The therapeutic approach for colon injury has changed continuously with the evolution of management strategies for trauma patients. In general, immediate laparotomy can be considered in hemodynamically unstable patients with positive findings on extended focused assessment with sonography for trauma. However, in the case of hemodynamically stable patients, an additional evaluation like computed tomography (CT) is required. Surgical treatment is often required if prominent mesenteric extravasation, free fluid, bowel infarction, and/or colon wall perforation are observed. However, immediate intervention in hemodynamically stable patients without indications for surgical treatment remains questionable. Three patients with colon and mesocolon injuries caused by blunt trauma were treated by nonoperative management. At the time of admission, they were alert and their vital signs were stable. Colon and mesocolon injuries, large hematoma, colon wall edema, and/or ischemia were revealed on CT. However, no prominent mesenteric extravasation, free fluid, bowel infarction, and/or colon wall perforation were observed. In two cases, conservative treatment was performed without worsening abdominal pain or laboratory tests. Follow-up CT showed improvement without additional treatment. In the third case, follow-up CT and percutaneous drainage were performed in considering the persistent left abdominal discomfort, fever, and elevated inflammatory markers of the patient. After that, outpatient CT showed improvement of the hematoma. In conclusion, nonoperative management can be considered as a therapeutic option for mesocolon and colon injuries caused by blunt trauma of selected cases, despite the presence of large hematoma and ischemia, if there are no clear indications for immediate intervention.
With time, the stable management of turbidity is becoming more important in the water treatment process. So optimization of coagulation is important for the improvement of the sedimentation efficiency. we evaluated the mixing and hydrodynamic behavior in the coagulation basin using Computational Fluid Dynamics (CFD). The items for evaluation are a location and the speed of agitator and angle of an injection pipe. The results of the CFD simulation, the efficacy of mixing in the coagulation basin was not affected according to one or two injection pipe and angle of an injection pipe. If there is a agitator near outlet of coagulation basin, the efficacy of mixing don't improve even though the speed of agitator increase. So location of agitator is perfect when it locate center at the inlet stream. The coagulation basin at this study, the proper speed of agitator is form 20rpm to 30rpm.
The efficiency of automated assembly line is increased by realizing the automation of each assembly cell, monitoring the line information and developing the real-time line control system it. which production flow is controllable. In this paper, the several modules which are important factors when constructing automated real-time control system, such as, line control S/W module, real-time model change module, error handling module and line production management S/W module, are developed. For developing these important programming modules, real-time control and multi-tasking techniques are integrated. In this paper, operating method of real-time line control in PCB automated assembly line is proposed and for effective control of production line by using multi-tasking technique, proper operating method for relating real-time line control with multi-tasking is proposed by defining the levels of signals and tasks. CIM-Oriented modular programming method considering expandability and flexibility will be added for further research in the future.
Large amounts of household water are required as common households change from the single-residence types of the past to group-residence types. Therefore, the management of reservoirs is urgently required to ensure the supply of clean household water to users. Important considerations for household water include the duration for which the water is stored in the reservoir, the disinfectant's dilution capacity, and the size of the reservoir to allow for the amount of water required for emergencies and firefighting. The drainage efficiency was analyzed in this study using computational fluid analysis for existing rectangular reservoirs and the newly proposed hexagonal reservoir. Thus, it was determined that the centrifugal force generated at the inlet was maintained until the outlet due to the approximately circular shape of the hexagonal reservoir. The findings of this study verified that the centrifugal force improved the flow rate by approximately 35% compared to existing rectangular reservoirs and that drainage was performed efficiently without stagnation zone.
In neurosurgical cases, problems related to wound healing can vary from simple wound dehiscence to multilayer defects. This study demonstrates an effective method to prevent persistent cerebrospinal fluid (CSF) leakage using reinforcing acellular dermal matrix in neurosurgical patients with wound dehiscence. A 52-year-old woman was admitted for management of recurrent glioblastoma. After tumor removal surgery, the patient experienced sustained CSF leakage from the wound despite reparative attempts. The plastic surgery team performed wound repair procedure after remnant tumor removal by the neurosurgery team. Acellular dermal matrix was applied over the mesh plate to prevent CSF leakage and the postoperative status of the patient was evaluated. No sign of CSF leakage was found in the immediate postoperative period. After 3 years, there were no complications including CSF leakage, wound dehiscence, and infection. We hereby propose this method as a feasible therapeutic alternative for preventing CSF leakage in patients experiencing wound problem after neurosurgical procedures.
The objectives of this study were to develop the optimal structures of recirculating aquaculture tank for improving the removal efficiency of solid materials and maintaining water quality conditions. Flow analysis was performed using the CFD (computational fluid dynamics) method to understand the hydrodynamic characteristics of the circular tank according to the angle of inclination in the tank bottom (0°, 1.5° and 3°), circulating water inflow method (underwater, horizontal nozzle, vertical nozzle and combination nozzle) and the number of inlets. As the angle in tank bottom increased, the vortex inside the tank decreased, resulting in a constant flow. In the case of the vertical nozzle type, the eddy flow in the tank was greatly improved. The vertical nozzle type showed excellent flow such as constant flow velocity distribution and uniform streamline. The combination nozzle type also showed an internal spiral flow, but the vortex reduction effect was less than the vertical nozzle type. As the number of inlets in the tank increased, problems such as speed reduction were compensated, resulting in uniform fluid flow.
