분노조절문제를 가진 아동에 대한 가족치료 사례연구 (A Case Study on Family Therapy for a Child with Anger Controling Problem)

  • This study aimed to explore the anger of child, the factors influencing the anger of child, the therapeutic techniques of therapist to resolve the marital conflict as well as the anger of child and the changes of family members after getting family therapy. The data was consisted of recording transcripts and note-taking of 8 sessions family therapy. The study used a constant comparative analysis and Miles & Huberman's matrix and network display as analysis method. The anger of child included aggressive and violent behavior, temperament, fear, anxiety, and negligence of rule. The study revealed parental dysfunctional communication patterns and parental experiences from family of origin as factors influencing the anger of child. The study found therapist's self-disclosure, sharing and comparing with similar cases, explanation of dysfunctional communication patterns, explanation about similarity in generational transmission process, and suggestion of new solutions as therapeutic techniques. The result of study showed the alleviation of marital conflict, the amelioration in the anger of child, and the change in the relationships of family members after getting family therapy.

위암환자 가족들의 경험세계에 관한 연구 (Lived Experience of the Family Members of Gastric Cancer Patients)

  • The purpose of the study is to explore and describe the lived experience of family members with gastric cancer patients using the grounded theory methodology. The participants were ten spouses of gastric cancer patients who had some kind of treatment at the hospital. They were asked open-ended and descriptive questions in order for them to talk about their experiences in their own terms. As the interveiw progressed the questions became more specific to discuss themes and working hypotheses that emerged from the analysis of previous interviews. All interviews were tape-recorded and transcribed for the analysis. Constant the core category that was emerged from the comparative analysis is “magmaggam” which can be described as a psychological distress due to a high level of uncertainty regarding the health of the patient and the future of the caregivers. Psychological distress includes several emotional feelings such as frustration. anxiety, fear, guilty, and self depreciation. Subcategories or strategies related to the core category are 1) managing illness, 2) using folk medicine, 3) giving the patient a reason to live, 4) being patient, 5) losing reality, 6) anticipatory experience on the patient's death and parting, and 7) changing interpersonal relationships. The results of this study would help clinical nurses to develop nursing intervention to help spouses of gastric cancer patients establish efficient coping strategies in dealing with the problems they face.

임상간호사에서 의료급여관리사로의 역할 전이 과정: '척박한 땅에 뿌리내리기' (Role Transition from Clinical Nurse to Case Manager for Medical-aid Beneficiaries: Taking Root in a Barren Land)

  • Purpose: The purpose of this study was to explain the role transition process to nurse case managers (NCMs) for Medical-aid beneficiaries in Korea. Methods: Fourteen NCMs were interviewed regarding their experiences of becoming proficient in the new role of case manger. Data were analyzed through the application of grounded theory. Results: 'Taking root in a barren land' was the core category explaining the role transition process of NCMs. They engaged in four stages: launching, trial and error, proficiency, and wait-and-see stages. NCMs showed not only fear but also passion for case management practice. Despite their passion and effort, NCMs went through a period of trial and error. After becoming skilled, NCMs went through a stage of wait-and-see often because of job insecurity related to temporary position or few opportunities for promotion. Factors influencing NCMs' role transition process included their understanding of client characteristics, belief in case management, and support from their colleagues and families. Conclusion: NCMs experience many challenges in the process of becoming proficient NCMs. To help with their role transition, there is a need for education programs, preceptorship programs, research on their roles and functions, and regulation for securing NCMs' employment and career stability.

고통(Suffering) 개념 분석 (Concept Analysis of Suffering)

  • There is a need to define the concept of suffering more appropriate in the context of Korean culture. This research is an attempt to reanalyze the concept of suffering by applying Walker & Avant's process of concept analysis. The steps of concept analysis are : 1. Select a concept. 2. Determine the aims or purpose of the analysis. 3. Identify all uses of the concept that you can discover. 4. Determine the defining attributes. 5. Construct model, contrary, borderline, related and invented cases. 6. Identify antecedents and consequences. 7. Define empirical referents. The concept of suffering can be defined as follows: Suffering is a fundamental and inevitable experience of all human beings. When each individual experiences loss, damage and pain which threaten one's personal integrity, suffering is perceived differently among each individual depending on their personal inner factors, one's significant others, exterior circumstances and stimuli, and the ultimate meaning of life. Suffering brings severe and unendurable distress which accompany despair, powerlessness, anxiety, bitterness, fear, anguish, guilt, depression, withdrawal and anger. The attributes of suffering were analyzed. Model case, contrary case, borderline case, related case, and invented case were described. Antecedents, consequences and empirical referents were explored. This concept analysis provides data for new insights of suffering. These findings suggests that appropriate nursing interventions aimed at relieving suffering of patients need to be developed and implemented more widely.

