Using Digital Infrared Thermal Image on Diagnosing Gastrointestinal Disease (위장병(胃腸病) 환자(患者)에서 적외선체열진단기(赤外線體熱診斷器)의 활용(活用))

  • Objective : Cold hypersensitivity is excessive sensitivity of each body part. particularly limbs and low backs, but sometimes it affects upper abdomen region in G-I trouble patients. Methods : We conducted this research on 69 patients who came and took the both exam of gastroscopy and D.I.T.I. in kangnam korean hospital kyung hee university. So, we researched the significance of temperature deviations on upper abdomen which was studied separately by gastritis group, digestive function group and H. pylori group. Results and Conclusion : The temperature deviation of Zhongwan(Ren12)-Danzhong(Ren17) of gastritis group and indigestive patients was significant, it means that patients who have severe gastritis and indigestion have more significant cold hypersensitivity in upper abdomen. The temperature deviation of Zhongwan(Ren12)-Danzhong(Ren17) of H.pylori positive patients in H.pylori group was not significant. The clinical relationship or tendency was not found both between H.pylori group and gastritis group and between H.pylori group and digestive function group.

A Case Study of 4 Patients with Reflux Esophagitis Who Had Improved Gastroesophageal Reflux Disease after Treatment with SokPyeonAnGel (속편안겔 투여 후 위식도 역류질환 증상의 호전을 보인 역류성 식도염 병발 환자 치험 4례)

  • Objectives : This study is to report the effects of SokPyeonAnGel (SPAG) on patients with reflux esophagitis. Methods : Four patients with gastroesophageal reflux disease (GERD) symptoms, who had suffered from different symptoms, were diagnosed with reflux esophagitis by gastroscopy. SPAG was orally administered 30 minutes after each meal, 3 times a day. Analysis of the Gastroesophageal Reflux Disease Questionnaire (GerdQ) for GERD symptoms and quality of life was performed before and 1~3 weeks after SPAG treatment. Results and Conclusions : The rating scores decreased by 1~4 points after treatment. In particular, there was a 2-point decrease in “How often did you take additional medication for your heartburn and/or regurgitation?” except for one patient who hadn't taken additional medication. This suggests that SPAG could be used to treat symptoms of reflux esophagitis.

  • Purpose: Ménétrier disease (MD) was first described in 1888, and 50 cases have been reported until now. We aimed to discuss the etiology, diagnostics, and management of MD in children. Methods: We searched for case reports published from 2014 till 2019 in English using PubMed. Articles were selected using subject headings and key words of interest to the topic. Interesting references of the included articles were also included. Results: The pathophysiology of MD is still uncertain. However, overexpression of transforming growth factor alpha with transformation of the gastric mucosa has been observed, which may be mediated by genetics and provoked by an infectious trigger. Clinically, MD is diagnosed by abdominal pain, vomiting, anorexia, and edema secondary to hypoalbuminemia. A gastroscopy with biopsy is the gold standard for the diagnosis of MD. In children, the disease is self-limiting and only requires supportive treatment. In general, children have a good prognosis and recover spontaneously within a few weeks. Conclusion: Few pediatric cases of MD have been described in recent years, and with all different etiology. Endoscopy with biopsy remains the golden standard for the diagnosis of MD, and in children, the disease is self-limiting.

  • A 21-year-old female Halla Horse weighing 248 kg was referred to the Jeju National University Equine Hospital with the chief complaint of anorexia accompanied by general weakness and depression for the previous three days suspected to be related to colic. Extensive diagnostic tests were performed for the following six days, including complete blood count (CBC), serum chemistry, gastroscopy, x-rays, and ultrasound imaging. The signalment, history, symptoms, and test results strongly suggested a chronic intestinal inflammatory disease with or without an alimentary tumor; hence, an exploratory laparotomy was performed. Almost the entire small intestine wall was severely thickened with diffuse ecchymosis on the serosa and protruded nodules on the mucosa. A presumptive diagnosis of an intestinal tumor was made, and the patient was euthanized, considering the patient's welfare with poor prognosis and low expectancy. A massive part of the small intestine was collected and submitted for macroscopic and microscopic pathology evaluations. The pathologic examination, including immunohistochemistry (IHC), indicated equine intestinal lymphoma showing strong positivity for T cell marker. This report describes the clinical signs, diagnosis and pathological features of intestinal lymphoma in a Halla Horse in detail.

