The Gender Comparison of Dietary and Health Behavior of First Grade Middle School Students in Suwon Gyeonggi, Korea (수원지역 중학교 1학년생의 성별에 따른 식행동과 건강행동 비교)

  • From the onset of puberty or during its progress, eating and health behaviors are important in establishing optimum growth and healthy fitness in middle school students. This study investigated the dietary and health behavior of first grade middle school students(male 260, female 255) selected from 3 middle schools in Suwon. Results were analyzed using the statistical program(SAS ver. 8.1). Usual dietary habits were scored by the Likert scale of 5 points and obtained the mean and standard deviation. Obtaining significant gender difference, chi-square and student t-test were also done. Daily breakfast intake was 70% of all students but some of them ate alone(27.3%). The amount of food intake was slightly less(34.8%) but sufficient food for dinner(53.2%). The purpose of each meal was solving hunger(57.4%). However, they want to change their current eating habits because of an unbalanced diet. Usual dietary habits were similar in gender, but 'apply nutrition knowledge' and 'consider food combination' were better in female students (p<0.01), 'regular meals' were higher in males(p<0.01). With respect to health behavior, the subjects went for daily walks for less than an hour(39.9%) and got 7-8 hours of sleep per day(33.8%). The amount of sleep was sufficient(22.6%) in males but in insufficient (33.8%) in female students(p<0.01). Weight control efforts for reducing(female 27.5% male 20.8%) and for increasing(male 10.6%) differed according to gender(p<0.01). As the result of the above analyses, the direction of nutrition education would be oriented to the nutrition knowledge and food combination applying to the diets of male students and regular meals for females. And also sleep dissatisfaction needs to be altered with taking care weight control trials.

Predictive factors for Relapse in Children with Steroid Responsive Nephrotic Syndrome (소아 스테로이드 반응성 신증후군에서 재발과 관련된 예측인자)

  • Purpose : Relapses are a major problem in children with steroid responsive nephrotic syndrome(SRNS). This study has been performed to determine the predictive factors for relapse in children with SRNS. Methods : The study group consisted of 7,3 children with SRNS who had been admitted to the Department of Pediatrics, Kyungpook National University Hospital, over 6 years from 1996 to 2001. The medical records were reviewed retrospectively and analyzed to determine significant relationships between selected variables[age at onset, sex, laboratory data, the rapidity of response(days to remission), interval to first relapse] and the frequency of relapse. Results : The age($mean{\pm}SD$) of patients was $4.53{\pm}2.53$ years old. The male to female ratio was 52:21. In 95$\%$, 39 out of the 41 children had a renal biopsy, and the final diagnosis was minimal change nephrotic syndrome. There was no significant correlation between the frequency of relapse and the following variables age at onset, sex, and presence of hematuria. However, the rapidity of response correlated well with the frequency of relapse, especially during the first year after the onset of the disease(P=0.005). Conclusion : The rapidity of response is expected to be one of the predictive (actors for relapse in children with SRNS. (J Korean Soc Pediatr Nephrol 2005;9:167-174)

  • Population screening of newborns is an extremely important and informative diagnostic approach that allows early identification of babies who are predisposed to the development of a number of serious diseases. Some of these diseases are known and have effective treatment methods. Neonatal screening enables the early diagnosis and subsequent timely initiation of therapy. This helps to prevent serious complications and reduce the percentage of disability and deaths among newborns and young children. Primary immunodeficiency diseases and primary immunodeficiency syndrome (PIDS) are a heterogeneous group of diseases and conditions based on impaired immune system function associated with developmental defects and characterized by various combinations of recurrent infections, development of autoimmune and lymphoproliferative syndromes (genetic defects in apoptosis, gene mutation Fas receptor or ligand), granulomatous process, and malignant neoplasms. Most of these diseases manifest in infancy and lead to serious illness, disability, and high mortality rates. Until recently, it was impossible to identify children with PIDS before the onset of the first clinical symptoms, which are usually accompanied by complications in the form of severe coinfections of a viral-bacterial-fungal etiology. Modern advances in medical laboratory technology have allowed the identification of children with severe PIDS, manifested by T- and/or B-cell lymphopenia and other disorders of the immune system. This review discusses the main existing strategies and directions used in PIDS screening programs for newborns, including approaches to screening based on excision of T-cell receptors and kappa-recombination excision circles, as well as the potential role and place of next-generation sequencing technology to increase the diagnostic accuracy of these diseases.

