Clinical Efficacy and Hormonal Change of GnRH Antagonist in Controlled Ovarian Stimulation for IVF-ET (체외수정시술을 위한 과배란유도에 있어 GnRH Antagonist의 임상적 효용성과 혈중 호르몬 농도의 변화)

  • Objectives: To evaluate the efficacy of GnRH antagonist cetrorelix in women undergoing controlled ovarian hyperstimulation (COH) for in vitro fertilization (IVF) and intracytoplasmic sperm injection (ICSI) and to determine changes in serum hormone concentrations during cetrorelix administration. Methods: We performed a clinical trial on 30 patients undergoing COH with highly purified follicular stimulating hormone (HP-FSH) and gonadotropin releasing hormone antagonist (GnRHant), cetrorelix. FSH was administrated from day 2 or 3 of cycle with fixed dose and adjusted according to individual response. 0.25 mg of cetrorelix was injected daily subcutaneously from stimulation day 5 until the day of hCG administration. Daily ultrasound monitoring was performed for growing follicles and serum levels of luteinizing hormone (LH), estradiol ($E_2$) and progesterone were measured daily during cetrorelix administration. Up to 4 embryos were transferred. Results: Mean age of enrolled patients was $32.0{\pm}3.4$ years (mean $\pm$ S.D.). All of 30 patients underwent oocyte pick-up, and embryo transfer was done in 28 patients. The total and mean numbers of received oocytes were 196 and $6.5{\pm}4.7$, the number of fertilized eggs was 111, and the fertilization rate was 56.6%. Total duration of FSH administration was $9.2{\pm}2.2$ days and mean of $24.3{\pm}7.7$ ampules of HP-FSH was administered. Total duration of cetrorelix administration was $5.7{\pm}1.9$ days. Serum LH and progesterone levels were maintained in the range of $1.4{\sim}2.9\;mIU/mL$ and $0.3{\sim}0.6\;ng/mL$, which respectively reflected effective prevention of premature LH surge. Clinical pregnancies were achieved in 9 patients, and overall clinical pregnancy rate was 30.0% per oocyte retrieval, and 32.1% per embryo transfer. Conclusion: GnRH antagonist is safe and convenient for COH for IVF-ET and effective with optimal pregnancy rate.

  • Two forms of gonadotropin releasing hormone (GnRH) are identified in the brain of adult mature spotted sea bass (Lateolabrax sp.) by immunohistochemical methods. Salmon GnRH immunoreactive (sGnRH-ir) cell bodies were distributed in the olfactory bulb, ventral telencephalon and preoptic region. Immunoreactive fibers were observed in the vicinity of the brain including the olfactory bulbs, the telencephalon, the optic nerve, the optic tectum, the cerebellum, the medulla oblongata and rostral spinal cord. In most cases, these fibers did not form well defined bundles. However, there was a clear continuum of immunoreactive fibers, extending from the olfactory bulbs to the pituitary. cGnRH-II-ir cell bodies were only found in olfactory bulbs. However, the distribution of cGnRH-II-ir fibers was basically similar to that of sGnRH-ir fibers except for the absence of their continuity between the olfactory bulbs and the pituitary. These data suggest that sGnRH and cGnRH-II are endogenous peptides and indicate the presence of multiple neuroendocrine functions in the brain of the spotted sea bass. It seems that sGnRH not only regulates GTH secretion but also functions as a neurotransmitter, whereas cGnRH-II functions only as a neurotransmitter.

Regulation of Luteinizing Hormone Release and Subunit mRNA by GnRH and Ovarian Steroids in Cultured Anterior Pituitary Cells (흰쥐 뇌하수체전엽 배양세포에서 GnRH 및 난소호르몬에 의한 $LH{\beta}$ subunit 유전자 발현 조절에 관한 연구)

