비디오 프로그램을 통한 정보제공이 위암조기발견에 대한 지식과 태도에 미치는 효과 (The Effect of Audiovisual Information with Videotape on Knowledge and Attitude of Early Detection of Gastric Carcinoma)

  • Gastric Carcinoma is very plentiful and significant disease in Korean community. The reason is that Gastric Carcinoma is not a simply finding disease with unclear symptoms for early detection. Early detection and early medical treatment of a cancer patient is an important instrument of reducing the mortility rate. The purpose of this study is to identify the effect of audiovisual information with videotape infulencing on knowledge and attitude of early detection of gastric carcinoma. The research design was a non-equivalent control group, pre-post test. The subjects of this study were 52 members of D-taxi drivers which was located in Pusan. The subjects were twenty six experimental group and twenty six control group. The experimental group was collected from January 15, 1999 to January 26, 1999. The control group was collected from the research was 28 minutes video program which was turning out the form of documentary on the based practical experience. And also the experimental tool was used measurung instrument which measured a dependent, variable throughout the consulatation of an percentage, average, standard deviation, ${\chi}^2-test$ and t-test using spss/pc program. The result of this research were as follows : 1)" The experimental group which was offered the video program, should be higher in knowledge of early detection of the gastric carcinoma than the control group" was supported. (t= -7.754, p=.000) 2) "The experimental group which was offered the video program, should be higher in attitude of the early detection of the gastric carcinoma than the control group" was supported. (t=-4.321, p=.000) Therefore, in conclusion, this study that the audiovisual information with videotape influencing on knowledge and attitude of early detection of gastric carcionma was very representational experience throughout the video of documentary form on the based practical experience was much effected the change of the knowledge and the attitude regarding to the early detection of the gastric carcinoma was verified.

Effect of Carrot Intake in the Prevention of Gastric Cancer: A Meta-Analysis

  • Purpose: Gastric cancer is the third leading cause of cancer-related mortality, with the incidence and mortality being higher in men than in women. Various studies have shown that eating carrots may play a major role in the prevention of gastric cancer. We conducted a meta-analysis to determine the relationship between carrot consumption and gastric cancer. Materials and Methods: We searched multiple databases including PubMed, Cochrane Library, Scopus, ScienceDirect, and Persian databases like Scientific Information Database (SID) and IranMedx. The following search terms were used: stomach or gastric, neoplasm or cancer, carcinoma or tumor, and carrot. Statistical analyses were performed using Comprehensive Meta Analysis/2.0 software. Results: We retrieved 81 articles by searching the databases. After considering the inclusion and exclusion criteria, 5 articles were included in this study. The odds ratio (OR) obtained by fixed effects model showed that a 26% reduction in the risk of gastric cancer has been associated with the consumption of carrots) OR=0.74; 95% confidence interval=0.68~0.81; P<0.0001). According to funnel graph, the results showed that the possibility of a publication bias does not exist in this study. Conclusions: The findings of this study showed an inverse relationship between the consumption of carrots and the risk of gastric cancer.

The Impact of Preoperative Low Body Mass Index on Postoperative Complications and Long-term Survival Outcomes in Gastric Cancer Patients

  • Purpose: The aim of this study was to investigate the impact of preoperative low body mass index (BMI) on both the short- and long-term outcomes in patients with gastric cancer. Materials and Methods: A total of 510 patients with gastric cancer were divided into the following 3 groups: low BMI group (${\leq}18.5kg/m^2$, n=51), normal BMI group ($18.6-24.9kg/m^2$, n=308), and high BMI group (${\geq}25.0kg/m^2$, n=151). Results: There were significantly more stage III/IV patients in the low BMI group than in the other groups (P=0.001). Severe postoperative complications were more frequent (P=0.010) and the survival was worse (P<0.001) in the low BMI group. The subgroup analysis indicated that survival was worse in the low BMI group of the stage I/II subgroup (P=0.008). The severe postoperative complication rate was higher in the low BMI group of the stage III/IV subgroup (P=0.001), although the recurrence rate and survival did not differ in the stage III/IV subgroup among all the BMI groups. Low BMI was an independent poor prognostic factor in the stage I/II subgroup (disease-free survival: hazard ratio [HR], 13.521; 95% confidence interval [CI], 1.186-154.197; P=0.036 and overall survival: HR, 5.130; 95% CI, 1.644-16.010; P=0.005), whereas low BMI was an independent risk factor for severe postoperative complications in the stage III/IV subgroup (HR, 17.158; 95% CI, 1.383-212.940; P=0.027). Conclusions: Preoperative low BMI in patients with gastric cancer adversely affects survival among those with stage I/II disease and increases the severe postoperative complication rate among those with stage III/IV disease.

