  • This research was conduced to 1) examine the influence of milk group intake on the dietary, considering dietary diversity and nutrient adequacy, 2) identify which foods truly contribute to the intake of specific nutrients 3) point out the dietary problems of this age group. The subjects were 664(male 357, female 307) healthy toddlers of the ages of 12-36 months, who had neither been taking any medicine-type supplements nor had any diseases that affect the dietary intakes, chosen among the outpatients of university and private hospitals in Seoul and Kyonggi province during Mach-August, 1999. The 24 hour recall method and CAN Pro(food analysis program) were used to analyze the amount of nutrient intake. The grouping of the subjects for the analysis by monthly age was based on the 98 Korean Child growth standard. The results that the milk group intake showed significant negative correlations with the grain group(r=-0.194, p<0.01), the vegetable group(r=-0.115, p<0.01) and the fruit group(r=-0.125, p<0.01). It also showed that the more the milk group intake, the lower the Dietary Diversity Score(r=-0.131, p<0.01), and the more the intake of raw milk, the more dificient the intake of iron(r=-0.211, p<0.01), vitamin A(r=-0.169,p<0.01), vitamin B$_1$(r=-0.078, p<0.05) and vitamin C(r=-0.187, p<0.01). Foods contributory to nutrient intake for of 12-14 months of were mostly infant formula, and the contribution ratio was high. In the age of 15-17 months, the subjects obtained most of the nutrients from raw milk., liquid-type yoghurt, soybean milk, cooked rice, etc. As for iron intake, the age of 21-23 month obtained the most from ionic drinks, and 24-29 months from raw milk, chocolate cakes, ionic drinks, etc. Though the NAR(Nutrient Adequacy Ratio) and MAR(Mean Adequacy Ratio) generally tended to increase accordingly with the DDs, as for nutrients Ca, vitamin A, vitamin B$_1$, and vitamin C the NAR marked the highest when the DDS marked the lowest(due to infant formula). And for iron, even when the DDS was 5 the NAR value was low. Im conclusion, the more the milk group intake, the lower was the dietary diversity. High dependency on infant formula and others from milk group over the age of 12 months proved to result in a decrease of nutritious solid food intake, iron deficiency and a low dietary diversity score, which can lead to an unbalanced diet. Therefore, nutrition education towards mothers/caretakers of toddler is of urgent need, in order to reduce the fluid milk group intake of low nutritional density and to provide various solid foods the children. • The food consumption of 148 healthy infants from 4 to 6 months of age have been measured. Three groups were assigned to change of feeding pattern. Formula group(FF, n=102) were fed formula from birth till 6 months of age. Convert 1 group (C1F, n=14) and Convert 2 group (C2F, n=32) were fed breast milk and mixed milk at 2months of age afterthat switched to formula milk, respectively. All infants received solids (solid foods) from 4 months of age. No significant difference was found in the intake of nutrients among three feeding groups excluding carbohydrate intake of C1F-female at 4 months of age. No significant difference was found in the intake of nutrients among three feeding groups excluding carbohydrate intake of C1F-female at 4 months of age. The FF-female(70.9g/d) and C2F-female(66.9g/d) had significantly higher carbohydrate intake when compared to the C1F-female (54.3g/d). The average total energy intake at 4, 6 months were 648.3 and 709.7kcal/d among all infants. At 4 and 6 months of age, mean intake of nutrients was as follows. Calcium intake was 526.7mg/d and 760.0mg/d at 4 and 6 months of age respectively. Iron intake was 8.3mg/d and 10/5mg/d at 4 and 6 months of age respectively. Calories from solids provided 22.5% of total energy intake at 4 months of age, and nearly 32% at 6 months of age. The average energy and protein intakes of all infants were less than the RDA for calcium and iron at 4, 6 months of age.

