V급 와동에서 심미성 수복재의 충전 방법에 따른 변연누출에 관한 실험적 연구 (AN EXPERIMENTAL STUDY ON THE MARGINAL LEAKAGE ACCORDING TO THE FILLING METHOD OF THE ESTHETIC MATERIALS IN CLASS V CAVITIES)

  • The purpose of this study was to evaluate the marginal leakage of esthetic restorative materials according to the filling method in class V cavities. 60 cavities were prepared on buccal surface for dye penetration test and divided into 5 experimental groups according to the filling methods of esthetic materials: group 1 was filled with glass ionomer cement, group 2, 3 and 4 were filled with the chamfer, knife edge and butt joint shaped glass ionomer cement respectively and silux plus/scotchbond 2$^{(R)}$ by sandwich technique, group 5 was lined with Dycal$^{(R)}$ and filled with silux plus/scotchbond 2$^{(R)}$ by bulk filling technique. All the specimens were then thermocycled in a range of $4^{\circ}C-60^{\circ}C$ by and immersed in 2% methylene blue dye solution for 24 hours, and sectioned mesiodistally with carborundum disk into two parts under water spray. All the specimens were observed at the occlusal and gingival margins and statistical analysis was performed. The obtained results were as follows : 1. At the occlusal margins, group 2, 3, 4 and 5 showed less marginal leakage than group 1 (p<0.05) and there were not significant differences among group 2, 3, 4, and 5. 2. At the gingival margins, group 5 showed the least marginal leakage and group 4 and 5 showed less marginal leakage than group 1, 2 and 3 (p<0.05). 3. In the laminated groups, group 4 showed less marginal leakage than group 2 and 3 at the gingival margins. 4. In comparison to the occlusal and gingival margins, all groups showed less marginal leakage at the occlusal margins than at the gingival margins (p<0.05).

Evaluation of internal adaptation of dental adhesive restorations using micro-CT

  • Objectives: The internal adaptation of composite restorations with or without resin modified glass ionomer cement (RMGIC) was analyzed non-destructively using Microcomputed tomography (micro-CT). Materials and Methods: Thirty intact human teeth were used. The specimens were divided into 3 groups. In the control group, the cavities were etched with 10% phosphoric acid for 15 sec. Composite resin was filled into the cavity without adhesive. In group 1, light cured glass ionomer cement (GIC, Fuji II LC, GC) was applied as a base. The cavities were then etched, bonded, light cured and filled with composites. In group 2, the cavities were then etched, bonded, light cured and filled with composites without base application. They were immersed in a 25% silver nitrate solution. Micro-CT was performed before and after mechanical loading. One-way ANOVA with Duncan analysis was used to compare the internal adaptation between the groups before or after loading. A paired t-test was used to compare internal adaptation before and after mechanical loading. All statistical inferences were made within the 95% confidence interval. Results: The silver nitrate solution successfully penetrated into the dentinal tubules from the pulp spaces, and infiltrated into the gap between restoration and pulpal floor. Group 2 showed a lower adaptation than the control group and group 1 (p < 0.05). There was no significant difference between the control group and group 1. For all groups, there was a significant difference between before and after mechanical loading (p < 0.05). Conclusions: The internal adaptation before and after loading was better when composites were bonded to tooth using adhesive than composites based with RMGIC.

Effect of casein phosphopeptide-amorphous calcium phosphate on fluoride release and micro-shear bond strength of resin-modified glass ionomer cement in caries-affected dentin

