Reconstruction of a scalp defect due to cochlear implant device extrusion using a temporoparietal fascia flap and a split-thickness skin graft from the scalp

  • Cochlear implant extrusion, which is a common complication of cochlear implants, is generally repaired by a well visualized soft-tissue flap. A 61-year-old female patient with a medical history of schizophrenia who had a skin ulcer that caused cochlear implant extrusion, but that would be a stronger statement was referred to our department for removal of the implant and reconstruction of the resultant scalp defect. Accordingly, the broad defect was covered via rotation of a temporoparietal fascia flap (TPFF) using the superficial temporal artery, with the pedicle in the preauricular region as the pivot point. Coverage of TPFF was achieved with a split-thickness skin graft using the scalp as the donor site, which led to a quick recovery after the operation and satisfactory results in terms of aesthetics. This case suggests that a TPFF might be used as a flexible flap with low donor site morbidity for reconstructing cases of cochlear implant extrusion accompanied by a large full-layer scalp defect.

  • In this contribution, we designed a general purpose input/output (GPIO) suitable for embedded systems, especially for Bluetooth baseband. Proposed architecture is compatible for the APB bus in AMBA bus architecture. General purpose I/O should be used as multi-functional and versatile interrupt sources. We considered the edge-sensitive mode as well as the level-sensitive mode for acquiring the interrupt sources. Also, we provided an option to select the operation polarity for flexible application to the embedded systems. The designed GPIO module was automatically synthesized, placed, and routed. Implementation was performed through the Altera FPGA and well operated at 25MHz clock frequency.

Power Flow Calculation Method of DC Distribution Network for Actual Power System

  • DC distribution system has been evaluated as an excellent one in comparison with existing AC distribution network because it needs fewer power conversion stages and the full capacity of the equipment can be used without consideration for power factor. Recently, research and development on the implementation of DC distribution networks have been progressed globally based on the rapid advancement in power-electronics technology, and the technological developments from the viewpoint of infrastructure are also in progress. However, to configure a distribution network which is a distribution line for DC, more accurate and rapid introduction of analysis technology is needed for the monitoring, control and operation of the system, which ensure the system run flexible and efficiently. However, in case of a bipolar DC distribution network, there are two buses acting as slack buses, so the Jacobian matrix cannot be configured. Without solving this problem, DC distribution network cannot be operated when the network is unbalanced. Therefore, this paper presented a comprehensive method of analysis with consideration of operating elements which are directly connected between neutral electric potential caused by the unbalanced of load in DC distribution network with bipolar structure.

The MFN Principle at Peril in Investment Treaties - with Particular References to Ansung Housing and Beijing Urban Construction

  • Purpose - This paper investigates the theories and practices of Most-Favored-Nation (MFN) clauses. The MFN clause became a controversial issue during the past two decades, especially in the context of investment arbitration. This paper aims to clarify a reasonable way to apply MFN clauses. It in particular focuses on the territoriality requirements and the scope of investment activity which are common features included in most of investment treaties. Design/methodology - This paper analyses two investment arbitration cases, Ansung Housing and Beijing Urban Construction. Through the case study, this paper reveals limitations of the currently dominant views on the operation of MFN clauses. It then tries to reconstruct the system of MFN application within the relevant arbitration principles. Findings - Tribunals of recent investment arbitration as represented in the two cases above employed strict literal interpretation of the treaty provisions, especially of the phrase "in its territory". This paper finds a more functional interpretation is appropriate and consistent with theories of public international law and developments of global economy. Originality/value - Existing studies either stuck to literal interpretation or suggested more flexible interpretation of the phrase "in its territory" without full explanation. This paper tries to fill the gap in the existing discussion by analyzing legal foundations and theoretical structure for an effective interpretation of MFN clauses.

An Efficient Positioning Method for Multi-GNSS with Multi-SBAS

  • The current SBAS service does not provide a method to integrate multiple SBAS corrections. This paper proposes a positioning method to effectively integrate multiple SBAS and multiple GNSS. In the method, the final position is obtained by the weighted sum of the positions obtained from the combination of GNSS and SBAS. Since each position is independently computed and combined using flexible weights, it has a simple structure that can easily cope with various environments. In order to verify the operation and performance of the proposed method, raw measurements of GNSS and SBAS were collected using commercial receivers. The experiments using real signals show that the combined use of two SBAS corrections was more accurate by 0.05~0.4m(2dRMS) than using only one SBAS correction. To improve the position accuracy, this paper considered the integration of multi-GNSS and multi-SBAS, which was not found in other existing studies. The proposed method is expected to be a core technology for designing multi-GNSS navigation receivers considering multi-SBAS corrections. The importance of the method will be increased as KPS and KASS also available in near future.

