A variety of surgical approach for ankle and tibiotalocalcaneal arthrodesis has been described. We used a transfibular approach between the sural nerve and lateral branch of the superficial peroneal nerve. This permits excellent visualization of the ankle and subtalar joint so that the fusion can readily be achieved under the direct visualization. Eight ankle fusions and four tibiotalocalcaneal fusions were carried out through a transfibular approach and reviewed. The resected fibula was utilized for bone graft. The follow up period was from 12 to 22 months. Ages of the patients ranged from 27 to 58 years. The postoperative regimen was six weeks nonweight bearing in a short leg cast, followed by weight bearing in a short leg cast until union occurred. All cases were fused except one who had preoperative pyogenic arthritis of the ankle and hindfoot. The results were as follows; 1. The chance of incisional neuroma is lessened through incision between the sural nerve and superficial peroneal nerve. 2. The possibility of a skin slough is reduced by using full thickness skin flaps. 3. Excellent visualization of the ankle and subtalar joint is easily achieved. So, we believed that the transfibular approach for ankle and tibiotalocalcaneal arthrodesis is the excellent surgical approach.

The objective of this study is to analysis the mechanical characteristics on the geometric nonlinear behavior of radial cable roof systems for long span retractable cable roof structures. The retractable roof is designed as a full control system to overcome extreme outdoor environments such as extreme hot or cold weather, strong wind or sunlight, and the cable roof greatly can reduce roof weight compared to other rigid structural system. A retractable cable roof system is a type of structures in which the part of entire roof can be opened and closed. The radial cable roof is an effective structural system for large span retractable roofs, the outer perimeter of the roof is a fixed membrane roof and the middle part is a roof that can be opened and closed. The double arrangement cables of a radial cable truss roof system with reverse curvature works more effectively as a load bearing cables, the cable system can carry vertical load in up and downward direction. In this paper, to analyze the mechanical characteristics of a radial cable roof system with central posts, the authors will investigate the tensile forces of bearing cables, stabilized cables, ring cables, and the deflection of roof according to the height of the post or hub that affects the sag ratio of cable truss. The tensile forces of the cables and the deflection of the roof are compared for the cases when the retractable roof is closed and opened.

Using light weight concrete as infill material in conventional cold-formed steel (CFS) shear wall systems can considerably increase their load bearing capacity, ductility, integrity and fire resistance. The compressive strength of the filler concrete is a key factor affecting the structural behaviour of the composite wall systems, and therefore, achieving maximum compressive strength in lightweight concrete while maintaining its lightweight properties is of significant importance. In this study a new type of optimum polystyrene lightweight concrete (OPLC) with high compressive strength is developed for infill material in composite CFS shear wall systems. To study the seismic behaviour of the OPLC-filled CFS shear wall systems, two full scale wall specimens are tested under cyclic loading condition. The effects of OPLC on load-bearing capacity, failure mode, ductility, energy dissipation capacity, and stiffness degradation of the walls are investigated. It is shown that the use of OPLC as infill in CFS shear walls can considerably improve their seismic performance by: (i) preventing the premature buckling of the stud members, and (ii) changing the dominant failure mode from brittle to ductile thanks to the bond-slip behaviour between OPLC and CFS studs. It is also shown that the design equations proposed by EC8 and ACI 318-14 standards overestimate the shear force capacity of OPLC-filled CFS shear wall systems by up to 80%. This shows it is necessary to propose methods with higher efficiency to predict the capacity of these systems for practical applications.
Objectives: This study investigated the anti-osteoarthritic effects of Kyejiinsam-tang (hereinafter referred to KIT) on the monosodium iodoacetate (MIA)-induced osteoarthritis rats. Methods: Anti-oxidative effects of KIT were measured by scavenging activities of DPPH, reactive oxygen species (ROS) and nitric oxide (NO). Scavenging activities of anti-oxidation in lipopolysaccharide (LPS)-treated RAW 264.7 cells were also measured for inhibitory effects against the production of inflammatory mediators (tumor necrosis factor-${\alpha}$, interleukin-$1{\beta}$, interleukin-6). Osteoarthritis was induced in rats by injecting MIA in the knee joint. Rats were divided into a total of 4 groups (n=6). The normal group were not treated at all without inducing osteoarthritis whereas the control group were induced for osteoarthritis by MIA and oral medicated physiological saline per day. The positive comparison group was injected with MIA and after 7 days, 2 mg/kg of Indomethacin. The experimental group was injected with MIA and after 7 days was medicated with 34 mg/kg of KIT. Indomethacin and KIT were orally-medicated for each substance a total of 4 weeks, once per day. Weight-bearing on hind legs was measured every week after MIA injection. At the end of the experiment (5 weeks after MIA injection), micro CT (computed tomography)-arthrography and histopathological examinations on the articular structures of knee joint were performed. The effect on inflammatory cytokines and immunological cells in synovial fluid was measured. Volume of cartilage was measured by micro CT-arthrography. Injury to synovial tissue was measured by H & E (hematoxylin and eosin), Safranin-O immunofluorescence. Results: 1. Cytotoxicity against hFCs was insignificant. 2. KIT showed the potent full term for DPPH. 1. NO was significantly reduced by KIT (at 100, $200{\mu}g/m{\ell}$) and ROS was also reduced, but not significantly, by KIT (at $200{\mu}g/m{\ell}$). 2. IL-6 and IL-$1{\beta}$ were significantly reduced by KIT (at 100, $200{\mu}g/m{\ell}$) and TNF-${\alpha}$ was also reduced, but not significantly, by KIT (at $200{\mu}g/m{\ell}$). 1. In hind legs weight-bearing measurement, level of weight increased. 2. Functions of liver and kidney were not affected. 3. IL-$1{\beta}$ was significantly reduced and TNF-${\alpha}$, IL-6 were also reduced but not significantly. 4. PGE2 (prostaglandin E2), LTB4 (leukotriene B4) were significantly reduced in the KIT group. 5. MMP-9 (matrix metalloproteinase-9), TIMP-1 (tissue inhibitor of metalloproteinases-1) and Osteocalcin were significantly reduced in the KIT group. 6. Destruction of cartilage on micro CT arthrography was reduced but had no significant differences. 7. Histopathologically, injury to synovial membrane of the KIT group was decreased and proteoglycan content of KIT group was increased. Conclusions: According to this study, Kyejiinsam-tang has inhibiting effect on the progression of arthritis in MIA-induced osteoarthritis rat. Kyejiinsam-tang has anti-oxidants and anti-inflammation effects, and is related to inhibiting the activity of inflammatory cytokine and injury of volume in cartilage.

