Breast-Feeding Protects Infantile Diarrhea Caused by Intestinal Protozoan Infections

  • This study investigated the effect of breast-feeding in protection against protozoan infection in infants with persistent diarrhea. Infants were classified into 2 groups; 161 breast-fed infants and the same number of non-breast-fed infants. Microscopic examinations of stool were done for detection of parasites and measuring the intensity of infection. Moreover, serum levels of IgE and TNF-${\alpha}$ were measured by ELISA. Cryptosporidium spp., Entamoeba histolytica/Entamoeba dispar, Giardia lamblia, and Blastocystis sp. were demonstrated in infants with persistent diarrhea. The percentage of protozoan infections was significantly lower in breast-fed infants than that in the non-breast-fed infants. The levels of IgE and TNF-${\alpha}$ were significantly lower in the breast-fed group than in the non-breast-fed group. There were significant positive associations between the serum levels of IgE and TNF-${\alpha}$ and the intensity of parasite infection in the breast-fed group. It is suggested that breast-feeding has an attenuating effect on the rate and intensity of parasite infection.

  • The status of intestinal parasitic infections and seroprevalence of cystic echinococcosis in the inhabitants In Dornod and Selenge, Mongolia, was observed with stool and serum samples from 165 and 683 residents from August 9 to August 15, 2003. A total of 10 inhabitants (6.1%) were found to be infected with protozoan cysts or oocysts by stool examinations; 7, 1 and 2 cases with Entamoeba coli, Giardia lamblia and Cryptosporidium parvum, respectively. A total of 62 people (9.1 %) revealed significantly high levels of specific antibodies against Echinococcus granulosus by enzyme-linked immunosorbent assay (ELISA). The surveyed areas are rural areas and the sanitized tab water is not available. There is a possibility of endemic transmission of water-borne protozoan diseases in these areas. The clinical echinococcosis is needed to be searched from the seropositive inhabitants.

Dientamoeba fragilis Infection in Patients with Digestive and Non-Digestive Symptoms: A Case-Control Study

  • In most developing countries, Dientamoeba fragilis infection is an obscure protozoan infection. We aimed to determine a frequency and clinical importance of D. fragilis infection in Taif, Saudi Arabia. A 1-year case control study included patients with gastrointestinal (cases, n=114) or non-gastrointestinal symptoms (controls, n=90). The fecal samples were examined with the classical parasitological methods for intestinal protozoa, and by real time PCR for D. fragilis. The infection by D. fragilis was detected in 5.8% by PCR and in 4.4% patients by microscopy. The infection was identified more in control group (n=9) than in cases (n=3); a sole infection in 11 patients and mixed with Giardia in 1 patient. The other enteric parasites detected were Blastocystis sp. (8.3%), Giardia sp. (5.3%), Cryptosporidium sp. (2.9%), Entamoeba histolytica (1.4%), Entamoeba coli (0.9%) and Hymenolepis nana (0.4%). Our results tend to reinforce the need to increase awareness of D. fragilis infection in Saudi Arabia.

  • The SWTR(Surface Water Treatment Rule) requires achieving removal 99.9% Giardia and 99.99% Virus. To remove Giardia and Virus, disinfection process is essential process. CT is important index for Disinfection performance. CT value is expressed by CXT. C is represented by disinfection concentration and T is represented by $T_{10}$ which means 90% contact time in clearwell. To improve Disinfection performance, it is desirable to increase $T_{10}$. Primary factor affected $T_{10}$ is flow Pattern. Because guide wall change that flow pattern is uniform, we have installed guide wall to increase $T_{10}$. In this paper, we have performed numerical analysis for clearwell installed symmetric guide wall and center divided guide wall using CFD. We have analyzed flow pattern and $T_{10}$ in each clearwell. And. we compare symmetric guide wall with center divided guide wall in flow pattern and $T_{10}$.

Prevalence of Intestinal Parasitic Infections and Associated Risk Factors Among in Children Ilam Town, West Iran

  • Purpose: Intestinal parasitic infections (IPIs) are a major health problem worldwide, especially in developing countries. According to the literature, children are more likely to get infected due to more frequent exposure to sources of infection. Methods: In this study, we collected 500 fecal samples from children under 15 years of age who referred to labs in Ilam city. All samples were microscopically examined using formalin-ether concentration and Trichrome staining techniques. The results were analyzed using SPSS 20.0. Results: Among 500 investigated samples, 4 types of parasites were detected. Giardia lamblia, Entameba coli, and Blastocystis hominis were identified as intestinal protozoa, and pinworm as intestinal worm. In this study, 23 cases (4.6%) had parasitic infections. 13 patients (2.6%) were infected with Giardia parasite, 4 patients (0.8%) with E. coli, 3 patients (0.6%) with B. hominis and 3 patients (0.6%) had pinworms. Conclusions: The findings of this study showed that the prevalence of IPIs among children aged under 15 years in Ilam is relatively down, which can be attributed of sufficient information of parents and children about the modes of parasite transmission and the important role of carriers in the pathogen cycle.

