충치는 사람의 구강질환 중 가장 흔한 질환으로 Streptococcus mutans (S. mutans)균이 초기 충치를 형성하는데 매우 중요한 역할을 담당한다. Porphyromonas gingivalis (P. gingivalis)는 대표적인 구취 유발균으로 구취 형성에 중요한 휘발성 황화합물을 생성하는데 관여한다. 치주질환은 치은결체조직과 치조골의 파괴를 유발하여 치아의 상실을 초래할 수 있는 만성 염증성 질환으로 Prevotella intermedia (P. intermedia)가 원인균이다. 이번 연구에서는 cetylpyridinium chloride (CPC), sodium fluoride (NaF), 녹차 추출액, 솔잎 추출액을 유효성분으로 하는 마우스워시 제품을 사용하여 S. mutans 균을 포함, 구강질환 균으로 널리 알려진 P. gingivalis, P. intermedia 대해 항균 효과를 확인하고자 하였다. 그 결과 시험군의 경우 S. mutans, P. gingivalis 에 대해 30 s 내에 4.00 Log, 4.68 Log의 사멸력을 확인하였고, P. intermedia의 경우 30 s 2.40 Log, 60 s 2.70 Log 사멸력을 확인하였다. 또한 Dentocult SM Strip mutans (SM Strip) 염색방법을 적용하여 S. mutans 균의 감소여부를 시각적 자료로 쉽게 확인할 수 있었다. 이와 같은 결과를 통해 CPC, NaF, 녹차 추출액, 솔잎 추출액을 포함한 마우스워시 제품은 구강균 사멸을 통해 충치 및 구취와 같은 구강질환 예방에 효과가 있을 것으로 기대한다.
본 증례는 전신성 홍반성 낭창으로 인해 2000년 4월부터 면역 억제제인 azathioprine 75mg과 prednisolne 35mg을 복용 중인 42세 여성 환자가 2004년 12월 상악 협측 치은의 종창, 증식, 동통 및 출혈을 주소로 내원하여 조직 생검 결과 모세 혈관종으로 진단되었으나, 2005년 1월 치은의 재발 병소의 절제 및 조직 생검으로 카포시 육종으로 진단되었고, 흉부의 피부 병변의 조직 생검과 흉부 전산화 단층 촬영 검사에서도 양측 폐에 다수의 결절을 보이는 카포시 육종으로 진단되었다. 의인성 면역억제제에 의한 카포시 육종으로 최종 진단되어, 면역 억제제와 부신피질호르몬제의 중단과, paclitaxel을 이용한 전신적인 항암화학 요법으로, 2008년11월 현재까지 재발의 징후 없이 구강, 피부, 폐의 병소에 대해 양호한 치료 결과를 얻었으나, 향후 전신성 홍반성 낭창의 악화 또는 카포시 육종의 재발 가능성이 있으므로 지속적인 추적 관찰이 요할 것으로 사료된다.

Background: The mandibular third molar (M3) is typically the last permanent tooth to erupt because of insufficient space and thick soft tissues covering its surface. Problems such as alveolar bone loss, development of a periodontal pocket, exposure of cementum, gingival recession, and dental caries can be found in the adjacent second molars (M2) following M3 extraction. The specific aims of the study were to assess the amount and rate of bone regeneration on the distal surface of M2 and to evaluate the aspects of bone regeneration in terms of varying degree of impaction. Methods: Four series of panoramic radiographic images were obtained from the selected cases, including images from the first visit, immediately after extraction, 6 weeks, and 6 months after extraction. ImageJ software® (NIH, USA) was used to measure linear distance from the region of interest to the distal root of the adjacent M2. Radiographic infrabony defect (RID) values were calculated from the measured radiographic bone height and cementoenamel junction with distortion compensation. Repeated measures of analysis of variance and one-way analysis of variance were conducted to analyze the statistical significant difference between RID and time, and a Spearman correlation test was conducted to assess the relationship between Pederson's difficulty index (DI) and RID. Results: A large RID (> 6 mm) can be reduced gradually and consistently over time. More than half of the samples recovered nearly to their normal healthy condition (RID ≤ 3 mm) by the 6-month follow-up. DI affected the first 6 weeks of post-extraction period and only showed a significant positive correlation with respect to the difference between baseline and final RID. Conclusions: Additional treatments on M2 for a minimum of 6 months after an M3 extraction could be recommended. Although DI may affect bone regeneration during the early healing period, further study is required to elucidate any possible factors associated with the healing process. The DI does not cause any long-term adverse effects on bone regeneration after surgical extraction.
