Clinical Study of Group B β-Hemolytic Streptococcal Meningitis (B군 연쇄상구균 뇌막염에 대한 임상적 고찰)

  • Purpose : Bacterial meningitis is a serious disease, especially in the neonatal period, and it carries a significant degree of mortality and morbidity. Group B streptococcus(GBS) is a common cause of neonatal bacterial meningitis. The purpose of this study was to evaluate the clinical manifestations, treatment results and complications of GBS meningitis. Methods : We analyzed 29 cases retrospectively who had been admitted to the pediatric ward or NICU in Asan Medical Center from May 1990 to January 2002. They had proven GBS in culture or latex agglutination test in CSF. Results : The male to female ratio was 1 : 1.9. There were two cases of early onset type and 27 cases of late onset type. All cases had normal birth weight with full term at delivery. The perinatal predisposing factors were premature rupture of membrane(two cases), and maternal colonization(two cases). The most common presenting symptoms were fever and irritability. Associated diseases were GBS sepsis(21 cases). There was relatively high sensitivity to penicillin derivatives. There were abnormal brain CT or MRI findings in 16 cases(64%), such as infarction, encephalomalatic change, effusion, hydrocephalus, hemorrhage and abscess. The intensive care unit admission rate and the incidence of DIC were higher in the group with complications. Two cases were discharged against advice. Conclusion : We recommend early detection and active treatment in Group B streptococcal meningitis to improve the prognosis.

  • Objectives: The aim of this study was to analyze case reports on and to study Korean medicine treatments of facial palsy. Methods: Studies published after 2000 were selected by searching for the terms "facial palsy" and "Guanwasa" in four databases and case reports using Korean medicine moderating variables were extracted by two individual researchers. Results: The House Brackmann (H-B) Scale (23 times, 65.7%), and Yanagihara Grading Scale (Y-system) (22 times, 62.8%) were used to assess facial palsy. All case reports used acupuncture, while 19 case reports (51.3%) used electro-acupuncture, and 8 case reports (21.6%) used pharmacopuncture. Main meridian points used were ST6, ST4, GB14, TE23, LI20, BL2. Thirty-two case reports used herbal medicine (86.4%) and both Ligigeopoong-san, Bojunglkgi-tang were used frequently. Conclusion: To treat facial palsy, acupuncture was mainly used in conjunction with electro-acupuncture and pharmacopuncture. Most case reports also used herbal medicine.

  • The efficienty of ATM is based on the statical multiplexing of fixed-length packets, which are called cells. The most important technical point for realizing ATM switching network is an arrangement of the buffers and switches. Current most ATM switching networks are being achieved by using the switching modules based on the unit switch of $8{\times}8$ 150Mb/s or $16{\times}16$ 150Mb/s, the unit switch of $32{\times}32$150Mb/s for a large scale system is under study in many countries. In this paper, we proposed a new $32{\times}32$(4.9Gb/s throughput) ATM switch using Shared buffer memory switch which provides superior traffic characteristics in the cell loss, delay and throughput performance and easy LSI(Large Scale Integrated circuit). We analytically estimated and simulated by computer the buffer size into it. We also proposed the configuration of the large capacity ATM switching network($M{\times}M$.M>1,000) consisting of multistage to improve the link speed by non-blocking.

Current Status of Intervention Studies on Acupuncture for Parkinson's Disease

  • Objectives : The purpose of this study was to investigate the effect of acupuncture treatment (AT) in the tendency of increase of the need for AT for the treatment of Parkinson's disease (PD) worldwide and to investigate the advancements in AT research in Korea and the future directions of research on this topic. Methods : Until May 2017, the PubMed, Scopus, Medline, and four Korean databases were searched. The searched keywords were "Parkinson's disease", "Acupuncture", and "Intervention study". The intervention groups from all screened original studies were analyzed and the methods used to determine the effect of AT on PD were examined. Results : A total of 17 studies were grouped by country on the basis of the first author's position, of which 10 studies were conducted in China, four in the United States, two in Korea, and one in Brazil. The most common type of intervention was electroacupuncture (nine studies), followed by AT (six studies), and a combination of AT and bee venom AT (two studies). The most frequently used acupoints in AT were Baihui (GV20), Taichong (LR3), Zusanli (ST36), Sanyinjiao (SP6), and Yanglingquan (GB34). The most commonly used tool for evaluation of PD was the Unified Parkinson's Disease Rating Scale III, which assesses motor functions. Conclusion : The screened studies reported that there were no adverse effects of AT on drug therapy, and AT reduced the dose of drugs used in PD treatment. Future studies on PD treatment with AT should use the acupoints GV20, LR3, ST36, SP6, and GB34, and the meridians Gallbladder meridian and Governor Vessel. Clinical studies on PD should use CONSORT or STRICTA to ensure the quality of national studies and allow the development of new tools for the assessment of the effect of AT on PD using the above criteria.