The torque converter is deformed according to the rotating speed and control pressure when engine power is transferred to the transmission. This deformation, which is called ballooning phenomenon, occurs mainly at the outer side by the centrifugal force of the automatic transmission fluid (ATF) and the control pressure from the valve body. Although the torque converter is slightly deformed when rotating, the ballooning phenomenon affects fluid performance, efficiency and durability. Thus, expansion characteristics analysis is important in determining torus size, control pressure and structure. In this paper, an analysis equation and FEM model was developed to investigate the expansion characteristics. Using this model, structural analysis was performed to investigate the relationships between deformation and the torus diameter. The results were confirmed by comparing with the test results.
유동 수치 해석 분야의 실험들은 복잡한 수학 방정식의 계산 과정으로 이루어져 있고, 이러한 실험을 수행할 수 있는 거대한 계산 자원과 그 자원을 효율적으로 관리할 수 있는 환경을 요구한다. 현재 수치해석을 위한 특정 목적의 실험을 수행할 수 있는 e-사이언스 환경에 대한 연구는 많이 진행되고 있으나 다양한 공학 교육 분야의 유동 해석을 위한 통합 수치해석 연구 프레임워크를 개발한 사례는 거의 없는 실정이다. 따라서 본 논문에서는 그리드 자원을 효율적으로 이용할 수 있는 UNICORE를 기반으로 다양한 유동 해석을 위한 기본 프레임워크를 설계하고, 이에 대한 프로토타입으로 세 가지 유동 응용들에 대한 수치해석 연구를 통합 환경에서 수행할 수 있는 e-사이언스 프레임워크를 개발하였다.

Identification of the pathogens causing infection is important in terms of patient's health management and infection control. Synovial fluids could be used as clinical samples to detect causative pathogens of prosthetic joint infections (PJIs) using molecular diagnostic assays, therefore, normalization and validation of clinical samples are necessary. Microbial culture is considered the gold standard for all infections, including PJIs. Recently, molecular diagnostic methods have been developed to overcome the limitation of microbial culture. Therefore, guideline for validating clinical samples to provide reliable results of molecular diagnostic assays for infectious diseases is required in clinical field. The present study aimed to develop an accurate validating method of synovial fluid clinical samples using GAPDH gene as an internal control to perform the quantitative PCR TaqMan probe assay to detect pathogens causing PJIs.
연구목적: 농흉은 강력한 새로운 항생제 사용에도 불구하고 아직 사망율과 이환율이 높은 질환이며 농흉의 치료에는 항생제 선택을 위해 흉강천자가 필요하며 치료적 배농술을 요하는 경우가 흔하다. 그러나 소방형성이 있는 경우는 흉강천자 및 흉강삽관이 어려운 경우가 많다. 1980년 Light 등이 늑막액의 생화학적 성상에 따른 배농의 적응증을 제시한 이래 이 기준이 통용되고 있으나 이 기준에 대해서 논란도 있으며, 최근 농흉의 진단 및 치료에 도움이 되고 있는 흉부 CT를 이용하여 늑막소견과 소방형성, 늑막비후 등의 합병증과의 관계에 대한 연구를 하였다. 방법: 1988년에서 1993년 동안 본원에 농흉으로 입원한 환자 147명중 흉부 전산화 단층촬영을 실시한 30명을 대상으로 늑막액 생화학적 검사 및 흉부 사진및 균동정을 실시하였다. 결과: 1) 원인균으로서는 그람음성균과 혐기성 세균이 9예(30%)에서 배양되었으며, 19예(63%)에서는 균이 동정되지 않았다. 2) 전산화단층을 촬영한 이유는 종양에 의한 2차적 농흉 및 종양과 감별을 위해서가 14예(46.7%), 항생제에 반응이 좋지 않아서 12예(40%)였으며, 그외 흉강천자가 잘되지 않아서가 3예(10%), 흉강삽관의 위치를 결정하기 위해서가 1예(3.3%)였으며, CT소견상 늑막강내 공기음영이 보이거나 늑막비후가 관찰된 환자가 24예(48%)였으며 소방형성을 보이는 경우는 21예(42%)였다. 3) 늑막액 생화학적 소견과 소방형성 유무와의 상관관계상 늑막액 pH만 소방형성과 상관관계가 관찰되었다(p<0.05). 4) 입원기간과 늑막액 생화학적 소견(Light's criteria), 소방형성유무, 늑막비후 삼출액 양과는 관계가 없었다(p>0.05). 5) 전산화 단층 촬영을 시행한 환자 30명중 소방형성군 9예(43%), 비소방형성으로 3예(33%)에서 전산화단층촬영소견에 따라 치료방법을 전환하였다. 결론: 이상의 결과로 보아 농흉에서 소방형성은 늑막액 pH와 상관관계가 있는 것이 관찰되었으며 배액이 힘든 환자에서 전산화 단층촬영을 실시하여 소방형성유무 및 소방의 정확한 위치, 늑막 비후를 관찰하여 흉강 천자 및 삽관과 늑막 박피술을 결정하므로써 이환률과 재원일수를 줄이는데 도움이 될 것으로 사료된다.
