사람에 있어서 단기간의 절식이 운동에너지원의 이용양상에 미치는 영향 (Effect of Short Termed Fasting on the Usage Patterns of Metabolic Energy Sources during Exercise in Man)

  • 본 실험의 결과를 요약하면 다음과 같다. 혈중유리지방산은 정상식이시 비운동선수군에서 운동부하시 증가하며 그증가의 정도는 운동부하량과 비례적인 관계를 보였으며 운동선수군에서도 같은 경향이었다. 18시간의 단기간 절식시에는 비운동선수군 및 운동선수군에서 정상식이시에 비하여 2.5배의 증가를 보였으나 비운동선수군은 운동부하량에 따라 증가한데 비하여 운동선수군에서는 변화가 없었다. 혈당은 정상식이시 비운동선수군 및 운동선수군에서 운동부하량에 비례하여 증가했으나 절식시에는 증가의 정도가 완만하였다. 혈중 중성지방 및 콜레스테롤은 비운동선수군 및 운동선수군에서 운동부하에 의한 변화는 없었으며 절식시 중성지방은 정상식이에 비하여 양군에서 현저히 감소하였다. 분당 심박동수는 식이시에 비운동선수군에서 운동부하량에 비례하여 증가하였으며 운동선수군은 비운동선수군보다 완만히 증가하였다. 절식시에는 비운동선수군에서 운동부하량에 비례하여 증가했으나 운동선수군은 운동부하초기에 증가한 후 변화가 없었으며 양군 모두 절식시에 정상식이에 비하여 운동부하량에 따른 증가의 정도가 완만하였다. 운동선수군에서 비운동선수군에 비하여 분당 맥박수가 현저히 낮았으며 운동부하량에 따른 증가용도 낮았다. 이상의 결과에서 단기간의 절식은 운동부하전 및 운동부하시에 정상식이에 비하여 유리지방산의 이용을 증대시켰으며 또한 운동선수군에서의 혈중 유리지빙산, 글루코스 및 심박동수의 변화 양상이 절식시의 그것과 유사한 것으로 보아 단기간의 절식이 체력단련과 비슷한 효과를 가져오는 것으로 사료된다.

Prevention of Olanzapine-induced Toxicities of Weight Gain and Inflammatory Reactions by Coadministration with Green Tea or its Major Component Phenolic Epigallocatechin 3-Gallate in Mouse

  • Chronic treatment with olanzapine (OLZ), an atypical antipsychotic drug, is associated with the adverse effects of weight gain, hyperglycemia and/or hypertriglyceridemia. Green tea or epigallocatechin gallate (EGCG), one of the most abundant green tea polyphenols, significantly reduces or prevents an increase in glucose levels, lipid markers and/or body weight. We hypothesized that combined treatment with OLZ and green tea extract (GTE) or EGCG may prevent body weight gain and increase of the lipid markers. ICR male mice weighing an average of 30.51 g (n=32) at the beginning of the experiment were used. OLZ, OLZ+GTE and OLZ+EGCG were administered for 27 d in the drinking water, and then the levels of fasting glucose, nitric oxide (NO), and a typical lipid marker triglyceride (TG) were determined in plasma. The body weight and food intake were also compared. The chronic treatment of OLZ increased the average body weight compared with that of controls. In the presence of GTE or EGCG, the OLZ-induced increase in body weight was significantly prevented. Furthermore, in the OLZ group, the plasma levels of glucose, NO and TG were significantly increased, whereas GTE or EGCG prevented these increases. These results implicate that OLZ may induce systematic inflammatory reaction, and suggest that GTE or EGCG can protect against OLZinduced weight gain, hyperglycemia and hypertriglyceridemia.