Earthquakes over 5.0 on the Richter scale have recently occurred in Korea, which has led to interest in the seismic safety of structures. If a water storage facility is damaged by an earthquake, the water could leak, and the insufficient water would make fire suppression difficult. Therefore, a water storage facility should satisfy safety requirements for earthquakes. In this study, the seismic performance of a water tank was improved by installing a lead rubber bearing between the foundation and the tank. It designed the lead rubber bearing available to the existed concrete foundation. ANSYS was used for modeling to consider the interaction between the fluid and structure of the tank and the hydrostatic and hydrodynamic pressure using four seismic waves. In the case of hydrostatic pressure at 2.5 water level, full level, the same stress appeared irrespective of whether the seismic isolation was installed. When hydrostatic pressure and hydrodynamic pressures are applied at the same time, the seismic-isolated water tank showed less seismic force, and the damping ratio was lower than that of general seismic isolation. This occurred because the weight of the water tank is much smaller than the stiffness of the seismic isolation. The result is expected to be used for further research on seismic capacity evaluation for water tanks.

Purpose: The purpose of this study was to analyze the correlation between the medial joint space on weight bearing simple X-ray and the ultrasonographic articular cartilage thickness. Materials and Methods: The articular cartilage thickness of 91 osteoarthritic knees of 73 patients were measured with weight bearing simple X-ray and ultrasonography between June 2010 and September 2010. Male were 13 and female were 60. Right knees were 35, left knees were 20 and bilateral involvements were 18. Medial joint spaces on X-ray were measured in full extension view and $45^{\circ}$ PA(Rosenberg) view. Femoral cartilage thicknesses at and those at the point between the middle 1/3 and posterior 1/3 area were measured by ultrasonography, that were scanned coronally at $30^{\circ}$ flexion and $130^{\circ}$ flexion respectively. The results were analyzed statistically by Pearson correlation test and Bland-Altman plot test. Results: Medial joint spaces measured in full extension view and femoral articular cartilage thicknesses at the point between the anterior 1/3 and middle 1/3 area measured with sonography were statistically correlated and medial joint spaces measured in $45^{\circ}$ PA view and those at the point between the middle 1/3 and posterior 1/3 area measured with sonography were also correlated (P<0.05, $SD{\pm}2$). Conclusion: The measurement of articular cartilage thicknesses by the ultrasonography was easy even in the patient who are unable to take weight bearing view and was thought to be a useful diagnostic and follow up method to examine the degenerative change in addition to simple radiographic examination in the knee osteoarthritis.
The heat produced by the fission in the fuel of HANARO, 30 MW of research reactor, was transferred from the primary cooling water to the secondary cooling water through heat exchangers. The secondary cooling water absorbing the heat was circulated by secondary cooling pumps and cooled through 33 MW of cooling tower. Each capacity of the three secondary cooling pumps was fifty percent ($50\%$) of full load. The two pumps were normally operated and the other pump was standby. One of the secondary cooling pumps has often made troubles by high vibration. To release these troubles the pump shaft has been re-aligned, the pump bearing has been replaced with new one, the shaft sleeve has been replaced with new one, the shaft and the impeller have been re-balanced representatively and/or the vibration of motor has been tested by disconnecting the shaft of pump. But the high vibration of pump cannot be cleared. We find out the weight balance trouble of the assembly in which the impeller is installed in the shaft. After clearing the trouble, the high vibration is relieved and the pump is operated smoothly. Irreducible knee dislocation is a rare injury and often need an open procedure with ligaments reconstruction. This report describes a case of arthroscopic treatment of a patient with traumatic knee dislocation unable to reduce in a closed method. MRI revealed incarceration of the medial collateral ligament and capsule in the medial compartment. And arthroscopic examination confirmed incarcerated medial capsuloligamentous structures which prevented the knee from reduction. Arthroscopic procedure without ligaments reconstruction was complete when the medial condyle was well visualized and the knee reduced. After 4 weeks of immobilization in extension, range of motion exercise and gradual increases in weight bearing was allowed. At the 3- year follow-up, mild laxity was remained but the patient did not have any discomfort of doing ADL activity and showed full range of motion of the knee.