정신분석적 관점에서의 불안 (Psychoanalytical View of Anxiety)

  • By the influence of the descriptive approach of DSM-III, the anxiety became the same thing as the anxiety disorder to the clinicians. This unfortunate result sacrificed psychodynamic model of symptom formations and simplified the anxiety as one of the disease entity not as the overdetermined symptoms. These phenomenon awakened the psychoanalytic interest which was in sleep. Freud was the first major articulator of the basic significance of anxiety in human behavior. He attributed the particular quality of the anxiety experience to the trauma of birth, and subsequently to the fear of castration. Such classification of the anxiety according to the psychosexual development is helpful for the clinicians in understanding the origin of anxiety which the patient shows during the psychotherapy. The other analytical view of interpersonal psychoanalysis came from Sullivan. A large part of his therapy is taken up with recognizing and correcting parataxic distortions that interfere with realistic self-appraisal of events and of oneself in relation to others. Perhaps no explanation is the 'most basic' explanation for human anxiety. Anxiety is a multifaceted entity consisting of aspects of realm of discourse. Existential anxiety is inescapable in Western culture but it can be transcended by the cultivation of mind in Eastern culture. The analysts need to stay attuned to their own propensities for anxiety and must permit their own experiences with anxiety to be the grist for the psychotherapeutic mill.

통증학설에 대한 문헌적 고찰 (Literature consideration on pain theory)

  • Pain, which is the most significant issue for the physical therapist, is the cause of various diseases until it disappears, and results in a lot of obstructions to treatment. Pain is very complicated. It is a subjective symptom that informs of a pathologic condition in the body, and one of the unpleasant experiences that people have. It is accompanied with anxiety and fear. Many researchers including Krause(1987) have identified the pain mechanism based on pain perception for many centuries and they have suggested many theories as they believed that pain management was possible. Reviewing the contents of psychoanalysis, uncontrollable pain that can't be explained is described as a defense mechanism to an unconscious psychological conflict. That is, mental pain is transferred to the body and the pain becomes unbearable. What is important is, like Keefe(1992) said, that the pain experience itself is primary stress and one should cope with it, whatever the cause of the pain. This paper investigates the background of the psychological theory of pain. Based on the efforts of previous studies, the next research generation will understand the treatment process for pain more dearly and will contribute to the prevention and protection from pain that humans undergo.

  • 본 연구는 이·통장을 활용한 자살예방 게이트키퍼 사업 담당자의 경험에 대해 알아보고자 하였다. 연구 참여자는 2년 이상 자살예방 게이트키퍼 사업을 통해 이·통장과 상호작용을 하고 있는 사업담당자 3명으로 하였으며, 각자의 경험을 심층적으로 이해하기 위하여 질적 연구방법인 내러티브 연구를 활용하였다. 연구결과, 이들은 자살예방사업 담당자로서 부담과 두려움을 가지고 사업을 시작하였다. 사업 진행과정에서는 소속기관 내외부적으로 느끼는 부담과 스스로의 부족함이라는 무거운 짐을 느끼고 있었다. 하지만 이러한 어려움 속에서도 업무를 유지할 수 있었던 것은 가족과 동료, 참여 이·통장들의 지지가 있었기 때문이었다. 아울러 연구 참여자들은 본 사업이 활성화될 수 있는 방안을 제시하였다.

  • Every one experiences death one day, however no one can knows exactly what it is because people can not experience death until it comes, it is therefore impossible to judge correctly on the phenomenon of the death. On the whole, man experiences indirect death through the mass communications such as TV drama, fiction, magazine etc because those methods can easily access by every one. In addition to this, people usually acquire the negative awareness of death through the dramatic change of story like dying of cancer for dramatic effect by giving scare and fear to the cancers. The purpose of this study is to provide basic information on the spiritual care that enables the facing death patients to accept death as a part of life and divert hope from scare about after death by comparing and analyzing of two aspects of death meaning I.e, Korean fiction and the end stage cancer patients. Additionally, for medical staff to understand the facing death cancer patients by making to aware patients correctly and provide the better quality of care. The study was performed from September 28, 2002 to February, 28 2003. The materials of this study were collected by direct data obtained from observation, interviews, note and diary of end stage of cancer patients and written materials acquired from Korean contemporary fiction. Participants of this study were 4 end stage cancer patients including 2 lung cancer patients, 1 liver cancer patient and 1 esophagus cancer patient. The methodology used in this study was divided into two types; Huberman & Miles methodology was used for fiction to find and categorize subject, and Colaizzi, one of phenomenological methodology was used for end stage cancer patients to find the major meaning, subject and categorization. 1.The death investigated in the fiction, was found as a progress of negative emotion, acceptance and sublimation, life related subjects in the negative emotion were tenacity for life, anxiety, lingering attachment, responsibility, abandonment and death related subjects were shock, isolation, fear, scare and rejection. Acceptance related subjects were acceptance, destiny, secularism, preparation and arrangement, and sublimation related subjects were sublimation through Christian and Buddhism. 2.The death showed in the participants was negative emotion, acceptance and sublimation, life related subjects were repentance, anxiety, responsibility and hopelessness, and death related subjects were dejection, solitude, anger, fear and scare. The acceptance was a type of religious acceptance that admitted instantly by reaching an understanding with the God, and death was accepted as a progress of preparation, arrangement, acceptance and hope. Sublimation related subjects were Christian sublimation and relief or destiny incurred from self-reflective sublimation through communications and thoughts. 3.The death in view of fiction and participants were positively accepted both death and negative emotion, and the study disclosed the fact that death was sublimated dependent on religion. 4.The progress of negative emotion, acceptance and sublimation was disclosed more complicated and various in the real end stage cancer patients and acceptance only found in the patients on the form of religious acceptance, according to the results compared with fiction and real end stage cancer patients. The death showed in the fiction was standardized, gradated and similar progress with psychological status of Kubler-Ross. However, death in the participants was showed complex and various feelings simultaneously, and sometimes they accepted death positively. The sublimation through religion was found in Buddhism and Christian in the fiction and mostly Christian in the participants due to a number of Hospice patients. It was found that negative emotion various types of death was more found in the participants than fiction. It is therefore necessary to study on the response of death in various types. In the participants death was incurred more systematic and variously, we knew that nursing practice focused on experience of participants is required and reality on death is much profound than we analyzed and presented, lots of situations and reactions should be premised because we can not completely rule out the negligence possibility of care mediation of participants. In caring for the facing death patients, we discovered and confirmed again through this study that the spiritual care should be needed as a mediation method.