  • The objectives of this study were to examine the growth pattern of infants by anthropometric measurement according to the 5 feeding practices of infants with the subject of two hundred healthy newborn babies from their birth till sixty month of age at intervals of two months. Breast group(BF, n=38), formula group(FF, n=102) and mixed group(ME, n=14) were fed breast milk, formula milk, breast and formula milk, from birth till 6 mo. of age, respectively. Convert 1 group (C1F, n=14) and covert 2 group(CF, n=32) were fed breast milk and mixed milk at 2 mo. of age afterthat switched to formula milk, respectively. From these, the following results were made. All the infants of this study showed superiority to Korean standard growth rate in regards to each growth item for each month age. In the case of males, at their birth, the subscapular skinfold thickness and the total skin fold thickness in the BF group was significantly larger than in MF group and FF group(p<0.05). At 6th month age, the chese circumference of MF group was 45.9cm, and significantly larger than those of BF, FF and C2F groups(p<0.05). In the case of females, at theri birth and 2nd month age, there was no difference among all the feeding groups in regards to each growth rate. At 4th month age, the Kauf index of C1F group was 16.21 and significantly lower than those of four groups(p<0.05). And total skinfold thickness in BF group was larger than in C1F group. The increase rate per month age of all growth items were larger at 2nd month age than at the later months both in males and in females. And until 2nd month age males showed more increase than females in regards to each growth item but after 2nd month age, this sapect did not show up. Multiple linear regression was used to determine predictive factors for infant growth. It was expected that at 6th month age, in the measurement of head circumference and chest circumference and cross-sectional fat area, BF-males were bigger by 22-39% of the explanation index than the infants of other groups. As a result, in spite of the significant lower intakes of energy and nutrients in breast-fed infants than in formula-fed infants, breast-fed infants showed more growth than the average of Korean infant standard growth rate at every month age, and showed no significant growth difference among feeding groups.

  • Purpose : A prospective, controlled trial was conducted to evaluate growth, efficacy, safety and nutritional status for very low birth weight infants fed with human milk fortified with Maeil human milk fortifier (Maeil $HMF^{(R)}$; Maeil Dairies Co., Ltd.). Methods : We enrolled 45 premature infants with a birth weight <1,500 g and gestational age <33 weeks, who were born at Dong-A University Hospital from October, 2006 through December, 2007. They were divided into 2 groups: infants in one group were fed with human milk fortified with $HMF^{(R)}$, and the second were fed with preterm formula. Growth, biochemical indices, feeding tolerance, and other adverse events in each group were assessed serially and compared relatively. Follow-up data were also collected after discharge at 1, 3, and 6 months corrected age. Results : Characteristics of the 2 groups including average gestational age, birth weight, sex, respiratory distress syndrome, patent ductus arteriosus, and other adverse events (sepsis, retinopathy of prematurity, and intraventricular hemorrhage) showed no significant difference. Average feeding start day ($8.00{\pm}3.27d$ vs. $8.86{\pm}5.37d$) (P=0.99) and the number of days required to reach full feeding after start feeding ($41.78{\pm}20.47d$ vs $36.86{\pm}20.63d$) (P=0.55) were not significantly different in the group fed human milk fortified with $HMF^{(R)}$ when compared with the group that was fed preterm formula. The duration of total parenteral nutrition and the incidence of feeding intolerance also showed no differences between the 2 groups. Although infants fed with human milk fortified with $HMF^{(R)}$ showed faster weight gain than those fed with preterm formula at the end stage of the admission period, other growth indices of the two groups showed no significant difference. No significant correlations were found between the 2 groups with regard to weight gain velocity, height gain velocity, head circumference velocity, and post-discharge follow up growth indices. Conclusion : Premature infants fed human milk fortified with $HMF^{(R)}$ showed no significant difference compared with those fed preterm formula in growth, biochemical indices, and adverse events. Using human milk fortifier can be an alternative choice for very low birth weight infants, who need high levels nutritional support even after discharge from NICU.

  • The objectives of this study were to examine the growth pattern and hair trace element contents of healthy infants who were fed breast milk(BF infant) and formula (FF infant) during the first 6 months and its relationship to intake of trace elements. Bimonthyl anthropometric measurements were obtained on 32 infants through 6 months of age. Mean calculated energy, iron, zinc and copper intake from breast milk at 2 months of age were 432.4kcal/d, 0.19mg/d, 1.18mg/d and 0.22mg/d. The values obtained from formular were543.7kcal/d, 6.68mg/d , 2.82mg/d and 0.33mg/d , respectively. In spite of the significantly lower intake of energy and trace elements in BF infants than in FF infants, BF infants showed growth above the average Kroean infant standard growth rate and showed no significant growth rate difference or hair trace element content. Hair iron content in the BF infants at 6 mo. of age was positively related to birth weight and iron intake at 2 mo. of age. In contrast, hair zinc and copper content in the FF infants at 6 mo. of age as negatively related to height increment and weight increment during 6 months, respectively. These results support the suggesting that BF infant's higher iron, zinc and copper intake is attributed to the superior bioabailability of these trace elements from breast milk.

