만성 위장장애를 호소하는 히스테리성 성격의 여자 (A Case of Woman with Histrionic Personality who Suffered from Chronic Gastrointestinal Dysfunction)

  • A case of a 38 year-old woman with histrionic personality who had chronic epigastric pain, dyspepsia and alternating bowel habit for more than 10 years was presented in detail on its course of two times of admission and follow-up. The diagnosis was thought as psychophy-siological disorder or gastrointestinal motility disorder of undefined etiology rather than hypo-chondriasis or Briquet's syndrome. She was characterized by sustained illness behavior and combined several physical illnesses. i.e. tuberculosis. anemia and hepatic stone. These physical diseases led to a blurring of psychological and physical boundaries regarding symptom formation. The points on consultation from medical part to psychiatric department were discussed and the supposed causal mechanisms in non-organic functional gastrointestinal disturbances were also reviewed. Physical and psychological modalities for the treatment and the abnormal illness behavior were mainly emphasized in this case.

Submucosal endoscopy: the present and future

  • Submucosal endoscopy or third-space endoscopy utilizes the potential space between the mucosal and muscularis layers of the gastrointestinal tract to execute therapeutic interventions for various diseases. Over the last decade, endoscopic access to the submucosal space has revolutionized the field of therapeutic endoscopy. Submucosal endoscopy was originally used to perform endoscopic myotomy in patients with achalasia cardia, and its use has grown exponentially since. Currently, submucosal endoscopy is widely used to resect subepithelial tumors and to manage refractory gastroparesis and Zenker's diverticulum. While the utility of submucosal endoscopy has stood the test of time in esophageal motility disorders and subepithelial tumors, its durability remains to be established in conditions such as Zenker's diverticulum and refractory gastroparesis. Other emerging indications for submucosal endoscopy include esophageal epiphrenic diverticulum, Hirschsprung's disease, and esophageal strictures not amenable to conventional endoscopic treatment. The potential of submucosal endoscopy to provide easy and safe access to the mediastinum and peritoneal spaces may open doors to novel indications and rejuvenate the interest of endoscopists in natural orifice transluminal endoscopic surgery in the future. This review focuses on the current spectrum, recent updates, and future direction of submucosal endoscopy in the gastrointestinal tract.

The Altered Signaling on EFS-Induced Colon Contractility in Diabetic Rats

  • Diabetes mellitus affects the colonic motility developing gastrointestinal symptoms, such as constipation. The aim of the study was to examine the role of intracellular signaling pathways contributing to colonic dysmotility in diabetes mellitus. To generate diabetes mellitus, the rats were injected by a single high dose of streptozotocin (65 mg/kg) intraperitoneally. The proximal colons from both normal and diabetic rats were contracted by applying an electrical field stimulation with pulse voltage of 40 V in amplitude and pulse duration of 1 ms at frequencies of 1, 2, 4, and 6 Hz. The muscle strips from both normal rats and rats with diabetes mellitus were pretreated with different antagonists and inhibitors. Rats with diabetes mellitus had lower motility than the control group. There were significant differences in the percentage of inhibition of contraction between normal rats and rats with diabetes mellitus after the incubation of tetrodotoxin (neuronal blocker), atropine (muscarinic receptor antagonist), prazosin (α1 adrenergic receptor antagonist), DPCPX (adenosine A1 receptor antagonist), verapamil (L-type Ca2+ channel blocker), U73122 (PLC inhibitor), ML-9 (MLCK inhibitor), udenafil (PDE5 inhibitor), and methylene blue (guanylate cyclase inhibitor). The protein expression of p-MLC and PDE5 were decreased in the diabetic group compared to the normal group. These results showed that the reduced colonic contractility resulted from the impaired neuronal conduction and decreased muscarinic receptor sensitivity, which resulted in decreased phosphorylation of MLC via MLCK, and cGMP activity through PDE5.