  • Recently, several deep learning studies have been reported to automatically identify the location of diagnostic devices using endoscopic data. In previous studies, there was no design to determine whether the configuration of the dataset resulted in differences in the accuracy in which artificial intelligence models perform image classification. Studies that are based on large amounts of data are likely to have different results depending on the composition of the dataset or its proportion. In this study, we intended to determine the existence and extent of accuracy according to the composition of the dataset by compiling it into three main types using larynx, esophagus, gastroscopy, and laryngeal endoscopy images.

  • This study was conducted to explore the relationship between locus of control and health belief. the life style of gastric cancer patients. The subjects of this study were 40 of the early gastric cancer and 90 of the advanced gastric cancer who first visiting patient to general surgery out patient department of Chungnam University Hospital for operation. The data was collected with structured questionnaire from July. 1998 to Feb. 1999 The tool were Moon's Health Belief Scale and Multiple Locus of Control of Wallston. Wallston, DeVellis. The data was analysed by SAS program using frequency, $X^2$-test, Pearson's correlation coefficient, ANOVA. Scheffe-test, t-test. The results were as follows 1. 30.8% of subjects were early gastric cancer anf 69.2% were advanced gastric cancer. The subject knowing about diagnosis was 83.1%. 16.9% did'nt know about diagnosis. 2. The correlationship between Health belief and Locus of control of subjects was not supported. 3. There was statistically no difference of life style between early and advanced gastric cancer patients. 4. There were statistically significant differences in perceived sensitivity according to weight. educational level. and birth order, in perceived barrier according to educational level. and in perceived severity according to occupation among demographic characteristics of study subjects. From above results. health professional has to educate general population to detect gastric cancer early to improve survival rate because early gastric cancer is high in survival rate, and to prevent recurrence and to maintain continuing healthy status. In the future, gastroscopy also has to expand to detect early. For there was no difference in life style between early and advanced gastric cancer. carcinogen related to diet should be emphasized through education. The perceived benefit among health belief model was not supported in this study therefore further study and comparison between gastric cancer and normal population are needed.

Involvement of MicroRNA-198 Overexpression in the Poor Prognosis of Esophageal Cancer

  • Objective: This study aimed to investigate whether the miR-198 expression level is related to clinicopathological factors and prognosis of esophageal cancer. Methods: MicroRNA was extracted from esophageal cancer patients who underwent surgery for assessment using the Taqman@ MicroRNA assay. The correlation between miR-198 expression and clinicopathological features was analyzed, and the significance of miR-198 as a prognostic factor and its relationship with survival was determined. Results: MicroRNA-198 (miR-198) expression was higher in patients with poor prognosis than those with good prognosis (P<0.05). Kaplan-Meier analysis results showed that the miR-198 expression level had a significant correlation with survival time (P=0.030) and that patients with a higher expression of miR-198 had a shorter survival time. Cox multi-factor model analysis showed that patient prognosis (P=0.014), tumor length (P=0.040) and expression (P=0.012), and survival time had a significant correlation; the corresponding risks were 7.268, 1.246, and 3.524, respectively. Conclusion: miR-198 overexpression is involved in the poor prognosis of esophageal cancer and can be used as a biomarker for selection of cases requiring especial attention.

