건강보험청구자료로 본 요양병원의 기능 유형 (A Taxonomy of Geriatric Hospitals Using National Health Insurance Claim Data)

  • Purpose: This study classified the actual functions of geriatric hospitals and examined the differences in their characteristics, in order to provide a basis for discussions on defining the functions of geriatric hospitals and how to pay for care. Methodology: This study used various administrative data such as health insurance data and long-term care insurance data. Cluster analysis was used to categorize geriatric hospitals. To examine the validity of the cluster analysis results, we conducted a discriminant analysis to calculate the accuracy of the classification. To examine cluster characteristics, we examined structure, process, and outcome indicators for each cluster. Findings: The cluster analysis identified five clusters. They were geriatric hospitals with relatively short stays for cancer patients(cluster 1; cancer patient-centered), geriatric hospitals with relatively large numbers of patients using rehabilitation services(cluster 2; rehabilitation patient-centered), geriatric hospitals with a high proportion of relatively severe elderly patients(cluster 3; severe elderly patient-centered), geriatric hospitals with a high proportion of mildly ill elderly patients with various conditions(cluster 4; mildly ill elderly patient-centered), and geriatric hospitals with a significantly higher proportion of dementia patients(cluster 5; dementia patient-centered). The largest number of geriatric hospitals were categorized in clusters 4 and 5, and the structure and process indicators for these clusters were generally lower than for the other clusters. Practical Implications: We have confirmed the existence of geriatric hospitals where the medical function, which is the original purpose of a geriatric hospital, has been weakened. It has been observed that the quality level of these geriatric hospitals is likely to be lower compared to hospitals that prioritize enhanced medical functions. Therefore, it is suggested to consider the conversion of these geriatric hospitals into long-term care facilities, and careful consideration should be given to the review of care-giver payment coverage.

노인병원의 운명 및 재무구조 특성에 관한 연구 (The research for the management and financial affairs of geriatric hospital)

  • According to the increase of the proportion of aged people, the medical demand for a senile chronic disease has been increased; therefore, aged people call for a geriatric hospital for special geriatric medical service. The main purpose of this study was to analyze the general characteristics and financial status of geriatric hospitals. For the study, a questionnaire was designed and sent to the geriatric hospitals to fill out the patient statistics, number of headcount by department, etc. to find out the stability, profitability, activity and so on financial statements of the hospitals were analyzed. The major findings of this study were as belows. 1. The ratio of the medical expenses to the revenue of the geriatric hospitals is much lower than acute care hospitals. But the probability of bankruptcy is higher due to the high ratio of the liabilities therefore it is required to stabilize the financial position by donating more money. 2. Government budget for the elderly people is not enough. To support the geriatric hospitals by going subsides, government should increase the budget. 3. Portion's of the patient of the geriatric hospitals are government support patient. Since the government doesn't pay the medical charges quickly, geriatric hospitals have a serious cash flow problem. Therefore, it is required that government is to prepay the bill. 4. Since geriatric hospitals treat elderly patient and most patients are government support patients, geriatric hospitals can be said to operate under the strict. 5. When we introduce the daily medical charge, the self-liability will be reduced on approximately 50% of current. This affection will bring a huge progressing financial structure to the medical profit of the geriatric hospital, and also patient family will feel less economical burden.

요양병원 소방안전관리의 효율화 방안에 관한 연구 (A Study on the Effective Method of Fire Safety Management of Geriatric Hospitals)

  • Currently many geriatric hospitals have been built in Korea because increasing aging rapidly. However the increasing geriatric hospitals make the increasing safety accidents. Therefore this study is conducted by survey and face-to-face talk with the fire safety managers for analyzing safety problem of geriatric hospitals which number is twelve among 15 geriatric hospitals in the north of Chung-buk area. The result of this study is that geriatric hospitals are very old. Fire safety managers holding the fire safety license are a level 2 issued by the Korea Fire Safety Association(KFSA). And the fire safety inspection ability of them is normal (58%). Furthermore a fire safety budget ratio is 83% (A little) in the company yearly budget. And ratio having the fire inspection equipment in the building is 42% (A little) and 33% (Nothing). Satisfaction ratio of their duty is 66% and they also get stress from heavy duty and heavy responsibility. In conclusion, CEO needs to invest and interest the improvement of job performance, satisfaction ratio and fire safety facility. Also government needs the improvement of system to support and supervise the fire safety of geriatric hospitals consistently.