  • This study aimed to reduce of traps incident of metro train door by suggesting preventive actions throughout analyzing why railway drivers and passengers commit unsafe behaviors which are human factors making occurrence of the incidents. The incident cases were analyzed and Incident Tree was structured by brainstorming with safety experts. In addition, the questionnaire survey was conducted for comparison with the analysis results. As the result, this study suggested driver's factors, passenger's factors, and public relation plan for safe use of metro in order to reduce the frequency of the incidents. For driver's factors, implementing job-rotation systems between railway and non-railway drivers, installing Object Detection Sensors between the metro doors and PSD, and flexible operation of dwell time were suggested. For passenger's factors, placing a platform safety person, installing a safety fence in front of the stairs and the elevators, and country wide public relations through mass media were suggested.

  • Robotic surgery facilitates surgical procedures by employing flexible arms with multiple degrees of freedom and providing high-quality 3-dimensional imaging. Robot-assisted nipple-sparing mastectomy with immediate reconstruction is currently performed to avoid breast scars. Four patients with invasive ductal carcinoma underwent robot-assisted nipple-sparing mastectomy and immediate robot-assisted expander insertion. Through a 6-cm incision along the anterior axillary line, sentinel lymph node biopsy and nipple-sparing mastectomy were performed by oncologic surgeons. The pectoralis major muscle was elevated, an acellular dermal matrix (ADM) sling was created with robotic assistance, and an expander was inserted into the subpectoral, sub-ADM pocket. No patients had major complications such as hematoma, seroma, infection, capsular contracture, or nipple-areolar necrosis. The mean operation time for expander insertion was 1 hour and 20 minutes, and it became shorter with more experience. The first patient completed 2-stage prosthetic reconstruction and was highly satisfied with the unnoticeable scar and symmetric reconstruction. We describe several cases of immediate robot-assisted prosthetic breast reconstruction. This procedure is a feasible surgical option for patients who want to conceal surgical scars.

  • As the spreading speed of electric vehicles increases rapidly, those are expected to be able to use them as flexible resources in the power system beyond the concern for the supply of its charging power. Especially when the Renewable Energy sources (RES) which have no intrinsic control capability have replaced the synchronous generators more and more, the power system needs to secure the additional frequency control resources to ensure its stability. However, the feasibility of using electric vehicles as the frequency control resources should be analyzed from the perspective of the power system operation and it requires the existing simulation frameworks for the power system. Therefore, this paper proposes the grid connected modeling of the primary frequency control provided by electric vehicles which can be integrated into the existing power system model. In addition, the proposed model is implemented considering technical performances constrained by the characteristics of the Vehicle-Grid Integration (VGI) system so that the simulation results can be accepted by the power utilities operating the power system conservatively.

Examination of excess electricity generation patterns in South Korea under the renewable initiative for 2030

  • According to the Renewable Energy 3020 Implementation Plan announced in 2017 by the South Korean government, the electricity share of renewable energy will be expanded to 20% of the total electricity generation by 2030. Given the intermittency of electricity generation from renewable energy, realization of such a plan presents challenges to managing South Korea's isolated national electric grid and implies potentially large excess electricity generation in certain situations. The purpose of this study is: 1) to develop a model to accurately simulate the effects of excess electricity generation from renewables which would arise during the transition, and 2) to propose strategies to manage excess electricity generation through effective utilization of domestic electricity generating capabilities. Our results show that in periods of greater PV and wind power, namely the spring and fall seasons, the frequency of excess electricity generation increases, while electricity demand decreases. This being the case, flexible operation of coal and nuclear power plants along with LNG and pumped-storage hydroelectricity can be used to counterbalance the excess electricity generation from renewables. In addition, nuclear energy plays an important role in reducing CO2 emissions and electricity costs unlike the fossil fuel-based generation sources outlined in the 8th Basic Plan.

  • As the 4th industrial revolution based on ICT is progressing in the manufacturing field, interest in building smart factories that can be flexible and customized according to customer demand is increasing. To this end, it is necessary to maximize the efficiency of factory by performing an automated process in real time through a network communication between engineers and equipment to be able to link the established IT system. It is also necessary to collect and store real-time data from heterogeneous facilities and to analyze and visualize a vast amount of data to utilize necessary information. Therefore, in this study, four types of controllers such as PLC, Arduino, Raspberry Pi, and embedded system, which are generally used to build a smart factory that can connect technologies such as artificial intelligence (AI), Internet of Things (IoT), and big data, are configured. This study was conducted for the development of a program that can collect and store data in real time to visualize and manage information. For communication verification by controller, data communication was implemented and verified with the data log in the program, and 3D monitoring was implemented and verified to check the process status such as planned quantity for each controller, actual quantity, production progress, operation rate, and defect rate.