  • The effects of gonadoropin-releasing hormone (GnRH) and ovarian steroid hormones on the release of luteinizing hormone (LH) and its subunit mRNA levels were investigated in anterior pituitary cells in culture. LH concentration was measured by a specific radioimmunoassay and mRNA levels of u and $LH{\beta}$ subunits by RNA slot blot hybridization assay. GnRH stimulated LH release in a dose-dependent manner from cultured pituitary cells. However, the basal LH release in the absence of GnRH was not changed during the course of 24h culture, strongly suggesting that release of LH is directly controlled by GnRH. The treatment of the pituitary cells with GnRH increased $LH{\beta}$ subunit mRNA levels in a dose-dependent manner, reaching the maximum with $2\;{\times}\;10^{-10}M$ GnRH while no significant increase in ${\alpha}$ subunit mRNA levels was observed after GnRH treatment. Estradiol did not augment GnRH-induced LH release while progesterone augmented GnRH-induced LH release in a dose-dependent manner at the level of pituitary. However, estradiol and progesterone increased basal and GnRH-induced $LH{\beta}$ subunit mRNA levels in a dose-dependent manner. The treatment of estrogen antagonist, LYI17018 blocked the effect of estradiol on GnRH-induced $LH{\beta}$ subunit mRNA levels in a dose-dependent manner while progesterone antagonist, Ru486 tended to block the effect of progesterone on GnRH-induced $LH{\beta}$ subunit mRNA levels. It is therefore suggested that GnRH Playa a major role in LH release and subunit biosynthesis by influencing the steady state $LH{\beta}$ subunit mRNA loves and ovarian steroid hormones modulate subunit biosynthesis via directly acting on pituitary gonadotropes.

  • The early studies demonstrated that the relative amount of FSH was important for stimulating normal ovarian activity and demonstrated the existence of a threshold level for FSH, above which follicular growth was activated. It was found that only a modest increase in circulating FSH level above the threshold (between 10 and 30%) was required to stimulate folliculogenesis. In addition, FSH is primary responsible for initiating estradiol production through the activation of the aromatase enzyme system in granulosa cells, follicular secretion and growth. LH on the other hand, plays a supportive role in ovarian steroidogenesis, stimulating the ovarian thecal cells to produce androgen, the precursor for estradiol synthesis. But there is now an increasing number of reports in the literature demonstrating an adverse effect of LH on fertility and miscarriage in infertile and fertile women. So HP-FSH is the drug of a highly purified FSH preparation which has a higher specific activity and far fewer impurities than FSH. This study was performed to evaluate the efficacy and safety of HP-FSH administered (SC; subcutaneous) versus FSH(IM; intramuscular) for ovulation induction. 20 candidates patients for ovulation induction were participated. All patients underwent pituitary desensitizing with a long gonadotropin-releasing hormone (GnRH) agonist protocol and ovulation induction was started with HP-FSH SC (10 patients; group I) or FSH IM (10 patients; group II). After ovulation, outcome of ovulation induction and local reaction of injection site were compared. There were no difference of outcome of ovulation in two groups except pregnancy rate/embryo transfer. Group I had a higher pregnancy rate/ embryo transfer than Group II (44.4% Vs 28.6%). Pain, redness, tenderness, bruising and itching when the injection received on the first 5 days of treated (50 SC and 50 IM injections) were assessed. There were no significant difference (P>0.05) in the incidence of tenderness, bruising and itching between the IM and SC injection. But IM injection (FSH) had a tendency of higher above incidence. The number of reports of pain, redness were significantly increased in IM injection group (P<0.05). These results indicate that SC administration of HP-FSH has been shown to be as effect for superovulation as traditional gonadotropins, with an improved safety profile due to the removal of extaneous proteins.

  • The purpose of the study is to investigate the change in the blood progesterone (P4) and estrogen (E2) levels when applying different estrus induction protocols to Korean black goats, and this was done to gain understanding about their reproduction physiology. For the experiment, we performed three estrus induction protocols that are commonly used in bovine: controlled internal drug release (CIDR) + prostaglandin $F2{\alpha}$ ($PGF2{\alpha}$), $PGF2{\alpha}$ + gonadotropin-releasing hormone (GnRH) + $PGF2{\alpha}$, and CIDR + $PGF2{\alpha}$ + PMSG. The P4 and E2 concentrations showed different patterns until the last treatment of the three protocols. However, similar concentration patterns were shown after the last treatment in all the protocols. In conclusion, we monitored the blood P4 and E2 levels in Korean black goats following three different estrus induction protocols. Our findings may be used in other breeding programs of Korean black goats, such as artificial insemination and embryo transfer.