  • Objectives: This study was conducted to evaluate fluoride release and the micro-shear bond strength of resin-modified glass ionomer cement (RMGIC) in casein phosphopeptide-amorphous calcium phosphate (CPP-ACP)-remineralized caries-affected dentin (CAD). Materials and Methods: Exposed dentin surfaces of 30 human third molar teeth were divided into 2 equal groups for evaluating fluoride release and the micro-shear bond strength of RMGIC to CAD. Each group was subdivided into 3 equal subgroups: 1) control (sound dentin); 2) artificially demineralized dentin (CAD); 3) CPP-ACP remineralized dentin (remineralized CAD). To measure fluoride release, 15 disc-shaped specimens of RMGIC (4 mm in diameter and 2 mm in thickness) were bonded on one flat surface of the dentin discs of each group. Fluoride release was tested using ion chromatography at different intervals; 24 hours, 3, 5, 7 days. RMGIC micro-cylinders were built on the flat dentin surface of the 15 discs, which were prepared according to the assigned group. Micro-shear bond strength was measured after 24 hours water storage. Data were analyzed using 1- and 2-way analysis of variance and the post hoc least significant difference test (${\alpha}=0.05$). Results: Fluoride detected in solutions (at all intervals) and the micro-shear bond strength of RMGIC bonded to CPP-ACP-remineralized dentin were significantly higher than those bonded to artificial CAD (p < 0.05). Conclusions: Demineralized CAD consumes more fluoride released from RMGIC into the solution for remineralization than CPP-ACP mineralized dentin does. CPP-ACP increases the micro-shear bond strength of RMGIC to CAD.

기능화 다중벽 탄소나노튜브를 첨가한 글라스아이오노머 시멘트의 항균효과 및 기계적 성질 (Antibacterial and Mechanical Properties of Glass Ionomer Cements Containing Functionalized Multi-wall Carbon Nanotube(MWCNT))

  • 본 연구는 상업용 글라스아이오노머 시멘트(GIC) 분말에 기능화 다중벽 탄소나노튜브(MWCNT)를 첨가하여 항균효과와 기계적 성질을 개선하고자 하였다. MWCNT를 기존의 GIC 분말에 0.125, 0.25, 0.5와 1.0 wt%를 첨가하여 조성하였다. 비커스 경도(VHN)는 (Ø$10mm{\times}2mm$) 각각 4개의 시편을 제작하여 증류수에 1일, 7일, 14일 보관 후 200 N 하중에서 20초 유지시간으로 경도를 측정하였으며 항균효과는 S. mutans와 S. aureus균주를 아가 디스크 확산법으로 억제대를 측정하여 평가하였다. 실험 결과 비커스 경도는 MWCNT를 첨가한 실험군에서 높은값을 보였으며 MWCNT를 첨가한 모든 실험군에서 침전시간이 증가할수록 유의하게 높은 경도값을 보였다(p<0.05). 항균실험은 S. mutans균에서 다소 높은 항균효과를 보였으나 통계적으로 유의한 차이는 나타나지 않았으며, S. aureus균은 MWCNT를 0.25 wt%첨가한 군에서 통계적으로 유의한 차이점을 보였다(p<0.05). 이상의 결과로 MWCNT를 첨가한 GIC는 항균효과와 기계적 성질에 효과가 있음을 입증하였다. 추후 MWCNT의 효율적인 기능화 처리에 대한 연구가 필요할 것으로 사료된다.

도재와 상아질의 표면 처리가 도재의 파절 강도에 미치는 영향 (THE EFFECT OF SURFACE TREATMENT ON FRACTURE STRENGTH OF DENTAL CERAMICS)

  • The major influencing factors on the strength of all-ceramic crowns are types of dental ceramics, fabrication techniques, methods of abutment preparation and cementation modes of all-ceramic restorations. Zinc phosphate cement and glass-ionomer cement were used as an early lot-ing media for all-ceramic crowns. Recently many studies have reported that resin cements have more advantages in increasing the fracture strength of restorations comparing with zincphosphate cement and glass-ionomer cement. The purpose of this study is to investigate the effect of etching, silane treatment, sandblasting and dentin bonding agents on fracture strengths of dental ceramics. 40 flat dentin specimens and 40 ceramic discs of 1.5mm thickness and 8mm diameter were fabricated, and divided into 4 groups according to surface treatments. Surface treatments before cementation were as follows Group I : (ceramic) : HF etching - silane treatment - application of bonding resin (dentin) : application of dentin bonding agent Group II : (ceramic) : sandblasting - application of bonding resin (dentin) : application of dentin bonding agent Group III : (ceramic) : application of bonding resin (dentin) : application of dentin bonding agent Group IV : (ceramic) : HF etching - silane treatment - application of bonding resin (dentin) : no dentin bonding procedure Dentin specimens and ceramic discs were cemented with dual cure resin cement, and went through thermocycling. Compressive stress es were loaded on the centers of ceramic discs with Instron test-ing machine, and fracture strengths resistance for catastrophic fracture were measured The results were as follows. 1. The group I showed the highest fracture resistance. The next was group II And group III, IV followed. 2. There was a significant difference in the mean value of fracture strengths between group I and group III (p<0.05), but no significant differences between group I and group II, and group II and group III (p>0.05). 3. There was a significant difference in the mean value of fracture strengths between group I and group IV (p<0.05).