Development of Molecular Diagnosis Using Multiplex Real-Time PCR and T4 Phage Internal Control to Simultaneously Detect Cryptosporidium parvum, Giardia lamblia, and Cyclospora cayetanensis from Human Stool Samples

  • This study aimed to develop a new multiplex real-time PCR detection method for 3 species of waterborne protozoan parasites (Cryptosporidium parvum, Giardia lamblia, and Cyclospora cayetanensis) identified as major causes of traveler's diarrhea. Three target genes were specifically and simultaneously detected by the TaqMan probe method for multiple parasitic infection cases, including Cryptosporidium oocyst wall protein for C. parvum, glutamate dehydrogenase for G. lamblia, and internal transcribed spacer 1 for C. cayetanensis. Gene product 21 for bacteriophage T4 was used as an internal control DNA target for monitoring human stool DNA amplification. TaqMan probes were prepared using 4 fluorescent dyes, $FAM^{TM}$, $HEX^{TM}$, $Cy5^{TM}$, and CAL Fluor $Red^{(R)}$ 610 on C. parvum, G. lamblia, C. cayetanensis, and bacteriophage T4, respectively. We developed a novel primer-probe set for each parasite, a primer-probe cocktail (a mixture of primers and probes for the parasites and the internal control) for multiplex real-time PCR analysis, and a protocol for this detection method. Multiplex real-time PCR with the primer-probe cocktail successfully and specifically detected the target genes of C. parvum, G. lamblia, and C. cayetanensis in the mixed spiked human stool sample. The limit of detection for our assay was $2{\times}10$ copies for C. parvum and for C. cayetanensis, while it was $2{\times}10^3$ copies for G. lamblia. We propose that the multiplex real-time PCR detection method developed here is a useful method for simultaneously diagnosing the most common causative protozoa in traveler's diarrhea.

Detection and genotyping of Giardia intestinalis isolates using intergenic spacer (IGS)-based PCR

  • Giardia intestinalis infections arise primarily from contaminated food or water Zoonotic transmission is possible, and at least 7 major assemblages including 2 assemblages recovered from humans have been identified. The determination of the genotype of G. intestinalis is useful not only for assessing the correlation of clinical symptoms and genotypes, but also for finding the infection route and its causative agent in epidemiological studies. In this study, methods to identify the genotypes more specifically than the known 2 genotypes recovered from humans have been developed using the intergenic spacer (IGS) region of rDNA. The IGS region contains varying sequences and is thus suitable for comparing isolates once they are classified as the same strain. Genomic DNA was extracted from cysts isolated from the feces of 5 Chinese, 2 Laotians and 2 Koreans infected with G. intestinalis and the trophozoites of WB, K1, and GS strains cultured in the laboratory, respectively. The rDNA containing the IGS region was amplified by PCR and cloned. The nucleotide sequence of the 3' end of IGS region was determined and examined by multiple alignment and phylogenetic analysis. Based on the nucleotide sequence of the IGS region, 13 G. intestinalis isolates were classified to assemblages A and B, and assemblage A was subdivided into A1 and A2. Then, the primers specific to each assemblage were designed, and PCR was peformed using those primers. It detected as little as 10 pg of DNA, and the PCR amplified products with the specific length to each assemblage (A1, 176bp; A2, 261 bp; B, 319 bp) were found. The PCR specific to 3 assemblages of G. intestinalis did not react with other bacteria or protozoans, and it did not react with G. intestinalis isolates obtained from dogs and rats. It was thus confirmed that by applying this PCR method amplifying the IGS region, the detection of G. intestinalis and its genotyping can be determined simultaneously.

환경수 중 크립토스포리디움 오시스트 및 지아디아 시스트 검출의 정확도 및 회수율 향상을 위한 연구 (Improvement of Validity and Efficiency for Detection of Cryptosporidium Ocysts and Giardia Cysts in Environmental Water Samples)