Differential Growth of the Reproductive Organs during the Peripubertal Period in Male Rats

  • In mammals, puberty is a process of acquiring reproductive competence, triggering by activation of hypothalamic kisspeptin (KiSS)-gonadotropin releasing hormone (GnRH) neuronal circuit. During peripubertal period, not only the external genitalia but the internal reproductive organs have to be matured in response to the hormonal signals from hypothalamic-pituitary-gonadal (H-P-G) axis. In the present study, we evaluated the maturation of male rat accessory sex organs during the peripubertal period using tissue weight measurement, histological analysis and RT-PCR assay. Male rats were sacrificed at 25, 30, 35, 40, 45, 50, and 70 postnatal days (PND). The rat accessory sex organs exhibited differential growth patterns compared to those of non-reproductive organs. The growth rate of the accessory sex organs were much higher than the those of non-reproductive organs. Also, the growth spurts occurred differentially even among the accessory sex organs; the order of prepubertal organ growth spurts is testis = epididymis > seminal vesicle = prostate. Histological study revealed that the presence of sperms in seminiferous tubules and epididymal ducts at day 50, indicating the puberty onset. The number of duct and the volume of duct in epididymis and prostate were inversely correlated during the experimental period. Our RT-PCR revealed that the levels of hypothalamic GnRH transcript were increased significantly on PND 40, suggesting the activation of hypothalamic GnRH pulse-generator before puberty onset. Studies on the peripubertal male accessory sex organs will provide useful references on the growth regulation mechanism which is differentially regulated during the period in androgen-sensitive organs. The detailed references will render easier development of endocrine disruption assay.

Changes of Sexual Behaviors in Rapamycin-injected Cichlid Fish Astatotilapia burtoni Males

  • Cichlid fish species exhibit characteristic sexual behaviors according to not only reproductive stages but also social status. In a reproductive season, Astatotilapia burtoni males compete for females and a small number of dominant winners finally obtain the chance of spermiation. In addition to the characteristic behaviors, the dominant males have relatively bigger gonadotropin-releasing hormone 1 (GnRH1) neurons in the preoptic area (POA) of brain compared to those of subordinate males. Although the stimulatory effect of GnRH1 in vertebrate reproduction is well established, little is known about the triggering signal pathway to control GnRH1 neurons and GnRH1-mediated sexual behavior. In the present study, we evaluated the potential effect of TOR inhibitor rapamycin in relation to the cichlid male behaviors and GnRH1 neuron. After 14 h and 26 h of intraventricular injection of rapamycin, behavior patterns of chasing and courtship display did not show significant changes between rapamycin- and DMSO-injected males. Behaviors of spawning site entry increased in rapamycin-injected fish at 26 h post-injection than at 14 h post-injection significantly (P<0.05). Meanwhile, there was a tendency that GnRH1 neurons' soma size in the POA shrank by rapamycin injection, whereas the testes did not show notable changes. Taken together, these results suggest the possible role of TOR signal on GnRH1-mediated sexual behavior in cichlid dominant males, although further biological characterization of the TOR signaling pathway will be required to clarify this matter.

Fertility preservation for patients with breast cancer: The Korean Society for Fertility Preservation clinical guidelines

  • With advances in the methods of cancer treatment used in modern medicine, the number of breast cancer survivors has been consistently rising. As the number of women who wish to become pregnant after being diagnosed with breast cancer increases, it is necessary to consider fertility preservation in these patients. However, medical doctors may be unaware of the importance of fertility preservation among cancer patients because most patients do not share their concerns about fertility with their doctors. Considering the time spent choosing and undergoing treatment, an early referral to a reproductive specialist is the best way to prevent a delay in cancer treatment. Since it is not easy to make decisions on matters related to cancer diagnosis and fertility, patients should be provided with enough time for decision-making, and to allow for this, an early referral will provide patients with sufficient time to choose an appropriate method of fertility preservation. The currently available options of fertility preservation for patients with breast cancer include cryopreservation of embryos, oocytes, and ovarian tissue and gonadotropin-releasing hormone agonist treatment before and during chemotherapy. An appropriate method of fertility preservation must be selected through consultations between individual patients and health professionals and analyses of the pros and cons of different options.

