This is a report of successful management of a patient with complicated native valvular endocarditis. Initially stable patient showed sudden collapse at the end of 4th week of antibiotics coverage. Echocardiography revealed that previous vegetation at the Aorto-mitral Fibrous Skeleton[AMFS developed into a false aneurysm, perforated to left atrium and caused fistulous communication between left ventricle and left atrium. Extensive debridement was performed including part of the ascending aorta, aortic cusps, the AMFS, anterior mitral cusp and roof of the left atrium. Reconstruction of the AMFS with tailored single piece of autologous pericardium enabled the implantation of mechanical valves at the aortic and the mitral position. Ascending aorta and roof of the left atrium were repaired with autologous pulmonary artery patch graft and another autologous pericardial patch. The patient was discharged on postoperative 16th day and followed - up till now without any residuae or sequelae.
Patients who have complex endocarditis with involvement of both the aortic and mitral valves and intervalvular fibrous skeleton are among the most difficult to treat and still have the highest surgical mortality and morbidity rates. We report one case of aortic and mitral valve replacement with reconstruction of the fibrous skeleton performed in a 55-year-old female patient who had an aortic annular abscess and both the aortic and mitral prosthetic valve endocarditis with destruction of the fibrous skeleton. Previously, she had undergone redo double valve replacement\`, Transesophageal echocardiogram showed the paravalvular defect at the noncoronary aortic sinus and abnormal sinus tract along the fibrous skeleton. Emergent operation was performed due to positive blood cultures of staphylococcus epidermidis and persistent sepsis despite appropriate antibiotic therapy. After aortotomy extended to the roof of left atrium, both prosthetic valves and destroyed fibrous skeleton were completely resected and the aortic annular abscess was debrided and closed with a bovine pericardial patch. Reconstructions of both aortic and mitral annuli and the fibrous skeleton were done by using two separate bovine pericardial patches in triangular shape and mechanical valves were implanted. Postoperatively, adequate antibiotic therapies were continued and the patient was discharged at the postoperative 72 days without evidence of recurrence of endocarditis. Transthoracic echocardiogram of the postoperative 8 months shows no paravalvular leakage or recurrence of endocarditis and the patient has been followed up with no symptom.
A new concept of an inorganic foaming process at low temperature was demonstrated for the production of inorganic thermal insulating materials with the properties of flexible light-weight, the advantages of organic-based thermal insulation material. The foaming process was proceeded by establishing a skeleton of the foam body by using inorganic fibrous sepiolite and aluminum silicate. A cavity was formed by the expansion of fibrous skeleton body, by the gas which was generated from foaming agent at low temperature. Then the multi-vesicular expanded perlite with low thermal conductivity was filled into the cavity in a skeleton of the foam body. Finally through these overall process, a new inorganic foamed body could be obtained at low temperature without the hot melting of inorganic materials. In order to achieve this object, various preparations such as fibrous sepiolite fibrillation process, heat treatment process of the fibrous slurry were needed, and the optimal compositional condition of slurry was required. The foam body produced showed the properties of flexible light-weight thermal insulation materials such as bulk density, yield strength, flexural strength, and high heat resistance.
We report here on a case of performing a redo-operation for a 65-years-old male patient who had prosthetic endocarditis after reconstruction of the fibrous skeleton due to infective endocarditis 8 years earlier. An aortic annular abscess with a 1cm sized subvalvular abscess and mobile mitral valve vegetation with destruction of the fibrous skeleton was shown on the preoperative echocardiography. An emergency operation was performed due to heart failure. Reconstruction of both the aortic and mitral annuli and the fibrous skeleton was done by using two separate bovine pericardial patches and then mechanical valves were implanted. The postoperative echocardiography shows no paravalvular leakage. The patient has been followed up with no symptoms.
Fibrous dysplasia is a slowly progressive, benign disorder characterized by fibrous tissue replacement of skeleton and may affect solitary or multiple bones. Monostotic fibrous dysplasia mainly occurrs in the rib, femur and tibia, however, rarely in the hand. We report a case of monostotic fibrous dysplasia confined to the 2nd metacarpal bone with findings of plain radiographs and MR imaging.