  • 환경수 중 크립토스포리디움 및 지아디아 분석방법은 현재 어떤 방법도 만족할만한 민감도, 특이도, 재현성을 인정 받지 못하고 있어, 다양한 형태의 정도관리 및 수행도 평가가 요구되고 있다. 본 연구는 1623방법(USEPA :Method 1623)을 사용해 한강 지표수에 대한 크립토스포리디움 및 지아디아 접종시험과 현장시료 동시분석시험에 기초하여, 환경수 중 이들의 존재여부와 농도를 측정함에 있어 분석결과의 정확도 및 신뢰도, 그리고 회수율에 미치는 영향요인을 검토하였다. 1623방법에 의한 결과, 평균적으로 한강 지표수시료에 존재하는 크립토스포 리디움의 46%와 지아디아의 60%를 검출할 수 있는 것으로 나타났으나, 각각의 회수율 범위가 13-73% 및 28-84%로 변이가 매우 커서 실제 환경 중 두 원생동물의 농도를 결정하기 위해서는 반복측정 및 통계학적 인 접 근이 수반되어 야 할 것으로 나타났다. 또한 IMS (immunomagnetic separation)중 산첨가에 의한 해체·분리과정을 1 회 더 반복함으로써 지표수시료에서의 총회수율의 약 10%를 향상시킬 수 있었으며, flow cytometry로 계수·준비된 (오)시스트((oo)cysts)를 정도관리에 사용할 경우 보다 신뢰할만한 결과를 얻을 수 있었다. 형광항체 및 DAPI염색은 현장시료특성 이나 전 처리과정에 따라 동일하지 않은 형광특징을 보인 바, 접종 된 현장시료 정제액에 대한 염색결과도 참조되어야 하며, 환경시료에는 다양한 비특이물질이 함유되어 있어 DAPI (4',6-diamidino-2-phenylindole) 염색 및 DIC(Differential Interference Contrast)에 의한 내부구조 관찰이 최종 동정기준이 되어야할 것으로 나타났다.

Cryptosporidium spp., Giardia intestinalis, and Enterocytozoon bieneusi in Captive Non-Human Primates in Qinling Mountains

  • Non-human primates (NHPs) are confirmed as reservoirs of Cryptosporidium spp., Giardia intestinalis, and Enterocytozoon bieneusi. In this study, 197 fresh fecal samples from 8 NHP species in Qinling Mountains, northwestern China, were collected and examined using multilocus sequence typing (MLST) method. The results showed that 35 (17.8%) samples were positive for tested parasites, including Cryptosporidium spp. (3.0%), G. intestinalis (2.0%), and E. bieneusi (12.7%). Cryptosporidium spp. were detected in 6 fecal samples of Macaca mulatta, and were identified as C. parvum (n=1) and C. andersoni (n=5). Subtyping analysis showed Cryptosporidium spp. belonged to the C. andersoni MLST subtype (A4, A4, A4, and A1) and C. parvum 60 kDa glycoprotein (gp60) subtype IId A15G2R1. G. intestinalis assemblage E was detected in 3 M. mulatta and 1 Saimiri sciureus. Intra-variations were observed at the triose phosphate isomerase (tpi), beta giardin (bg), and glutamate dehydrogenase (gdh) loci, with 3, 1, and 2 new subtypes found in respective locus. E. bieneusi was found in Cercopithecus neglectus (25.0%), Papio hamadrayas (16.7%), M. mulatta (16.3%), S. sciureus (10%), and Rhinopithecus roxellana (9.5%), with 5 ribosomal internal transcribed spacer (ITS) genotypes: 2 known genotypes (D and BEB6) and 3 novel genotypes (MH, XH, and BSH). These findings indicated the presence of zoonotic potential of Cryptosporidium spp. and E. bieneusi in NHPs in Qinling Mountains. This is the first report of C. andersoni in NHPs. The present study provided basic information for control of cryptosporidiosis, giardiasis, and microsporidiosis in human and animals in this area.

국내 송아지 분변 내 설사병 병원체 검출과 설사 발병율 조사 (Detection of diarrheagenic pathogens from feces and incidence of diarrhea in Korean calves)

  • Diarrhea is reported as one of the most common diseases in calves. It is thought to be a major cause of productivity and economic loss to cattle producers. The aim of this study is to provide a better understanding of well-known diarrheagenic pathogens and incidence of diarrhea in Korean calves. In this study, the relationship of calf diarrhea and pathogens were investigated from calves under 60 days of age in five areas of Korea from April to July, 2016. Of examined fecal samples, 38.3% was positive for any pathogens, and Giardia was the most common pathogen (25.5%). The incidence of diarrhea was 31% from pathogen-negative fecal samples whereas 61.1% from pathogen-positive fecal samples, suggesting high correlation between pathogenic factor and diarrhea. In addition, 80% of E. coli (K99) positive calves showed diarrhea, suggesting E. coli (K99) could be highly pathogenic. The incidence of diarrhea and infection rate increased with age. Rotavirus was revealed as a major pathogen in calves under 20 days of age, and the infection rate of Giardia increased rapidly in calves 20 to 39 days of age. The information on interconnections between clinical diarrhea and pathogens would contribute to developing strategies for treatment of calf diarrhea.
