  • In the early stage of stroke, the symptoms of constipation, chest discomfort and insomnia may appear in addition to the general symptoms such as specific limb weakness, dysphasia, headache, dizziness etc.. We administered Daeseungkitang(大承氣湯) to the patient diagnosed with cerebral infarction by MRI. He clearly displayed the usual syndromes of stroke(實證), endogenous wind stirring in the liver(肝風內動) and acceleration of upper chest functions(上焦火盛), ascertainned by Oriental Medical methods. After treatment with Daeseungkitang(大承氣湯), the constipation, chest discomfort and insomnia improved along with improvement of general symptoms of stroke.

A study on menopausal symptoms and health needs among middle aged women. (중년여성의 갱년기 증상과 건강요구에 대한 기초조사)

  • During the middle age of a woman's life cycle. several health changes and problems occur. Therefore. middle aged women must manage their health and maintain quality life by coping with bodily changes. However. today there is not enough research and health programs for middle aged women. Data from the study will be used for health promotion program development of middle aged women. Data was collected from January 21 to 24. 1995 by telephone interview. Four hundred middle aged women between 40 and 59 years old and living in Chon Ju City were interviewed. The results of this study were as follow: 1. Frequency rate of menopausal symptoms was 38.4%. The most serious menopausal symptom was psychosomatic symptom. Mean score orders of experienced symptoms were back pain, joint pain(1.80). nervousness(1.80). general weakness(1.67). 2. The most important problems as perceived by the clients were children(45.8%), health (24.0%) and economics(7.8%). The most serious health problems were concerning the muscle-skeletal system(45%) such as arthritis. spinal disk problems and osteoporosis. Adult diseases04.S%) such .as hypertension and diabetes were also health concerns. Health management activities reported were exercise(22.5%), social activity02%) and inactivity(53%)' 33% of clients were interested in health groups and they wanted a program of health education, exercise and social activity to be provided. 3. General characteristic variables were significantly related to the level of menopausal symptoms as follows: age(t=-2.06, p=0.040), status of marriage(t=-3.56, p=0.000), educational level (F=4.35. p=0.05) and menopausal status(t=4.37, p=0.000).

  • 1. Objectives: This case study reports clinical improvement of a Soyangin benign arrhythmia patient who showed improvement of palpitation symptoms after treatment with Hyeongbangjihwang-tang. 2. Methods: The patient's subjective and objective symptoms were observed daily, and the VAS scores and duration times of palpitation were recorded daily throughout the hospitalization period. 3. Results: The general condition of the patient improved by the end of hospitalization. The VAS score dropped from 4-10 to 0-1, and the duration of palpitation dropped from 50-160 min to 0-5 min. Other symptoms such as diarrhea and lower back pain also showed improvement. 4. Conclusion: This case study shows the effectiveness of Hyeongbangjihwang-tang in the treatment of palpitation. Although the literature on Hyeongbangjihwang-tang does not report its usage on palpitation, the prescription was shown to be effective in the case where the palpitation symptom is concomitant to the general symptomatology of the Mangeum disease, a pathology indicative of Hyeongbangjihwang-tang.

Study on the Change of Fatigue in Gastrointestinal Cancer Patients With the Time Relapse After Chemotherapy (소화기암환자의 항암요법 시간 경과에 따른 피로도 측정 연구)

  • Purpose: Fatigue is one of the most common complaints of cancer patients. In this study, we analyzed the change of fatigue level and general symptoms as time go by, so that, we could explain more on the mechanism and change of fatigue in relation with treatment, and explore the influencing factors. Method: The subjects of this study were 50 GI cancer patients who have visited the cancer center of A hospital in Suwon. We measured fatigue by using the Revised Piper Fatigue Scale(RPFS) at the time of starting and finishing induction chemotherapy, and starting the 2nd cycle of chemotherapy. Results: 1) The fatigue score was 2.81, 3.73, and 3.82 in a 10 point scale at the time of starting and finishing induction chemotherapy, and starting the 2nd chemotherapy, respectively. This means fatigue persisted until after the treatment. 2) Fifty two percent of participants complained of some kinds of symptoms when starting the treatment, and the proportion increased up to 92% when finishing the treatment. 3) Fatigue scores were significantly high in patients with fatigue-related symptoms than for patients without those symptoms. 4) Fatigue scores showed significant differences according to patients' general characteristics such as age, educational level, economic status, occupation, diagnosis, hematocrit, weight, and amount of sleepy. Conclusion: We have to develop intervention strategies to reduce fatigue in cancer patients in the consideration of influencing factors.