일반병원의 노인전문병원으로의 전환에 관한 건축계획 연구 (A Study on the Architectural Planning for Conversion from Acute-Care Hospital to Geriatric Hospital)

  • Considering the rapid growth of elderly population and the increase of medical demand for aged, many of geriatric hospitals are needed in Korea. On the other hand, supplies of hospitals for acute care are considered to be excessive. So many of small and medium hospitals facing management problems have been converged to geriatric hospitals. These attempts are considered resonable for efficient utilization of health care resources in Korea. This paper aims to identify the concept of geriatric hospital and to analyze the problems and then to seek the alternatives for architectural planning on the basis of surveying the geriatric hospitals converged from hospitals for acute care.

노인전문병원 입원 노인의 인지훈련 프로그램이 인지기능, 스트레스 및 우울에 미치는 효과 (Effects of the Cognitive Training Program on Cognitive Function, Stress and Depression of Elderly Patients in Geriatric Hospitals)

  • Purpose: The purpose of this study was to identify the effects of the cognitive training program on cognitive function, stress and depression in geriatric hospitals. Methods: The subjects of the experimental group were 17 elderly patients who attended the cognitive training program in Geriatric Hospitals and the subjects of the control group were 15 elderly patients who attended in geriatric hospitals. The persons in training group must take cognitive training program for 4 weeks by 3 times a week. The Mini-mental State Examination Korea version (MMSE-K), Geriatric depression Scaleshort form-Korea version (GDS-K) and Korean Computerized Neurobehavioral Test (KCNT) were utilized to determine cognitive function, stress and depression. The collected data were analyzed by descriptive statistics, t-test and ANOVA using SPSS/PC 12.0 program. Results: Cognitive function (t=-7.625, p=.000) in the elderly after receiving the cognitive training program was significantly more improved than before intervention. Also, Stress and depression (t=2.73, p=.004) was significantly more reduced than before the intervention. Conclusion: This cognitive training program was partially effective in improving cognitive function, neurobehavioral performance and reduce stress and depression. Therefore, it is recommended that this program be used in clinical practice as an effective nursing intervention in geriatric hospitals.

요양병원 간호사의 직무만족이 이직의도에 미치는 영향 (The Influence of Job Satisfation on Turnover Intention of Nurses in Geriatric Hospital)

  • This study aims to analyze the effects of job satisfaction on turnover intention among the nurses in geriatric hospitals. In terms of study methods, this study developed structured questionnaires, and distributed them to nurses working in 11 geriatric hospitals in metropolitan area. A total of 418 questionnaires (response rate: 87.0%) were used for final analysis. We conducted multiple regression analyses to analyze the influence of job satisfaction on turnover intention, adjusting for the characteristics of the research subjects. The results of this study are as follows. First, both job satisfaction and turnover intention were statistically significantly related to individual and hospital characteristics. Second, turnover intention was lower in older age group, and higher in smaller hospitals. As job satisfaction increases, turnover intention decreases. Third, the results of the regression analysis adjusting for subject's characteristics and job satisfaction's sub-categories show that the turnover intention is lower in nurses who are older, day worker, and work in bigger hospitals. Fourth, the result of subgroup analysis by hospital volume show that both of two groups are influenced by hospital ownership and satisfaction about compensation. In addition, professionalism had a significant effect on decreasing turnover intention in smaller hospitals. However, there was no statistically significant relationship between job satisfaction's sub-categories and turnover intention in larger hospitals. The results of this study confirms that job satisfaction lowers turnover intention among nurses in geriatric hospitals. It is suggested that the administrators of those hospitals continue to monitor critical factors associated with job satisfaction, and thus, reduce turnover intention, which helps improve quality of services.