일 지방 간호대학생의 수도권 대학병원 임상실습 경험 (Clinical Practicum Experiences of Local Nursing College Studentsat a University Hospital in the Seoul Metropolitan Area)

  • 본 연구는 일 지방 간호대학생이 수도권 대학병원에서 임상실습을 하면서 어떤 경험을 하는가를 간호대학생의 시각에서 살펴보고자 수행되었다. 연구대상은 J시에 소재하는 간호대학 4학년 12명이었고 자료 수집은 면담 질문지를 사용하여 2017년 9월 29일부터 10월 12일까지였다. 수집된 자료는 Colaizzi의 현상학적 분석방법을 사용하여 분석하였다. 본 연구의 결과, 3개의 범주와 9개의 주제묶음으로 나타났다. '질 높은 실습교육'의 범주에는 '수도권 대학병원 실습에 대한 기대감', '체계적인 실습교육에 대한 만족감', '진로결정에 구체적 기준설정 기회'로 나타났다. '독립된 사회생활의 경험'은 '일상에서 벗어난 자유로움', '혼자 해결해야 함', '주위 인간관계의 소중함을 깨달음'이며 '체계적 실습생 지원 부족'은 '정보부족으로 인한 실습준비의 어려움', '경제적 지원 부족', '안전이 보장되지 않은 생활환경에 대한 두려움'으로 나타났다. 본 연구 결과는 일 지방 간호대학생이 수도권 대학병원에서 어떤 임상실습을 경험하는가를 파악하여 양질의 임상실습 교육환경을 모색할 수 있는 기초자료로 활용할 수 있을 것이다.

Appraisal of Breast Cancer Symptoms by Iranian Women: Entangled Cognitive, Emotional and Socio-Cultural Responses

  • Background: Breast cancer is the most common cancer in Iranian women and usually features delayed presentation and late diagnosis. Interpretation of symptoms, as the most important step, has a significant impact on patient delay in seeking treatment. There is a dearth of studies on symptom appraisal and the process leading to seeking help in breast cancer patients. This study explored the perceptions and experiences of Iranian women with self-detected possible breast cancer symptoms. Materials and Methods: A qualitative method was conducted involving in-depth semi-structured interviews with 27 Iranian women with self-discovered breast cancer symptoms. Participants were purposefully selected from women who attended Cancer Institute of Tehran University of Medical Sciences during June 2012 to August 2013. The audiotaped interviews were transcribed and analyzed using conventional content analysis with MAXQDA soft ware version 10. The trustworthiness of the study was verified by prolonged engagement, member validation of codes, and thick description. Results: The main concepts emerging from data analysis were categorized in four categories: symptom recognition, labeling of symptoms, interactive understanding, and confronting the fear of cancer. Symptom recognition through breast self-examination, symptom monitoring and employing prior knowledge distinguished normal from abnormal symptoms and accompanied with perception of being at risk of breast cancer led to symptom labeling. Social interaction by selective disclosure and receiving reassurance from a consultant led to confirmation or redefinition of the situation. Perceived seriousness of the situation and social meanings of breast cancer as a stigmatized and incurable illness associated with loss of femininity were reasons for patient worries and fear. Conclusions: This study emphasized that entangled cognitive, emotional and socio-cultural responses affecting understanding of symptom seriousness require further investigation. It is suggested that programs aimed at shortening patient delay in breast cancer should be focused on improving women's knowledge and self-awareness of breast cancer, in addition to correcting their social beliefs.
