Author examined clinical procedure, clinical application and clinical failure of castable glass ceramic (Dicor) crowns which were made at the Dept. of Prosthodontics, Seoul National University Hospital during 19 months from Jan. 1988 to July, 1989(Observation period:19 months). The author obtained the following results. 1. A total of 86 crowns were constructed ; Most of them has been set on maxillary central incisors(53 cases, 62%):maxillary lateral incisors(20 cases, 23%):maxillary canines(6 cases, 7%):others(7 cases, 8%) 2. Dicer could be used as a lamiate veneer for discolored tooth. 3. During the examination period(1-19 months), there was no fracture case. 4. Dicer could be applied on the maxillary anterior tooth with minimal occlusal force.

Objectives: The aim of this study was to perform a clinical and radiographic analysis of endodontically treated teeth (ETT) restored with cast metal posts (CMPs) or prefabricated glass fiber posts (GFPs) and crowns. Materials and Methods: Fifty ETT were restored with 25 CMPs and 25 GFPs at a private dental clinic between 2001 and 2016. The restorations consisted of 12 all-ceramic crowns, 31 metal-ceramic crowns, and 7 composite resin crowns. Demographic data, type of teeth, type of post-and-core system, time of placement, crown restorations, the number of proximal contacts, the type of antagonist, and reports of any complications after post-and-core placement were recorded for each patient. Assessments were performed at baseline (radiographic) and follow-up (radiographic and clinical). Data were analyzed by the McNemar test, the Pearson χ2 test, and Kaplan-Meier survival curves (α = 0.05). The mean follow-up was 67.6 months. Results: No significant difference was observed for any of the radiographic parameters when the baseline and final radiographs were compared. In the clinical evaluation, anatomical form (p = 0.009) and occlusion (p = 0.001) showed significant differences according to the type of crown restoration; specifically, metal-ceramic and all-ceramic crowns outperformed composite resin crowns. Conclusions: CMPs and GFPs showed favorable results for restoring ETT after 6 years of follow-up. All-ceramic and metal-ceramic crowns showed higher survival rates and better clinical outcomes.
PURPOSE. This study was conducted to evaluate the effects of full-coverage all-ceramic zirconia, lithium disilicate glass-ceramic, leucite glass-ceramic, or stainless steel crowns on antagonistic primary tooth wear. MATERIALS AND METHODS. There were four study groups: the stainless steel (Steel) group, the leucite glass-ceramic (Leucite) group, the lithium disilicate glass-ceramic (Lithium) group, and the monolithic zirconia (Zirconia) group. Ten flat crown specimens were prepared per group; opposing teeth were prepared using primary canines. A wear test was conducted over 100,000 chewing cycles using a dual-axis chewing simulator and a 50 N masticating force, and wear losses of antagonistic teeth and restorative materials were calculated using a three-dimensional profiling system and an electronic scale, respectively. Statistical significance was determined using One-way ANOVA and Tukey's test (P<.05). RESULTS. The Leucite group ($2.670{\pm}1.471mm^3$) showed the greatest amount of antagonist tooth wear, followed by in decreasing order by the Lithium ($2.042{\pm}0.696mm^3$), Zirconia ($1.426{\pm}0.477mm^3$), and Steel groups ($0.397{\pm}0.192mm^3$). Mean volume losses in the Leucite and Lithium groups were significantly greater than in the Steel group (P<.05). No significant difference was observed between mean volume losses in the Zirconia and Steel groups (P>.05). CONCLUSION. Leucite glass-ceramic and lithium disilicate glass-ceramic cause more primary tooth wear than stainless steel or zirconia.
