A Study on the Diversity of Shanghan(傷寒) Concept in Gangpyeong-Sanghanlun(康平傷寒論) (『강평상한론(康平傷寒論)』 내 '상한(傷寒)' 개념의 다양성에 대한 고찰)

  • Objectives : Usually medical terminology of oriental mecidine has a multiple meaning. But concept of Shanghan(傷寒) should be simple, because Shanghanlun(傷寒論) is a clinical guideline book. So I researched to suggest many concept of Shanghan, which are suitable for each chapter of Shanghanlun. Methods : I enumerated provisions including Shnaghan from the original texts of Gangpyeong-Shanghanlun(康平傷寒論). And I translated and reviewed them. Results : 1. Shanghan of Preface(序文) means a disease of high fatality. 2. Shanghan of Shanghanrye(傷寒例) means diseases due to physical damage of cold weather. 3. Shanghan of Diagnosis of Daeyang Disease(辨大陽病) - Neck stiffness(痙), Dampness(濕), Sun stroke(暍) means certain disease names accompanying fever, chill. 4. Shanghan used in Diagnosis of Diseases is a premise of many provisions of Shanghanlun. And Shanghan is made up of finished fever, expected fever, chill, body pain, loss of appetite, image of tension. Conclusions : We can use a appropriate translation on Shanghan of each chapter of Gangpyeong-Sanghanlun. Especially Shanghan used in "Diagnosis of Diseases" should have more accurate meaning.

A Conceptual Study of "Mai (脉)" in Gangpyeong-Shanghanlun Based on Comparative case analysis and Paleography (『강평상한론(康平傷寒論)』 맥(脉)에 관한 고찰: 임상 증례 분석 및 고문자학적 해석을 통하여)

  • Objective : The objective of this study was to explore the meaning of Mai (脉) in the fifteen letters provisions of Gangpyeong-Shanghanlun through the comparative analysis of 10 cases and paleography. Methods : We have collected 10 cases that have been diagnosed and treated by Gangpyeong-Shanghanlun Mai (脉) provision. And then, comparative analysis has been done among those 10 cases. Based on result of case analysis, paleographic meaning of each Mai (脉) have been studied to redefine Gangpyeong-Shanghanlun Mai (脉)'s meaning. Results : After administration of Mai (脉) provision's herbal medicine, all 10 patients recovered their mobility problem and pain. From this result, we could fomulate the hypothesis that Mai (脉) in Gangpyeong-Shanghanlun could mean mobility problems. After through further paleography studies, we could elaborate each Mai (脉)'s clinical meaning and related diseases. Conclusions : Mai (脉) in Gangpyeong-Shanghanlun could mean mobility problems caused by various diseases, which include musculoskeletal pain, neuropathy, syncope, fatigue, developmental disability and major depressive disorder.

Diagnostic definition of Chest bind(結胸) in GangPyeongShanghanlun(康平傷寒論) 15 letters provision. (『강평상한론(康平傷寒論)』 15자행(字行) '결흉(結胸)'의 진단적 의미 고찰)

  • Objectives : Purpose of this study is to suggest a diagnostic definition of Chest bind(結胸) in GangpyeongShnghanlun 15 letters provision. Methods : Using The Etymology Dictionary of Chinese Characters, we interpreted the meaning of Chest bind(結胸) and symptoms on 134th provision of Shanghanlun. Results : The meaning of Chest bind is endurance of worries. Symptoms on 134th provision can be interpreted as follows. Worrywart person whose behavior is dilatory, who suffer from an inferiority complex, feel pressure on chest, feel heavy with worries, unstable, talkative, suffer from a feeling of anger, think himself as fallen into a trap, feel always nervous and is stubborn can be in a situation of Chest bind. Conclusions : Chest bind is one of the Names of disease in Shanghanlun. The Name of disease has its own meaning that can be used as diagnostic evidence. Further studies are needed to verify a hypothesis of diagnostic definition of Chest bind.

  • Objective : We Explored the meaning of the 厥逆 in Gangpyeong-Shanghanlun through two cases. Methods : We analyzed the original form of 厥逆 and the contexts in Shanghanlun. At the same time, we analyzed 2 clinical cases treated with 大靑龍湯 (Daecheongryong-tang) according to the newly deduced definition of 厥逆. Results : 厥逆 can be interpreted as 'Being indulged and indigestive'. We analyzed the cases and found that both cases were related to eating disorder even though their chief complaints were different. We also found that 大靑龍湯 was effective for both bulimia nervosa and anorexia nervosa. Conclusions : 厥逆 is symptoms of reflux in human body such as, gastroesophageal reflux disease, as a result of concentration or indulgence. We analyzed the two cases with DSM-5 and found that the two cases also can be diagnosed with eating disorder along with the provision (38條, 大靑龍湯). We found out the possibility of correlation between eating disorder and 38條, however, more advanced clinical studies are needed in the future.

