Clinical usefulness of fixation of absorbable implants with cyanoacrylate in comminuted fractures of the maxilla

  • Background: The open reduction of craniofacial bone fractures requires internal fixation using metal plates and screws, which have been considered the gold standard. However, metal implants pose a risk of palpation, protrusion, and foreign body reaction, and they may require an additional operation for removal. Recently, good results have been reported for absorbable implants which complement the disadvantages of metal implants. This study presents the results of using absorbable mesh, plates, and screws with cyanoacrylate for more accurate and firmer fixation of comminuted fractures of the maxilla. Methods: In total, 235 patients underwent operations for comminuted fractures of the maxilla. From January 2012 to December 2014, absorbable mesh and screws were used in 114 patients, while from January 2015 to December 2017, absorbable mesh, plates, and screws with cyanoacrylate were used in 121 patients. Open reduction of the bone fragments was performed, after which absorbable implants were accurately molded and fixed by screws. Results: All patients underwent postoperative computed tomography scans, which showed highly accurate reduction and firm fixation in the patients who underwent procedures using absorbable implants, screws, and cyanoacrylate. There were no postoperative complications or cases of abnormal facial contour. Conclusion: When absorbable implants and screws are used for maxillary fractures, no additional surgery to remove the metal plate is required. In addition, the use of cyanoacrylate enables accurate and firm fixation of the tiny bone fragments that cannot be fixed with screws.

Cloninger의 이론을 활용한 사상체질 생리심리 지표 연구 (Universal index for Sasang typology using Cloninger's biopsychological theory)

  • Introduction Cloninger's Novelty-Seeking (NS) and Harm-Avoidance (HA) were found to be the biopsychological characteristic of Sasang typology, and the So-Yang type has high NS and low HA however the So-Eum type as contrary. The purpose of this study was to show the clinical usefulness of these measures as objective clinical index of Sasang typology in foreign countries lacking validated measures. Methods The Sasang type of 103 university students was diagnosed using Questionnaire for Sasang Constitution Classification (QSCC) II and clinical specialist, and biopsychological and physical features with NS, HA, Sasang Personality Questionnaire (SPQ) and BMI. These measures were normalized to make a noble index of 'NS(100-HA)', and the correlation among these were examined with Pearson's correlation. The differences of biopsychological and physical features between Sasang type groups were attested with ANCOVA, and the comparison of clinical usefulness of SPQ and NS(100-HA) were examined with discriminant analysis. Results SPQ and BMI were clinically useful for Sasang typology as shown in previous studies, and the substitution of SPQ with normalized NS(100-HA) was found acceptable. The NS(100-HA) is significantly correlated with SPQ (r=0.466, p<0.01), however not with BMI (r=0.079, ns). The SPQ and NS(100-HA) scores were found to be distinctive between Sasang type groups, and these were found to predict Sasang type of participants with similar correctness. Discussion and Conclusion The current study reviewed the theoretical backgrounds and confirmed the clinical usefulness of Cloninger's biopsychological theory in Sasang typology. These might provide foundations for integrative medicine and cross-cultural biopsychology of the East and West.

  • 본 연구는 옥외저장탱크에서 발생한 화재의 근본적인 원인분석을 통해 저장탱크의 안전을 유지할 목적으로 수행되었다. 옥외저장탱크는 위험물을 장기간 저장하기 위한 고정장치로 탱크본체 및 부속장치로 구성되며, 부속장치는 벤트 시스템, 브레더 밸브, 화염방지기 등으로 구성된다. 화염방지기는 옥외저장탱크의 화재 폭발을 방지하기 위해 필요한 안전장치이다. 하지만 최근 발생한 옥외저장탱크 화재를 계기로 기존에 설치되어 운용중인 옥외저장탱크에 안전성 확보 차원에서 화염방지기를 의무적으로 설치해야 하는지 국내 외 기준을 비교 분석하여 화염방지기 설치가 필요함을 제시하였고, 화염방지기의 성능을 검증할 수 있는 국내기준이 부족하므로 외국 기준을 참고하여 보완할 필요성이 있다.

