Laminar Cortical Necrosis (Polioencephalomalacia) caused by Postoperative Fluid Overload in a Dog with Pyometra

  • A 7-year-old, intact, female Siberian husky presented with vomiting and diarrhea after a fight with a dog with which it lived. The bitch was diagnosed with pyometra and severe dehydration. The patient received IV fluid therapy for 2 day pre- and post-operatively. Four days post-surgery, the patient had a decreased level of consciousness and suffered a tonic-clonic generalized seizure. On magnetic resonance imaging and histopathological findings, a diagnosis of laminar cortical necrosis caused by fluid overload was made. This case provides important information on the potential for fluid overload in a dehydrated patient giving rise to critical condition and death.

  • Pressure oscillations triggered by the unstable interaction of dynamic flow features of the hydraulic turbine with the hydraulic plant system - including the electrical design - can at times reach significant levels and could lead to damage of plant components or could reduce component lifetime significantly. Such a problem can arise for overload as well as for part load operation of the turbine. This paper discusses an approach to analyze the overload high pressure oscillation problem using computational fluid dynamic (CFD) modeling of the hydraulic machine combined with a network modeling technique of the hydraulic system. The key factor in this analysis is the determination of the overload vortex rope volume occurring within the turbine under the runner which is acting as an active element in the system. Two different modeling techniques to compute the flow field downstream of the runner will be presented in this paper. As a first approach, single phase flow simulations are used to evaluate the vortex rope volume before moving to more sophisticated modeling which incorporates two phase flow calculations employing cavitation modeling. The influence of these different modeling strategies on the simulated plant behavior will be discussed.

  • Acute kidney injury (AKI) is associated with mortality and may lead to increased medical expense. A modified criteria (pediatric RIFLE [pRIFLE]: Risk, Injury, Failure, Loss, and End-stage renal disease) has been proposed to standardize the definition of AKI. The common causes of AKI are renal ischemia, nephrotoxic medications, and sepsis. A majority of critically ill children develop AKI by the pRIFLE criteria and need to receive intensive care early in the course of AKI. Factors influencing patient survival (pediatric intensive care unit discharge) are known to be low blood pressure at the onset of renal replacement therapy (RRT), the use of vasoactive pressors during RRT, and the degrees of fluid overload at the initiation of RRT. Early intervention of continuous RRT (CRRT) has been introduced to reduce mortality and fluid overload that affects poor prognosis in patients with AKI. Here, we briefly review the practical prescription of pediatric CRRT and literatures on the outcomes of patients with AKI receiving CRRT and associations among AKI, fluid overload, and CRRT. In conclusion, we suggest that an increased emphasis should be placed on the early initiation of CRRT and fluid overload in the management of pediatric AKI.

Relationship between Nursing Task Overload and Aseptic Technique Performance in Clinical Nursing Skills (임상 간호사의 역할과부담과 기본간호술 수행 시의 무균술 이행의 관계)

  • Purpose: The study was investigated to identify the relationship between nursing workload and aseptic technique performance by clinical nurses, and to decrease the incidence rate of nosocomial infection. Methods: Participants (N=283) were recruited in B city from April to June 2007. The data were collected by a structured questionnaire and analyzed with descriptive statistics, t-test, ANOVA and Pearson's correlation coefficient. Results: Nursing workload was rated 9.85 out of a total score of 15. The level of aseptic technique performance as the basis for insertion of a Foley catheter was 42.72 out of a total score of 50, and as basis for insertion of intravenous catheter for fluid therapy was 40.11 out of a total score of 55. There was not a significant relationship between aseptic technique performance and nursing workload. There was a significant positive relationship between the aseptic technique performance in insertion of Foley catheter and that of intravenous catheter for fluid therapy (r=.279, p<.001). Conclusions: Attention to asepsis by nurses is crucial in nosocomial infection-related clinical nursing skills.

  • In this paper the thermal aging characteristics of transformer fluid were considered. First, the temperature rise characteristic in transformer winding was discussed. The temperatures were rapidly increased immediately with applying load and then the temperature was followed the exponential temperature rise model. Next, Capacitance and leakage current characteristics of the transformer fluid were measured during the over-loading operation. The capacitance of vegetable fluid and leakage current were evidently increased wit aging time. So the simple methods could be applied to diagnosis of the transformer aging

  • Prediction of performance and operating characteristics of a state-of-the-art ultra-supercritical (USC) steam turbine is an important issue in many ways. Theoretical and empirical correlation equations, developed a few decades ago, have been widely used in commercial programs for a prediction of performance. To improve of these correlation equations and apply them to the high pressure turbine of a USC steam turbine, computational fluid dynamic analysis was carried out and correlation equations to calculate efficiency variation of each stage were made. Both fluid dynamic characteristic and thermodynamic performance was analyzed for the development of the correlation equations. In particular, the impact of flow addition through an overload valve (OLV) between stages was examined throughly. The trend of pressure drop due to the flow mixing by the OLV flow addition was analyzed and an efficiency correlation equation considering the OLV flow was also made.

