Purpose: This study was conducted to describe the effects of aroma inhalation on sleep and fatigue in night shift nurses. Methods: The research design was one group pre-posttest design. The subjects were 17 night shift nurses who work at neurosurgery intensive care unit of K hospital of C university in Seoul. During three consequent night shifts, the participants were worn the Actigraph to measure the sleep pattern on their wrist since their first night shift work. The quality of sleep and fatigue was measured by survey when they awoke at 6 pm after second night shift work. And these measures were repeated during the night shifts in second and third week. Aroma inhalation was applied before going to bed after finishing each night shift work during second and third week. SPSS 12.0 program was used for data analysis. Results: There were no statistically significant differences in sleep latency and total sleeping time. However, significant differences were found in the sleep efficiency, awakening number, sleep quality, subjective symptom and intensity of fatigue. Conclusion: These results showed that aroma inhalation had positive effects on the pattern and quality of sleep and reducing fatigue in night shift nurses.
Purpose : The aim of this study was to compare shift satisfaction, sleep, fatigue, quality of life (QOL), and patient safety incidents between a newly implemented two-shift system and a traditional three-shift system. Methods : A total of 127 intensive care unit nurses (48 two-shift nurses and 79 three-shift nurses) working in a tertiary hospital in Seoul were recruited from January 1, 2017, to March 31, 2017. They completed a self-reported questionnaire about their work hours, shift satisfaction, sleep patterns, sleep quality, fatigue, QOL, and patient safety incidents in the past 2 weeks. Data were analyzed using SPSS version 23.0. Results : The two-shift group showed higher shift satisfaction scores compared with the three-shift group (6.93 vs. 4.37, p<.001). Sleep latency was shorter and sleep quality was better in the two-shift group compared with the three-shift group. There were no significant differences in other sleep parameters, fatigue, QOL, and patient safety incidents between the two groups. Conclusion : Although a two-shift system did not improve nurses' fatigue or QOL in this study, it may effectively serve as an alternative shift-work system that can increase sleep quality and shift satisfaction without increasing patient safety incidents.
This study was to investigate the level of fatigue and its associated variables in patients with systemic lupus erythematosus(SLE) in Korea. From March to June, 2001, 100 patients, registered at one university hospital rheumatology clinic in Seoul, were accepted as subjects for this study. The sampling method was adopted a non-probability, purposive technique. The instruments used for this study were The Multidimensional Assessment Fatigue scale developed by Tack and Beck Depression Instrument develped by Beck. The collected data were analyzed by SAS program using t-test, ANOVA with Scheffe-test, Pearson correlation coefficients and stepwise multiple regression. The results were as follows: 1. Total scores of fatigue of the subjects averaged $24.46(\pm10.85)$, degree of fatigue was $5.08(\pm2.29)$, and influence of fatigue was $3.52(\pm2.12)$. 2. Regarding characteristics, more depressive(p=.0001) and more painfuI(p=.0122) patients revealed more fatigue. Also, the subjects with spouse(p=.0337) and having poor quality of sleep(p=.0445) revealed more fatigue. 3. The subjects' total fatigue score, depression, pain and age was correlated positively(r=.53; r=.48; r=.24), and total fatigue score, and exercise time, quality of sleep was correlated negatively(r=-.45; r=-.21). 4. The main influencing factors on the fatigue were depression$(52.92\%)$ and quality of sleep $(8.10\%)$. These two main variables made it possible to explain $61.02\%$ of the varience in fatigue. In conclusion, this study revealed depression and quality of sleep is an important factor that can improve quality of life in patients with SLE. It is recommended that nursing intervention for SLE patients would be focused to decrease depression and to enhance quality of sleep.
본 연구는 간호대학생들의 수면의 질을 조사하고, 그에 영향을 미치는 요인을 확인하기 위한 서술적 조사연구이다. 설문도구는 피로증상척도, 한글판 스트레스 자각척도, 한국판 피처버그 수면의 질 평가척도를 사용하였으며, 수면의 질 도구에서 제시한 절단점 점수를 기준으로 비숙면군과 숙면군으로 나누어 분석하였다. 대상자들의 수면의 질은 비숙면군 30.5%, 숙면군 69.5%이며, 피로는 4.39점 vs. 4.27점(t=577, p=.564), 스트레스는 22.2점 vs. 19.0점(t=3.565, p=.000), 수면의 질은 11.2점 vs. 5.7점(t=20.375, p=.000)이었다. 두 군간 수면의 질의 하위 영역 7개는 모두 유의한 차이가 나타나, 비숙면군이 숙면군보다 피로가 높고, 수면의 질이 낮은 것으로 나타났다. 수면의 질에 영향을 미치는 요인으로는 잠자리에 드는 시간과 스트레스로 설명력은 22.1%로 나타났다. 간호대학생들의 적절한 수면을 위한 중재프로그램 계획시 잠자리에 드는 시간과 스트레스 관리를 위한 전략이 필요하다.
