for magnetizing the coil of the EMF. For the characterization of the developed EMF, the uncertainty of calibrator was estimated within $\pm$0.5 %. The evaluation procedure of the uncertainty followed the ISO Guide to the Expression of Uncertainty in Measurement. It was found that the flow signals between the electrodes were about $\pm$60-$\pm$300$\mu$V, which were sufficient for the discrimination of flowmeter and the protection of noise. The test results against the calibrator showed the good linearity in the range of 3 ㎥/h and 70 ㎥/h. A commercialized design of the EMF based on the current study will be technically more competitive in domestic and foreign market.

Francis turbines are often operated over a wide load range due to high flexibility in electricity demand and penetration of other renewable energies. This has raised significant concerns about the existing designing criteria. Hydraulic turbines are not designed to withstand large dynamic pressure loadings on the stationary and rotating parts during such conditions. Previous investigations on transient operating conditions of turbine were mainly focused on the pressure fluctuations due to the rotor-stator interaction. This study characterizes the synchronous and asynchronous pressure and velocity fluctuations due to rotor-stator interaction and rotating vortex rope during load variation, i.e. best efficiency point to part load and vice versa. The measurements were performed on the Francis-99 test case. The repeatability of the measurements was estimated by providing similar movement to guide vanes twenty times for both load rejection and load acceptance operations. Synchronized two dimensional particle image velocimetry and pressure measurements were performed to investigate the dominant frequencies of fluctuations, vortex rope formation, and modes (rotating and plunging) of the rotating vortex rope. The time of appearance and disappearance of rotating and plunging modes of vortex rope was investigated simultaneously in the pressure and velocity data. The asynchronous mode was observed to dominate over the synchronous mode in both velocity and pressure measurements.
