A Study on Prevention Measure Establishment through Cause Analysis of Chemical-Accidents (화학사고 원인분석을 통한 예방대책 수립에 관한 연구)

  • Even if several chemical accident prevention systems such as PSM(Process Safety Management), RMP(Risk Management Plan), etc. have been carried out, many chemical accidents have still occurred at industrial plants in Korea. We describe the status of chemical industry and the trend of occurrence of chemical accidents in Korea. And this paper analyzes the recent chemical accidents in eight ways. These ways include chemical accident forms, ignition sources, sources of chemical equipment, human vs equipment/material causes, worker's working situation, employee scale, hazardous substances, week & time, fatalities of manufacture & contractor's workers. Finally we proposes the four representative prevention measures brought to result of cause analysis by accident statistics.

  • Almost all companies have paid much attention to the safety management ranging from maintenance to operation even at the stage of designing in order to prevent accidents, but fatal accidents continue to increase throughout the world. In particular, it is essential to systematically prevent such fatal accidents sa fire, explosion or leakage of toxic gas at factories in order to not only protect economic losses and environmental pollution. In addition, HRA may be used to detect the human errors which may cause accidents or trace back to any mistake on the part of workers. This work focuses on the coincidence of human error and mechanical failure for management of human error, and on some important performance shaping factors to propose a method for improving safety effectively of the process industries.

In addition, HRA may be used to detect the human errors which may cause accidents or trace back to any mistake on the part of workers. Usually, HRA technique is used in association with other risk assessment techniques. Moreover, it can serve to enumerate the human errors which may occur during operation or down-time or correct the existing system to reduce the mistakes. This work focuses on the coincidence of human error and mechanical failure for management of human error, and on some important performance shaping factors to propose a method for improving safety effectively of the process industries.

  • Chemical facility system have dangerous elements that can injure the human like an explosion and a fire, gas poisoning by a leakage of the harmful chemical material. In addition to a vibration of the machine occurs the leakage. Therefore, the chemical factory requires for periodic monitoring of the vibration. But, until now, the operator has executed a monitoring of the machine by the senses. So, the diagnostic expert system by which the operator can judge easily and expertly a condition of the machine is developed.   • Close to high power radio/radar transmitters, there is a possibility that electrical sparks may occur at discontinuities in metallic structures. If these structures are in an area where flammable mixtures are present, there is a danger that fire or explosion may happen by these sparks. Voltage may be induced on these metallic structures by the radio-frequency transmitter. In this case, a person who comes into contact with these structure may be undergone a severe electrical shock. In this paper, assessment of the these hazards was investigated through experimental and evaluation for actual tower cranes near AM radio transmitters.

A Study on Asphyxiation Accidents occurred in the confined space, and their Prevention (밀폐공간 질식재해 발생현황 고찰 및 예방에 관한 연구)

  • A confined space means places where the risk of damage to health caused by lack of oxygen or hazardous gases in inadequate ventilation conditions and the risk of fire and explosion caused by flammable substances. Asphyxiation accidents in a confined space occured every year and also occurred more than two people at the same time. In this study, we surveyed the domestic statistical data occurred the lack of oxygen in confined space for the last 10 years(2006-2015) and, analyzed the accident by industries sector, workplace size etc. 17 fatal work accidents that occurred in confined spaces in Korea between 2013 and 2015 were investigated and analyzed using the database of the KOSHA and suggested interventions to minimize asphyxiation accidents in confined spaces. This paper is expected to be used to establish interventions planning and training as a preventive measures in workplace having confined spaces.

  • The flash point is generally used as a hazardous index of fire or explosion of a flammable liquid. In this study, the lower flash points and the upper flash points according to the composition of Toluene-o-Xylene and Toluene-Methyl Ethyl Ketone mixtures are determined by air-blowing method. As results, relations between the flash points and the compositions of mixtures ; (1) for Toluene-o-Xylene mixtures $T_{fL}$=25.23 $\alpha$ +5.34 $T_{fu}$=27.36 $\alpha$ +40.50 (2) for Toluene-Methyl Ethyl Keton mixtures $T_{fL}$=10.00 $\beta$-5.00 $T_{fu}$=16.91 $\beta$+20.45.

Proposal of the Safety Technical Standards for Incineration Process in the Chemical Plants (화학공장 소각공정에 대한 안전기술기준 제안)

  • The purpose of this study is to suggest new safety technical standards to improve the safety in the incineration process. Firstly, we analyzed the major accidents occurred in the incineration process since 1996 and proposed 4 articles which is required to be added newly to the existing KOSHA GUIDE. Secondly, we also performed the HAZOP study for each study node and also suggested 3 articles. Finally, we analyzed recommendations commented for PSM reports which have been submitted from the enterprise located in Honam province since 2005 and proposed 2 articles as well. We understand that all 9 articles proposed above, should be added to the KOSHA GUIDE in order to improve the safety in the incineration process and to prevent the major fire and explosion accidents in the design stage.

Quantitative Risk Assessment Method for Deep Placed Underground Spaces (대심도지하공간의 정량적위험성 평가기법)

  • As the necessity to utilize deep-placed underground spaces is increasing, we have to seriously consider the safety problems arising from the U/G spaces which is a restricted environment. Due to the higher cost of land compensation for above ground area and environmental issues, the plan to utilize deep-placed U/G spaces is currently only being established for the construction of U/G road network and GTX. However it is also expected that the U/G spaces are to be used as a living space because of the growing desires to change the above ground areas into the environmentally green spaces. Accordingly it is necessary to protect the U/G environments which is vulnerable against desasters caused by fire, explosion, flooding, terrorism, electric power failure, etc. properly. We want to introduce the principles of the Quantitative Risk Assessment(QRA) method for preparedness against the desasters arising from U/G environments, and also want to introduce an example of QRA which was implemented for the GOTTHARD tunnel which is the longest one in Europe.

A Comparative Study on the Methods of Consequence Estimation in Fire and Explosion Hazards (화재 및 폭발재해의 강도 산정에 관한 비교 연구)

  • Many methods to assess hazards caused by the risks increased with the growth of petrochemical industry. The manual of International Atomic Energy Agency which was much more applied to quantitative analysis of the real situation and the CPQRA is introduced to verify the theoretical background of this manual. Than other methods, as a result, we can see that this manual, which is simple to use and requires a little information, shows similar results to those of calculation by numerical formula. Also, the program code of this manual was materialized and if it is possible to obtain adequate parameters to our circumstance, the manual will be quite useful in early risk analysis.
