Factors Associated with the Long-Stay Admissions in Geriatric Hospitals - Focused on Dementia's Inpatients - (요양병원 치매노인의 장기입원 관련 요인)

  • Purposes: The purpose of this study was to identify the factors related to the long-stay hospitalization of dementia patients aged 65 years or older who had received inpatient care at geriatric hospitals according to the minute facility characteristics and patient features. Methodology: This study was conducted on 317,353 cases of 1,512 geriatric hospitals using the Health Insurance Review and Assessment Service dataset. The data collected were processed using the SAS Enterprise Guide 4.3 for descriptive statistics, the chi-square test, and the binary logistic regression analysis. Findings: As a result of the study, in the facility characteristics of geriatric hospitals, the long-stay hospitalization of the aged with dementia were found to be related to the type of facility establishment, the number of hospital beds, the number of medical specialists, the number of nursing personnel, and the number of geriatric hospitals by region and province. In the personal features of patients, the long-stay hospitalization was found to be associated with the gender, age, insurance, and the patient classification groups. Practical Implication: Considering the results of this study, it seems that securing the sufficient medical personnel in a geriatric facility, providing the good quality medical services, and preparing the appropriate discharge plan can reduce the unnecessary long-stay hospitalization and spend the medical expenses for the older patients.

  • This is a reprospective study on the care of odontogenic infections in admission patients with geriatric diseases. The study was based on a series of 480 patients at Dong San Medical Center, Wonju Christian Hospital and Il San Health Insurance Hospital, From Jan. 1, 2000, to Dec. 31, 2002. The Obtained results were as follows: 1. The systemic malignant tumor was the most frequent cause of the geriatric diseases with odontogenic infectious diseases, and refractory lung disease, systemic heart disease, type II diabetes mellitus, cerebrovascular disease, bone & joint disease, senile psychologic disease were next in order of frequency. 2. Male prediction(57.5%) was existed in the odontogenic infectious patients with geriatric diseases. But, there were female prediction in senile psychologic disease, systemic heart disease and cerebrovascular disease. 3. The most common age group of the odontogenic infectious patient with geriatric disease was the sixty decade(47.9%), followed by the seventy & eighty decade in order. 4. In the contents of chief complaints on the odontogenic infectious patients with geriatric disease, peak incidence was occurred as toothache(52.7%), followed by extraction wish, tooth mobility, oral bleeding, oral ulcer, fracture of restoration, gingival swelling in order. 5. In the diagnosis group of odontogenic infectious diseases, periodontitis, pulpitis & periapical abscess were more common. 6. In the treatment group of odontogenic infectious diseases, the most frequent incidence(34.2%) was showed in primary endodontic treatment (pulp extirpation, occlusal reduction and canal opening drainage) and followed by scaling, incision & drainage, only drugs, pulp capping, restoration in order.

A study on oral health perception and oral health-related quality of life of the elderly patients in a geriatric hospital (요양병원 입원 노인환자의 구강건강인식도와 구강건강관련 삶의 질에 관한 연구)

  • Objectives: The purpose of the study was to investigate the relationship between oral health perception and oral health-related quality of life-14(OHIP-14) in a geriatric hospital. Methods: A self-reported questionnaire was completed by 230 elderly people in a long-term care institution. The questionnaire was consisted of items on general characteristics, oral health perception and OHIP-14. Except incomplete answers, 226 data were analyzed statistically with one-way ANOVA, t-test, Pearson's correlation coefficient and post hoc Tukey test and ANOVA when significance was set at 0.05. Results: The OHIP-14 showed a negative linear relationship(p<0.01) of r=-0.272 with the oral health perception. Among the subgroups of the OHIP-14, quantitative linear relationship(p<0.01) was shown in functional limitation(r=0.822), physical pain(r=0.825), psychological discomfort(r=0.568), physical disability(r=0.895), psychological disability(r=0.652), social disability(r=0.804) and handicap(r=0.818). Conclusions: In order to improve the oral health perception and OHIP-14 in a geriatric hospital, it is necessary to develop continuously a variety of oral health education and systematic oral health promotion program.

  • The purpose of this study is to clarify the actual condition of the daily living activities of the elderly with dementia in medical facilities. The thorough investigation and observation works were made to them from the view points of daily living behaviors and behavioral places of the recuperating elderly. This article discusses about the basic characteristics of the elderly and the actual condition of the daily living activities of the elderly with dementia in 2 geriatric hospitals and 1 special hospital for the aged with dementia. The results of this study are summarized as follows; There is no wide difference between geriatric hospital and special hospital for the aged with dementia on the characteristic of living behaviors of the elderly with dementia. The usage of behavioral places in hospitals are influenced by the physical environments and the basic characteristics of the elderly with dementia.

