Factors Influencing Driving ability and Its Measurements in Older Driver: A Systematic Review (고령자의 운전능력 영향요인 및 측정도구에 대한 체계적 문헌고찰)

  • Self-driving is meaningful activity for older persons because it enlarges the range of activity and provides opportunities for social participation. Driving is a complex activity that requires integration of physical, cognitive and sensory functions and is influenced by human and psychological factors. Age related functional deterioration affects the driving ability of older drivers. The purpose of this study is to investigate the factors affecting the risk of accidents and driving cessation of elderly drivers through systematic literature review. MEDLINE, EMBASE, Cochrane Library, KoreaMed, Pubmed were used for searching articles published from 2007 to 2017. 'aged', 'aging', 'automobile driving', 'age factors' were used as search terms and 18 articles were finally selected for analysis among 1,458 articles. In result of the study, the most significant effect showed in the physical domain, the driving habit and the performance function. The most frequent used tools evaluated driving habit and the cognitive function. In demographic characteristics, there was a correlation with the driving discontinue according to sex and age. This study emphasizes the necessity of preparing measures for safety driving with elderly. In addition, it suggests the necessity of systematically services such as individual education programs based on various driving cessation related factors of the elderly.

An Integrated Review on Main Caregiver's Burden of Elderly in Korean Nursing Home (한국 요양시설 노인의 주 돌봄자 부담감에 대한 통합적 고찰)

  • The purpose of this study is to investigate the burden of caring for the care-givers of the elderly in Korea by using an integrated literature review method. A total of 23 studies were analyzed using a search database. When care-givers had higher sense of filial and guilt or more than two diseases, they showed high level of feeling of burden. Also, they had high level of feeling of physical burden by their oldness, service period (especially at the period of 1-3 years). In the feeling of economic burden, they had high level of feeling of burden by their oldness, or elderly's disease periods. The feeling of burden by psychological condition was found in elderly in aged and the beginning of admission of nursing home. The feeling of burden by environment situation was found when the functional status of the elderly was bad. Therefore, we need to concern care-givers's feeling of burden with elderly people in the nursing home. In the future, I believe that the findings of this study will be helpful for development of the intervention program for alleviate burden for the care-giver.

The Correlation between Motor Capacity, Capability, and Performance in Children with Cerebral Palsy (뇌성마비 아동의 능력, 수행능력, 수행 간 상관관계)

  • The aim of this study was to distinguish 3 concepts(capacity, capability, and performance) for the motor activities of children with cerebral palsy(CP) and examinate relation between capacity(can do in a standardized environment), capability(can do in daily environment), and performance(does do in daily environment). Cross-sectional analysis with a assessment record of children with CP (n=40; 19males, 21 females; mean age 6y 6mo, SD 3y 8mo) was performed. Levels of severity according to the Gross Motor Function Classification System(GMFCS) included level1(13%), level2(10%), level3(43%), level 4(33%), and level 5(3%). Motor activities capacity was assessed by the Gross Motor Function Measure (GMFM-66). Capability and performance were assessed using 2 scales(functional skill, caregiver assistance) of the Pediatric Evaluation of Disability Inventory(PEDI). Correlations between capacity and capability was 0.811(p < .05), and between capability and performance were high(r=0.711, p < .05). And the correlation between capacity and performance is the lowest(r=0.711, p < .05). Motor performance levels are only partly reflected by the motor capacity and motor capability levels in children with CP. Because performance is influenced by Contextual factors (particularly, social factors such as family function). This study suggests that it is necessary to distinguish and evaluate the capacity, capability, and performance in children with cerebral palsy.

  • This study evaluated the actual status of special needs of the hearing-impaired person for disaster response. The analysis revealed a significant level of unmet needs in disaster response for hearing-impaired person. The 5 special needs in disaster response include: 1) communication needs, which involve securing the means to make an emergency rescue request and communicating information during the rescue process; 2) transportation needs, which indicate the effective evacuation capacity and the level of training; 3) medical needs, which address the degree of preparedness for physical and mental emergency measures and the delivery of health information for rescue and first aid process; 4) maintaining functional independence needs, which refer to the level of self-preparedness to minimize damage in disaster situations, and; 5) supervision needs, which correspond to a personalized support system provided to disaster-vulnerable groups.

