Clinical Study of Group B β-Hemolytic Streptococcal Meningitis

Purpose : Bacterial meningitis is a serious disease, especially in the neonatal period, and it carries a significant degree of mortality and morbidity. Group B streptococcus(GBS) is a common cause of neonatal bacterial meningitis. The purpose of this study was to evaluate the clinical manifestations, treatment results and complications of GBS meningitis. Methods : We analyzed 29 cases retrospectively who had been admitted to the pediatric ward or NICU in Asan Medical Center from May 1990 to January 2002. They had proven GBS in culture or latex agglutination test in CSF. Results : The male to female ratio was 1 : 1.9. There were two cases of early onset type and 27 cases of late onset type. All cases had normal birth weight with full term at delivery. The perinatal predisposing factors were premature rupture of membrane(two cases), and maternal colonization(two cases). The most common presenting symptoms were fever and irritability. Associated diseases were GBS sepsis(21 cases). There was relatively high sensitivity to penicillin derivatives. There were abnormal brain CT or MRI findings in 16 cases(64%), such as infarction, encephalomalatic change, effusion, hydrocephalus, hemorrhage and abscess. The intensive care unit admission rate and the incidence of DIC were higher in the group with complications. Two cases were discharged against advice. Conclusion : We recommend early detection and active treatment in Group B streptococcal meningitis to improve the prognosis.

B군 연쇄상구균 뇌막염에 대한 임상적 고찰

목 적 : 세균성 뇌막염은 특히 신생아 시기에 발병하는 경우, 비특이적인 증상으로 인해 다른 질환과의 구별이 힘들어 진단과 치료가 늦어지고, 합병증을 초래하며 장기적인 후유증을 남기는 경우가 적지 않다. 이에, 가장 많은 원인균 중에 하나인 GBS에 의한 뇌막염에 대하여 임상 양상, 치료 결과 및 합병증에 대해서 살펴보고자 하였다. 방 법 : 1990년 5월부터 2002년 1월까지 울산의대 서울아산병원 소아과 병동 및 신생아 중환자실에 입원하여, 뇌척수액 배양과 라텍스 응집 반응 검사상 GBS에 의한 뇌막염으로 진단된 29명의 환아를 대상으로 후향적으로 조사 분석하였다. 결 과 : 1) 남녀의 발생 비는 1 : 1.9이었고, 발병 당시 연령은 2명을 제외하고는 생후 60일 이전의 환아로, 이 중 생후 7일 이내의 조발형이 2례, 생후 8일 이후 발생한 지발형이 27례이었다. 2) 29례 모두 만삭아로 2례에서 조기 양막 파열이 동반되었고, 2례에서 산모의 질 배양 검사상 각각 그람 양성 구균과 GBS가 배양되었다. 3) 내원 당시 임상 증상은 전례에서 발열이 동반되었고, 보챔, 경련, 수유 곤란, 호흡 곤란의 순으로 그 외 기면, 구토, 상기도 호흡기 증상, 빈맥, 청색증, 발진, 두통 등이었다. 4) 말초 혈액의 백혈구 수는 17례(58.6%)에서 $5,000/mm^3$ 이하로 감소되어 있었고, 파종 혈관내 응고 소견은 10례(34%)에서 동반되었으며, 뇌척수액 소견상 당은 18례(62.1%)에서 40 mg/dL 이하로 감소되어 있었고, 단백은 16례(55%)에서 300 mg/dL 이상으로 증가되어 있었다. 5) 21례(72.4%)에서 GBS에 의한 패혈증이 동반되었고, 항생제 감수성 검사에서 penicillin에는 100% 감수성을 보였다. Ampicillin, cephalothin은 감수성 검사가 시행된 각각 15례와 17례 모두에서 감수성을 나타내었고, erythromycin과 clindamycin에는 각각 14례에서 감수성을 보였다. Tetracycline에는 감수성 검사가 시행된 8례 모두에서 내성을 나타내었고, tetracycline, clindamycin, gentamicin, erythromycin, chloramphenicol에 대해 내성균이 보고되었다. 6) 치료에는 penicillin 혹은 ampicillin과 3세대 cephalosporin(cefotaxime, ceftriaxone)의 병합 요법이 쓰였으며, 재발한 2례의 경우도 같은 약제의 병합 요법으로 치료하였다. 7) 뇌전산화 단층 촬영 또는 자기공명 영상을 시행한 25례중 뇌 경색, 뇌연화증, 경뇌막하삼출, 뇌수종, 뇌출혈, 뇌농양 등의 합병증이 발생한 경우는 16례(55%)였고, 이 중 중환자실 치료가 필요하거나 파종 혈관내 응고가 동반된 경우가 뇌 영상 소견이 정상이었던 환자 군에 비해 통계적으로 유의하게 많았다(유의수준 0.05). 9례에서 경련의 조절을 위해 항경련제를 지속 투여하였으며, 자의 퇴원한 2례를 제외하고 사망한 경우는 없었다. 결 론 : GBS에 의한 뇌막염은 신생아 시기에 뇌막염을 일으키는 중요한 원인균 중에 하나로, 심각한 합병증을 동반하고 후유증을 남길 수 있으므로 적극적인 진단과 효과적인 치료를 필요로 한다.