A histochemical study of argentaffin endocrine cells in the gastrointestinal tract of ovariectomized rats

  • The regional distributions and frequencies of argentaffin endocrine cells in gastrointestinal (GI) tract of osteoporotic Sprague-Dawley rat induced by ovariectomy were studied by Masson-Hamperl silver stain. The experimental animals were divided into two groups, one is non-ovariectomized group (Sham) and the other is ovariectomized group (OVX). Samples were collected from each part of GI tract (fundus, pylorus, duodenum, jejunum, ileum, cecum, colon and rectum) at 10th week after ovariectomy or sham operation. Argentaffin cells were detected throughout the entire GI tract with various frequencies regardless of ovariectomy except for the rectum of OVX in which no cells were detected. Most of these argentaffin cells in the mucosa of GI tract were generally spherical or spindle in shape (open type cell) while cells showing round in shape (close type cell) were rarely found in gland regions. Significant decrease of argentaffin cells was detected in OVX compared to that of Sham except for the fundus and jejunum. However, in the fundus and jejunum, argentaffin cells in OVX showed similar frequency compared to that of Sham. In conclusion, the endocrine cells are the anatomical units responsible for the production of gut hormones that regulate gut motility and digestion including absorption, and a change in their density would reflect the change in the capacity of producing these hormones and regulating gut motility and digestion. Ovariectomy induced severe quantitative changes of GI argentaffin endocrine cell density, and the abnormality in density of GI endocrine cells may contribute to the development of gastrointestinal symptoms in osteoporosis such as impairments of calcium and some lipids, frequently encountered in patients with postmenopausal osteoporosis.

Effects of Cheonggukjang on Immune Responses and Gastrointestinal Functions in Rats

  • Effects of cheonggukjang on immunohistochemical reactions in gastrointestinal (GI) tract of rats were investigated. $CD4^+/CD8^+$ immunoreactive cells of cheonggukjang-fed diet groups were more strongly stained in lamina propria of mucosa and submucosa than those of basal diet group. Universal nitric oxide synthase immunoreactive density in colon was mildly stained in surface epithelium and mucous secretory gland, and strongly stained in submucosa and myenteric plexus in muscle layers of all cheonggukjang-fed diet groups. Protein kinase C-${\alpha}$ immunoreactive cells in colons of 15 and 25% cheonggukjang-fed diet groups were more strongly stained in mucosa, submucosa, and muscle layers than those of basal diet group. These results indicate mucosal immune activity, gastrointestinal motility, blood circulation, and physiological activities of enteroendocrine cells in GI tract could be increased with cheonggukjang intake.

생강(生薑) 추출물의 위장관 기능개선 및 체온저하 억제효과에 대한 실험적 연구 (Effect on Promoting Gastrointestinal Function and Inhibiting of Decreasing Body Temperature of Ginger Extracts(Zingiber Officinale))

  • This study was performed to investigate the effect of promoting gastrointestinal function and inhibiting of decreasing body temperature of ginger extract(Zingiber officinale) in rats. In order to elucidate the gastrointestinal function and inhibiting effect of body temperature of native ginger and improved ginger, water extracts of ginger were orally administrated into rats. The results are as follows: The gastrointestinal transit time was significantly decreased in native ginger(7.66hrs) and improved ginger(7.72hrs) extract administrated groups compare to control group(8.44hrs). The mean red faecal weight was increased in native ginger(30.6%) and improved ginger(31.1%) extract administrated groups compare to control group(24.9%) for 24hrs. Inhibiting effect of decreasing body temperature induced by serotonin was increased in native ginger($1.116^{\circ}C$) and improved ginger($1.416^{\circ}C$) extract administrated groups compare to positive control group($0.384^{\circ}C$) during 40 minutes. Gastrin and CGRP immunoreactive density was more strongly expressed in native ginger and improved ginger extract administrated groups compare to control group. Serotonin immunoreactive density was more weakly expressed in native ginger and improved ginger extract administrated groups compare to control group. These results suggest that ginger extracts may enhance physiological activity such as gastrointestinal motility, protection of mucosa and gastric acid secretion in gastrointestinal tracts, and inhibits decreasing body temperature