치과용 합착제의 선택 (Selection of Dental Cements)

  • Zinc phosphate cement has long been the material of choice for permanent luting of cast restorations, and through many years of use has been considered effective to retain castings. However, cast restorations cemented with this material have been susceptible to secondary caries. Glass ionomer luting agents become available in the late 1970s. These material s, through release of fluoride, show considerable promise as a means of reducing secondary caries. Other favorable traits include significantly less disintegration in vivo than zinc phosphate cements, a film thickness comparable to that of zinc phosphate cement, and adhesion to tooth structure. Compomer materials were created in 1993 as a filling material for deciduous teeth, cervical lesions, and class III cavities. In the meantime, compomer have been developed as chemical hardening cements for cast gold restorations. The aim of this paper is to review the articles on luting cements to help the choice of dental cements.

  • 결손된 치질과 변색된 법랑질을 수복하기 위해 많은 재료와 술식이 개발되어왔다. 즉 silicate cement, PMMA resin, BISGMA composite resin, glass ionomer cement, porcelain등이 이용되어 왔으나 이것들은 biocompatible하지 않아 이상적인 재료는 아니다. 따라서 인간의 결손된 enamel을 수복할 수 있는 새로운 생역학적인 재료가 필요하게 되었다. 필자는 최근 관심의 대상이 되고 있는 porcelain laminate veneer system(층상 도재 전장관)에 관하여 문헌을 중심으로 고찰해 보고자 한다(국내에서는 장완식 교수께서 1987년 6월10일 보철학교실 세미나에서 video tape를 통해 소개한바 있음). 여기서는 Hobo, Iwata(1985)등이 소개한 castable apatite ceramic material을 이용한 laminate veneer술식에 관해 기술하고 내화성 모형(refractory cast)을 이용한 laminate술식과 비교 기술하고자 한다.

Effect of abutment neck taper and cement types on the amount of remnant cement in cement-retained implant restorations: an in vitro study

  • PURPOSE. The present study aims to analyze the effect of abutment neck taper and types of cement on the amount of undetected remnant cement of cement-retained implant prostheses. MATERIALS AND METHODS. Three neck taper angles (53°, 65°, 77°) and three types of cement (RMGI: resin-modified glass ionomer, ZPC: zinc phosphate cement, ZOE: zinc oxide eugenol cement) were used. For each group, the surface percentage was measured using digital image and graphic editing software. The weight of before and after removing remnant cement from the abutment-crown assembly was measured using an electronic scale. Two-way ANOVA and Duncan & Scheffe's test were used to compare the calculated surface percentage and weight of remnant cement (α = .05). RESULTS. There were significant differences in remnant cement surface percentage and weight according to neck taper angles (P < .05). However, there were no significant differences in remnant cement surface percentage and weight on types of cement. No interaction was found between neck taper angles and types of luting cement (P > .05). The wide abutment with a small neck taper angle showed the most significant amount of remnant cement. And the types of luting cement did not influence the amount of residual cement. CONCLUSION. To remove excess cement better, the emergence profile of the crown should be straight to the neck taper of the abutment in cement-retained implant restoration.