A Conceptual Study of "Pyo (表)" in Gangpyeong-Shanghanlun Based on Two Cases of Bipolar Disorder Improved with Mahwang-tang (『강평상한론(康平傷寒論)』 '표(表)'에 대한 고찰 : 마황탕(麻黃湯)으로 호전된 양극성 장애 2례를 바탕으로)

  • Objective : We explored the meaning of "Pyo (表)" in Gangpyeong-Shanghanlun through two cases. Methods : We analyzed the original form of 表 and its context in Shanghanlun. We also analyzed 2 clinical cases of bipolar disorder treated with Mahwang-tang (麻黃湯) according to the newly deduced definition of 表. Results : 表 can be interpreted as "pursuing something high-class seen from the outside." Both cases had various psychological symptoms including insomnia, mood swings, anxiety, and depression. We determined that the diseases in both cases occurred when 表 had not been achieved. We also found that 麻黃湯 can improve psychological status. Conclusions : 表 is different from "surface," which is the most widely used definition. After diagnosing these two patients with bipolar disorder using the Diagnostic and Statistical Manual of Mental Disorders (5th edition), we found that symptoms could be improved by administration of 麻黃湯 (46條). Although its potential for neuropsychiatric diagnosis has been suggested through etymological interpretation of 表 and analysis of two cases, more advanced clinical studies are needed in the future.

A Study on Greater yin Disease in Gangpyeong-Shanghanlun by Analyzing Four Mental Illness Cases (정신과질환 4례를 통한 강평본(康平本) 『상한론(傷寒論)』의 태음병(太陰病) 진단 고찰)

  • Objectives : This study aimed to suggest distinct signs of Greater yin disease (tàiyīnbìng, 太陰病) by researching the etymology of three terms: zi-tong (自痛), jie-ying (結硬), and yin-er (因爾), which are exclusive found only in Greater yin disease (太陰病) in Gangpyeong-shanghanlun, but not in other categories. Methods : We studied the etymologies of the three terms that are unique in Greater yin disease, then found some pathological signs that are related with them through the four mental illness cases, which include somatization, obsessive-compulsive behavior, delusion, and panic disorder. Results : Based on the definitions of each term, we diagnosed the four patients who had mental illnesses as having Greater yin disease, and we observed meaningful improvements after administering herbal medication. After Gyejigajakyak-tang administration, the Insomnia Severity Index (ISI) score decreased from 18 to 7 and the Beck's Anxiety Index (BAI) score decreased from 36 to 18 in the first case, the ISI score decreased from 27 to 16 and the BAI score decreased from 50 to 33 in the second case, and the ISI score decreased from 23 to 4 and the BAI score decreased from 34 to 5 in the third case. In the last case, the ISI and BAI scores were 16 and 22, respectively, at the first visit, and it was found that the scores had changed to 6 and 22, respectively, at the last visit. Conclusions : We found that the unique terms in Greater yin disease, including zi-tong (自痛), jie-ying (結硬), and yin-er (因爾), can be interpreted as groundless arbitrary assumption, resting strong tension, and psychological projection based on the etymological hypothesis. Therefore, we suggest these as specific signs of Greater yin disease.

The Fundamental Meaning of Mai and Shanghanlun Provisions Based on Paleography (『상한론(傷寒論)』 맥(脈) 및 기타 조문(條文)의 고문자학적(古文字學的) 해석(解釋)을 통한 본원적(本源的) 의미(意味) 고찰(考察))

  • Objectives : The purpose of this study is to show the importance of Shanghanlun(傷寒論) paleography. Shanghanlun(傷寒論) must be thoroughly reinterpreted through graphonomy evidence. Methods : The object of this study is Gangpyeong-Shanghanlun(康平傷寒論), which is known as the oldest edition of Shanghanlun(傷寒論). This study compares the paleographic meaning of Shanghanlun(傷寒論), Mai(脈), Shanghanlun provisions 96 and 103 to existing interpretations. Results : Mai(脈) doesn't mean 'Radial pulse(Chon.Gwan.Cheok)' in Shanghanlun(傷寒論), so the meaning of Mai(脈) should be redefined. Mai(脈) means 'Continuous movement of human body' based on Chinese paleography. Also it's being verified as reasonable hypothesis in recent clinical researches. In addition, Shanghanlun(傷寒論) paleography makes it possible to grasp the true intention of Zhang Zhongjing(張仲景). Conclusions : Shanghanlun(傷寒論) paleography has a limit that it cannot be perfectly applied to clinical diagnosis. Also, it may not represent the whole meaning of Shanghanlun (傷寒論). Therefore, the value of Shanghanlun(傷寒論) reinterpretation should be recognized through this study, and we should continuously find out future values from Shanghanlun(傷寒論).