The pupose of this study was to compare the fracture strength of five kinds of all-ceramic crowns(Vintage, Dicor Empress-staining, Empress-layering, In-Ceram) luted with glass ionomer cerment and composite resin cement and to evaluate the effect of cements on the fracture stregth of all ceramic crowns. Five groups of twelve uniform sized all-ceramic crown specimens were fabricated. Six specimens of each group were cemented with glass ionomer cement(Fuji G.I. Cement) and the remaining six specimens of each group were etched, silane-treated, and cemented with composite resin cement(Bistite resin cement). The crowns were stored in water$(37^{\circ}C)$ for 1 day prior to loading in an Instron, using a steel ball(diameter 4mm) at a crosshead speed of 0.5mm/min. The crowns were angled $30^{\circ}$, so the steel ball contacted with the crowns 2mm lingual from the mid-incisal edge. The results obtained were summarized as follows ; 1. With G.I. cement, mean fracture load(Kg) Were : Intage : $18.33{\pm}1.47$ ; Empress-staining : $23.92{\pm}6.67$ ; Dicor : $24.0{\pm}5.81$ ; Empress-layering : $26.92{\pm}2.80$ ; In-Ceram : $51.58{\pm}6.87$ ; ANOVA revealed a significant difference existed(p<0.05) between the group A(Vintage, Dicor, Empress-staining, Empress-layering) and group B(In-Ceram). 2. With Resin cement, mean fracture load(Kg) were : Intage : $22.75{\pm}4.97$ ; Dicor : $42.75{\pm}7.07$ ; Empress-staining : $44.08{\pm}7.99$ ; Empresslayering : $50.42{\pm}5.43$ ; In-Ceram : $52.58{\pm}6.51$ ; ANOVA revealed a significatnt difference existed(p<0.05) between the group A(Vintage) and B(Dicor, Empress-staining Empress-alyering, In-Ceram). 3. Resin cement significantly increased the fracture strength of the all-ceramic crowns for Dicor(156%), Empress-staining(185%), Empress-alyering(187%)(p<0.05); but did not increase the fracture strength of Vintage(128%) and In-Ceram(101%)(p>0.05). 4. Majority of the all-ceramic crowns show a wedge fracture extending through proximal surfaces to an apex, usually apical third(with G.I. cement) or middle third(with Resin cement) of the facial surface.
All-ceramic restorations have become an attractive alternative to porcelain-fused-to-metal crowns. In-Ceram, and more recently IPS Empress 2 were introduced as a new all-ceramic system for single crowns and 3-unit fixed partial dentures. But their strength and marginal fit are still an important issue. This study evaluated the fracture resistance and marginal fit of three systems of 3 unit all-ceramic bridge fabricated on prepared maxillary anterior resin teeth in vitro. The 3 all-ceramic bridge systems were: (1) a glass-infiltrated, sintered alumina system (In-Ceram) fabricated conventionally, (2) the same system with copy-milled alumina cores (copy-milled In-Ceram), (3) a heat pressed, lithium disilicate reinforced glass-ceramic system (IPS Empress 2). Ten bridges of each system with standardized design of framework were fabricated. All specimens of each system were compressed at $55^{\circ}$ at the palatal surface of pontic until catastrophic fracture occurred. Another seven bridges of each system were fabricated with standard method. All of the bridge-die complexes were embedded in epoxy resin and sectioned buccolingually and mesiodistally. The absolute marginal discrepancy was measured with stereomicroscope at ${\times}50$ power. The following results were obtained: 1. There was no significant difference in the fracture strength among the 3 systems studied. 2. The Weibull modulus of copy-milled In-Ceram was higher than that of In-Ceram and IPS Empress 2 bridges. 3. Copy-milled In-Ceram($112{\mu}m$) exhibited significantly greater marginal discrepancy than In Ceram ($97{\mu}m$), and IPS Empress 2 ($94{\mu}m$) at P=0.05. 4. The lingual surfaces of the ceramic crowns showed smaller marginal discrepancies than mesial and distal points. There was no significant difference between teeth (incisor, canine) at P=0.05. 5. All-ceramic bridges of three systems appeared to exhibit sufficient initial strength and accept able marginal fit values to allow clinical application.

All-ceramic restorations have had a more limited life expectancy than metal ceramic restorations because of their low strength. Their relatively lower strength and resistance to fracture have restricted the use of all-ceramic crowns to anterior applications where occlusal loads are lower. But there has been increasing interest in all-ceramic restorations because patients are primarily concerned with improved esthetics. Many efforts have been made to in prove the mechanical properties of dental ceramics. This study was designed to elucidate the influence of the luting agent on the strength of the Empress 2 crown (staining technique) cemented on human teeth. Seventy extracted human permanent molar teeth were chosen. Teeth were prepared for Empress 2 crowns with milling machine on a surveyor. A dental bur was placed in the mandrel that was positioned so that the long axis of the bur was perpendicular to the surveyor base. Dimensions of the Empress 2 crown preparation were $6^{\circ}$ taper on each side, $1.5{\pm}0.1mm$ shoulder margin, and 4mm crown height. The luting cements used in this study were as follow: 1. Uncemented 2. Zinc phosphate cements (Confi-Dental) 3. Conventional glass ionomer cement : Fuji 1 (GC) 4. Resin-modified glass ionomer cements : Fuji plus (GC) 5. Adhesive cements : Panavia F (Kuralay), Variolink II (Vivadent), Choice (Bisco). Fracture test using Instron. The crowns were loaded in compressive force to evaluate the effect of these cements on the breaking strength of these all-ceramic crowns. A steel ball with a diameter of 4mm was placed on the occlusal surface and load was applied to the steel ball by a cylindrical bolt with a crosshead speed of 0.5mm per minute until fracture occurred. The fractured surface was examined using Scanning Electron Microscopic Image (SEM) to discover the correlation between fracture strength and bonding capacity. Within the limitation of this in vitro study design, the results were as follows : 1. fomentations significantly increased the fracture resistance of Empress ceramic crowns compared to control. Uncemented (206.9 N): ZPC (812.9 N): Fuji 1 (879.5 N): Fuji Plus (937.7 N): Choice (1105.4 N): Variolink II (1221.1 N): Panavia F (1445.2 N). 2. Resin luting agent, treated by a silane bond enhancing agents, yielded a significant increase in fracture resistance. In some of the Panavia F group, a fracture extended into dentin. 3. According to SEM images of fractured Empress crowns, the stronger the bond at both interfaces(crown and die), the more fracture strength was acquired.