Purpose: The purpose of this study was to identify associations among the sleep disturbance, fatigue, job stress, and blood and body fluid (BBF) exposure of shift-work nurses. Methods: A total of 299 shift-work nurses from two tertiary hospitals were enrolled in this study. We used the Insomnia Severity Index (ISI), and the Fatigue Severity Scale (FSS), and the Korean Occupational Stress Scale-Short Form (KOSS-SF) to evaluate sleep disturbance, fatigue, and job stress, respectively. The data were analyzed using t-test or chi-squared test and Logistic regression analysis using the SPSS 23.0 program. Results: We found that 43.8% of participants reported BBF exposure over the past year. Splash or exposure to broken skin of BBF were most frequent (56.9%), and followed by needlestick injuries (30.4%) and sharp injuries (12.8%). Age, hospital, working period, level of stress, sleep disturbance ($ISI{\geq}15$), fatigue (FSS $score{\geq}4$), job demand and organizational climate subset in KOSS-SF were significantly associated with BBF exposure in shift-work nurses. In multivariate analysis after adjusting age and hospital, the risk factors of BBF exposure in shift-work nurses were the level of stress and fatigue (FSS $score{\geq}4$). Conclusion: Fatigue and job stress were related to BBF exposure in shift-work nurses. Our results suggest that management of sleep disturbance, fatigue, and high job stress in shift-work nurses is needed to reduce risk of BBF exposure.
Pilots' fatigue is one of the most serious threat to flying safety. Fatigue is influenced by many factors like sleep deprivation, daily activities, aging, stress, etc. This study was designed to investigate military pilots' daily activities. 20 pilots' daily activity was monitored by activity monitor, Fitbit(R), and the survey about sleep time and flight time was conducted. They placed Fitbit(R) on their waist for 5 days. During flight, they removed Fitbit(R) that had been used for checking their steps and walking distances. It was found that the average sleep time is 6.7 hours for fighter pilots. It was a little shorter than average sleep time for adults, 8 hours. Average steps per day was 6,838 which is more than sedentary worker's but less than active worker's. Their daily activities were not as high as we had expected. But their sleep time was very short. Flight surgeons should recommend them to take a rest and get sleep during rest period.
Purpose: The purpose of the study is to investigate the degree of fatigue and its related factors in women with rheumatoid arthritis. Method: The subjects were 143 patients with rheumatoid arthritis. Data were collected by questionnaires including Multi-Dimensional Assessment of Fatigue(Tack, 1991), Korean Health Assessment Questionnaire(Bae et al., 1991), numeric scale of pain(Lee & Song, 1987), Center for Epidemiologic StudiesDepression, and Korean Sleep Scale(Oh, et al., 1998). The data were analyzed by SPSS WIN 12.0 program using descriptive statistics, Pearson Correlation, and Stepwise Multiple Regression. Result: The result were as follows. 1. The scores of fatigue of subjects averaged $4.95{\pm}1.83$, degree of fatigue was $5.85{\pm}1.98$, and influence of fatigue was $4.04{\pm}2.09$. 2. The mean score of the degree of physical dysfunction, pain, sleep disorder, and depression were $1.42{\pm}0.38,\;8.15{\pm}3.58,\;1.86{\pm}0.67,\;and\;1.85{\pm}0.46$ points respectively. 3. The subject's total fatigue score, physical dysfunction, pain, sleep disorder, and depression was correlated positively(r=.44, r=.28, r=.29, r=.27, p< .01). 4. The main influencing factors on the fatigue were physical dysfunction and sleep disorder. These two main variables made it possible to explain 23.0% of the variance in fatigue. Conclusion: Therefore, nursing interventions for fatigue experienced women with rheumatoid arthritis would be focused to decrease physical dysfunction and sleep disorder.