해계 유침과 전침이 loperamide로 저하된 흰 쥐의 장운동에 미치는 영향 (The Effect of Acupuncture and Electro-acupuncture at ST41 on Intestinal Hypomotility Induced with Loperamide in Rats)

  • Objectives : The purpose of this study was to compare the effect of acpuncture and electro-acupuncture of low(EA(L)) and high(EA(H)) frequency at Haegye(ST41) on intestinal hypomotility induced with loperamide in rats. Methods : We made suppressed state of intestinal motility with loperamide in rats and carried out needle retention acupuncture, low frequency electro-acupuncture and high frequency electro-acupuncture at ST41 in rats devided into pre-treatment group and post-treatment group. We fed charcoal to them after the treatment and measured the travel rate of charcoal in the gastrointestinal track to analyze which treatment is more effective in state of intestinal hypomotility. Results : None of acupuncture, EA(L) and EA(H) at ST41 had significant influences on intestinal motility of rat in normal state. Needle retention at ST41 did not significantly increase intestinal motility suppressed with loperamide in rats. Pre-treatment of EA(L) and EA(H) at ST41 significantly increased intestinal motility suppressed with loperamide in rats. Post-treatment of EA(L) and EA(H) at ST41 did not have significant influences on intestinal motility of rat in normal state. Conclusions : These results suggest that treatment of EA(L) and EA(H) at ST41 may be effective on gastric disorders such as intestinal hypomotility and its effect had more prevention than cure. Further study is necessary to know more effects of ST41 and electro-acupuncture of low and high frequency.

Preliminary pharmacological evaluation of an Ayurvedic formulation Dasamularista

  • In this study the Ayurvedic formulation Dasamularista was studied for its preliminary pharmacological properties using laboratory mice. Dasamularista showed a decrease in food intake and stool formation, while the water content of stool and water intake was higher and the volume of the urine was less. Dasamularista in a slight extent reduced the intestinal motility. This constipating effect was further supported by the significant anti-diarrhoeal property of the formulation in castor oil induced dairrhoea. The tested formulation markedly increased the latent period of diarrhoea and reduced the purging index value. Dasamularista did not alter the acetic acid induced abdominal writhing. Significant reduction on the onset of sleeping time and increased duration of sleep was observed in pentobarbital induced sleeping time test.

Evaluation of antidiarrhoeal activity of Cardamom (Elettaria cardamomum) on mice models

  • Diarrhoea is a major health care problem in developing countries. Elettaria cardamomum Maton fruits, commonly known as cardamom are widely used for flavoring purposes in food. In this study we evaluated the antidiarrhoeal activity of hot water extract of cardamom against experimental diarrhoeal models on mice. Cardamom extract showed significant antidiarrhoeal activity against castor oil and magnesium sulphate induced models. Whereas, the gastrointestinal motility was slightly increased.

기능성 소화불량증 (Functional Dyspepsia)

  • The functional dyspepsia is a complex disease reaction that ocurs due to the presence of various factors and its pathophysiology has not been clearly verified until now. This disease is diagnosed after the organic diseases are ruled out. Therefore, the main treatments are limited down to compensating for revealed symptoms through exercising the supportive care. The international prevalence of the functional dyspepsia under the Rome I criterior is approximately 25%. For the cases of Koreans, there are very omnifarious symptoms in gastrointestinal disorders and the medical vocabularies which describe the diagnostic standard are not closely associated with the daily vocabularies, thus there would be a lot of restrictions to directly apply the Rome II criterior. However, the Korean Society of Gastrointestinal motility had decided to promote the use of the international standardized Rome II criterior in order for Korea to be comparable to the clinical studies from the other nations. There should be more extensive studies to differentiate the definitions of symptom descriptions from Korea to foreign countries. The extended researches concerning the pathophysiology are highly recommended which in return, would help enabling the reliable treatment techniques for this disease.