Characterization of Plasma Carnitine Level in Obese Adolescent Korean Women

  • Carnitine is known to be involved in lipid metabolism and affects body composition as well as energy metabolism of the whole body. Improvement of obesity by L-carnitine supplement suggests that obesity can be related with the abnormality of carnitine metabolism and therefore, plasma carnitine level in normal and obesity groups was investigated. For the characterization of plasma carnitine level in obese people, 60 plasma samples collected from Korean women subjects were analyzed using LC/MS and plasma fatty acid level was also determined using GC/MS. Additionally, several clinical chemical parameters including fasting glucose, cholesterol, AST, and ALT level were measured. All the data obtained were combined and pattern recognition analysis was carried out with the dataset. Obese group showed a different metabolic pattern compared with normal group. Plasma acylcarnitine level of the obese group was found to be $11.7{\mu}g/ml$, which was higher than that of normal group ($8.0{\mu}g/ml$). Statistically significant differences in plasma fatty acid level were not observed between the two groups. Other clinical parameters for the obese group were within normal ranges but AST and ALT levels were slightly elevated compared to normal group. The obese group showed elevated plasma acylcarnitine level.

Anti-diabetic Effect and Mechanism of Korean Red Ginseng in C57BL/KsJ db/db Mice

  • The present study was designed to investigate the anti-diabetic effect and mechanism of Korean red ginseng in C57BL/KsJ db/db mice. The db/db mice were divided into three groups: diabetic control group (DC), Korean red ginseng group (KRG, 100 mg/kg) and metformin group (MET, 300 mg/kg), and treated with drugs once per day for 10 weeks. Compared to the DC group, fasting blood glucose levels were decreased by 19.8% in KRG-, 67.7% in MET-treated group. With decreased plasma glucose and insulin levels, the insulin resistance index of the KRG-treated group was reduced by 27.6% compared to the DC group. The HbA1c levels in KRG and MET-treated groups were also decreased by 11.0% and 18.9% compared to that of DC group, respectively. Plasma triglyceride and non-esterified fatty acid levels were decreased by 18.8% and 16.8%, respectively, and plasma adiponectin and leptin levels were increased by 20.6% and 12.1%, respectively, in the KRG-treated group compared to those in DC group. Histological analyses of the liver and fat tissue of mice treated with KRG revealed significantly decreased number of lipid droplets and decreased size of adipocytes compared to the DC group. From the pancreatic islet double-immunofluorescence staining, we observed KRG has increased insulin contents, but decreased glucagon production. To elucidate action mechanism of KRG, effects on AMP-activated protein kinase (AMPK) and its downstream target proteins responsible for fatty acid oxidation and gluconeogenesis were explored in the liver. KRG activated AMPK and acetyl-coA carboxylase (ACC) phosphorylations, resulting in stimulation of fatty acid oxidation. KRG also caused to down regulation of SREBP1a and its target gene expressions such as FAS, SCD1 and GPAT. In summary, our results suggest that KRG exerted the anti-diabetic effect through AMPK activation in the liver of db/db mice.

서울지역 일부 성인 여성에서 혈청 Ferric Reducing Antioxidant Power와 대사 위험요인간의 상관성에 대한 연구 (Relationship between Ferric Reducing Antioxidant Power and Metabolic Risk Factors in Korean Women Living in Seoul)

  • The present study was conducted to examine metabolic risk factors and total antioxidant capacity (TAC) of Korean females living in Seoul and to investigate the relationship between the metabolic risk factors and serum TAC. A total of 353 females aged between 20 and 64 participated in the study. Obesity indicators, blood pressure, serum lipid profile and fasting blood glucose were measured as metabolic risk factors. Ferric reducing antioxidant power (FRAP) assay was employed to determine serum TAC of subjects. Obesity indicators such as body mass index, waist circumference and waist-hip ratio were significantly higher in the participants aged $\geq$ 50 y (older group) than in the participants aged 20-49 y (younger group) (p < 0.001). Blood pressure, serum total cholesterol (TC), triglyceride (TG) and fasting blood glucose were also significantly higher in the older group than in the younger group (p < 0.001), demonstrating significant positive correlations between age and MS risk factors. The association between FRAP and MS risk factors were also investigated. FRAP values showed significant positive correlations with age (p = 0.001), serum TG (p = 0.002) and TC (p = 0.03). A tendency of positive association between FRAP and waist circumference was observed without any significant difference (p = 0.06). Increased serum FRAP with central obesity and serum lipids may be interpreted as results of activation of antioxidant defense system against oxidative stress induced by metabolic syndrome (MS) constituent factors. However, to verify the function of FRAP as a potential biomarker of susceptibility to MS various contributors to the plasma antioxidant capacity and their biological relevance related to MS should be elucidated further.