  • Bacterial biofilm is a community of bacteria that are embedded and structured in a self-secreted extracellular matrix. An important clinical-related characteristic of bacterial biofilms is that they are much more resistant to antimicrobial agents than the planktonic cells (up to 1,000 times), which is one of the main causes of antibiotic resistance in clinics. Therefore, infections caused by biofilms are notoriously difficult to eradicate, such as lung infection caused by Pseudomonas aeruginosa in cystic fibrosis patients. Understanding the resistance mechanisms of biofilms will provide direct insights into how we overcome such resistance. In this review, we summarize the characteristics of biofilms and chronic infections associated with bacterial biofilms. We examine the current understanding and research progress on the major mechanisms of antibiotic resistance in biofilms, including quorum sensing. We also discuss the potential strategies that may overcome biofilm-related antibiotic resistance, focusing on targeting biofilm EPSs, blocking quorum sensing signaling, and using recombinant phages.

Sitagliptin attenuates endothelial dysfunction independent of its blood glucose controlling effect

  • Although the contributions of sitagliptin to endothelial dysfunction in diabetes mellitus were previously reported, the mechanisms still undefined. Autophagy plays an important role in the development of diabetes mellitus, but its role in diabetic macrovascular complications is unclear. This study aims to observe the effect of sitagliptin on macrovascular endothelium in diabetes and explore the role of autophagy in this process. Diabetic rats were induced through administration of high-fat diet and intraperitoneal injection of streptozotocin. Then diabetic rats were treated with or without sitagliptin for 12 weeks. Endothelial damage and autophagy were measured. Human umbilical vein endothelial cells were cultured either in normal glucose or in high glucose medium and intervened with different concentrations of sitagliptin. Rapamycin was used to induce autophagy. Cell viability, apoptosis and autophagy were detected. The expressions of proteins in c-Jun N-terminal kinase (JNK)-Bcl-2-Beclin-1 pathway were measured. Sitagliptin attenuated injuries of endothelium in vivo and in vitro. The expression of microtubuleassociated protein 1 light chain 3 II (LC3II) and beclin-1 were increased in aortas of diabetic rats and cells cultured with high-glucose, while sitagliptin inhibited the over-expression of LC3II and beclin-1. In vitro pre-treatment with sitagliptin decreased rapamycin-induced autophagy. However, after pretreatment with rapamycin, the protective effect of sitagliptin on endothelial cells was abolished. Further studies revealed sitagliptin increased the expression of Bcl-2, while inhibited the expression of JNK in vivo. Sitagliptin attenuates injuries of vascular endothelial cells caused by high glucose through inhibiting over-activated autophagy. JNK-Bcl-2-Beclin-1 pathway may be involved in this process.

A Study on the Relationship Among the Patient Caregiver's Consumption Value and Hospital Satisfaction, Psychological Well-Being in Geriatric Hospitals (노인요양병원 환자보호자의 소비가치와 병원 만족도 및 심리적 안녕감 간의 관계 연구)

  • The purpose of this study was to investigate the relationship among the consumption value and hospital satisfaction and patient caregiver's psychological well-being(PWB) in geriatric hospitals. Data from 355 subjects were collected and analyzed. The findings are as follows. Functional value and emotional value influenced hospital satisfaction positively, while rarity value influenced negatively. Hospital satisfaction influenced patient caregiver's "PWB" positively. And family's support, which had a positive effect on patient 'caregiver's "PWB", played a role in moderating variable between functional value and hospital satisfaction. From this study, the implications of the findings were discussed and hospital management research project that geriatric hospitals should improve hospital satisfaction and "PWB" of patient caregiver was proposed.

  • As the population of the elderly has increased and the nuclear family has become popular, the demands of long-term care services for the elderly have arisen. However, a lot of patients, who need long-term care services, are in general hospitals because existing facilities for the elderly are not sufficient in terms of both quality and quantity. In this study, it is considered a good alternative to set geriatric wards, offering long-term care services for the elderly, in general hospitals. This study shows an analysis of geriatric wards in general hospitals, especially, based on the case of N hospital, and suggests the spacial elements of geriatric wards, which is different from other wards in the aspects of space program and operating system.

A study of Relationship between Death Perception, Spiritual Well Being and Terminal Care Stress of Geriatric Hospital Nurses (노인요양병원 간호사의 죽음 인식, 영적 안녕과 임종간호 스트레스의 관계 연구)

  • The purpose of this study was to understand death perception, spiritual well-being, and terminal care stress in geriatric hospital nurses and analyze related factors affecting terminal care stress. The participants were 181 nurses working in six geriatric hospitals located in Seoul and Gyonggi Province, Korea. Data were gathered from July 16 to August 1, 2015. The data were analyzed using ANOVA, t-test, Duncan test, and logistic regression. Death perception of geriatric hospital nurses had an average score of 4.30, spiritual well-being 3.40, and terminal care stress 3.84. Terminal care stress in relation to general characteristics showed a meaningful difference in religion and total clinical career. The terminal care stress level was divided into two groups using average score as a standard, and the results revealed that spiritual well-being had a significant impact on terminal care stress of geriatric hospital nurses. These factors need to be considered when developing an educational program to reduce terminal care stress of nurses working in a geriatric hospital.