There are evidences that exogenous electric currents are capable of enhancing bone formation and resolution, and that the conversion of the bioelectric response to biochemical activity provides the directional component of orthodontic tooth movement. In addition, evidence has implicated cyclic nucleotides in alveolar bone cellular activation mechanism during orthodontic tooth movement. In view of these evidences, this study was performed to investigate the effects of exogenous electric currents on cyclic nuclotide levels in feline alveolar bone and the possible clinical application of electric currents as an additional orthodontic tool. In the first study, three groups of three adult cats were subjected to application of a constant direct current of $10{\pm}2$ microamperes to gingival tissue near maxillary canine noninvasively for 1, 3, and 7 days respectively. In the second study, three groups of three adult cats each were treated by an electric-orthodontic procedure for 1, 3, and 7 days respectively. The left maxillary (control) canine received an orthodontic force of 80gm alone at time of initiation, while the right maxillary (experimental) canine received combined force-electric stimulation (80gm of force and $10{\pm}2$ microamperes of a constant D.C. currents). Alveola, bone samples were obtain from the mesial (tension and/or cathode) and the distal (compression and/or anode) sites surrounding maxillary canines as well as from contralateral control sites. The samples were extracted, boiled, homogenized, and the supernatants were assayed for cyclic nucleotides (cAMP, cGMP) by a radioimmunoassay method. And also the amount of tooth movement was measured in the second study. On the basis of this study, the following conclusions can be drawn: 1. The fluctuation pattern of cyclic nucleotide levels in alveolar bone treated by exogenous electric currents was similar to that treated by orthodontic force. 2. The cAMP levels in alveolar bone of electrically treated teeth significantly elevated above the control values. And of electrically treated teeth, the values of the anode sites were higher than those of the cathode sites. 9. The cGMP levels in alveolar bone of electrically treated teeth elevated above the control values at the initiation phase of treatment, but dropped below the control values at time of termination. And of electrically treated teeth, the values of the cathode sites were higher than those of the anode sites. 4. The rate of tooth movement in teeth . treated by force-electric combination increased with the length of treatment as compared to that treated by mechanical force alone.
오늘날 삶의 질에 대한 관심이 고조되면서 일반 대중에게서도 레져 및 스포츠가 활성화됨에 따라 구강악안면영역 외상의 발생빈도도 높아지고 있다. 이러한 구강악안면영역의 외상을 예방하기 위해 구강보호장치 사용의 중요성이 커지고 있으며, 그 중 가장 많이 사용되는 것이 마우스가드이다. 마우스가드는 1) 치아에 가해지는 충격을 흡수하고 변형시켜 치아의 손상을 방지; 2) 입술, 혀, 치은 등 연조직의 열상을 예방; 3) 충격의 발생 시 반대측 치아가 접촉되는 것을 예방; 4) 하악골을 안정화시켜 하악각 또는 하악과두의 파절을 유발할만한 충격을 흡수시킴; 그리고 5) 목 또는 뇌손상을 예방할 수 있다. 이렇듯 마우스가드가 구강악안면영역의 외상방지에 효과적이지만, 운동선수나 일반 대중들에게는 널리 보급되어 있지 않고 그 중요성에 대한 인식률도 저조한 실정이다. 레져나 스포츠 활동 시 나타날 수 있는 구강악안면 외상 예방에 가장 효과적인 마우스가드의 종류, 재료, 제작 시 고려할 사항 등을 알아보고, 사용실태에 대한 고찰을 통해 마우스가드의 중요성에 대한 인식과 대중화에 도움이 되고자 한다.