PURPOSE. Fracture of the veneering material of zirconia restorations frequently occurs in clinical situations. The purpose of this in vitro study was to compare the fracture strengths of zirconia crowns veneered with various ceramic materials by various techniques. MATERIALS AND METHODS. A 1.2 mm, $360^{\circ}$ chamfer preparation and occlusal reduction of 2 mm were performed on a first mandibular molar, and 45 model dies were fabricated in a titanium alloy by CAD/CAM system. Forty-five zirconia copings were fabricated and divided into three groups. In the first group (LT) zirconia copings were veneered with feldspathic porcelain by the layering technique. In the second group (HT) the glass ceramic was heat-pressed on the zirconia coping, and for the third group (ST) a CAD/CAM-fabricated high-strength anatomically shaped veneering cap was sintered onto the zirconia coping. All crowns were cemented onto their titanium dies with Rely $X^{TM}$ Unicem (3M ESPE) and loaded with a universal testing machine (Instron 5583) until failure. The mean fracture values were compared by an one-way ANOVA and a multiple comparison post-hoc test (${\alpha}$= 0.05). Scanning electron microscope was used to investigate the fractured interface. RESULTS. Mean fracture load and standard deviation was $4263.8{\pm}1110.8$ N for Group LT, $5070.8{\pm}1016.4$ for Group HT and $6242.0{\pm}1759.5$ N for Group ST. The values of Group ST were significantly higher than those of the other groups. CONCLUSION. Zirconia crowns veneered with CAD/CAM generated glass ceramics by the sintering technique are superior to those veneered with feldspathic porcelain by the layering technique or veneered with glass ceramics by the heat-pressing technique in terms of fracture strength.
Due to high esthetic demands from patients, we are driven to fabricate natural-looking(form and color) restorations. In the past decade, the glass-ceramic in dentistry has been used as a material with variable crystals that affects the appearance, characteristics, and mechanical properties of metal-free fixed restoration. The conventional IPS Empress glass-ceramic is a heat-pressed, leucite reinforced material using the "lost wax" principle. This glass-ceramic has been successfully used for esthetic restorations such as all-ceramic crowns, inlays/onlays, and veneers for about 15 years. This article presents that the form and surface texture of clinical crown are duplicated by IPS-Empress system using "lost-wax technique".
PURPOSE. The aim of the present study was to evaluate the fracture resistances of zirconia, cast nickel-chromium alloy (Ni-Cr), and fiber-composite post systems under all-ceramic crowns in endodontically treated mandibular first premolars. MATERIALS AND METHODS. A total of 36 extracted human mandibular premolars were selected, subjected to standard endodontic treatment, and divided into three groups (n=12) as follows: cast Ni-Cr post-and-core, one-piece custom-milled zirconia post-and-core, and prefabricated fiber-glass post with composite resin core. Each specimen had an all-ceramic crown with zirconia coping and was then loaded to failure using a universal testing machine at a cross-head speed of 0.5 mm/min, at an angle of 45 degrees to the long axis of the roots. Fracture resistance and modes of failure were analyzed. The significance of the results was assessed using analysis of variance (ANOVA) and Tukey honest significance difference (HSD) tests (${\alpha}=.05$). RESULTS. Fiber-glass posts with composite cores showed the highest fracture resistance values ($915.70{\pm}323N$), and the zirconia post system showed the lowest resistance ($435.34{\pm}220N$). The corresponding mean value for the Ni-Cr casting post and cores was reported as $780.59{\pm}270N$. The differences among the groups were statistically significant (P<.05) for the zirconia group, as tested by ANOVA and Tukey HSD tests. CONCLUSION. The fracture resistance of zirconia post-and-core systems was found to be significantly lower than those of fiber-glass and cast Ni-Cr post systems. Moreover, catastrophic and non-restorable fractures were more prevalent in teeth restored by zirconia posts.
