사상사고에 노출된 기관사의 경험에 대한 현상학적 연구 (A Phenomenological Study on the Lived Experience of Train Operators Exposed to Person under Train (PUT))

  • Purpose: To understand the meaning of the lived experience with Person under train (PUT) for train or subway operators. Methods: The study was built on hermeneutic phenomenological themes from individual interviews of present-day train or subway operators in South Korea. Eight participants were selected to participate in the study. All qualitative data were analyzed using the heuristic guides of Van Manen. Results: Four fundamental lifeworld themes and eight sub themes emerged in the findings. The first theme of spatiality had'the place pressed by the darkness' and 'the train drags me there'. The second theme, corporeality had 'a foreboding fear', and 'debris of death that gets stuck in the whole body'. The temporality theme had'distorted time in chaos', and 'memory trapped in time'. Finally the last existential theme of relationality had 'intrapersonal encounter' and 'resentment and guilt'. Conclusion: The four existential lifeworld themes provided a framework for in-depth investigation of the operator's "lived experience." This leads to clear understanding of effects of PUT experience on related individuals. The findings imply that specific active nursing intervention strategies are necessary in order to treat affected train operators, and to prevent further issues in their work and private life.

ASIC2a-dependent increase of ASIC3 surface expression enhances the sustained component of the currents

  • Acid-sensing ion channels (ASICs) are proton-gated cation channels widely expressed in the nervous system. Proton sensing by ASICs has been known to mediate pain, mechanosensation, taste transduction, learning and memory, and fear. In this study, we investigated the differential subcellular localization of ASIC2a and ASIC3 in heterologous expression systems. While ASIC2a targeted the cell surface itself, ASIC3 was mostly accumulated in the ER with partial expression in the plasma membrane. However, when ASIC3 was co-expressed with ASIC2a, its surface expression was markedly increased. By using bimolecular fluorescence complementation (BiFC) assay, we confirmed the heteromeric association between ASIC2a and ASIC3 subunits. In addition, we observed that the ASIC2a-dependent surface trafficking of ASIC3 remarkably enhanced the sustained component of the currents. Our study demonstrates that ASIC2a can increase the membrane conductance sensitivity to protons by facilitating the surface expression of ASIC3 through herteromeric assembly.

  • Extracellular acidification occurs not only in pathological conditions such as inflammation and brain ischemia, but also in normal physiological conditions such as synaptic transmission. Acid-sensing ion channels (ASICs) can detect a broad range of physiological pH changes during pathological and synaptic cellular activities. ASICs are voltage-independent, proton-gated cation channels widely expressed throughout the central and peripheral nervous system. Activation of ASICs is involved in pain perception, synaptic plasticity, learning and memory, fear, ischemic neuronal injury, seizure termination, neuronal degeneration, and mechanosensation. Therefore, ASICs emerge as potential therapeutic targets for manipulating pain and neurological diseases. The activity of these channels can be regulated by many factors such as lactate, $Zn^{2+}$, and Phe-Met-Arg-Phe amide (FMRFamide)-like neuropeptides by interacting with the channel's large extracellular loop. ASICs are also modulated by G protein-coupled receptors such as CB1 cannabinoid receptors and 5-$HT_2$. This review focuses on the physiological roles of ASICs and the molecular mechanisms by which these channels are regulated.

질적연구 진술문을 이용한 중환자실 생존자의 감성분석 (Sentiment Analysis of the Quotations of Intensive Care Unit Survivors in Qualitative Studies)

  • Purpose : As the intensive care unit (ICU) survival rate increases, interest in the lives of ICU survivors has also been increasing. The purpose of this study was to identify the sentiment of ICU survivors. Method : The author analyzed the quotations from previous qualitative studies related to ICU survivors; a total of 1,074 sentences comprising 429 quotations from 25 relevant studies were analyzed. A word cloud created in the R program was utilized to identify the most frequent adjectives used, and sentiment and emotional scores were calculated using the Artificial Intelligence (AI) program. Results : The 10 adjectives that appeared the most in the quotations were 'difficult', 'different', 'normal', 'able', 'hard', 'bad', 'ill', 'better', 'weak', and 'afraid', in order of decreasing occurrence. The mean sentiment score was negative ($-.31{\pm}.23$), and the three emotions with the highest score were 'sadness'($.52{\pm}.13$), 'joy'($.35{\pm}.22$), and 'fear'($.30{\pm}.25$). Conclusion : The natural language processing of AI used in this study is a relatively new method. As such, it is necessary to refine the methodology through repeated research in various nursing fields. In addition, further studies on nursing interventions that improve the coherency of ICU memory of survivors and familial support for the ICU survivors are needed.