Factors Affecting the Quality of Life of Patients With Dementia: A Systematic Review of Literature Focusing on the Subjects Admitted to an Overseas Long-term Care Facility (치매 환자의 삶의 질에 미치는 요인 분석: 국외 요양시설에 입소한 대상자를 중심으로 체계적 문헌고찰)

  • Objective: The purpose of this study was to perform a systematic literature review focusing on international studies to identify various factors affecting the quality of life(QOL) of dementia patients admitted to long-term care facilities. Methods: From January 2000 to July 2018, the articles published in foreign journals were searched through CINAHL and MEDLINE databases. The main search terms were'dementia or Alzheimer's'. 'quality of life', 'long-term care', 'care home', 'nursing home', 'care institution', 'residential care', 'small- scale setting'. The first 1706 articles were searched, but 10 studies were selected using the selection and exclusion criteria. Results: Analysis of ten cross-sectional studies showed that factors such as physical functioning status, ability to perform activities of daily living, and cognition showed a positive correlation with QOL. Depression and anxiety, severity of dementia, and neuropsychiatric symptoms were found to be negatively correlated with QOL. In addition, two longitudinal studies have confirmed that factors affecting QOL are affected by individual functional status and social environmental factors rather than the size and form of care facilities. Conclusion: This study summarized 10 papers and analyzed them through a systematic review of literature. We found that factors such as individual characteristics and social environment determine QOL of patients with dementia. In this study, various intervention methods to improve QOL of patients with dementia should be developed and used in long-term care facilities by identifying the factors affecting QOL of dementia patients and using them.

Development and Usability of a Cognitive Rehabilitation System Based on a Tangible Object for the Elderly (고령자를 위한 실감객체기반 인지재활 시스템의 개발과 사용성 연구)

  • Objective: To develop and verify the usability of a cognitive rehabilitation system with diverse cognitive functional levels based on tangible objects for the elderly population. Methods: A study was conducted to investigate the system's strengths and weaknesses by upgrading it with responses from two groups of 15 patients and 4 occupational therapists. After undergoing three forms of training - regarding executive function, memory, and concentration for a total of 20-30 min, the participants were asked to answer a structured questionnaire about contents of the three forms of training, hardware including the tablet PC functioning as a CPU and display media and tangible objects, and satisfaction of experiential usage of the system. Results: Both groups responded that the most interesting training area was executive function while the least interesting was concentration. Six participants reported that the size of the screen of the tablet PC was inappropriate, and five responded that the size of the tool was inappropriate. All therapists and 40% of the patients responded that they were satisfied with this system. Conclusion: This system's features include easy manipulation of tangible tools for performing training tasks, easy selection of and training in cognitive areas based on users' needs, and automatic adjustment of difficulty level based on users' performance. The training environment was designed to be similar to the natural environment by using tangible objects in both hands as input devices for the system, and the system was considered as an alternative to the lack of community cognitive rehabilitation specialists.

Effects of Sling Exercise With Vibration on Range of Motion, Muscle Strength, Pain, Disability in Patients With Shoulder Injuries (진동을 동반한 슬링 운동이 어깨 손상 환자의 관절가동범위, 근력, 통증, 기능장애 수준에 미치는 영향)

  • Background: Sling exercises are frequently used for the rehabilitation process of patients with shoulder joint injuries, but research on the significant frequency intensity and appropriate treatment duration for sling exercises with local vibration stimulation is lacking. Objects: The aim of this study was to investigate the effects of sling exercise with vibration on shoulder range of motion (ROM), muscle strength, pain, and dysfunction in patients with a medical diagnosis of shoulder joint injury. Methods: Twenty-two patients were randomly assigned to the experiment and control groups. Six sling exercises with and without 50 Hz vibrations were applied in the experiment and control groups, respectively. Each exercise consisted of 3 sets of 5 repetitions performed for 6 weeks. The assessment tools used included shoulder joint range of motion, muscle strength, pain level, and shoulder pain and disability index for functional disability. We conducted re-evaluations before and 3 and 6 weeks after intervention. The changes in the measurement variables were analyzed and compared between the two groups. Results: The ROM of the external rotation of the shoulder joint had a significant interaction between the group and the measurement point (F=3.652, p<.05). In both groups, we found a significant increase in external rotation angle between the measurement points (p<.05). The flexor strength of the shoulder joint significant interaction between the group and the measurement point (F=4.247, p<.05). Both the experiment (p<.01) and control groups (p<.05) showed a significant increase in shoulder flexor strength at the measurement points. After 6 weeks of the interventions, both the groups showed significantly improved VAS (p<.01), SPADI (p<.01), and orthopedic tests (p<.01). However, there was no significant difference between the group and the measurement point in terms of the clinical outcomes observed. Conclusion: The sling exercise with local vibration of 50 Hz affected the external rotation of the shoulder range of motion and improved shoulder flexor strength in the patients with shoulder injuries. Therefore, we propose the use of the sling exercise intervention with vibration in the exercise rehabilitation of patients with shoulder joint injuries.