Purpose: Radiation damage is produced and viable cell number is reduced. We need to know the type of cell death by the ionizing radiation and the amount and duration of cell cycle arrest. In this study, we want to identified the main cause of the cellular damage in the oral cancer cells and normal keratinocytes with clinically useful radiation dosage. Materials and Methods: Human gingival tissue specimens obtained from healthy volunteers were used for primary culture of the normal human oral keratinocytes (NHOK). Primary NHOK were prepared from separated epithelial tissue and maintained in keratinocyte growth medium containing 0.15 mM calcium and a supplementary growth factor bullet kit. Fadu and Hep-2 cell lines were obtained from KCLB. Cells were irradiated in a /sup 137/Cs γ-irradiator at the dose of 10 Gy. The dose rate was 5.38 Gy/min. The necrotic cell death was examined with Lactate Dehydrogenase (LDH) activity in the culture medium. Every 4 day after irradiation, LDH activities were read and compared control group. Cell cycle phase distribution and preG1-incidence after radiation were analyzed by flow cytometry using Propidium Iodine staining. Cell cycle analysis were carried out with a FAC Star plus flowcytometry (FACS, Becton Dickinson, USA) and DNA histograms were processed with CELLFIT software (Becton Dickinson, USA). Results: LDH activity increased in all of the experimental cells by the times. This pattern could be seen in the non-irradiated cells, and there was no difference between the non-irradiated cells and irradiated cells. We detected an induction of apoptosis after irradiation with a single dose of 10 Gy. The maximal rate of apoptosis ranged from 4.0% to 8.0% 4 days after irradiation. In all experimental cells, we detected G2/M arrest after irradiation with a single dose of 10 Gy. Yet there were differences in the number of G2/M arrested cells. The maximal rate of the G2/M ranges from 60.0% to 80.0% 24h after irradiation. There is no significant changes on the rate of the G0/G1 phase. Conclusion: Radiation sensitivity was not related with necrosis but cell cycle arrest and apoptosis. These data suggested that more arrested cell is correlated with more apoptosis.
Purpose: Both chronic periodontitis (CP) and iron deficiency anemia (IDA) induce oxidative stress in the body and cause an imbalance between reactive oxygen species and antioxidants, such as superoxide dismutase (SOD). This study explored the SOD enzyme activity of saliva and serum in CP patients with and without IDA and analyzed the impact of IDA on CP. Methods: A total of 82 patients were divided into four groups: control group (CG, 22), periodontally healthy IDA patients (IDA-PH, 20), CP patients (CP, 20), and IDA patients with CP (IDA-CP, 20). After clinical measurements and samplings, serum and salivary SOD levels were determined using an SOD assay kit. Results: IDA-CP patients exhibited a higher gingival index, bleeding on probing, probing pocket depth, and percentage (%) of sites with a clinical attachment loss (CAL) of ${\geq}6mm$ (P<0.008) than CP patients. The mean salivary and serum SOD levels were significantly lower in the IDA-PH, CP, and IDA-CP patients than in the CG group (P<0.008). A significant positive correlation between salivary and serum SOD activity was observed in IDA (P<0.05). Furthermore, serum and salivary SOD levels were significantly and negatively correlated with all periodontal parameters including the percentage of sites with CAL of 4-5 and ${\geq}6mm$ (P<0.05) except the significant correlation between salivary SOD activity and mean CAL and the percentage of sites with CAL of 4-5 mm (P>0.05) in these patients. Conclusions: Within the limits of this study, it may be suggested that IDA patients with chronic periodontitis have more periodontal breakdowns than patients with chronic periodontitis. Serum and salivary SOD activity levels were lower in the IDA-PH, CP and IDA-CP groups than in the CG. Iron deficiency anemia influenced the serum SOD activity but did not seem to affect the salivary SOD activity in these patients.