연하곤란을 초래하는 구인두 후벽에서 발생한 거대육아종 (Dysphagia Caused by Giant Granuloma Arising from the Oropharyngeal Posterior Wall)

  • Granuloma is an uncommon benign disease that develops in the process of wound healing. Pharyngeal or laryngeal granuloma can be associated with gastric reflux, mechanical injury or trauma including intubation, voice abuse, or foreign body. 50-year-old female was transferred to our institute with a huge mass occupying the upper aerodigestive tract causing dysphagia. The patient has been suffering from a brain hemorrhage for several months and was kept in bed due to the quadriplegia with stuporous mental status, and was tracheotomized. On examination, the whole oropharynx and hypopharynx was covered by a smooth-surfaced soft big diffuse granular mass, which extended down to the upper trachea through the larynx. The huge granuloma was successfully removed with surgery and was found to have a pedunculating stalk on the oropharyngeal posterior wall with a small mucosal defect, suggestive of the origin of the mass. The defect was closed primarily after the cauterization. The patient is now followed up regularly without any recurrence of the disease.

  • 움직임은 삶의 모든 것이며 음악은 우리의 전반적인 삶과 함께하고 있다. 움직임과 소리를 바탕으로 탄생한 예술은 기술과 밀접한 관계를 가지고 발전하며 시대를 반영해 왔으며 디지털 시대에 새로운 문화콘텐츠를 만들어가고 있다. 융합 예술과 교육에 있어 음악과 신체의 움직임이 접목되는 빈도에 비해 다양한 분야의 학문적 지식과 경험을 가진 전문가와 연구기관의 부재는 현실적 문제이다. 외국 대학의 선례를 통해 음악, 움직임을 구성하는 가장 기초적인 원리인 '리듬'을 연구하는 학문인 리드믹을 소개하고 함께 연구되는 분야인 음악생리학과 음악인의학 그리고 행위예술과학에 대해 살펴보며 국내 도입의 필요성을 제기한다. 격변하는 디지털 시대에 유일하게 '나 자신'이 주체가 되는 움직임을 생리학과 의학을 통해 체계적으로 인지하고 음악을 매개체로 과학적으로 움직이고 표현하는 연구 분야를 개척해 융합시대와 교육에 새로운 방향을 제시한다.

미세먼지 위험 단계 예측을 위한 1-D CRNN 모델 설계 (Design of a 1-D CRNN Model for Prediction of Fine Dust Risk Level)

  • 최근 국내 미세먼지 발생의 증가에 따라 발생하는 인체에 유해한 영향을 줄이기 위하여, 미세먼지 수치를 예측하고 사전 조치를 취할 수 있도록 돕는 기술이 필요해지고 있다. 본 논문에서는 국내 미세먼지 위험 수준을 예측하기 위한 1D Convolutional to Recurrent Neural Network (1-D CRNN) 모델을 제안한다. 제안 된 모델은 딥러닝 신경망의 CNN과 RNN을 결합한 구조이며, 다른 종류의 데이터로 구성된 시계열 데이터 세트에서 데이터 예측을 수행 할 수 있다. 데이터 예측을 위해 국내·외 미세먼지, 풍향, 풍속 데이터를 사용한다. 제안된 모델은 약 76%(부분 최대 84%)의 정확도를 달성했으며, 일반 RNN 모델(53%)보다 정확한 예측 결과를 얻었을 수 있었다. 제안된 모델은 향후 여러 개의 시계열 데이터 세트를 고려해야 하는 데이터 예측 모델 학습 및 실험을 목표로 한다.