Antioxidant and anti-inflammatory activities of different extraction solvents of Cryptoporus volvatus (한입버섯의 추출 용매별 항산화 및 항염증 활성)

  • In this study, we analyze the antioxidant and anti-inflammatory activities of various extracts of Cryptoporus volvatus. Three extracts were prepared using hot-water, 70% ethanol, and 70% methanol. Compared to the hot-water and 70% methanol extracts, the 70% C. volvatus ethanol extracts showed significantly higher DPPH radical scavenging activities (76.1%), nitrite scavenging activities (29.5%), and total polyphenol content (9.17 mg GAE/g). The DPPH radical and nitrite scavenging activities of C. volvatus both correlated significantly with the total polyphenol content. The Pearson's correlation coefficient of DPPH radical and nitrite scavenging activities with total polyphenol content were r = 0.744 (p<0.05) and r = 0.921 (p<0.01), respectively. To test their anti-inflammatory effects, nitric oxide production and cell viability were measured by performing nitric oxide (NO) and MTT assays on lipopolysaccharide-treated RAW 264.7 cells. The NO production levels of the 70% ethanol extract-treated cells were slightly lower than those of the others. In our MTT assay, the hot-water extracts showed higher cell viabilities (81.4%) than those of 70% ethanol and 70% methanol extracts. The ${\beta}$-glucan content of C. volvatus was 37.8%. From the results of this study, it is suggested that C. volvatus, an easily accessible wild mushroom, has potential to be used in the development of medicines, cosmetics, and functional health foods.

Natural and synthetic pathogen associated molecular patterns modulate galectin expression in cow blood

  • Pathogen-associated Molecular Patterns (PAMPs) are highly conserved structural motifs that are recognized by Pathogen Recognition receptors (PRRs) to initiate immune responses. Infection by these pathogens and the immune response to PAMPS such as lipopolysaccharide (LPS), Peptidoglycan (PGN), bacterial oligodeoxynucleotides [CpG oligodeoxynucleotides 2006 (CpG ODN2006) and CpG oligodeoxynucleotides 2216 (CpG ODN2216)], and viral RNA Polyinosinic-Polycytidylic Acid (Poly I:C), are associated with infectious and metabolic diseases in animals impacting health and production. It is established that PAMPs mediate the production of cytokines by binding to PRRs such as Toll-like receptors (TLR) on immune cells. Galectins (Gal) are carbohydrate-binding proteins that when expressed play essential roles in the resolution of infectious and metabolic diseases. Thus it is important to determine if the expression of galectin gene (LGALS) and Gal secretion in blood are affected by exposure to LPS and PGN, PolyI:C and bacterial CpG ODNs. LPS increased transcription of LGALS4 and 12 (2.5 and 2.02 folds respectively) and decreased secretion of Gal 4 (p < 0.05). PGN increased transcription of LGALS-1, -2, -3, -4, -7, and -12 (3.0, 2.3, 2.0, 4.1, 3.3, and 2.4 folds respectively) and secretion of Gal-8 and Gal-9 (p < 0.05). Poly I:C tended to increase the transcription of LGALS1, LGALS4, and LGALS8 (1.78, 1.88, and 1.73 folds respectively). Secretion of Gal-1, -3, -8 and nine were significantly increased in treated samples compared to control (p < 0.05). CpG ODN2006 did not cause any significant fold changes in LGALS transcription (FC < 2) but increased secretion of Gal-1, and-3 (p < 0.05) in plasma compared to control. Gal-4 was however reduced in plasma (p < 0.05). CpG ODN2216 increased transcription of LGALS1 and LGALS3 (3.8 and 1.6 folds respectively), but reduced LGALS2, LGALS4, LGALS7, and LGALS12 (-1.9, -2.0, -2.0 and; -2.7 folds respectively). Secretion of Gal-2 and -3 in plasma was increased compared to control (p < 0.05). Gal-4 secretion was reduced in plasma (p < 0.05). The results demonstrate that PAMPs differentially modulate galectin transcription and translation of galectins in cow blood.

Effects of commonly used infusion method on catechin content and antioxidant capacities of pure green tea packaged in tea bags (음용 조건으로 추출한 티백 형태 순수 녹차의 카테킨 함량 및 산화방지능)

  • To evaluate antioxidant capacities and total phenolic, flavonoid, and catechin contents of pure green tea packaged in tea bags, green tea bags of 10 commercial brands were purchased and analyzed. Considering commonly used infusion methods, green tea bags were extracted using 150 mL of spring water at $70^{\circ}C$ for 2 min. Ten brands of green tea showed a wide range of total phenolic content (46.3-93.9 mg gallic acid equivalents/g), total flavonoid content (10.3-21.7 mg catechin equivalents/g), and antioxidant capacities (88.8-175.6 mg and 87.9-183.5 mg vitamin C equivalents/g for DPPH and ABTS assays, respectively). Total catechin content of green tea ranged from 33.8-74.7 mg/g, which corresponded to 43.0-108.3 mg/150 mL of cup. Collectively, these results suggest that daily consumption of 3.8-12.5 cups of green tea packaged in tea bags meets the recommended daily intake of catechins in green tea extract as a health functional foods in Korea.
