교통사고 후 두통을 호소하는 환자의 풍지혈.후계혈 자침 효과에 대한 임상적 고찰 -무작위 임상 연구- (The Effect of Acupuncture at Fengchi($GB_{20}$) and Houxi($SI_3$) for Acute Headache Due to Whiplash Injury -Randomised Clinical Trial-)

  • Objectives : The main purpose of this study is to evaluate effects of acupuncture at Fengchi($GB_{20}$) and Houxi($SI_3$) for acute headache due to whiplash injury. Methods : This study was observed among 50 patients with acute headache due to whiplash injury who admitted to Bundang oriental medicine hospital of Dongguk University from June 16, 2009 to January 31, 2010. These patients were divided into two groups; One was control group that was acupunctured at general acupuncture points except Fengchi($GB_{20}$), Houxi($SI_3$)(Control group) and the other was acupunctured at Fengchi($GB_{20}$), Houxi($SI_3$) with general acupuncture points(Experimental group). Each patient was acupunctured once a day. The effect was assessed through self-rating headache index. Results & Conclusions : As a result of evaluation by using self-rating headache index, pain score at 8th day declined in both groups. But they were not difference between two groups at 8th day.

The Analysis of Korean Clinical Studies on the Effect of Pharmacopuncture for Whiplash Injury after Traffic Accidents

  • Objectives: Whiplash injury is one of the major diseases in recent times because of increasing traffic accidents. This review aims to analyze the overall trend of studies on pharmacopuncture for whiplash injury after traffic accidents. Methods: We searched through 4 Korean electronic databases from 2001 up to October 2020 for relevant clinical studies for whiplash injury after traffic accidents, regardless of the patients' age, gender, or race. We included studies that had an intervention group receiving pharmacopuncture therapy with or without other additional treatments, and also included studies that had a control group receiving sham treatment or active treatment such as physical therapy and herbal medication. For the clinical outcomes, we did not place any restrictions on evaluation scales if they are objective metrics. Results: We included 6 randomized controlled trials (RCTs) and 10 non-randomized controlled trials (nRCTs). 10 nRCTs were divided into 4 categories that were case-control studies, case series, case report, and retrospective observational study. In RCTs, Hwangryun-haedoktang (黃蓮解毒湯) pharmacopuncture was the most frequently used. In nRCTs, Jungsong-ouhyul (中性瘀血) pharmacopuncture, and bee venom pharmacopuncture were the most frequently used. As target points of Hwangryun-haedoktang pharmacopuncture, Ashi-points, Jianjing (GB21), and Fengchi (GB20) were the most frequently used. As target points of Jungsong-ouhyul pharmacopuncture, Ashi-points were the most frequently used. And as target points of Bee venom pharmacopuncture, Fengchi (GB20) was the most frequently used. Conclusion: Hwangryun-haedoktang pharmacopuncture, bee venom pharmacopuncture, and Jungsong-ouhyul pharmacopuncture were mainly used for whiplash injury, and their usual target points were Jianjing (GB21), Fengchi (GB20), and Ashi-points. However, a high level of evidence should be conducted through studies with systematic methodology in the future.

만성 긴장성 두통환자의 매선요법과 침의 효과에 대한 예비적 비교 연구 (A Preliminary Comparison of Efficacy of Needle-Embedding Therapy with Acupuncture for Chronic Tension-type Headache Patients)

  • Objectives : The purpose of this study was to compare the efficacy of Needle-Embedding Therapy with Acupuncture for Chronic Tension-type Headache (CTTH) patients. Methods : The study recruited Chronic Tension-type Headache patients. A total of twenty-six patients were divided into 2 groups. 13 subjects were placed into a needle-embedding therapy group and 13 subjects into an acupuncture therapy group by a randomization table. The needle-embedding therapy group was treated with needle-embedding at Fengchi (GB20), while the acupuncture therapy was acupunctured at Fengchi (GB20). Then, a comparative analysis was conducted by comparing the results with those measured by a Blood pressor, Heart rate variability (HRV), Henry Ford Headache Disablity Inventory (HDI) Internet Headache Impact Test (HIT), and Visual Analogue Scale (VAS). Results & Conclusions : As a result of evaluation by using the self-rating headache index (VAS, HDI, HIT), pain score declined in both groups. But they were not difference between the two groups.

돌발성 난청의 침치료에 대한 최근 임상 연구 동향 (Recent Clinical Research on Effect of Acupuncture on Sudden Hearing Loss)

  • Objectives : The purpose of this study is to investigate recent clinical studies on the effect of acupuncture on hearing loss. Methods : Based on the PubMed search with the key search terms of 'hearing loss, acupuncture', dated from 2004 to 2017, 1 controlled trials, 3 case reports and 3 case series was found, and were analyzed for this study. Results : 1. The most commonly used acupoints were Shuaigu(GB8) and Fengchi(GB20). 2. The most commonly used meridians were the du channel, the gall bladder meridian of foot-shaoyang, and the triple bunner meridian of hand shaoyang. 3. Acupuncture treatment was effective for conductive and sensorineural hearing loss, especially sudden sensory neural hearing loss. 4. More clinical studies are needed to prove the effectiveness of the acupuncture on hearing loss. To be more objective on the study results, we can measure auditory brainstem response for hearing loss after acupuncture treatment.

