Study of Clinical Research Acupuncture Treatment on Post-stroke Depression (뇌졸중후 우울증 침치료의 임상연구에 대한 고찰)

  • The aim of this study is to collect and analyze clinical studies on acupuncture therapies of Post-stroke depression, and to suggest a method on study about acupuncture therapies. In this thesis, electronic searches were performed. Also, 6 major journals associated with oriental medicine in Korea were searched manually. Through searching, 27 studies were selected and analyzed. There were 24 controlled studies and 3 non-controlled studies. Treatment methods were acupuncture, electronic-acupuncture, eye-acupuncture, abdominal-acupuncture and pharmaco-puncture. The percentages of these treatment method were 59.2%, 29.6%, 3.7%, 3.7% and 3.7%, respectively. Acupuncture points such as GV20, PC6, ST36, SP6, LV3, GV24, GV26, LI14, LI11, H7, ST40, GB20, GB34 and LI15 were used. In comparison of results before and after acupuncture treatment, there were statistically significant improvement in 26 studies. Based on the results from above, the studies on acupuncture for Post-stroke depression needs to be standardized and more appropriate high-quality study models should be introduced.

Identification of Developmental Related Genes in the Lab Animal (실험동물 발생 유전자의 확인)

  • The present study was conducted to gain insights into oocyte maturation and early embryo development, SELDI-TOF-MS was used to find the protein candidates that are specifically or prominently expressed in mouse oocytes at the in vitro matured metaphase II (MIl) and germinal vesicle (GV) stages. By using selected CM10 chip, found 16 candidates which were up-regulated in GV stage oocytes compared with in MIl stage oocytes, molecular weight are 8180 (2 candidates), 10226 (5 candidates), 15767 (5 candidates) and 16770 (4 candidates) Da respectively. And 29 candidates were higher in MIl than in GV stage oocytes, molecular weight are 10832 (3 candidates), 17744(8 candidates), 20122 (3 candidates), 22131 (3 candidates), 24857 (7 candidates) and 33507 (5 candidates) Da, respectively. All (45) candidate (0.2 and 1.0 % error tolerances) were performed real time RT-PCR analysis and further selected 13 more potential candidates.

Engineered Recombinant PON1-OPH Fusion Hybrids: Potentially Effective Catalytic Bioscavengers against Organophosphorus Nerve Agent Analogs

  • Organophosphorus nerve agents (OPNAs), including both G- and V-type nerve agents such as sarin, soman, tabun and VX, are extremely neurotoxic organophosphorus compounds. Catalytic bioscavengers capable of hydrolyzing OPNAs are under development because of the low protective effects and adverse side effects of chemical antidotes to OPNA poisoning. However, these bioscavengers have certain limitations for practical application, including low catalytic activity and narrow specificity. In this study, we generated a fusion-hybrid form of engineered recombinant human paraoxonase 1 (rePON1) and bacterial organophosphorus hydrolase (OPH), referred to as GV-hybrids, using a flexible linker to develop more promising catalytic bioscavengers against a broad range of OPNAs. These GV-hybrids were able to synergistically hydrolyze both G-type OPNA analogs (paraoxon: 1.7 ~ 193.7-fold, p-nitrophenyl diphenyl phosphate (PNPDPP): 2.3 ~ 33.0-fold and diisopropyl fluorophosphates (DFP): 1.4 ~ 22.8-fold) and V-type OPNA analogs (demeton-S-methyl (DSM): 1.9 ~ 34.6-fold and malathion: 1.1 ~ 4.2-fold above) better than their individual enzyme forms. Among the GV-hybrid clones, the GV7 clone showed remarkable improvements in the catalytic activity toward both G-type OPNA analogs (kcat/Km (106 M-1 min-1): 59.8 ± 0.06 (paraoxon), 5.2 ± 0.02 (PNPDPP) and 47.0 ± 6.0 (DFP)) and V-type OPNA analogs (kcat/Km (M-1 min-1): 504.3 ± 48.5 (DSM) and 1324.0 ± 47.5 (malathion)). In conclusion, we developed GV-hybrid forms of rePON1 and bacterial OPH mutants as effective and suitable catalytic bioscavengers to hydrolyze a broad range of OPNA analogs.

