  • Purpose: As the use of computed tomographic scanning spread, the diagnosis of blow-out fractures of the medial orbital wall increased. Conventionally, the surgery of blow-out fractures in medial orbital wall was performed by various approaches with external incision or endoscopic approach. Although the field of orbital surgery has progressed significantly during the last decade, accurate realignment and replacement of component is difficult due to lack of visualization of the fracture site, blind dissection of the orbital wall, and difficulty in insertion of implant. In order to overcome these shortcomings, we explored the use of endoscopic transnasal approach together with subciliary approach. Methods: The entrapped periorbital tissues in the ethmoid sinus were completely reduced endoscopically, and the bone defect of medial orbital wall was reconstructed with $Medpor^{(R)}$ insertion via subciliary approach. This technique was applied to 13 patients who had medial orbital wall fracture. Results: The patients were followed-up for 3 to 24 months with an average of 9 months. The postoperative courses were satisfactory in all cases. Conclusion: The conjunction of endoscopic transnasal and subciliary approach technique seems to produce good results in medial orbital wall fracture.

  • Equal channel angular pressing (ECAP) has been studied intensively over the decade as a typical top-down process to produce ultrafine/nano structured materials. ECAP has successfully been applied for a processing method of severe plastic deformation to achieve grain refinement of magnesium and to enhance its low ductility. However, difficult-to-work materials such as magnesium and titanium alloys were susceptible to shear localization during ECAP, leading to surface cracking. The front pressure, developed by Australian researchers, can impose hydrostatic pressure and increase the strain level in the material, preventing the surface defect on workpiece. In the present study, we investigated the deformation and fracture behavior of pure magnesium using experimental and numerical methods. The finite element method with different ductile fracture models was employed to simulate plastic deformation and fracture behavior of the workpiece.

  • When the external force was applied to the orbit the most thin area might be displaced. These were usually orbital floor and/or medial wall of orbit. Among these cases some who showed the entrapment of the muscle between the fractured fragments needed the surgical treatment. We had operated 4 cases of the "blow-out" fracture via subciliary approach. The entrapped muscles were freed from fragments and the bony defect was restored with micro-titanium mesh. Mean follow-up periods was 15 months and there were no complication observed.

  • The authors describe a rare case of tension pneumocephalus, caused by ventriculoperitoneal[V-P] shunting for communicating hydrocephalus. The patient had a history of a right frontal skull fracture and pneumocephalus after a traffic accident five months prior to the present presentation of gait disturbance and memory impairment. A CT scan showed hydrocephalus and a V-P shunt was put in place. On the fourth day after surgery, the mental status of the patient gradually deteriorated due to a tension pneumocephalus; this was treated by repairing a fistula in the frontal sinus and a dural defect. The patient's mental status improved and symptoms were completely recovered. We report a case of tension pneumocephalus following V-P shunt for hydrocephalus in a patient who sustained a right frontal skull fracture.

  • PHILOS plate fixation in osteoporotic proximal humerus fracture of old age is well-known for high complication rate, especially metal failure, providing various augmentation techniques, such as calcium phosphate cement, allogenous or autologous bone graft. We report a case of polymethyl methacrylate augmentation to provide appropriate reduction with a significant mechanical support. This can be a treatment option for displaced unstable osteoporotic proximal humerus fracture with marked bony defect.

  • Derailment due to wheel failure would cause a tremendous social and economical cost in service operation. It is necessary to evaluate quantitatively the safety with respect to high-speed train. Although the safety of railway wheel has been ensured by an regular inspection, all critical defects cannot be detected in inspection cycles and the wheel has been replaced because a defect quickly become critical for safety. Therefore, it is important to calculate quantitatively the fracture limit and remnant life of damaged railway wheel in wheel-rail system. In present paper, the critical crack size of wheel for high-speed train is simulated based on fracture scenario and the safety of wheel is evaluated.

  • In order to study on the effect of SiC whisker in flexural strength characterization of macro defect-free (MDF) cement composites, which composed of high alumina cement and polyvinyl alcohol, microstructural characterization of the composite specimens fabricated by the addition of SiC whiskers was investigated. Microproes are created around the SiC whisker, MDF cement didn't react with the SiC whisker. However, flexural strength of the composites have been improved. Fracture morphology of the composites, presents mainly intergranular type fracture passing around the unhydrated particles and siC whiskers, and partially transgranular type fracture. The main strengthening mechanisms of the MDF cement composites reinforced with SiC whiskers are characterized by crack deflection, microcracking, and bridging of cracks.

  • Complications of pelvic fractures and sacroiliac joint luxation include hemoabdomen, pulmonary trauma, and soft tissue injury. Among them, rectal perforation accounts for 1% of pelvic fracture complications. Delayed diagnosis is commonly associated with a poor prognosis. A mixed-breed dog was presented to Jeonbuk Animal Medical Center (JAMC) after a traffic accident with no signs of rectal perforation. After fracture surgery, rectal perforation was seen as a perianal fistula. This report describes a case of a perianal fistula progressed from rectal perforation after a traffic accident that was curatively treated with lateral caudal axial pattern flap surgery to the perianal skin.