False Aneurysm of Common Carotid Artery by Trauma (외상에 의한 가경동맥루 1례 보고)

  • Aneurysms of the extracranial carotid artery are rare. This is a case report of the rapidly expanding false carotid aneurysm at left common carotid artery, which was repaired surgically with internal shunt This 20 year old male patient had a large pulsatile mass on left lateral aspect of neck at the time of admission. About 1 month before admission, he had received a trauma on left neck by glass piece and noted massive blood loss. And its skin of lesion was sutured simply at local clinic and well healed. 10 days before the admission, he had the sudden onset of the adult thumb sized and pulsatile mass and the mass had been enlarged more and more to the adult first-sized one. The cervical film showed a egg-sized and soft tissue mass. There was systolic bruit on the mass. The diagnosis was confirmed with the angiogram of left carotid artery and this showed the man`s thumb tip-sized extravasation at the point 2 Cm below the bifurcation of Internal and external carotid arteries. The emergency operation was performed by the internal shunt with carotid artery. The aneurysm was enclosed with the adventitia and carotid sheath, and the intima and media were Intact and had the opening of 0.5 cm in diameter. The opening was sutured by the one-hand mattress suture method and firmed with the Aron Alpha-A "Sankyo." The postoperative course was uneventful and the patient was discharged with good general condition.

Dynamics of an elastic beam and a jumping oscillator moving in the longitudinal direction of the beam

  • An oscillator of two lumped masses linked through a vertical spring moves forward in the horizontal direction, initially at a certain height, over a horizontal Euler beam and descends on it due to its own weight. Vibration of the beam and the oscillator is excited at the onset of the ensuing impact. The impact produced by the descending oscillator is assumed to be either perfectly elastic or perfectly plastic. If the impact is perfectly elastic, the oscillator bounces off and hits the beam a number of times as it moves forward in the longitudinal direction of the beam, exchanging its dynamics with that of the beam. If the impact is perfectly plastic, the oscillator (initially) sticks to the beam after its first impact and then may separate and reattach to the beam as it moves along the beam. Further events of separation and reattachment may follow. This interesting and seemingly simple dynamic problem actually displays rather complicated dynamic behaviour and has never been studied in the past. It is found through simulated numerical examples that multiple events of separation and impact can take place for both perfectly elastic impact and perfectly plastic impact (though more of these in the case of perfectly elastic impact) and the dynamic response of the oscillator and the beam looks noisy when there is an event of impact because impact excites higher-frequency components. For the perfectly plastic impact, the oscillator can experience multiple events of consecutive separation from the beam and subsequent reattachment to it.

Association of wheezing phenotypes with fractional exhaled nitric oxide in children

  • Asthma comprises a heterogeneous group of disorders characterized by airway inflammation, airway obstruction, and airway hyperresponsiveness (AHR). Airway inflammation, which induces AHR and recurrence of asthma, is the main pathophysiology of asthma. The fractional exhaled nitric oxide (FeNO) level is a noninvasive, reproducible measurement of eosinophilic airway inflammation that is easy to perform in young children. As airway inflammation precedes asthma attacks and airway obstruction, elevated FeNO levels may be useful as predictive markers for risk of recurrence of asthma. This review discusses FeNO measurements among early-childhood wheezing phenotypes that have been identified in large-scale longitudinal studies. These wheezing phenotypes are classified into three to six categories based on the onset and persistence of wheezing from birth to later childhood. Each phenotype has characteristic findings for atopic sensitization, lung function, AHR, or FeNO. For example, in one birth cohort study, children with asthma and persistent wheezing at 7 years had higher FeNO levels at 4 years compared to children without wheezing, which suggested that FeNO could be a predictive marker for later development of asthma. Preschool-aged children with recurrent wheezing and stringent asthma predictive indices also had higher FeNO levels in the first 4 years of life compared to children with wheezing and loose indices or children with no wheeze, suggesting that FeNO measurements may provide an additional parameter for predicting persistent wheezing in preschool children. Additional large-scale longitudinal studies are required to establish cutoff levels for FeNO as a risk factor for persistent asthma.