Effects of Adenosine, Guanosine and Azaserine on Maturation of Mouse Oocytes In Vitro (생쥐 미성숙난자의 체외성숙에 미치는 Adenosine, Guanosine 및 Azaserine의 영향)

  • Normal maturation of the mammalian oocytes is prerequisite for the fertilization and the early embryonic development. We have been tested the effects of purine and its de novo synthetic inhibitor, azaserine(Aza) on the maturation of germinal vesicle(GV) and germinal vesicle breakdown(GVBD) mouse oocytes. Denude-immature oocytes were cultivated in the media containing adenosine, guanosine, and/or azaserine, and checked the matruation stage by monitoring the prominent morphological changes. In GV stage oocytes, GV was arrested temporarily by the adenosine(1.0%) and protractedly by the guanosine(65.9%, P<0.001). The regression was increased significantly at the adenosine(90%, P<0.001) but decreased at the guanosine(1.6%, P<0.05). Inhibiting the de novo synthesis of purine, nuclear maturation rate was increase(90.4% : 96.7%), but GV arrest was significantly increased by cotreatment with guanosine(P<0.001). Polar body extraction significantly was increased at the Aza(P<0.05), but not in others. In GVBD oocytes, adenosine itself did not affect GVBD arrest. Guanosine, on the other hand, elevated GVBD arrest rate(P<0.001), but co-treated with Aza, decreased GVBD arrest(P<0.001). Aza increased GVBD arrest rate(20.2%, P<0.05) compared with control. From those results, we know that guanosine shows more prominent effect on the inhibition of nuclear maturation at the GV stage, and of the 1st polar body extrusion at the GVBD stage. Adenosine showed the cytoplasmic toxicity at GV stage oocyte. Our data speculate that cytoplasmic cAMP level is auto-regulated by endogenous adenylate cyclase while GVBD is inhibited by guanosine, since purine toxicity is not observed in the GVBD stage. And it is showed that purine metabolism is concerned with nuclear maturation, that the amounts of purine metabolism is not even during the oocyte maturation.

Effects of prematuration culture with a phosphodiesterase-3 inhibitor on oocyte morphology and embryo quality in in vitro maturation

  • Objective: The study assessed the developmental potential of germinal vesicle (GV) oocytes subjected to in vitro maturation (IVM) after prematuration culture with cilostamide (a phosphodiesterase-3 inhibitor) and the impact of cilostamide exposure on the morphology of meiosis II (MII) oocytes and subsequent embryo quality. Methods: In total, 994 oocytes were collected from 63 patients. Among 307 GV oocytes, 140 oocytes were selected for the experimental group and 130 oocytes for the control group. The denuded GV-stage oocytes were cultured for 6 hours with cilostamide in the experimental group and without cilostamide in the control group. After 6 hours, the oocytes in the experimental group were washed and transferred to fresh IVM medium. The maturational status of the oocytes in both groups was examined at 26, 36, and 48 hours. Fertilization was assessed at 18 hours post-intracytoplasmic sperm injection. Embryo quality was assessed on days 3 and 5. Results: In total, 92.1% of the oocytes remained in the GV stage, while 6.4% converted to the MI stage (p<0.01) after cilostamide exposure. In both groups, more MII oocytes were observed at 36 hours (25.8% vs. 21.5%) than at 26 hours (10.8% vs. 14.6%) and 48 hours (13% vs. 7.9%) (p>0.05). With the advent of cilostamide, blastocyst quality was better in the experimental group than in the control group (p<0.05). Conclusion: Cilostamide effectively blocked nuclear maturation and promoted cytoplasmic growth. Prematuration culture with cilostamide enabled synchronization between cytoplasmic and nuclear maturity, resulting in better blastocyst outcomes.

Impact of Additional Therapeutic Exercises on Functional Performance of the Lower Extremities in Stroke Inpatients Within 3 to 6 Months After Stroke Onset (발병 3~6개월의 뇌졸중 환자에서 부가적 운동치료가 하지의 기능수행능력에 미치는 영향)

  • The purposes of this study were to determine the effects of rehabilitation programs on functional performance of the lower extremities and whether additional therapeutic exercise with routine rehabilitative therapy improves functional performance more than just routine rehabilitative therapy by it self for inpatients who have suffered a stroke with below moderate severity within 3 to 6 months after the onset of the stroke. Fifty-eight subjects were divided into two groups. Group I was given routine rehabilitative therapy and group II was given additional therapeutic exercise along with the routine rehabilitative therapy. Each group received 6 weeks of rehabilitation. The timed get-up and go test (TUG), the Fugl-Meyer score (FMS), functional independence measure (FIM), functional reach (FR), gait velocity (GV), and the strength of knee extensor and flexor were selected to measure effect of rehabilitation programs. The main results were measured and analysed at baseline, 3 weeks, and 6 weeks after the start of the rehabilitation programs. The results revealed that all of functional performance of the lower extremities in 3 weeks after the start of the rehabilitation programs were significantly improved compared with before the rehabilitation programs in both groups. In 6 weeks, TUG, FIM, FR, GV, and the strength of knee extensor in group I, TUG, FMS, FIM, FR, GV, and the strength of knee extensor and flexor in group II were significantly improved compared with the results after 3 weeks. At 3 weeks after rehabilitation programs, group II made significantly greater gains in TUG, FR, GV, and the strength of the knee flexor compared to the group I. At 6 weeks, group II made significantly greater gains in TUG, FR, GV, and the strength of the knee extensor and flexor compared to the group I. In conclusion, rehabilitation programs for stroke patients within 3 to 6 months after stroke onset significantly contributed to improve functional performance of the lower extremities. It is desirable for improvement of functional performance of the lower extremities to perform additional exercise with routine rehabilitative therapy.