A Literature Study on Acupuncture for Spastic Cerebral Palsy (강직성 뇌성마비의 침자치료에 대한 문헌고찰 - 최근 중의잡지를 중심으로 -)

  • Objectives The purpose of this study is to look into the methods of acupuncture for spastic cerebral palsy. Methods 27 cases of the clinical literature were selected from 43 studies about cerebral palsy and spastics. Theses literature were published between the January 2010 to September 2015 by using the China Academic Journal (CAJ) of China National Knoweldge Infrastructure (CNKI). Results Most studies mentioned the effect of acupuncture (scalp, general) or acupoint-injection, derma needle, warm acupoint. It was more effective with other herbal complex rehabilitation therapy such as acupuncture, physical therapy occupational therapy, rehabilitation, and the complex speech therapy than just the herbal therapy alone. Acupuncture of acupoint-injection has overall 86~96% of rehabilitation rate. The acupuncture was often used with the general acupuncture and scalp acupuncture. Commonly used major acupuncture points were sishencong (四神總), bohui (百會), yundongqu (運動區), and pinghengqu (平衡區), Li11, Li04, Gb39, St41. The main acupoint-injection points were fengchi (Gb21) and jiexi (St41). For the injection, brain actiator, astragalus injection, cerebroprotein hydrolysate, Vit B1, Vit B2 were commonly used. Conclusion Acupuncture and acupoint-injection have been shown as an effective treatment on spastic cerebral palsy. The acupuncture was used often with the general acupuncture and scalp acupuncture all together.

Hwa-Byung Treated by Using Ascending Kidney Water and Descending Heart Fire Pharmacopuncture: Three Case Studies

  • Objectives: We report the results of three case studies on the clinical efect of ascending kidney water and descending heart fre (AKDH) pharmacopuncture on patients with Hwa-Byung. Methods: Tis study involved three patients, all female, who had been admitted to a hospital of traditional Korean medicine and had been diagnosed as having Hwa-Byung by using the Hwa-Byung Structure Clinical Interview for DSM-IV. AKDH pharmacopuncture is administered by injecting pharmacopuncture at a specifc acupoint to change from a state of water-fre disharmony to one of water-fre harmony. For our three patients, Hwangyeonhaedoktang (黃連解毒湯) pharmacopuncture was injected at GB21 and GB20 on both sides; a total of 0.4 cc was injected, with 0.1 cc being injected at each point. Fel ursi, Bezora bovis and moschus (BUM) pharmacopuncture was injected at CV17, CV12, and CV6; a total of 0.15 cc was injected, with 0.05 cc being injected at each point. Treatment was done daily. Progress was evaluated using the Hwa-Byung diagnostic interview (HBDIS), beck depression inventory (BDI), and numeric rating scale (NRS) scores. Results: For the frst patient (case 1), after treatment, the HBDIS score was reduced from 44 to 30 points, the BDI score from 37 to 14, and the NRS score from 10 to 7. For the second patient (case 2), after treatment, the HBDIS score was reduced from 41 to 27 points, the BDI score from 13 to 7, and the NRS score from 10 to 5. For the third patient (case 3), after treatment, the HBDIS score was reduced from 42 to 28 points, the BDI score from 12 to 9, and the NRS score from 10 to 4. Conclusion: Ascending kidney water and descending heart fre pharmacopuncture treatment can be efective for improving ascending kidney water and descending heart fre energy and can be used to alleviate Hwa-Byung.

Defects and Grain Boundary Properties of Cr-doped ZnO (Cr을 첨가한 ZnO의 결함과 입계 특성)

  • In this study, we investigated the effects of Cr dopant (1.0 at% $Cr_2O_3$ sintered at $1000^{\circ}C$ for 1 h in air) on the bulk trap (i.e. defect) and interface state levels of ZnO using dielectric functions ($Z^*$, $M^*$, $Y^*$, $\varepsilon^*$, and $tan{\delta}$), admittance spectroscopy (AS), and impedance-modulus spectroscopy (IS & MS). For the identification of the bulk trap levels, we examine the zero-biased admittance spectroscopy and dielectric functions as a function of frequency and temperature. Impedance and electric modulus spectroscopy is a powerful technique to characterize grain boundaries of electronic ceramic materials as well. As a result, three kinds of bulk defect trap levels were found below the conduction band edge of ZnO in 1.0 at% Cr-doped ZnO (Cr-ZnO) as 0.11 eV, 0.21 eV, and 0.31 eV. The overlapped defect levels ($Zn^{..}_i$ and $V^{\cdot}_0$) in admittance spectra were successfully separated by the combination of dielectric function such as $M^*$, $\varepsilon^*$, and $tan{\delta}$. In Cr-ZnO, the interfacial state level was about 1.17 eV by IS and MS. Also we measured the resistance ($R_{gb}$) and capacitance ($C_{gb}$) of grain boundaries with temperature using impedance-modulus spectroscopy. It have discussed about the stability and homogeneity of grain boundaries using distribution parameter ($\alpha$) simulated with the Z"-logf plots with temperature.