Twelve cases in eleven patients with segmental bone defects were treated with contralateral fibula free flap and ipsilateral island fibula flap in an antegrade, retrograde or bidirectional flow fashion. Five cases were managed with free flaps and seven were with ipsilateral fibula island transfer. Among seven cases, antegrade fashion was three, retrograde was three, and bidirectional was one. All patients were related with open tibial fractures and its sequelae except one who had open foot bone fracture. According to Gustilo's classification, ten patients were type IIIb and one was type IIIc. Basically, antegrade-flow flaps based on the peroneal vessels as in the conventional free flap were used for the proximal or middle one-third tibial defects. On the contrary, retrograde-flow flaps based on the communicating branch between the peroneal and posterior tibial vessels were used for the middle or distal one-third of the tibia. Bidirection-flow flap based on intact peroneal vessels were used for the middle portion of the tibia. The patients who have undergone ipsilateral fibula island flap had one of the following problems: a previously failed free flap, below-knee amputation of the opposite leg because of open tibial fracture, refusal to use the contralateral sound leg, or poor general condition to stand a lengthy operation. Six of the patients who have got ipsilateral fibula island flap also had an associated fibula fracture on the same leg, which was ultimately used as one of the osteotomy sites. The follow-up period was from 1 to 10 years. Two cases of free flap were failed: one patient had below-knee amputation and the other patient had ipsilateral fibula transfer. Other cases were successful and excellent hypertophy of the transferred fibula was achieved. Time to bone union ranged from 4 to 11 months. Time to full weight bearing was from 5 to 13 months after surgery. All of the transferred fibulas showed hypertrophy after weight bearing. In one case, stress fracture was developed during ambulation, which was healed conservatively. Nonunion occurred in two cases, which were treated with a long leg cast and cancellous bone graft, respectively. Length discrepancy of the legs was noted. The limb was shorter by an average 0.5 cm in three cases, longer by 1.1 cm in one case. In the case of island fibula transfer, limited arc of rotation was not a problem. Other disabling complications were not seen. We believe that these diverse modalities using a vascularized fibula will make us more comfortable to handle major bone defects.

BACKGROUND: Defoliation in grapevine cultivation is practically used to improve light environment within the canopy and thereby fruit quality. Effects of defoliation in five-year-old 'Pione' grapevine during cultivation with heating were investigated to find out optimum ratio of leaf area to fruit cluster weight (L/F). METHODS AND RESULTS: The grapevines were defoliated with berry-thinning 20 days after full bloom to provide various levels of L/F. At harvest, total leaf area values of fruit bearing branches were between 0.23 and $0.60m^2$. With increasing L/F, soluble solids and anthocyanin contents curvilinearly increased ($R^2=0.76^{**}$). At L/F over $0.6m^2/kg$, soluble solids content (SSC) leveled off. With increasing L/F, titratable acidity (TA) linearly decreased ($R^2=0.87^{**}$), but the ratio of SSC to TA linearly increased ($R^2=0.86^{**}$). Anthocyanin content was significantly correlated with SSC and the ratio of SSC to TA ($R^2=0.80^{**}$ and $0.82^{**}$, respectively). When total leaf area per fruit bearing branch was maintained $0.40m^2$, soluble solids and anthocyanin contents linearly decreased ($R^2=0.79^{**}$ and $0.85^{**}$, respectively), but TA linearly increased with increasing fruit cluster weight ($R^2=0.70^{**}$). Fruit was low in quality when the L/F was below $0.6m^2/kg$. CONCLUSION: L/F is recommended to be maintained at least $0.6m^2/kg$ in 'Pione' grapevine during cultivation with heating to produce higher-quality fruits.
