손가락 경직을 모사하는 로봇 시뮬레이터를 이용한 경직도 검진의 신뢰도 평가 (Reliability of Modified Ashworth Scale Using a Haptic Robot Finger Simulating Finger Spasticity)

  • 본 연구에서는 환자의 손가락 경직을 모사하는 손가락 시뮬레이터를 통해 손가락 경직도 검진에서의 검진자간 신뢰도에 대해 연구하였다. 시뮬레이터를 제어하기 위해 경직에서 나타나는 토크를 간단하게 모델링 하고 손가락 경직 환자로부터 간단한 측정 모듈을 이용해 각각의 Modified Ashworth Scale(MAS) 등급에 맞는 파라미터를 얻어냈다. 또한 중수지 관절에 모터가 위치한 손가락 형태의 로봇을 설계하여 경직 토크 모델을 따르는 시뮬레이터를 개발하였다. 이 시뮬레이터를 통해 일곱 명의 재활의학과 전문의들의 검진 결과를 평가해본 결과 중수지 관절에서 0.619, 근위지간 관절에서 0.514의 Cohen's Kappa 값을 보였다. 검진자간의 Fleiss' Kappa 값은 중수지 관절에서 0.513, 근위지간 관절에서 0.486으로 나타났다. 또한 검진자들은 각각의 주관적인 MAS 검진 기준을 가진다는 것을 확인하였다. 결과적으로 같은 환자에 대한 MAS 검진 신뢰도가 높지 않기 때문에 개발된 로봇 시뮬레이터는 검진자의 신뢰도를 높일 수 있는 교육용 도구로서 활용 가능함을 확인하였다.

침, 뜸, 봉약침으로 호전된 듀피트렌 구축 환자 증례 보고 (A case report on a patient with Dupuytren's contracture improved by acupuncture, moxibustion and bee venom pharmacopuncture)

  • Objectives : To treat the progression of fibroproliferative disease that affects the flexion contracture of the fingers for patients with Dupuytren's contracture, the purpose of this study is to report a case of a patient with Dupuytren's contracture after complex Korean medical treatment. Methods : A patient was treated with acupuncture, moxibustion and bee venom pharmacopuncture on their left palmar aponeurosis. Six rounds of acupuncture and moxibustion were administered from November 30, 2015 through to January 2, 2016. Three rounds of bee venom pharmacopuncture was administered from December 14, 2015 through to January 2, 2016. The degree of flexion contracture and the Tubiana's stage were measured to evaluate the clinical improvement. Results : After 30 treatment sessions the flexion contracture degrees of the 4th finger's metacarpophalangeal joint and proximal interphalangeal joint improved as much as $25^{\circ}$, $15^{\circ}$, respectively. And the flexion contracture degrees of the 5th finger's metacarpophalangeal joint, proximal interphalangeal joint and distal interphalangeal joint improved as much as $15^{\circ}$, $10^{\circ}$, $5^{\circ}$, respectively. The Tubiana's stage of each finger decreased from 4 to 3. Conclusion : This study suggests that acupuncture, moxibustion and bee venom pharmacopuncture could be effective for patients with Dupuytren's contracture.

The Oblique Extended Reverse First Dorsal Metacarpal Artery Perforator Flap for Coverage of the Radial-Volar Defect of the Proximal Interphalangeal Joint in the Index Finger: A Case Report

  • The dorsal metacarpal artery perforator flap is a flap that rises from the hand dorsum. Owing to its reliability and versatility, this flap is used as a workhorse for finger defect. However, to cover the radial-volar defect of the proximal interphalangeal joint (PIPJ) of the index finger, a longer flap is required than before. Here, we introduce the oblique extended reverse first dorsal metacarpal artery (FDMA) perforator flap to cover the radial-volar aspect defect of the index finger. A 45-year-old man got injured to the radial-volar defect of PIPJ of the left index finger caused by thermal press machine. The wound was 2 × 1 cm in size, and the joint and bone were exposed. We used FDMA perforator from anastomosis with palmar metacarpal artery at metacarpal neck. Since the defect was extended to the volar side, the flap was elevated by oblique extension to the fourth metacarpal base level. The fascia was included to the flap, and the flap was rotated counterclockwise. Finally, PIPJ was fully covered by the flap. Donor site was primarily closed. After 12 months of operation, the flap was stable without complication and limitation of range of motion. The oblique extended reverse FDMA perforator flap is a reliable method for covering the radial-volar defect of the PIPJ of the index finger. This flap, which also has an aesthetic advantage, will be a good choice for hand surgeons who want to cover the PIPJ defect of the index finger using a nonmicrosurgical option.