  • 선(lines)은 인류 역사에서 가장 오래된 시각적 요소이자 인간의 삶과 밀접하게 연관됐으며, 동굴벽화나 바위에 선으로 그려진 그림 및 기호는 인간의 생존을 위한 대체 이미지로 존재해 왔다. 미술에서 선은 단시간 내에 대상의 핵심을 드러내며, 미술치료에서는 내담자의 심리적 상태를 진단하고 치료적으로 개입할 수 있는 매개체가 된다. 그러나 선 그리기, 즉 선화(line drawing)는 진단뿐 아니라 과정상에서 치료적 중요성을 지님에도 불구하고 그 효용성과 치료적 특성을 고찰한 연구는 활발히 이루어지지 못했다. 따라서 본 연구에서는 미술치료에서 선화와 연관된 국내외 문헌을 이론 삼각검증, 질적 내용분석, 공통부분 찾아내기를 통해 우선 도출한다. 그 결과 선화의 핵심적인 치료적 요소인 운동성을 뇌 구조와 연결하여 살펴본다. 이를 통해 선화의 운동성이 신체, 뇌, 마음에 미치는 치료적 메커니즘을 고찰하고 미술치료 안에서 어떻게 치료적으로 활용 가능한가를 검토 및 제안하는 데 학문적 의의를 둔다.

International Legal Measures of Protection of Critical Infrastructure Facilities in Banking Sphere

  • Based on the obtained results of the study, the most problematic issues and legal conflicts are identified, which are related to the ratio of norms of domestic and foreign legislation, taking into account the requirements of the Constitution of Ukraine and the provisions of the Law of Ukraine "On international agreements". Along with this, it is stated in this scientific article that there are a number of provisions and examples of positive practice on the specified topic abroad and in international legal acts today, which should be used by Ukraine both in improving legislation on the issues of banking activity and in increasing the level of criminal legal protection of relevant critical infrastructure facilities, especially those that are substantively related to prevention and counteraction of activity, with regard to the legalization (laundering) of criminally obtained funds, financing of terrorism and the financing of the proliferation of weapons of mass destruction, which is quite relevant for our state, given the military conflict that is taking place on its territory in the Donbass. Again, in the same context, the need for more active cooperation between Ukraine and the FATF (international body developing a policy to combat money laundering) has been proven.

의료용 폴리머 소재를 활용한 3D 프린팅 인공치아용 사면체 비정질 카본 코팅 기술 연구 (A Study on the Tetrahedral Amorphous Carbon (ta-C) Coating on Medical Polymer Materials for 3D Printing Artificial Teeth)

  • This research presents tetrahedral amorphous (ta-C) coating on the artificial tooth for improving the durability and functionality (esthtics, foreign body of tooth) by filtered cathodic vacuum arc (FCVA). A differentiated coating method is required for a ta-C coating on polymer owing to the low melting point of the polymer, inter-facial adhesion, low friction, and non-conductivity. Herein, ta-C coating is applied below 50℃, and the potential difference of the carbon plasma drawn to the substrate was controlled by applying a positive duct bias voltage without using a substrate bias voltage. Consequently, the ta-C coating with a thickness of 70nm using the duct bias condition of 20V with the highest plasma intensity satisfies the esthetics of the artificial tooth and had a 5B level of inter-facial adhesion. In addition, the composite hardness of ta-C/polymer is 380 MPa, and correlations with esthetics, sp3 bonding, and mechanical properties. The friction coefficient (CoF) of the ta-C coating in a water-lubricated environment is 0.07, showing a six-fold reduction in CoF compared with that of a polymer.

뇌파를 평가도구로 사용한 침 중재 임상연구 동향 (A Review on Clinical Research of Acupuncture Using Electroencephalogram)

  • Objectives: The purpose of this study was to investigate the recent trends of clinical research on acupuncture using electroencephalogram (EEG) as the outcome measurement. Methods: Nine domestic and foreign databases were searched to collect related studies published up to November 3, 2021. The participants, intervention, outcomes, results of the included studies were extracted and analyzed. Results: A total of 18 studies were selected. Neurological diseases and mental disorders were included in most studies, and vascular dementia were most frequently investigated. Electroacupuncture and body acupuncture intervention were most frequently conducted in seven studies. The most commonly used outcomes using EEG was EEG abnormality. However, in most studies there was accurate description of the EEG measurement. Most studies showed significant difference in EEG outcomes after intervention. The quality of included studies was poor. Conclusions: EEG as diagnostic markers and outcome measurements is increasingly studied. Standardized EEG measurement and the consistent EEG finding for specific diseases are needed to perform the future rigorous studies on EEG as diagnostic and outcome tools.