  • 무(한국 품종 및 일본 품종)를 사용하여 H. pylori에 의한 위세포독성 및 IL-8생성에 미치는 영향을 살펴보았다. 그 결과 무는 H. pylori에 의한 위세포독성을 농도의존적으로$(2\~10 mg/ml)$ 억제하였으며 또한 위암세포인 KATO III에서 분비되는 IL-8의 생성을 억제시킴$(5\~10mg/ml)$을 알았다. 이러한 사실은 무가 H. pylori감염에 의한 위점막세포의 손상을 방지할 수 있음을 나타낸다.

Phlegmonous Gastritis with Early Gastric Cancer

  • Phlegmonous gastritis is a rare and rapidly progressive bacterial infection of the stomach wall, with a high mortality rate. Antibiotics with or without surgical treatment are required for treatment. We present a case in which phlegmonous gastritis occurred during the diagnostic evaluation of early gastric cancer. The patient showed improvement after antibiotic treatment, but attempted endoscopic submucosal dissection failed because of submucosal pus. We immediately applied argon plasma coagulation since surgical resection was also considered a high-risk procedure because of the submucosal pus and multiple comorbidities. However, there was local recurrence two years later, and the patient underwent subtotal gastrectomy with lymph node dissection. Considering the risk of incomplete treatment immediately after recovery from phlegmonous gastritis and that recurrent disease can be more difficult to manage, delaying treatment and evaluation until after complete recovery of PG might be a better option in this particular clinical situation.

Outcome of Gastric Cancer Surgery in Elderly Patients

  • Purpose: Owing to increased life expectancy, the number of elderly patients with gastric cancer has increased. This study aimed to identify the outcomes of gastric cancer patients aged 80 years or older through comparison of their clinicopathological characteristics, surgical outcomes, and oncologic outcomes. Materials and Methods: Between January 2006 and December 2013, the records of 478 patients who underwent surgery for gastric cancer were retrospectively evaluated. Patients were divided into two groups: patients <80 years old (n=446) and patients ${\geq}80$ years old (n=32). Results: There were no significant differences in sex, body mass index, length of hospital stay, duration of surgery, depth of invasion, nodal metastasis, histologic type, or tumor size between the two groups. However, significant differences were found for the American Society of Anesthesiologist score and the serum albumin level between the two groups. Postoperative morbidity, mortality, disease-free survival, and recurrence rate did not differ between curatively resected patients in the two groups. Conclusions: In elderly patients with gastric cancer, active treatment including radical gastrectomy is necessary.

위선암에서 Survivin과 KAI-1의 발현에 대한 연구 (Expression of Survivin and KAI-1 in Gastric Adenocarcinomas)

  • Purpose: The aim of this study was to investigate the impact of survivin expression and the decrease or loss of KAI-1 on the clinical stage and the survival rate in gastric adenocarcinomas. Materials and Methods: Expressions of survivin and KAI-1 were immunohistochemically determined in 40 cases of gastric adenocarcinomas. The survivin and KAI-1 expressions were also analyzed by using western blots in 14 cases among them. Results: Resected gastric cancer specimens from 40 patients (intestinal type: 15 cases and diffuse type: 25 cases) were evaluated immunohistochemically. Survivin protein expressions were significantly higher in diffuse types (P=0.03) and in advanced clinical stages (UICC TNM II and III, P=0.02). In contrast, a decrease or loss of KAI-1 expression had no statistically significant correlation with the Lauren classification or the clinical stage. Survivin protein positivity was associated with an unfavorable prognosis. Decrease or loss of KAI-1 was associated with a shorter disease free survivial rate (P < 0.01). The western blot data (n=14) indicated that neither survivin protein over-expression nor KAI-1 down-expression had an significant correlation with the Lauren classification or the clinical stage. Conclusion: In gastric carcinomas, survivin over-expression and decrease or loss of KAI-1 were associated with unfavorable prognosis, being independent prognostic factors along with the clinical stage and the disease free survival rate.