Up to now, many researchers developed anthropomorphic robotic hands as end effectors. In this paper, we will discuss a problem on the development of a human-scale and motor-driven anthropomorphic robot hand. In this paper, design concept, actuator and transmission, kinematic design and sensing device are presented. By imitating the physiology of human hands, we devised new metacarpalphalangeal joint and interphalangeal joint suitable for human-size robot hands

웨어러블 소프트 센서 장갑의 손가락 관절 관절가동범위 측정에 대한 신뢰도 분석 (Reliability Analysis of Finger Joint Range of Motion Measurements in Wearable Soft Sensor Gloves)

  • Purpose: The purpose of this study was to compare universal goniometry (UG), which is commonly used in clinical practice to measure the range of motion (ROM) of finger joints with a wearable soft sensor glove, and to analyze the reliability to determine its usefulness. Methods: Ten healthy adults (6 males, 4 females) participated in this study. The metacarpophalangeal joint (MCP), interphalangeal joint (IP), and proximal interphalangeal joint (PIP) of both hands were measured using UG and Mollisen HAND soft sensor gloves during active flexion, according to the American Society for Hand Therapists' measurement criteria. Measurements were taken in triplicate and averaged. The mean and standard deviation of the two methods were calculated, and the 95% limits of agreement (LOA) of the measurements were calculated using the intraclass correlation coefficient (ICC) and Bland-Altman plot to examine the reliability and discrepancies between the measurements. Results: The results of the mean values of the flexion angles for the active range of motion (AROM) of the finger joints showed large angular differences in the finger joints, except for the MCP of the thumb. In the inter-rater reliability analysis according to the measurement method, the ICC (2, 1) value showed a low level close to 0, and the mean difference by the Bland-Altman plot showed a value greater than 0, showing a pattern of discrepancy. The 95% LOA had a wide range of differences. Conclusion: This study is a preliminary study investigating the usefulness of the soft sensor glove, and the reliability analysis showed a low level of reliability and inconsistency. However, if future studies can overcome the limitations of this study and the technical problems of the soft sensor glove in the development stage, it is suggested that the measurement instrument can show more accurate measurement and higher reliability when measuring ROM with UG.

수지 골절-탈구 치료의 최신 지견 (The Current Concepts in Treatment of Fracture-Dislocations of the Finger)

  • 수지는 복잡한 구조와 필수적인 기능을 가진 신체 부위로, 외상에 노출되기 가장 쉬운 부위이다. 수지 골절의 치료 원칙은 적절한 고정으로 골절부의 안정성을 획득하는 것과 안정성을 담보로 한 조기 관절 운동 간의 균형을 신중하게 결정하는 것이다. 치료에도 불구하고 합병증으로 강직이나 변형 등으로 인해 기능적, 미용적 문제가 되는 경우가 많다. 본 종설은 수지 골절 치료의 최신 지견을 소개하고, 치료가 어렵고 결과가 좋지 않은 것으로 알려진 관절면의 손상을 포함한 지절 관절 주위의 골절 및 탈구 위주로 정리하였으며, 치료 증례 소개와 합병증에 대한 접근 및 치료 순서로 기술하였다.

수지골에 발생한 관절내 골연골종 - 증례 보고 - (Intraarticular Osteochondroma of the Phalanx of the Hand - A Case Report -)

  • 골연골종은 양성 골종양의 약 40%를 차지하는 매우 흔한 종양이나 수지골의 관절내에서 발생한 예는 아주 드물며, 국내 보고는 아직까지 없었다. 12세 남아가 제 3수지의 근위지 관절의 무통성 종물을 주소로 내원하였다. 이학적 검사상 관절 운동 범위는 정상이었고, 운동시 동통이나 압통은 없었다. 종물의 크기는 $8{\times}3$mm 정도였으며, 경계는 약간 불분명하였고 가동성은 없었다. 단순 측면 사진상 근위지골의 원위 골간단부에 원형의 종물이 피질골로부터 발생하였으며 피질골을 파괴하지는 않았다. 육안 소견상 연골 조직으로 둘러싸인 종괴를 형성하였으며, 현미경적 소견상 연골모에 둘러싸인 정상적 해면골의 소견을 보였다. 이에 저자들은 수지골의 관절내에서 발생한 골연골종을 문헌 고찰과 함께 보고하는 바이다.