Relationship among Dysfunctional Attitudes, Stress Coping Strategies and Depressive Symptoms in Psychiatric Patients (정신질환자들의 역기능적 태도, 스트레스 대처 방식 및 우울증상 간의 관계)

  • This study was aimed to investigate dysfunctional attitudes, stress coping strategies and depressive symptoms in psychiatric patients. The subjects of this study consisted of 210 patients(138 schizophrenic patients, 29 depression patients, 43 alcohol dependence patients) according to DSM-IV criteria. Futhermore, the instruments were K-BDI(Beck Depression Inventory-Korean version), DAS(Dysfunctional Attitude Scale) and multidimensional coping strategy scale. The results were the following. 1) There were statistically significant correlations between depressive symptoms and dysfunctional attitudes in psychiatric patients. 2) In terms of coping strategies, there were positive correlations between depressive symptoms and focus on and venting emotions, accommodation, active forgetting, self-criticism, positive comparison, fatalism, passive withdrawal. Whereas, there was significant negative correlation between depressive symptom and active coping. 3) In terms of coping strategies, there were significant correlations between dysfunctional attitudes and focus on and venting emotions, active forgetting, self-criticism, positive comparison, fatalism, passive withdrawal. 4) Depression groups reported significantly higher BDI scores than schizophrenia groups. 5) In depression groups, DAS scores were significantly higher than those in schizophrenia groups. 6) In terms of coping strategies according to diagnosis, there were significant differences in venting emotions, active forgetting and self-criticism. As for venting emotions, alcoholic groups were scored significantly higher than schizophrenic groups. As for active forgetting, depression groups were scored significantly higher than schizophrenic groups. In self-criticism, depression groups and alcohol dependence groups reported significantly higher scores than schizophrenic groups.

Industrial Fatigue and Low Back Pain of the Nurses in General Hospital (종합병원 간호사들의 피로자각증상과 요통)

  • To investigate industrial fatigue and low back pain, the questionaire survey for subjective symptoms of fatigue and low back pain was carried out 305 nurses in general hospital in Korea. Nurses participated to this study were divided into low back pain group(LBP) and control group, according to the self-reports by written questionaires. The subjective sysptoms of fatigue comprised three groups of 10 items each, representing dullness and sleepiness(level of cerebral activation), difficulty in concentration(level of motivation) and bodily projection of fatigue. The resultant data were processed for $x^2$-test, t-test to confirm the associations. The results were as follows: 1. 28 items of fatigue subjective symptoms exept two item, 'feel thirsty and 'feel ill', were directly associated with low back pain. 2. The percentage of fatigue complaint were significantly higher in LBP group than control. 3. Of the 30 items of fatigue subjective symptoms, the highest percentage was accounted for 'legs feel heavy', 'eye strain', 'feel stiffness in the neck or the shoulders' followed by 'feel like lying', 'whole body feels tired', 'feel a pain in the low back', 'feel drowsy' and in the order of sequence. 4. The average weighted score for the first group of fatigue items(dullness and sleepiness) was the largest among three groups and was followed by the second group(difficulty in concentration) and the third group (bodily projection of fatigue) in the order of sequence, suggesting the heavier mental and shift work stress of the female workers.

A Study on Premenstrual syndrome and Menstrual Attitude (여대생의 월경전증후군과 월경에 대한 태도에 관한 연구)

  • The study was intended to investigate the bothersome level of premenstrual symptoms, their pattern and to examine the relationships between menstrual attitude and the premenstrual symptoms. Two hundred sixty eight female students were recruited from a college located in Kyungido from March 1, 2001 to July 1, 2001. A general characteristics questionnaires, the premenstrual assessment form(PAF) and the menstrual distress questionnaire(MDQ) were used to measure the bothersome level of the premenstrual symptoms and the menstrual attitude. The data were analyzed by SPSS-PC+ program. The results of this study were as follows ; 1. All subject who were participated in the research reported more than one symptom in premenstrual period and the mean score of total categories in PAF was low(1.89). The subject had more symptoms of fatigue, abdominal bloating and discomfort, backache and muscle stiffness and among the 21 categories fatigue feature, hysteroid feature, water retention feature and miscellaneous mood/behavior change feature were prevalent. On the other hand organic mental feature and increased well-being feature were rare that premenstrual symptom has negative aspect than positive. 2. Degree of discomfort in premenstrual symptom was related with dysmenorrhea but other general characteristics. 3. In Menstruation attitude, the student in college recognized menstruation as natural but bothersome and causes negatives effects on body and emotion. 4. There were significant correlation(r=.395, p<0.000) between premenstrual symptom and level of Menstrual attitude. 5. Menstrual attitude explained 15.3% variance of PMS and five categories of menstrual attitude, especially factor 1(menstruation is a phenomena that weakens women physically and psychologically) was most highly correlated with PMS and explained 21.1% variance of PMS.