Benjaminian Ruskin: Redemptive Myth and Modernity

  • The Queen of the Air, John Ruskin-s highly elliptical publication of 1869, elaborates a complex mythology as a way of responding to the prevalence of scientific thinking, widespread environmental degradation, the pernicious effects of political economy, and mechanistic labor. Benjamin-s desire to rescue human experience from prevailing scientific conceptions is reminiscent of Ruskin-s fear that the peculiar power that shapes the unities of the natural world is simultaneously being "beaten down by the philosophers into a metal or evolved by them into a gas" and obscured by the dreams and theories of philosophers and theologians. As a critic remarks, in Benjamin-s-and, we would add, Ruskin-s-view, "what the modern era lacked was a basis for continuity which would prevent experience from disintegrating into a desultory and meaningless series of events." Despite its frenetic hyper-associativity, then, The Queen of the Air contains a key element that Benjamin believes is necessary for "redemption": the desire for a new form of consciousness that recognizes links to the past and thus to the longings and dreams of our forebears. Thus, although Ruskin most immediately influences Proust, who in turn influences Benjamin, Benjamin-s thought is far more Ruskinian than critics have heretofore observed. Just as Benjamin helps us make sense of the ways in which The Queen of the Air is caught in the grip of the shocking associativity of modern life, so Ruskin assists us in discerning similar impulses in Benjamin-s attraction to a form of archaic consciousness that can, by altering the modern form of perception, reenchant the present.

심장수술환아의 ICU에서의 스트레스원과 심리적 손상반응에 관한 탐색적 연구 (Stressors of Post Cardiac Surgery Children in the ICU and Their Impaired Psychological Responses)

  • The purpose of this study was to explore the stressors in pediatric intensive care unit and impaired psychological responses of children after open heart surgery. Sixteen children aged 6 to 11, who were admitted to the hospital for open heart surgery during the period from July, 1991 to February, 1992 were the subjects of the study. Observations, drawings, and interviews were used to collect data for study. Behavioral responses about intensive care unit phenomena were analyzed according to the 4 categories identified to assess children's perceived stressors in ICU. Impaired psychological responses were examined using observational and interview data. Drawings were analyzed by content and color by this researcher and validated by the psychologist. The findings were as follows : 1. The most frequently perceived stressors by children in ICU were the physical stressors causing pain and discomfort(68.5%). It was followed by social stressors which denote disruption of relationship with family and friends(13.0% ), environmental stressors which denote unfamiliar surroundings, noise, staff, and other patients (11.2%), and psychological stressors which denote factors affecting self-esteem such as inability to communicate and inadquate knowledge of the situation (7.3%). 2. 81.3% of the children showed of least one of the impaired psychological responses. Three children (18.8%) experienced time disorientation. An equal number of subject experienced perceptual illusion. Two children(12.5%) experienced hallucination. Vivid dream about ICU phenomena was reported by 2 children. Seven children(43.8%) were identified as having exaggerated fear. They feared about oxygen mask and ICU environment in general. Seven children(43.8%) experienced impairment of memory about treatments, procedures, and environment of ICU. 3. The analysis of children's drawing revealed that 56.3% of children experienced fear in the ICU. 75% of children included nurses in the drawings and 62.5% of children drew other children. 81.3% of children drew and identified himself in the drawing. The colors used most by children were green, purple, and brown. From this result, it is recommended to prepare children before open heart surgery to reduce their stressors and impaired psychological responses in ICU.

청소년의 약물남용 경험 (The Adolescents' Experience in Drug Abuse)

  • This study was to provide understanding for the juvenile problem of drug brought about by industrialization, and the importance people that we should take a role in helping to resolve the problem. The purpose of this study lies in approaching the effective methods to prevent Korean adolescents from being involved in drug by examining the factors of their experience. The method of this study is designed to grasp the adolescents' drug experience, its meaning, its structure and the nursing intervention ways. By applying a phenomenological approach which is based on a qualitative research method, the researchers hoped to reveal key data in each group. It has been carried on from October 1998 until September 1999 with sample of 5 adolescent inmates in K mental hospital located in Seoul. The data were collected by sereous personal deep interviews and observations. Collected data were analyzed according to the phenomenological method suggested by Giorgi. The results of this study are as follows: First, pleasure seeking: curiosity, joy, and fun; Second, affiliation: formation of clique with the same age; Third, escape from reality: loneliness, emptiness, heroic mind, self- confidence, self-absorption, and comfort; Forth, feeble- mindedness: anxiety, fear, and temptation; Fifth, psychological unadaptation: wandering, forfeiture, and incompetence; Sixth, physical counteraction: headache, vomiting, loss of memory, loss of appetite and loss of sensibility; Seventh, psychological confusion: illusion, auditory hallucination and dream; Eighth, recovery of self-consciousness: repentance, desire and awareness This study results in the following suggestions of the nursing intervention 1. To cultivate a wholesome game culture for the adolescents. 2. To make up the group activities so that the adolescents can establish their sense of identity through a variety of group counseling program. 3. To foster the capability to cope with the change in the process of social accommodation. 4. To enforce the social life skill for the adolescents. 5. To practice the systematic education about drug use. 6. To develop the social education programs. 7. To help the adolescents build up their sense of identity by applying the adolescent cure programs. Above all, it is necessary that school and community as well as home should establish the educational environment paying special concern and consideration to the adolescents.