Purpose: For the best possible aesthetic reconstruction after craniofacial bone tumor resection, pasteurization has been adopted to devitalize neoplastic cells while maintaining osteoinductive properties and mechanical strength. This case report aims to demonstrate a long-term follow-up result of a monostotic fibrous dysplasia in frontal bone which was reconstructed by pasteurized tumoral autograft in situ. Methods: A 14-year-old girl presented with a hard, nontender, slowly growing mass of 6-year duration on her left supraorbital area. CT showed $5{\times}4{\times}3cm$ sized well defined bony mass confined to frontal bone with heterogeneous density. Tumor was excised completely through bicoronal approach and reimplanted to its original site after pasteurization at $60^{\circ}C$ for 30 minutes. The pathologic examination confirmed fibrous dysplasia. Results: She revisited our clinic 5 years later after suffering some assault on her face. On CT examination, pasteurized tumoral autograft was incorporated to host bone except the fractured upper orbital rim without any evidence of recurrence. She has been satisfied with the result. Conclusion: Pasteurization offers a simple, reliable, cosmetic, economic, and durable reconstruction method for craniofacial skeletal tumor. It has advantages of both biologic incorporation ability and mechanical strength without risk of recurrence. So, it should be considered as one of the primary options in benign as well as resectable malignant tumors of craniofacial skeleton.
Fibrous dysplasia is a chronic developmental disease of the skeleton involving formation of immature bone. Involvement of facial bones can result in deformation of facial contour. Prominent cheek area is often treated with malarplasty, which has a variety of modifications, depending on the surgeon's preference. The authors report on a case of polyostotic fibrous dysplasia in which the patient's right cheek had a prominent appearance. The prominence was corrected with malarplasty without internal fixation. The soft nature of bone involved in fibrous dysplasia could provide greater flexibility for modification of the traditional surgery.
McCune-Albright syndrome (MAS) is a rare multisystem disorder characterized by a clinical triad of polyostotic fibrous dysplasia (FD), skin pigmentation, and hyperfunctioning endocrinopathies. A 42-year-old man visited our medical hospital for the treatment of intermittent headaches and was diagnosed with MAS with acromegaly. This patient showed various clinical features of MAS, including pituitary adenoma, polyostotic FD, and hypogonadotropic hypogonadism. The FD lesions showed characteristic radiographic features, such as widespread, sclerotic bony lesions in the cranial bones, mixed radiolucent-radiopaque multilocular lesions in the mandible, and radiolucent lesions in the axial and appendicular skeleton. Over the years, the patient had been hospitalized multiple times due to accidental bony fractures associated with the fragile bony state of FD. This report presents a retrospective description of a case of MAS, with a review of the relevant literature.
Bone shaving for surgical correction is general method in facial asymmetrical patient with fibrous dysplasia. Therefore, decision of bone shaving amount on the preoperative planning is very difficult for improvement of ideal occlusal relationship and harmonious face. Preoperative planning of facial asymmetry with fibrous dysplasia is generally confirmed by the simulation surgery based on evaluation of clinical examination, radiographic analysis and analysis of facial study model. However, the accurate postoperative results can not be predicted by this method. By using the computed tomography based RP(rapid prototyping) model, simulation of facial skeleton can be duplicated and 3-dimensional simmulation surgery can be perfomed. After fabrication of postoperative study model by preoperactive bone shaving, preoperative and postoperactive surgical index was made by omnivaccum and clear acrylic resin. Amount of bone shaving is confirmed by superimposition of surgical index at the operation. We performed the surgical correction of facial asymmetry patients with fibrous dysplasia using surgical index and prototyping model and obtained the favorable results.
We describe herein a three-dimensionally diverse micropatterning of poly(lactic acid), as a biopolymer, using 1-butyl-3-methylimidazolium-based room-temperature ionic liquids (bmim-based RTILs), [bmim]X (X = $SbF_6$, $PF_6$, $NTf_2$, Cl). Utilizing the hydrophobic bmim-based RTILs, [bmim]X (X = $SbF_6$, $PF_6$, $NTf_2$) and a phase separation technique, we were able to produce white and opaque membranes with a three-dimensional structure closely packed with particles ($10-50{\mu}m$ in diameter). The particlulate structure, made by the assistance of [bmim]$NTf_2$ and DCM, interestingly transformed to a fibrous structure by using a cosolvent, e.g., DCM/$CF_3CH_2OH$. When we used an increased amount of [bmim]$NTf_2$, the particles were effectively detached and macrosized ($100-500{\mu}m$ in diameter) and the oval-shaped beads were obtained in a powder form. By varying the counter-anion type of the imidazolium-based RTIL, for example from $NTf_2^-$ to $Cl^-$, the particulate 3D-morphology was once more transformed to a porous structure. These reserch results could be potentially useful, as a method to fabricate particulate scaffolds, fibrous or porous scaffolds, and beads as a biopolymer device in diverse fields including drug delivery, tissue regeneration, and biomedical engineering.