Urosodeoxycholic Acid Therapy in a Child with Trimethoprim- Sulfamethoxazole-induced Vanishing Bile Duct Syndrome

  • We present a case of a 7-year-old boy who had cholestasis after trimethoprim-sulfamethoxazole combination therapy. Liver biopsy was performed 36 days after the onset of jaundice because of no evidence of improving cholestasis. Liver histology revealed portal inflammation, bile plug, and biliary stasis around the central vein with the loss of the interlobular bile ducts. Immunohistochemical stains for cytokeratin 7 and 19 were negative. These findings were consistent with those of vanishing bile duct syndrome (VBDS). Chlestasis was progressively improved with dose increment of urosodeoxycholic acid from conventional to high dose. This is the first case report of trimethoprime-sulfamethoxazole associated VBDS in Korean children. The case suggests that differential diagnosis of VBDS should be considered in case of progressive cholestatic hepatitis with elevation of alkaline phosphatase and gamma-glutamyl transpeptidase after or during taking medicine to treat nonhepatobiliary diseases illness.

  • Recently, onset of bacterial disease, especially infiltration of chronic respiratory diseases have been increasing at high rate. The main cause of these diseases are originated from inappropriate sanitary management and slow progress in introducing system and herd free system, which are the base of productivity improvement in bigger pig management, Methods for the prevention and removal of these diseases, are divided into four categories. The first category includes prevention and removal of infectious diseases by organizing strict animal quarantine, enforcing vaccination, and legal regulation and disposal at the time of outbreak. The second category includes improvement of production systems. This purpose can be achieved by discontinuing the open herd system, which brings on the invasion of foreign pathogens and replacing the system by the closed type of farm. To Continue eradication process step-wisely by performing the all-in all-out system at every pig pen or herd level is also effective for this purpose. The third category includes introduction of herd free system can be achieved by repopulating conventional pigs with SPF pigs. If these means are unrealizable from economic or technical viewpoint, medicated early weaning system is perfomed. This system consists of disinfection of pigsty at pre-postpartum stage, application of drugs to pregnant sows and newborn piglets in accordance with the medication program, and early weaning and rearing of newborn piglets in isolation. The fourth category is sanital leading by veterinarians, aimed to eliminating incentives for diseases through diagnoses. Their main activities include periodical monitoring and improvement leading on each responsible subject.

  • This study was to explore lived experience of middle-aged women with aging. The ultimate purpose of this inquiry was to discover the essence of middle-aged women's experience of aging and to promote understanding. Method: This inquiry was performed by using Van Manen's hermeneutical phenomenological approach to make more plausible interpretation of experience. First-handed experiences were explored through multi-stage in-depth interview with 6 women aged between 40 and 64. Second- handed experiences were explored with text such as essay, novel, and photographs. Result: As the process of reflecting and analysing the data of experience were performed, essential themes were emerged: striking onset of event, discomfort and tired body and mind, everything in ones mind, age of harvest gaining much more than loss. Conclusion: This inquiry would be a cornerstone for humanistic nursing care for the mid-aged women.

Scabies mimicking graft versus host disease in a hematopoietic cell transplant recipient

  • Scabies is a highly contagious skin infestation caused by the mite, Sarcoptes scabiei var. hominis. Complex responses to scabies mites in the innate, humoral, and cellular immune systems can cause skin inflammation and pruritus. Diagnosis can be challenging because scabies resembles other common skin conditions. We report the first Korean case of scabies in a hematopoietic cell transplant (HCT) recipient, initially suspected of skin graft versus host disease (GVHD). A T-cell acute lymphocytic leukemia patient underwent a sibling-matched allogeneic HCT and developed pruritus after cell engraftment. Treatment for GVHD did not improve the symptoms. He was diagnosed with scabies 30 days after the onset of symptoms.