혈액투석 환자들이 경험한 건강문제 분석 (An Analysis of Health Problems Experienced by the Clients Receiving Hemodialysis)

  • The purpose of this study was to discover practical health problems which hemodialysis clients experienced so as to present basic data for development of a health assessment tool. The research subjects were 70 clients receiving hemodialysis in Seoul and Inchon from Mar. 1996 to Sep. 1996. Data were collected by researcher's informal indepth interview and nurses' open ended question. Content analysis was applied to collect similar contents and common experiences in order to derivate concepts and categories for better understanding of hemodialysis clients' experiences. As a result, 9 categories derivated to identify the health problems of clients receiving hemodialysis were as follows : 1) They experienced 'the decreased digestive function' which contained the changed appetite, nausea, vomiting, constipation and diarrhea. 2) They experienced 'the decreased respiratory and circulatory function' which contained dyspnea, changed blood pressure, tingling sensation and the fear of aggrevated vascular condition. 3) They experienced 'the aggrevated oral condition' which contained dry mouth and destruction of teeth and their soft tissue. 4) They experienced 'the decreased sensory function' which contained visual disturbances, sensation difficulty, and hearing loss. 5) They experienced 'the aggrevated skin condition' which contained dark brown skin color, dry skin (and hyperpigmentatic freckle, seborrheric keratosis, scale), itching sense, and alopethia. 6) They experienced 'the decreased urinary reproductive function' which contained anuria or oliguria, dysmenorrhea, sterility and decreased libido. 7) They experienced 'the restricted activity' which contained decreased activity, muscle cramp and stiffness of joint. 8) They experienced 'the changed mental status' which contained memory disturbance, decreased cognition, disorientation, neurosis and psychosis. 9) They experienced 'the aggrevated general condition' which contained kyphosis, weight loss, fatigue, sleep disturbance, bleeding tendency, inflammation, generalized edema and foul oder of uremia.

북한이탈주민의 남한이주 과정의 외상적 체험에 대한 현상학적 연구 (A Phenomenological Approach to Traumatic Experiences Among North Korean Defectors)

  • 본 연구의 목적은 북한이탈주민들이 남한이주 과정에서 경험한 외상 사건의 의미와 본질이 무엇인지를 이해하는 것이었다. 이를 위하여 현상학적 연구방법으로 접근하였으며, 연구 참여자는 탈북과정에서 고문, 공포, 폭행, 기아, 질병 등의 외상을 경험한 7명의 북한이탈주민들이었다. 연구결과, 6개의 주제묶음(Theme clusters)을 '삶과 죽음의 경계에서의 몸부림', '살아남음과 살아가는 것의 차이'로 범주화 하는 과정에서 연구 참여자들의 남한이주 과정에서의 외상은 수년이 지난 현재와 연결되어 있음을 발견하였다. 연구 참여자들이 외상 사건을 경험하며 살아남은 과정에서 각인된 이미지와 기억은 수 년 동안 지속되어 남한에서의 일상생활과 관련을 맺고 있었다. 본 연구를 통하여 북한이탈주민들의 부정적 특성이라고 간주되었던 일부 요인들이 외상을 경험한 개인들에게는 피할 수 없는 개별적 특수성임을 알 수 있었다.

W. H. 오든의 시학과 정치성 (W. H. Auden's Poetics and the Political)

  • Controversies over W. H. Auden's "political" poetry remind us of an old but perhaps never easily resolved problem about the relationship between poetry (literature) and politics. Auden has arguably been referred to as a "leftist" or "Marxist" because of his political viewpoint registered in "Spain" or "September 1, 1939," which embodies his contemporaries' loss and fear, brought by the socio-political turmoil, economic depression, and moral conflicts of the 1930s. Interestingly, however, Auden is known to have an ambivalent position toward the political reality. He once disavowed the above "political" poems as "dishonest" in the preface of the 1966 edition of Collected Poems and declared, "poetry makes nothing happen" in "In Memory of W. B. Yeats," which seemingly acknowledges the political incapacity of poetry. Auden's position and poetry should be understood as the result of complicated interactions between his perspectives on society, human beings, and poetics. Auden definitely believed in the role of poetry in such a politically demanding time, yet was not concerned with the anticipation of certain immediate changes effected by poetry in real situations. Instead, he sought the intellectual and moral effects that poetry could give his readers to help them survive the dismal circumstances of the 1930s. This is what distinguished Auden's poetry from political propaganda. In doing so, Auden's poetry captures the zeitgeist of his generation and has privileged him as the leading voice of his time, but it has also encouraged the following generations to confront different socio-political difficulties. This is something poetry can make happen politically, and the survival of Auden's "memorable speech" proves the legitimacy of his frequently misunderstood poetics.