Clinical Significance of the Pattern of Lymph Node Metastasis Depending on the Location of Gastric Cancer

  • Purpose: When performing a laparoscopic assisted gastrectomy, a function-preserving gastrectomy is performed depending on the location of the primary gastric cancer. This study examined the incidence of lymph node metastasis by the lymph node station number by tumor location to determine the optimal extent of the lymph node dissection. Materials and Methods: The subjects consisted of 1,510 patients diagnosed with gastric cancer who underwent a gastrectomy between 1996 and 2005. The patients were divided into three groups: upper, middle and lower third, depending on the location of the primary tumor. The lymph node metastasis patterns were analyzed in the total and early gastric cancer patients. Results: In all patients, lymph node station numbers 1, 2, 3, 7, 10 and 11 metastases were dominant in the cancer originating in the upper third, whereas station numbers 4, 5, 6 and 8 were dominant in the lower third. In early gastric cancer patients, the station number of lymph nodes with a metastasis did not show a significant difference in stage pT1a disease. On the other hand, a metastasis in lymph node station number 6 was dominant in stage pT1b disease that originated in the lower third of the stomach. Conclusions: When performing a laparoscopic-assisted gastrectomy for early gastric cancer, a limited lymphadenectomy is considered adequate during a function-preserving gastrectomy in mucosal (T1a) cancer. On the other hand, for submucosal (T1b) cancer, a number 6 node dissection should be performed when performing a pylorus preserving gastrectomy.

정전가미이진탕(正傳加味二陳湯)이 흰쥐의 위배출능(胃排出能)에 미치는 영향(影響) (Effect of Jungjungamiyjintang(JGT) on Gastric Emptying of Spregue-Dawley rat)

  • Background & Objective : The herbal medicine JGT has been used for the treatment of functional dyspepsia, generally categorized as a gastric disease. However, its effect and mechanism are not yet well known. Therefore, the effects of JGT on gastric emptying in rats was investigated to know its effectiveness and mechanism. Methods : Gastric emptying effect was measured by the number of glass beads expelled from the stomach within an hour after glass beads and test drugs were administered. In another series of experiments to evaluate the mechanisms of JGT under delayed conditions, the rats were treated with atropine sulfate Results : Intragastric administration of JGT significantly increased gastric emptying of glass beads Under the gastric emptying delayed with atropine sulfate, JGT insignificantly increased gastric emptying of glass beads. Conclusions : These suggest that JGT has a significant effect to stimulate gastric emptying. Results are indicative of JGT as an especially effective remedy for dysmotility-like functional dyspepsia with impaired reservoir function such as gastric adaptive relaxation.

Liquid Biopsy: An Emerging Diagnostic, Prognostic, and Predictive Tool in Gastric Cancer

  • Liquid biopsy, a minimally invasive procedure that causes minimal pain and complication risks to patients, has been extensively studied for cancer diagnosis and treatment. Moreover, it facilitates comprehensive quantification and serial assessment of the whole-body tumor burden. Several biosources obtained through liquid biopsy have been studied as important biomarkers for establishing early diagnosis, monitoring minimal residual disease, and predicting the prognosis and response to treatment in patients with cancer. Although the clinical application of liquid biopsy in gastric cancer is not as robust as that in other cancers, biomarker studies using liquid biopsy are being actively conducted in patients with gastric cancer. Herein, we aimed to review the role of various biosources that can be obtained from patients with gastric cancer through liquid biopsies, such as blood, saliva, gastric juice, urine, stool, peritoneal lavage fluid, and ascites, by dividing them into cellular and acellular components. In addition, we reviewed previous studies on the diagnostic, prognostic, and predictive biomarkers for gastric cancer using liquid biopsy and discussed the limitations of liquid biopsy and the challenges to overcome these limitations in patients with gastric cancer.