변형-내초점 핀 고정술을 이용한 골성 망치 수지의 해부학적 직접 정복 (Anatomical Direct Reduction of Bony Mallet Finger Using Modified-Intrafocal Pinning Technique)

  • 목적: 변형-내초점 핀 고정술을 통한 골성 망치 수지의 해부학적인 직접 정복에 대한 임상적 결과를 알아보고자 한다. 방법: 2014년 3월부터 2017년 10월까지 18명의 골성 망치 수지 환자를 대상으로 K-강선을 이용하여 골편을 직접 정복하는 방법인 변형-내초점 핀 고정술을 시행하였다. 수술 후 통증, 관절 운동 범위, 방사선적인 평가를 시행하였다. 또한 골유합 시기, 기능 회복 정도, 합병증 발생률을 평가하였고, 수술 후 기능적 예후를 판정하기 위해 Crawford의 평가 기준을 이용하였다. 결과: 평균 6주(5-7주)에 방사선적 골유합을 얻었다. 전체 환자에서 평균 $2.8^{\circ}$ ($0^{\circ}-10^{\circ}$)의 신전 소실(extension loss)이 발생하였다. 모든 환자에서 관절면의 일치와 만족스러운 관절면의 재형성이 관찰되었으며, 최종 외래 추시에서 원위지간 관절의 평균 굴곡각은 $72.2^{\circ}$ ($70^{\circ}-75^{\circ}$)였다. Crawford의 평가 기준으로 아주 만족이 12명(66.7%), 만족이 6명(33.3%)이었다. 결론: 변형-내초점 핀 고정술은 골편을 직접 정복 후 고정하여 해부학적 정복을 얻는 방법으로 기존의 다른 경피적 핀 고정술들과 결합하여 적절한 적응증에 적용한다면 좋은 결과를 얻을 수 있을 것으로 기대한다.

손가락에 발생한 고립성 교원종 (A Case of Isolated Collagenoma on the Finger)

  • Purpose: Collagenoma is an intradermal harmatomatous collagen proliferation among connective tissue nevi of the skin. Although there are some reports of isolated collagenomas that occurred on the sole and the palm, collagenoms at the finger has not been reported in Korea. Methods: An 11-year-old girl presented with a growing mass on the distal interphalangeal joint of the left 5th finger. It was a skin-colored, oval and mild tender mass. There were no associated cutaneous or systemic abnormal findings. Results: The nodule, at the subcutaneous level on the distal phalanx, was completely removed by excision. Grossly it was covered with normal skin, and its surface was smooth having a definite margin with a size of $1.5{\times}1.2cm$. Histopathological examination, the epidermis showed to be normal, increased thickness of collagens arranged with a whirl formation was found in the dermis. No signs of cellular component increase were observed. Conclusion: Isolated collagenoma can be aroused as a solitary nodule in at any place of the body. We report a rare case of a female patient with an isolated finger collagenoma.

Open Reduction of Proximal Interphalangeal Fracture-Dislocation through a Midlateral Incision Using Absorbable Suture Materials

  • Background Fracture-dislocation of the proximal interphalangeal (PIP) joint is a relatively common injury. Various treatments for fracture-dislocation of the PIP joint have been reported. In the present study, we performed open reduction through a midlateral incision using absorbable sutures to reduce the small bone fragments and performed volar plate repair. Methods We treated nine patients with fracture-dislocation of the PIP joint with small fractured bone fragments too small for pinning or screw fixation. Patients with volar plate injury were treated with open reduction and volar plate repair at the periosteum of the middle phalangeal bone base by the modified Kessler method using absorbable sutures. All patients were placed in a dorsal aluminum extension block splint, which maintained the PIP joint in approximately 30 degrees of flexion to avoid excessive tension on the sutured volar plate. Results At a mean final follow-up of postoperative 9 months, all patients were evaluated radiographically and had adequate alignment of the PIP joint and reduction of the displaced bone fragments. Range of motion was improved and there were no complications. Conclusions This technique is an excellent alternative to the current method of treating patients with fracture-dislocations that include small fragments that are too small for pinning or screw fixation. It is a less invasive surgical method and enables stable reduction and early exercise without noticeable complications.