The purpose of this study This study was performed on the basis of clinical consideration about rhinitis patients who suffered from sneeze, rhinorrhea, rhinocleisis and etc. And we checked the clinical inclination of this disease and the characteristic treatments of Sasang Constitutional Medicine. 2. The method of this study For 17 months from October in 1999 to April in 2001 we selected outpatients who had consulted for rhinitis more than twice in the department of the Sasang Constitution of the Bundang Oriental medical hospital, Dongguk University, and 31 of those were confirmed of their course of treatment. From their medical records and history, we learned the general characterisics and patterns of rhinitis, and the qualities and effects of its various treatment. 3. The result of this study The result were as follows ; 1) The rate of patients who have had the symptoms over the two years was 74.2%. 2) In the distribution of chief symptoms, sneeze occupied 80.6% of all appealed symptoms, rhinocleisis 77.4%, rhinorrhea 71.0%, intranasal pruritus 54.8%, postnasal drip 16.1%, orderly. 3) The treatment period was about 30days. The method of treatment was the Herb-med Tx, acupuncture Tx and etc. The herb-med ingestion was about two times in that period. 4) On the effect of treatment, Sneeze and rhinocleisis were improved. 5) The symptoms of rhinitis was more depravated by fatigue and stressful condition than weather condition. This result shows that the improvement of general condition has an effect on the rhinitis treatment.

A Clinical Case Report of Soeumin Patient with Chronic Hand Eczema (만성 수부 습진을 호소하는 소음인 환자 치험 1례)

  • Introduction Hand eczema is dermatitis that occurs on the hand and fingers. In this case, the patient had chronic hand eczema that had relapsed for 3 years, worsened for 6 months, and did not improve even with standard dermatological treatment. We report significant improvement of the patient with chronic hand eczema, diagnosed with Soeunmin Greater Yin Symptomatology based on Sasang Constitutional Medicine. Methods We prescribed herbal medicine, Sibimigwanjung-tang(十二味寬中湯), and the western medications were continued throughout the treatment period. To evaluate the treatment progression, we observed objective and subjective symptoms and took pictures of the patient's hands. Results At the first time of the outpatient visit, the patient complained of pain, oozing, scaling and severe itching even though she had taken dermatological medication for over 6 months. After 8 weeks of Korean medicine treatment, most symptoms showed improvement, and after about 47 weeks of treatment, the symptoms maintained 90% improvement even after stopping western medications and herbal medications. Conclusion The patient in this case showed a significant improvement of chronic hand eczema and the improvements have been maintained even stopping western medications and herbal medications during follow-up for 15 weeks. In addition, the patient showed improvement and maintenance of general conditions as well as skin-related symptoms. In this case, we can concern the effect of the Sasang Constitutional Medicine focused on treating the patient's general pathology as well as the patient's chief complaint at chronic hand eczema not responding to standard dermatological treatment.

Olfactory Dysfunction in COVID-19 from a Korean Medical Perspective (COVID-19 후각 이상에 대한 한의학적 고찰)

  • Objectives : To analyze symptoms of olfactory dysfunction caused by COVID-19 from a Korean Medical Perspective. Methods : Previous studies dealing with olfactory dysfunction accompanying COVID-19 were studied and analyzed for general characterization. Physiology and pathology of olfactory functions within the classical texts of Korean Medicine were collected and analyzed, through which symptoms of olfactory dysfunction in COVID-19 were examined. Results : Olfactory dysfunction manifested in high ratios in the early stages of confirmed COVID-19 cases, at times independent of other nasal symptoms such as blockage or discharge. There was a high chance of loss of taste being accompanied, while mental problems such as a tendency to have difficulty concentrating were present as well. In most cases, recovery took one to two weeks. From a Korean Medical perspective, physiology of olfactory function is closely linked to the Lungs, Ancetral Qi[宗氣], and the Heart, while its dysfunction could be explained by pathological factors such as Wind-Cold, Fire stagnation, Qi deficiency, Wind stroke, etc. Conclusions : In the context of external contraction disease[外感病], olfactory dysfunction could be caused by problems in the Lungs and Stomach that are responsible for breathing, or the Heart which is involved in recognizing and differentiating scent. General characteristics of COVID-19 imply it to be closely related to the Heart. In clinical application, overall symptoms need to be considered in diagnosis and treatment planning, after which further approaches could made to determine the problem to be of the Lung and Stomach, or of the Heart.