A Study on Emotional Characteristics Using Upper Gastrointestinal Endoscopic Findings and The Core Seven Emotions Inventory-Short Form (상부위장관내시경 소견과 핵심칠정척도 단축형을 활용한 정서적 특성 연구)

  • Objectives: To perform correlation analysis between the Core Seven Emotions Inventory-Short Form (CSEI-s), Beck Anxiety Inventory (BAI), Beck Depression Inventory (BDI), upper gastrointestinal symptoms, and gastroscopic findings and to compare emotional characteristics with upper gastrointestinal endoscopic findings. Methods: In this study, medical records of 38 participants who visited the hospital for examination purposes or complained upper gastrointestinal symptoms and completed the CSEI-s, BAI, BDI, and Questionnaire for upper gastrointestinal symptoms were analyzed using SPSS (Statistical Package for the Social Sciences) version 25.0. Frequency Analysis, Descriptive Analysis, Independent t-test, Mann-Whitney's U-test, and Correlation analysis were performed. Results: The upper gastrointestinal symptom group showed higher levels of anger, sorrow, depression, and anxiety than the asymptomatic group. On gastroscopy, the normal group had higher levels of Joy than the abnormal group. The reflux esophagitis group showed higher levels of thought, depression, sorrow, and anxiety than the non-reflux esophagitis group. Joy showed a negative correlation with BDI score. However, anger, depression, sorrow, BAI, and BDI score showed positive correlations. Conclusions: Results of this study suggest that CSEI-s can be used to treat patients with upper gastrointestinal symptoms and reflux esophagitis.

Hookworm Infection Caused Acute Intestinal Bleeding Diagnosed by Capsule: A Case Report and Literature Review

  • Hookworm infections are rare causes of acute gastrointestinal bleeding. We report a middle aged man with primary nephrotic syndrome and pulmonary embolism. During the treatment with steroids and anticoagulants, the patient presented acute massive hemorrhage of the gastrointestinal tract. The results of gastroscopy showed red worms in the duodenum. Colonoscopy and CT angiogram of abdomen were unremarkable. Capsule endoscopy revealed fresh blood and multiple hookworms in the jejunum and ileum. Hookworms caused the acute intestinal bleeding. The patient responded well to albendazole. Hematochezia was markedly ameliorated after eliminating the parasites. Hence, hookworm infection should be considered in the differential diagnosis of a patient with obscure gastrointestinal bleeding. Capsule endoscopy may offer a better means of diagnosis for intestinal hookworm infections.

Bowel Sound Recording for Structural Deformity in the Pylorus-Duodenum: Report of 3 Cases (각기 다른 유문-십이지장의 기질적 병변 3증례에 대한 장음 측정 보고 : 장음의 주주파수를 중심으로)

  • Objectives: The purpose of this study is to report bowel sound patterns recorded in 3 diseases associated with structural deformity of the pylorus-duodenum. Methods: : Bowel sound recording is a useful non-invasive tool for the evaluation of gastrointestinal motility. However, the difficulty in manual attachment of the electronic stethoscope on the abdominal wall and noise production against bowel sound signals have prohibited its widespread use. Therefore, I developed a new apparatus that eliminates the noise; it contains a sound absorbing device and a holder for the head of stethoscope. Using the new bowel sound recording system, bowel sounds of the patients were recorded repeatedly. The endoscopic and abdominal computed tomography findings were obtained from other hospitals' medical record, and all patients were confirmed to have structural deformity in the pylorus-duodenum. The records of each patient were compared and assessed. Results: : Patients had either duodenal ulcer scar, pyloric stricture, or far advanced malignant stomach cancer, as diagnosed by the gastroscopy. Their dominant frequency of bowel sound obtained from the new system was checked more than 2 times at regular intervals. All 8 recordings in the 2 patients with duodenal ulcer scar (5 times) or pyloric stricture (3 times) showed a decrease in postprandial than fasting dominant frequency. One patient with stomach cancer showed no significant change between postprandial and fasting states at 2 recordings. Conclusions: The analysis of dominant frequency in bowel sounds can indicate the existence of pyloric obstruction that could delay gastric emptying.