Treatment of Canine Cervical and Lumbar Disc Disease by Injection-Acupuncture (개 경부 및 요부 디스크의 수침치료)

  • Two cases of canine intervertebral disc disease (IVDD) were treated twice a week with injection-acupuncture (injection-AP) using 0.1 ml/acupoint. In case 1, a paralysed dog with cervical intervertebral disc disease (C3-C4), was submitted to injection-AP with dexamethasone (1 mg/ml) at session 1-2 and thiamine (25 mg/ml) at session 3-5. Injected acupoints included GV-16, GB-20, BL-10, LU-7, LI-4 and SI-6. The acupoints GV-6, GV-20 and SP-6 were added at session 2-5. Trigger point (TP) therapy with 0.2 ml of 2% lidocaine was used in session 3-5 at TP in the infraspinatus and triceps muscles. Treatment was stopped when paralysis disappeared after 5 injection-AP treatments; there were no recurrent symptoms in the follow-up period of 5 months. In case 2, an ataxic dog with lumbar IVDD (L1-L2), was submitted to injection-AP with dexamethasone at session 1 and 2 and thiamine at session 3-4. Injected acupoints included GV-6 as the main point, ST-36, GB-30, ST-40, GB-34, ST-41 and BL-40. TP therapy with 0.2 ml of 2% lidocaine was used at TP in the ileocostorum lumborum and quadriceps muscles. Treatment was stopped when ataxia disappeared after 4 treatments; there were no recurrent symptoms In the fallow-up period of 5 months. Injection-AP using dexamethasone and thiamine, combined with TP therapy using lidocaine, effectively alleviate the symptoms of canine cervical and lumbar IVDD.

The Development of Acupuncture- Moxibustion Device for 4 points around GV20(Baihui) (백회와 사신총 혈위에 적합한 침구 소자의 개발)

  • Objectives This paper focused on developing the acupuncture-moxibustion device for head. Disadvantages of the existing devices for head moxibustion are its hume and inconvenience in operating because of the hair. Methods The proposed acupuncture-moxibustion device for GV20(Baihui) is 10mm long as needle body and 6mm long as needle root whose feature are 1~2 turns spiral coil. The spiral coil contains the moxa in order not to drop the moxa while operating Results The direct temperature on the burning moxa were measured by TK-305 infrared thermometer. But the direct temperature on the needle of acupuncture-moxibustion device were not able to measure by TK-305 infrared thermometer. By definition equation, the indirect temperatures on the needle of acupuncture-moxibustion device were able to be calculated. Conclusion A very safe, small and cheap acupuncture-moxibustion device for GV20(Baihui) of head is proposed. It will be useful to generate the heat shock protein and to operate in preventing and curing the Alzheimer's disease.

Safe Needling Depth of Pungbu(GV16) with MRI-a Retrospective Study (MRI를 통한 풍부혈(GV16)의 안전 자침 깊이에 대한 연구)

  • Objectives : The purpose of this study is to determine the safe needling depth of Pungbu($GV_{16}$) retrospectively by using magnetic resonance imaging (MRI). Methods : We chose 114 Brain or C-spine MRI images from the Sang-Ji hospital picture archiving communication system. We measured the shortest distance from skin to cerebral dura mater passing by posterior edge of the foramen magnum on the sagittal view for the depth of Pungbu. We analyzed the differences between male and female measured values by using a student t-test. Results : The average depth of male insertion was $49.71{\pm}6.32mm$ and the shortest depth of insertion was 36.29 mm. The average depth of female insertion was $39.84{\pm}5.25mm$ and the shortest depth of insertion was 30.02 mm. The results showed a significant difference according to gender (p=0.00). Conclusions : The depth of male insertion is deeper than that of female, and the safe needling depth in the case of males is 36.29-67.35 mm, while the safe needling depth in the of females is 30.02-52.18 mm.

Effects of Wheel Profile on KTX Dynamic Characteristics (차륜답면 형상변화에 따른 KTX의 동특성)

  • The running safety of a railway vehicle depends on the design parameters and contact condition between wheel and rail. In this study, the effect of the conicity of wheel tread is analyzed using ADAMS/RAIL software on running situation. Modal analysis shows in 0.6 Hz natural frequency of lateral mode in fully arranged the KTX cars. The excessive vibration of the tail cars occurs in the 17th car as the speed and the stiffness of the secondary suspension increases, and especially for 1/40 conicity of the GV40 wheel. Also, the analysis shows that combination of wheel profile, GV40 for power cars and XP55 for passenger cars can reduce the lateral vibration of the tail cars.