노인병원 환자 죽음에 대한 간호사의 경험 (Nurses' Experiences of the Death of Patients in Geriatric Hospitals)

  • Purpose: The purpose of this study was to identify and understand nurses' experiences of the death of patients in geriatric hospitals. Methods: Van Manen's hermeneutic phenomenological analysis was applied in the interpretation of experiential descriptions of seven nurses who had experienced the death of patients in a geriatric hospital. Results: The essential subjects derived from the experience of the nurses on the death of patients in a geriatric hospital are covered in the following 7 themes. 'Placed in death site', 'Difficult repetition of death and farewell', 'Emotional waves that rushes in after farewell', 'Dilemmas in a place with no preparation to greet expected death', 'Getting dull from continually being struck with sorrow', 'Being together with living death', and 'Showing courtesy for a good farewell and living well'. Conclusion: The results of this research will contribute to the development of policy on all the deaths of patients in geriatric hospitals and suggest basic data that need to be applied in real practice and directions to introduce plans for realistic improvements in nursing care of deathbed patients in geriatric hospitals.

노인특성을 고려한 노인전문병원의 색채환경에 관한 연구 (Study on Color Environment for Geriatric Hospital Considering Characteristics of the Old)

  • Decrease of children caused by economic growth, scientific and technological advancement, long life, spread of nuclear families, increase of women's entry in public affairs, etc. in modern society has given rise to aging societies as another social problem. This has resulted in the advent of problems of the aged and necessity of geriatric hospitals specializing in providing medical services for the old. Even though the geriatric hospitals currently operated are mainly used by aged persons, however, their color environment has been decided, for the most pin, not in view of characteristics of the old but in view of supporting families' criteria for selection of facilities. This study intends to help geriatric hospital designers recognize importance of color environment considering characteristics of the old and select the relevant colors In designing geriatric hospitals in future to elevate remedial value and prevent accidents in space use. To this end, this study suggests problems of color environment found in surveys of the existing geriatric hospitals currently operated throughout the nation and further make some proposals for improvement.

노인요양병원 재이용 의사에 영향을 미치는 소비가치 요인 (Consumption Values on the Intention to Re-use on Geriatric Hospitals)

  • 노인장기요양보험 도입 이후 노인요양병원은 양적으로 빠르게 증가하였다. 그러나 노인요양병원 이용자의 만족도에 관한 연구는 많이 이루어지지 않았다. 이 연구는 Sheth의 소비가치이론을 우리나라 노인요양병원 환자들의 특성에 맞게 부분적으로 수정하여, 시립노인요양병원과 민간노인요양병원에 입원한 환자들을 대상으로 만족도의 중요한 요소 중 하나인 재이용 의사를 분석하여 노인요양병원 경영전략 수립에 필요한 기초자료를 도출하였다. 서울과 경기지역에 소재하고 있는 시립노인요양병원 2개소와 민간노인요양병원 3개소에서 진료를 받고있는 60세 이상의 입원환자 475명을 조사 대상으로 선정하였다. 구조화된 설문지를 개발하여 2012년 8월 1일부터 2012년 9월 30일 까지 자기 기입방법과 면담 설문방법으로 자료를 수집하였다. 분석방법은 SPSS(version14.0)을 사용하여 요인분석을 통해 범주를 추출하고, 위계적 로지스틱 회귀분석을 시행하였다. 연구결과 소비가치 요인 중에서 기능적 가치인 재활치료의 만족도가 병원을 재이용하도록 하는 가장 강한 요인이었으며, 감정적 가치인 첨단 고급성과 진료서비스의 신뢰성, 건강적 가치인 운명적인 건강관의 순으로 유의한 영향을 미치는 것으로 추정되었다. 따라서, 노인요양병원의 경영에 있어서 이러한 연구결과에 기초하여 병원 이용 만족도를 향상시킬 수 있는 재활치료 프로그램의 향상 및 시설, 장비의 개선, 직원의 친절도 향상 등의 전략을 수립하고, 병원 재이용 의사를 제고하기 위한 경영 전략을 수립하는 것이 필요하다.