Purpose: This study aims to investigate the factors affecting fatigue of care helpers in long-term care facilities. Methods: The subjects consisted of 131 care helpers at six long-term care facilities in Chungcheongnam-do. A survey was conducted between July and September 2019 to collect data, which were analyzed using t-test, ANOVA, Pearson's correlation coefficients, and multiple regression analysis.Results: An increase in fatigue of care helpers owing to taking care of over 10 patients a day, increase in emotional labor and sleep disturbance, and a decrease in economic status was observed. Their fatigue decreased when there was an increase in their subjective health status and job satisfaction. Work-related stress (β=.37, p<.001), emotional labor (β=.21, p=.014), and sleep disturbance (β=.18, p=.026) significantly predicted fatigue. These factors explained 37% of the total variance in fatigue (F=10.48, p<.001).Conclusion: In addition to improving the work environment, interventions that alleviate emotional labor and improve sleep quality, are needed to lower the fatigue of care helpers.
Objectives : Daytime drowsiness or sedation and changes in night sleep are commonly seen in patients treated with clozapine. There is, however, very limited information on their degree and nature during the course of treatment. The purpose of this study was to understand the sleep patterns in chronic schizophrenic patients with clozapine treatment over a period of 24 weeks. Method : The sleep pattern was evaluated using a set of 5-point scale questionnaire, to record subjective impressions of the night sleep induction, maintenance and quality, and daytime drowsiness and fatigue. In addition, unusual experiences associated with night sleep were recorded. The sleep questionnaire was repeatedly administered at baseline and at 1, 2, 4, 8, 12 and 24 weeks of drug treatment. At present, data on 12 patients has been collected. Results : All the components of night sleep were significantly improved in the 1st through the 12th week after treatment with clozapine. Daytime drowsiness was significantly higher in the 1st to the 2nd week after the treatment and fatigue was also significantly higher in the 1st to the 4th week after the treatment. Eight patients experienced noticeable increases in salivation during night sleep, and of these, one also reported frequent nocturnal urination and even enuresis. However, all these adverse factors did not affect the major sleep patterns. Conclusions : These findings suggest that the beneficial effects of clozapine on night sleep might last much longer than the undesirable effect of daytime drowsiness and fatigue. In other words, tolerance of the hypnotic action of clozapine might develop late and tolerance of the daytime drowsiness and fatigue might be evident earlier.
목 적 : 저자들은 40시간의 전수면박탈을 시행하여 수면 박탈이 정상인의 불안, 기분, 졸리움, 피로감에 어떠한 영향을 주는 지를 평가하고, 동시에 개인의 평소의 불안 성향이 수면박탈에 의한 상태불안, 기분, 졸리움, 피로감에 어떠한 영향을 주는가를 연구하고자 하였다. 방 법 : 대학생 중 정신적, 신체적으로 건강하며 수면각성주기에 이상이 없는 자원자 29명(남:여=22:7, 평균 연령=$24.59{\pm}1.35$)을 대상으로 하였다. 피험자들은 전날 충분한 수면을 취한 후 기상하였으며 이후 다음날 오후 10시까지 총 40시간동안 수면박탈을 시행하였다. 상태-특성불안척도, 일반기분척도, 스텐포드 졸리움 척도, 피로도 설문을 3시간 간격으로 시행하였으며 이의 시간에 따른 변화를 분석하였고, 또한 피험자의 평소 불안성향에 따라서 상태불안, 기분, 졸리움, 피로도의 변화에 어떠한 차이가 있는 지를 살펴보았다. 결 과 : 수면박탈에 따라서 상태불안은 증가하며, 기분은 부정적이 되며, 졸리움과 피로도가 증가하였다(p<.001). 특성불안이 높은 군과 낮은 군을 나누어 비교하였을 때, 상태 불안은 특성불안이 높은 군과 낮은 군간에 차이가 있었으나(p<.05) 상태불안의 증가 양상의 차이가 있는 것은 아니었다. 기분은 특성불안이 높은 군과 낮은 군간에 차이가 없었으나(p>.05) 기분의 변화의 양상과 특성불안사이에는 교호작용이 있어(p<.05), 특성불안이 높은 군에서 수면박탈에 따라서 기분의 악화가 심하였다. 또한 수면박탈에 의한 상태불안, 부정적 정서, 졸리움, 피로도의 변화는 새벽 4시에서 아침 7시 사이에 심한 악화를 보였다. 결 론 : 본 연구의 결과는 수면박탈이 졸리움을 유발하고, 부정적인 기분과 불안감 및 피로도를 증가시킨다는 것을 보여준다. 개인적 특성으로서의 특성불안의 수준에 따라 수면박탈에 의한 일반적 기분상태의 악화가 보여진다. 또한 수면박탈에 의한 상태불안, 부정적 정서, 졸리움, 피로도의 변화는 일주기 리듬의 영향으로 새벽에 심한 악화를 보여 실제생활에서 주의를 요한다.