  • Objective: The purpose of this study is to investigate recent clinical studies on the effects of acupuncture on dementia. Method: We searched RCTs (randomized controlled trials) using the database PubMed (2000-2018). Clinical RCT studies of acupuncture used to treat dementia in humans were included. Nine randomized controlled trials were included. Results: Various acupoints such as Baihui (GV20), Sishencong (EX-HN1), Fengchi (GB20), and Shenting (GV24) were used to treat symptoms. The studies reported a decrease in evaluation criteria, such as the Mini Mental State Examination (MMSE) and activities of daily living (ADL), for measuring cognitive ability. The acupuncture process, the results for outcome measure, and the effectiveness of studies were analyzed. We then assessed "risk of bias." Conclusions: Through this review, we found that acupuncture is applicable for the treatment of dementia. This can be used for future study.

  • Objectives : The purpose of this study was to obtain some knowledge concerning the scalp acupuncture treatment on patients with dementia for the future practice and research from clinical studies in China. Methods : The literatures were searched using the data base-China Academic Journals(CAJ) (2001-2011). Results : We found out 1 case study, and 17 randomized controlled trials(RCT) which met the inclusion criteria in most studies. There were 3 types of scalp acupuncture-applying method, cho-si scalp acupuncture(焦氏頭鍼法), combination of using ordinary acupoints(正經鍼) and extraordinary acupoints(經外寄穴), standard method of scalp acupuncture(標準頭鍼法). The often used acupoints are Yundongqu(運動區), Ganjuequ(感覺區), Yuntingqu(暈聽區), Zuyunganqu(足運感區), Sishencong (HN23), Baihui(GV20), Shenting(GV24), Fengchi(GB20), Dingnieqianxiexian(MS6), and Dingniehouxiexian(MS7). Conclusions : There were significant improvements on the scalp acupuncture-treated groups, compared to the control groups(western medication group or other acupuncture treatment group). Results of this study could be used for future studies concerning the scalp acupuncture for dementia.

Effects of acupuncture in postmenopausal women with prehypertension or stage 1 hypertension: study protocol for a prospective, comparative, interventional cohort study

  • Background: Hypertension is a major cause of cardiovascular disease and associated mortality, and postmenopausal women are at a high risk of hypertension. We aim to investigate the hypotensive effect and safety of acupuncture, focusing on postmenopausal women with prehypertension and stage 1 hypertension. In addition, we aim to investigate whether the effect of acupuncture treatment differed, depending on Sasang Constitution and cold-heat pattern. Methods: This study is designed as an intervention cohort study. Two hundred postmenopausal women aged <65 years with prehypertension or stage 1 hypertension living in Daejeon city in Korea will be recruited, and randomly assigned to either an acupuncture or no-treatment control group. The intervention will consist of four sessions; one session will include acupuncture performed 10 times for 4 weeks. There will be a 20-week observation period after each session, and the total study duration will be 96 weeks. Acupuncture will be applied at the bilateral Fengchi (GB20), Quchi (LI11), Zusanli (ST36), and Sameumgyo (SP6) acupoints. The effect of acupuncture will be evaluated by comparing the change in systolic and diastolic blood pressure between the acupuncture and control groups every 4 weeks until the end of the study. Discussion: To evaluate the success of blood pressure management, long-term observation is required, but no long-term studies have been conducted to evaluate the effect of acupuncture on blood pressure in postmenopausal women. To our knowledge, this study will be the first long-term study to investigate this issue for more than 6-8 weeks.

알레르기 비염 한의표준임상진료지침 방향 제언 - 2018 중국 임상진료지침을 바탕으로 (Suggestions for developing Korean Medicine Clinical Practice Guideline for Allergic Rhinitis - Based on 2018 Chinese Clinical Practice Guideline)

  • Objective : The purpose of this study is to examine the treatment of allergic rhinitis introduced in the Chinese guideline, and to find out the direction of evidence-establishment and applicability in developing Korean Medicine clinical practice guideline for allergic rhinitis. Method : We studied Traditional Chinese Medicine treatment of allergic rhinitis introduced in the 2018 Chinese society of allergy guidelines for diagnosis and treatment of allergic rhinitis. The treatment are classified into three categories; herbal medicines, acupuncture and the others. And we compared this guidelines with other guidelines for how they differ in description of the database, evidence of level, and strength of recommendation. Results : Herbal medicines are presented based on syndrome differentiation. The basic acupoints for allergic rhinitis are introduced as follows; Fengchi(GB20), Yingxiang(LI20), Feishu(BL13) and Taiyuan(LU9). And in comparison with other guidelines for allergic rhinitis, the Chinese guideline showed lack of description in the database, evidence of level, and strength of recommendation, though they used evidence-based models. Conclusions : Clinical practice guideline projects are also underway in Korea for benefit expansion and improving quality of medical services. It is important to develop guidelines which should be evidence-based and reflect Korean medical environment.