  • We investigated the selenium (Se) nutrition status in Korean infants. The mean serum Se concentration in infants was 66.9 ${\mu}g/L$, and it increased with increasing in infant age: 57.6 ${\mu}g/L$ at 0-5 months, 71.8 ${\mu}g/L$ at 6-11 months, and 75.5 ${\mu}g/L$ at 12-24 months. Serum glutathione peroxidase (GPx) activity also increased with infant age. Serum Se concentration in infants was positively correlated with serum GPx activity (r = 0.565, p < 0.01). At 0-5 months, human milk-fed infants tended to have higher Se concentrations and GPx activity than those of formula-fed infants, but the result was not significant. With the introduction of supplemental feeding at 6-24 months of age, serum Se concentration was not different between the groups. Therefore, human milk feeding seemed to be more appropriate for infant Se nutrition than infant formula feeding during the first 6 months of life, but supplemental feeding became more important later to maintain good Se nutrition status.

  • The surveys of food intakes were carried out on 49 healthy infants aged 4-9 months at the first interview and repeated 3 more times at the interval of 2 months by using food diary recorded by their mothers. Of the subjects 12 were breast-fed, 28, formula-fed, and 9, mixed type-fed. Foods introduced first as the weaning food were commercial weaning foods, fruit juices, yoghurt, egg and rice. Supplemental food was introduced at the age of 4 months in 57% of the infants, but it amounted to a significant proportion of overall food consumption from the age of 6 months. The levels of nutrients except energy, iron and niacin were similar or in excess of RDA, and breast-fed infants tended to have lower intakes of energy and protein compared to infants formula-fed or mixed type-fed. Average intakes of vitamin A, vitamin B1, vitamin B2, vitamin C and calcium were above RDA, but iron intake did not meet RDA of infants of all ages. In conclusion, the average status of nutrient intakes of infants was fairly good, however, food consumption besides milk was less in breast-fed infants than in formula-fed infants, and iron status seemed to be poor, Although it is well-known that breast-milk compared to formulas is more beneficial for infants, mothers feeding breast-milk to their infants should be educated for the importance of supplemental food and its practice to support good nutrition.

  • The purpose of this research was to develop an optimum composition model for a new synbiotic fermented dairy product with high probiotic cell counts, and to experimentally verify this model. The optimum composition model indicated the growth promoter ratio that could provide the highest growth rate for probiotics in this fermented product. Different levels of growth promoters were first blended with milk to improve the growth rates of probiotics, and the optimum composition model was determined. The probiotic viabilities and chemical properties were analyzed for the samples made using the optimal formula. The optimal combination of the growth promoters for the synbiotic fermented milk product was 1.12% peptides, 3% fructooligosaccharides (FOS), and 1.87% isomaltooligosaccharides (IMO). A product manufactured according to the formula of the optimum model was analyzed, showing that the model was effective in improving the viability of both Lactobacillus spp. and Bifidobacterium spp.

  • This study was conducted to evaluate the nutrition quality of the commercial infant formulas and to seek the solution to the establishment of the standard of nutrient requirements for infant formula in Korea. Nutrition informations were obtained from the labels of nineteen commercial infant formulas manufactured by 3 different companies, and the actual amounts of some nutrients were analyzed and compared to the labeled amounts. In addition, the nutrient composition of the commercial infant formulas was compared to the composition of breast milk, RDA for infants, and Codex standard for infant formula. The kind of minerals, vitamins and special components added to the commercial infant formulas were the major differences among 3 manufacturers. For some nutrients, the analyzed amounts were lower than the labeled amounts. In addition when different batches of the same kind of infant formula were analyzed, the large variations in the amounts of certain nutrients were noted. These data suggest that the nutrition labeling informations need to be validated, and nutrients added to the formulas are to be homogenized thoroughly. In order to solve these problems, therefore, like other countries, we need to establish the standard of nutrient requirements for infant formulas. Considering the available data on breast milk composition, RDA for infants and coordination with the international standard, we suggest the adoption of the Codex standard for infant formula may be the best way to manage the nutrition quality of commercial infant formulas at the present time.