Study on Prediction of Net Thrust of Multi-Pod-Driven Ice-Breaking Vessel Under Bollard Pull and Overload Conditions According to the Change of Water Depth Using Computational Fluid Dynamics-Based Simulations (수심 변화에 따른 볼라드 당김 및 과부하 조건에서의 다중 포드 추진 쇄빙선박의 여유추력 추정에 대한 수치해석적 연구)

  • In this paper, a numerical analysis technique using a body force model is investigated to estimate the available net thrust of multi-pod-driven ice-breaking vessels under bollard pull and overload conditions. To employ the body force model in present flow simulations, drag and thrust components acting on the pod unit are calculated by using Propeller Open Water (POW) test data. The available net thrusts according to the direction of operation are evaluated in both bollard pull and overload conditions under deep water. The simulation results are compared with the model test data. The available net thrusts, calculated by the present analysis for ahead operating modes at 3~6 knots which are typical speeds of the target vessel in arctic field, are agreed well with the model test results. It is also found that the present result for astern operating mode appears approximately 6 % larger than the model test result. In addition, the available net thrusts are calculated under the both operating conditions accompanied by shallow water effects, and the main cause of the difference is studied. Based on the result of the present study, it is confirmed that the body force model can be applied to the performance evaluation of multi-pod propulsion system and the main engine selection in early design stage of the vessel.

Internal Flow Characteristics of a Steelworks Sintering Cooler by the Duct Shape of Cooler Fan Outlet (제철소 소결냉각용 축류송풍기 출구 덕트 형상에 따른 내부유동특성)

  • Because of overload working condition of sintering cooler, the cooler fan often suffers the break or damage of rotor blade and fixing shaft. Therefore, internal flow characteristics of a steelworks sintering cooler fan by the duct shape of the cooler fan outlet, such as duct outlet opening ratio, duct height and dividing wall shape on the duct outlet flow pattern are examined in detail. The results show that relatively short duct wall height and attachment of dividing wall shape improves flow patterns considerably.

Outcome and Prognosis in Critically III Children Receiving Continuous Renal Replacement Therapy (소아 중환자에서 지속적 신대체요법의 치료 결과와 예후)

  • Purpose : Continuous renal replacement therapy(CRRT) has been the first choice for the treatment of acute renal failure in critically ill children not only in western countries but also in Korea. However, there are very few studies that have analyzed the outcome and prognosis of this modality in Korean children. We performed this study to evaluate the factors associated with the outcome and prognosis of patients treated with CRRT. Methods : We retrospectively reviewed the medical records of 32 children who had received CRRT at Severance hospital from 2003 to 2006. The mean age was 7.5 years(range 4 days-16 years) and the mean body weight was 25.8 kg (range 3.2-63 kg). Results : Eleven(34.4%) of the 32 patients survived. Bone marrow transplantation and malignancy were the most common causes of death and underlying disease leading to the need for CRRT Mean patient weight, age, duration of CRRT, number of organ failures, urine output, estimated glomerular filtration rate(eGFR), C-reactive protein, and blood urea level did not differ significantly between survivors and nonsurvivors. (1) Pediatric risk of mortality(PRISM) III score at CRRT initiation($9.8{\pm}5.3$ vs. $26.7{\pm}7.6$, P<0.0001), (2) maximum pressor number ($2.1{\pm}1.2$ vs. $3.0{\pm}1.0$, P=0.038), and (3) the degree of fluid overload($5.2{\pm}6.0$ vs. $15.0{\pm}8.9$, P=0.002) were significantly lower in survivers than in nonsurvivors. Multivariate analysis revealed that fluid overload was the only independent factor reducing survival rate. Conclusion : CRRT was successfully applied to the treatment of acute renal failure in a wide range of critically ill children. To improve survival, we suggest the early initiation of CRRT to prevent the systemic worsening and progression of fluid overload in critically ill children with acute renal failure. A Study of the Fluid Balance of the Patients on Soft Diets (유동식을 섭취하는 대상자의 수분 섭취와 배설의 균형 분석)

  • The purpose of this study was to investigate the fluid balance of the patients who were either on soft fluid diet or total parenteral nutrition. We studied 19 patients with neurologic disorders and 22 patients with oromaxillary surgery who were admitted to either D university hospital in Choognam or S general hospital in Seoul between May and November 1995. The mean age for the patients who had oromaxillary surgery was 24 years and their average hospital stay was 9 days. The mean age of the patients with neurologic disorders was 54 years and they were bedridden for average of 71 days. For the maxillary bone surgery patients we did not limit the range of their activities in the ward during data collection period. The patients with neurological disorders were bedridden and did not move around the ward. They were all either on soft fluid diet, or total parenteral nutrition. The findings of this study are as follows ; 1) The difference of the triceps skinfold thickness between the baseline and the final measurement was 0.4cm for neurologic patient group and 0.5cm for oromaxillary surgery patient group. The difference was not statistically significant in each group. 2) In the oromaxillary surgery patient group, the daily intake of fluid in the form of pure water, other beverages, fluid diet as well as IV fluid was 4581m1 while urine output was 2979ml. The difference between fluid intake and output was statistically significant, indicating that fluid intake was far more than urine output. In neurologic patient group, the daily intake of fluid including water from fluid diet and IV fluid was 2701m1 whereas urine output was 2253m1 and they were statistically significant. 3) For a more accurate assessment we adjusted the fluid balance based on weight changes during data collection period. In the oromaxillary surgery patient group. the difference between fluid intake and output was 1238m1 after weight changes being adjusted. The difference was statistically significant, suggesting fluid overload in this patient group. In neurologic patient group, the difference between fluid intake and output considering weight changes was 124ml. The difference was not statistically significant, suggesting that the fluid intake and output was well balanced in this patient group.