Purpose: The decontamination procedure is a challenging aspect of surgical regenerative therapy (SRT) of peri-implantitis that affects its success. The purpose of the present study was to determine the impact of additional topical gaseous ozone therapy on the decontamination of implant surfaces in SRT of peri-implantitis. Methods: A total of 41 patients (22 males, 19 females; mean age, $53.55{\pm}8.98years$) with moderate or advanced peri-implantitis were randomly allocated to the test group (ozone group) with the use of sterile saline with additional ozone therapy or the control group with sterile saline alone for decontamination of the implant surfaces in SRT of peri-implantitis. Clinical and radiographic outcomes were evaluated over a period of 12 months. Results: At the 12-month follow-up, the plaque and gingival index values were significantly better in the ozone group (P<0.05). Probing depth decreased from $6.27{\pm}1.42mm$ and $5.73{\pm}1.11mm$ at baseline to $2.75{\pm}0.7mm$ and $3.34{\pm}0.85mm$ at the end of the 12-month observation period in the ozone and control groups, respectively. Similarly, the clinical attachment level values changed from $6.39{\pm}1.23mm$ and $5.89{\pm}1.23mm$ at baseline to $3.23{\pm}1.24mm$ and $3.91{\pm}1.36mm$ at the 12-month follow-up in the ozone and control groups, respectively. According to the radiographic evidence, the defect fill between baseline and 12 months postoperatively was $2.32{\pm}1.28mm$ in the ozone group and $1.17{\pm}0.77mm$ in the control group, which was a statistically significant between-group difference (P<0.05). Conclusions: Implant surface decontamination with the additional use of ozone therapy in SRT of peri-implantitis showed clinically and radiographically significant. 상악 전치의 맹출장애는 임상적으로 비교적 흔한 치열발육 이상으로 환자측에서 인지하기 쉬우며 심미적, 사회 심리적으로 중요한 문제가 될 수 있다. 맹출장애의 치료방법으로는 주기적 관찰, 공간 확보, 치은 절제술을 포함한 단순 노출, 교정적 견인, 외과적 노출과 교정적 견인의 병행, 발거 후 자가치아이식 등 여러 방법이 있다. 이 중 적절한 치료방법을 선택하기 위해서는 우선 환자의 내과적, 치과적 병력을 고려해야 하고 치아의 위치, 상태, 치근 형성정토 및 만곡여부, 맹출을 위한 공간 확보 여부, 주위 치조골 및 치주조직의 상태, 견인 방법, 이동에 소요되는 기간 등을 고려해야 한다. 저자는 상악 전치부의 맹출장애를 보이는 두 증례를 치험한 바, 첫 번째 증례는 상악 우측 중절치의 이소맹출과 우측 견치의 맹출지연을 주소로 내원한 환자로서, 견치를 노출시킨 후 견인을 시행하였으며 현재 우측 중절치를 배열중이다. 두 번째 증례는 상악 좌측 중절치의 맹출지연을 주소로 내원한 환자로서, 상악 좌측 중절치를 발거 한 후 자가치아이식을 시행하였다. 위 증례를 통해서 치아의 맹출력을 기대할 수 없거나 맹출방향이 비정상적인 경우에는 외과적 노출과 견인을 동반한 방법을 우선 적용할 수 있으나, 치아의 위치 이상이 심각하거나 치근의 위치가 과도하게 이탈된 경우에는 자가치아이식을 포함한 외과적 술식이 맹출유도에 도움이 된다는 사실을 확인하였다.
수직적, 수평적 골흡수가 심한 환자에서 임플란트를 이용한 전악수복의 경우 경조직과 연조직 이식을 통해서 임플란트를 원하는 위치에 식립할 수도 있지만, 치은과 치아의 기능과 심미를 회복할 수 있는 fixed detachable prostheses를 대체 술식으로 사용할 수도 있다. 이러한 증례에서 다양한 재료가 수복물의 제작에 사용 가능하지만, metal/acrylic 보철물에서는 레진치아의 파절 및 탈락이 일어날 수 있고, metal/ceramic 혹은 zirconia/ceramic 보철물에서는 도재의 chipping이나 파절과 같은 문제가 발생할 수 있다. 이에 최근에 심미적이면서도 기능적인 보철수복을 위해 zirconia에 도재를 축성하지 않고 임상적으로 적용가능한 monolithic zirconia framework이 출시되어 사용되고 있다. 본 임상 증례는 심미적인 요구도가 높은 완전 무치악 환자에서 임플란트를 식립하고 chipping이나 파절의 위험을 감소시키기 위해 monolithic zirconia framework을 이용해 만들어진 complete fixed detachable 보철물을 이용한 수복에 대해 보고하고자 한다. 이번 증례에서 보철물은 심미적, 기능적으로 만족스러웠으며, 2년 간의 정기검사에서 임상적인 합병증은 보고되지 않았다.

① 당 사이트는 회원이 서비스 이용내용에 있어서 본 약관 제 11조 내용을 위반하거나, 다음 각 호에 해당하는

- 기타 정상적인 서비스 운영에 방해가 될 경우
② 상기 이용제한 규정에 따라 서비스를 이용하는 회원에게 서비스 이용에 대하여 별도 공지 없이 서비스 이용의
일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.

1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