Expression of Human Serum Albumin in Milk of Transgenic Mice Using Goat β-casein/Human Serum Albumin Fusion Gene

  • The gene encoding human serum albumin (HSA) was cloned from human liver cDNA library by PCR. The HSA cDNA in size of 2,176 bp, including 1,830 bp of open reading frame, was cloned into the plasmid carried with the 5'flanking sequence of goat $\beta$-casein gene (-4,044 to +2,025 bp) to get a tissue specific expression vector in mammary gland named pGB562/HSA (12.5 kb). A 9.6 kb DNA fragment in which the sequence is in order of goat $\beta$-casein gene regulatory sequence, HSA cDNA and SV40 polyadenylation signals was isolated from the pGB562/HSA by SacI and DraIII cutting, and used to microinject into the pronuclei of mouse fertilized eggs to produce transgenic mice. Three transgenic mice (2 female and 1 male) were identified by PCR and dot Southern blot analysis. The copy numbers of integrated transgene were more than 10 copies in line #21 and #26 as well as over 50 copies in line #31 of transgenic mice. HSA protein collected from the milk of lactating transgenic mice was confirmed by immuno-detection of Western and slot blot. The concentrations of HSA in the milk were from 0.05 to 0.4 mg/ml. An obvious antigen and antibody conjugate could be observed in immunohistochemical stain of mammary gland tissue from lactating day 11 of HSA transgenic mice. The transmission of transgene and its expression was recognized according to the results of RT-PCR and sequences analyses of their progeny.

A Systematic Review of Acupuncture-Moxibustion Treatments for Nonspecific Chronic Neck Pain (비특이적 목 통증에 사용된 침구치료에 대한 체계적 문헌고찰)

  • Objectives : The aim of this study was to describe the details and to assess the clinical evidence of acupuncture and moxibustion for non-specific chronic neck pain. Methods : We searched seven databases including Korean, English, and Chinese databases through July 2016. Studies using acupuncture, moxibustion, pharmacopuncture, electroacupuncture, auricular-acupuncture, acupressure for non-specific chronic neck pain were included. Only controlled clinical trials or randomized clinical trials were assessed. Study design, number of subject, inclusion criteria, intervention, and results were extracted. In addition, details of intervention including needle type, retention time, acupoints were analyzed. Results : Total 64 studies(39 acupuncture, 9 laser, 6 pharmaco-acupuncture, 3 electro-acupuncture, 3 auricular-acupuncture, 3 moxibustion, 1 acupressure) were included. Among 39 acupuncture studies, 35 used acupuncture as sole intervention. Sham treatment was the most common intervention for control group, followed by no intervention. Various outcome including pain, disability, quality of life, range of motion was used as outcome measurement. The effect of acupuncture and moxibustion was different depending on the type of control and outcome measurement. The most commonly used method in acupuncture for neck pain was GB21, SI3, GB20, LI4, BL11 acupoints, 10~30 mm insertion depth, 20~30 retention time, and 1~2 times per week. Conclusions : Analyzing the details of acupuncture and moxibustion treatment could be helpful for researches and clinics. Further studies should consider the characteristics of study design, intervention, and outcome to assess the effect of TKM.

Research Trends on the Thread Embedding Therapy of Neck pain in Traditional Chinese Medicine (경항통에 대한 매선 임상연구의 중국 현황 분석)

  • Thread embedding therapy is used increasingly for various disease including neck pain. However, the evidence of thread embedding therapy on neck pain and the assessment of their methodology are still limited. This study aimed to investigate the clinical research methodology using thread embedding therapy on neck pain. Thread embedding therapy is used increasingly for various disease including neck pain. This study aimed to investigate the clinical research methodology using thread embedding therapy on neck pain. Total 31 studies were included in analysis. Thread embedding therapy usually was used once a week(32.3%), once per 10days(29.0%), once per two weeks (25.8%). The most common concurrent treatment used with thread embedding therapy was Chinese medicine. Among acupuncture points, EX-B2 (61.3%) was most commonly used, followed by GV14(45.2%), GB20(29.0%), GB21(22.6%). For control group, acupuncture were most commonly used(58.1%). All studies reported that the effect of thread embedding therapy was more effective compared to control group. and 11 studies reported side effects. Only 13 studies(41.9%) reported the appropriate randomization method, and the mean Jadad score of included studies was 1.52. Previous clinical trials included in this study showed the effect of thread embedding therapy for neck pain. However, the quality of the studies was not high. Further rigorous clinical trials are need to assess the effect of thread embedding therapy.