글래스 아이오노머 이장재와 복합레진간의 결합강도에 관한 연구 (A STUDY ON THE BOND STRENGTHS BETWEEN GLASS IONOMER CEMENT BASES AND COMPOSITE RESINS)

  • 본 연구는 현재 널리 쓰이고 있는 sandwich technique을 임상에 적용함에 있어서, GIC 이장재에 대한 이중중합형 레진과 비교적 근래에 소개된 compomer의 결합력을 측정하여 기존의 광중합형, 화학중합형 복합레진과 비교하고, 가장 우수한 결합력을 보이는 glass ionomer-composite resin의 조합을 밝혀내고자 시도되었다. 이장용 재료로는, 광중합형 GIC인 Vitrebond와 화학중합형 GIC인 Ketac-fil을 사용하였으며, 이 두 가지의 이장재에 대해, 광중합형 복합 레진인 Z-100, 화학중합형 복합 레진인 Clearfil, compomer인 Dyract, 그리고 이중중합형 복합 레진인 Bis-core를 축조하여 각 군당 10개씩, 총 80개의 시편을 제작하였다. 제작된 시편은 $37^{\circ}C$의 증류수에 24시간동안 보관한 후, full load scale 50kg, cross-head speed 1mm/min 조건의 만능 시험기에서 그 전단결합강도를 측정하였으며 실험결과는 student t-test로 검정하였다. 본 연구의 결과는 다음과 같다. 1. Vitrebond를 이장재로 사용한 경우, Z-100이 가장 낮은 결합강도를 보였으며, 나머지 세 재료는 결합력의 차이를 보이지 않았다(P>0.05). 2. Ketac-fil을 이장재로 사용한 경우, Clearfil이 가장 높은 결합강도를 보였고, Dyract, Bis-core가 중등도의 결합강도를, Z-100이 가장 낮은 결합강도를 보였다(P<0.05). 3. Clearfil은 Vitrebond 상방에서는 타 재료들과 비슷한 결합강도를 보였으나, Ketac-fil 상방에서는 가장 강한 결합강도를 보임으로써, GIC 이장재의 종류에 따른 결합력의 차이를 보였으나 (P<0.05), 나머지 세 재료의 경우에는 차이를 보이지 않았다(P>0.05). 4. Vitrebond를 사용할 때보다 Ketac-fil을 이장재로 사용할 때, 상부에 축조되는 복합레진의 종류에 따른 결합력의 차이가 더 크게 나타났다(P<0.05).

접착용 시멘트의 결합강도에 임시 접착제와 탈감작제가 미치는 영향 (THE EFFECT OF TEMPORARY CEMENT AND DESENSITIZER ON THE BOND STRENGTH OF LUTING CEMENTS)

  • This study investigated the effect of temporary cement and desensitizer on the bond strength of luting cements. Total 96 dentin specimens were divided into two groups with and without temporary cementation. For temporary cement-tread group, specimens were cemented with $Temp-bond^{(R)}$ and all specimens were stored in distilled water at $37^{\circ}C$ for 7 days. Each cup was further divided into 3 subgroups with $Gluma^{(R)},\;One-step^{(R)}$ application and without desensitizer After desensitizer application, Ni-Cr specimens were luted to dentin surface with $Panavia-F^{(R)}$ and $Vitremer^{(R)}$ Specimens were placed in distilled water at $37^{\circ}C$ for 24 hours and shear bond strength between metal and dentin was measured by a universal testing machine. The results were as follows : 1. In $Panavia-F^{(R)}$ cemented groups, the combination of $One-step^{(R)}$ without temporary cement showed the greatest strength. Among the desensitizer types, $One-step^{(R)}$ showed the highest bond strength, followed by No-desensitizer, $Gluma^{(R)}$. 2. In $Vitremer^{(R)}$ cemented groups, the combination of no temporary cement and without desensitizer showed the greatest bond strength. Among the desensitizer types, No-desensitizer group showed the highest bond strength. 3. The use of $Gluma^{(R)}$ significantly reduced the shear bond strength in $Panavia-F^{(R)}$ and $Vitremer^{(R)}$ groups. 4. All temporary cement-treated groups showed a significant lower shear bond strength than without temporary cement groups. 5. Desensitizer application significantly influenced the bond strength of the resin cement and resin modified glass ionomer cement.