The Effect of GnRH on the Ovaries and Uterus in Postpartum Korean Native Goats (한국재래산양에 있어서 분만후 Gonadotropin Releasing Hormone(GnRH)의 처리가 난소 및 자궁에 미치는 영향)

  • These studies were carried out to examine the estradiol-17$\beta$ levelsin plasma and ovarian tissues, as well as the contents of collagen and catecholamines in the uterus, and to determine the effects of GnRH administrations of uterine involution in postpartum Korean native goats. Plasma concentrations of estradiol-17$\beta$ were 63.81$\pm$8.00 pg/ml at day 1 of kidding, declined to 36.78$\pm$22.90 ng/ml at day 24 and decreased progressively to 27.81$\pm$17.06 and 12.46$\pm$8.13 pg/ml at days 30 and 36 postpartum, respectively. In ovarian tissues, the concentrations of estaiol-17$\beta$ were increased just before parturition and decreased immediately after parturition. The plasma estradiol-17$\beta$ levels were slightly higher on days 12 and then decreased gradually after parturition. The concentraitons of estradiol-17$\beta$ in the ovaries of postpartum goats were increased at day 36 after treatments with GnRH. The total hydroxyproline contents in the uterus was slightly higher prior to parturition and decreased gradually with the postpartum intervals after parturition. Hydroxyproline concentraitons in the uterus were decreased at days 24 and 36 postpartum after treatments with GnRH. The norepinephrine concentrations in myometrium from the pregnant and postpartum goats were correspondingly low both immediately before and after partuition. Norepinephrine concentrations in the pregnant horn of the uterus were increased from days 12 to 36 of postpartum and those levels of the non-pregnant horn were also increased from days 24 to 36 postpartum. Slightly higher concentrations were present in the non-pregnant horn in comparison to the pregnant horn but these differences were not significant. Postpartum, the uterine norepinephrine concentration was slightly increased at day 36 after treatments with GnRH. Dopamine concentrations were greater than those of norepinephrine. The concentrations of dopamine in the uterus of pregnant goats was not significantly different from that in the postpartum animals. Dopamine concentraitons of pregnant horn in postpartum goats were increased at day 24 after treatments with GnRH.

Usefulness of pelvic ultrasonography for the diagnosis of central precocious puberty in girls

  • Purpose: It is difficult to differentiate between central precocious puberty (CPP) and premature thelarche (PT) in girls. The aim of this study was to investigate the diagnostic usefulness of pelvic ultrasonography to distinguish between CPP and PT in girls with early breast development. Methods: This study included girls with early breast development who visited the clinic between January 2012 and December 2013. Clinical, laboratory, and pelvic ultrasonographic data were evaluated. CPP and PT were confirmed using the gonadotropin-releasing hormone stimulation test. Results: A total of 248 girls aged 7-8 years were included, among whom 186 (75.0%) had CPP and 62 (25.0%) had PT. The uterine length, transverse diameter, fundus, volume, and cross-sectional area were significantly larger in the CPP group (uterine length, $2.45{\pm}0.50cm$ vs. $2.63{\pm}0.49cm$, P=0.015; uterine volume, $0.95{\pm}0.62cm^3$ vs. $1.35{\pm}0.76cm^3$, P<0.001). However, there were no differences in the fundus/cervix ratio and ovarian measurements. In receiver operating characteristic analysis, a uterine volume of at least $1.07cm^3$ was the most predictive parameter for CPP with an area under the curve of 0.670 (95% confidence interval, 0.593-0.747). Conclusion: Uterine measurements by pelvic ultrasonography in girls with early pubertal development were significantly larger in the CPP group. However, the diagnostic value of ultrasonographic parameters was not high because of a considerable overlap of values between the two groups. Therefore, pelvic ultrasonography in combination with clinical and laboratory tests may be useful to distinguish between CPP and PT in girls.