합병증을 동반하지 않은 2형 당뇨병에 대한 갈근황금황련탕의 혈당개선 효과와 안전성 연구 : 체계적 문헌고찰과 메타분석 (A Study on the Blood Sugar Improvement Effect and Safety of Galgunhwangryunhwanggum-tang for Type 2 Diabetes without Complications: A Systemic Review and Meta-Analysis)

  • Objectives: The purpose of this study is to assess the antidiabetic effect and safety of Galgunhwangryunhwanggum-tang for type 2 diabetes without complications by analyzing related research. Methods: For a systematic review and meta-analysis, we searched for the antidiabetic effect and safety of Galgunhwangryunhwanggum-tang for type 2 diabetes without complications in 10 databases up to September 2021. Only randomized controlled trials were chosen. Results: In the treatment effectiveness analysis and meta-analysis, Galgunhwangryunhwanggum-tang had significant improvement effects on fasting plasma glucose level, 2-hour postprandial glucose level, glycated hemoglobin, fasting insulin, and homeostasis model assessment for insulin resistance compared to the control group when treated in parallel with oral glycemic drugs. Conclusion: Galgunhwangryunhwanggum-tang is effective in improving blood sugar and insulin resistance in type 2 diabetes patients without complications and can especially be considered in parallel treatment with oral hypoglycemic drugs. A large-scale randomized controlled clinical trial is required to complement the limitations presented in this study in the future.

Effects of starvation-induced negative energy balance on endoplasmic reticulum stress in the liver of cows

  • Objective: Endoplasmic reticulum (ER) stress engages the unfolded protein response (UPR) that serves as an important mechanism for modulating hepatic fatty acid oxidation and lipogenesis. Chronic fasting in mice induced the UPR activation to regulate lipid metabolism. However, there is no direct evidence of whether negative energy balance (NEB) induces ER stress in the liver of cows. This study aimed to elucidate the relationship between the NEB attributed to feed deprivation and ER stress in bovine hepatocytes. Methods: Blood samples and liver biopsy tissues were collected from 6 non-lactating cows before and after their starvation for 48 h. The blood non-esterified fatty acids (NEFA), β-hydroxybutyric acid (BHBA) and glucose level were analyzed. Real-time quantitative polymerase chain reaction and Western blotting were used to explore the regulation of genes associated with UPR and lipid metabolism. Results: The starvation increased the plasma BHBA and NEFA levels and decreased the glucose level. Additionally, the starvation caused significant increases in the mRNA expression level of spliced X-box binding protein 1 (XBP1s) and the protein level of phosphorylated inositol-requiring kinase 1 alpha (p-IRE1α; an upstream protein of XBP1) in the liver. The mRNA expression levels of peroxisome proliferator-activated receptor alpha and its target fatty acid oxidation- and ketogenesis-related genes were significantly upregulated by the starvation-mediated NEB. Furthermore, we found that the mRNA expression levels of lipogenic genes were not significantly changed after starvation. Conclusion: These findings suggest that in the initial stage of NEB in dairy cows, the liver coordinates an adaptive response by activating the IRE1 arm of the UPR to enhance ketogenesis, thereby avoiding a fatty liver status.

치주질환과 당뇨병의 관련성 (An Association of Periodontitis and Diabetes)

  • 2009년 제 4기 3차년도 국민견강영양조사 자료를 이용하여 우리나라 성인의 치주질환과 당뇨병의 관련성에 대해 평가하고자 하였다. 지역사회치주지수와 혈액검사를 통해 당뇨병을 진단받거나 공복혈당장애에 대한 검사와 당화혈색소 자료가 모두 있는 만 19세 이상의 성인 4,830명이 최종분석대상자가 되었다. 치주질환의 유병률은 성별에 따라서는 남자인 경우, 연령에 따라서는 나이가 많은 경우, 월평균가 수수입과 교육수준이 낮을수록, 흡연 경험이 있고 현재 흡연자일수록, 음주량이 많을수록 높게 나타났다. 당뇨병의 경우 남자인 경우, 나이가 많고, 현재 흡연자일수록, 교육수준과 소득수준이 낮고 음주량과 간식섭취가 많을수록 유병률이 높게 나타났다. 공복혈당수치에 따른 치주질환 유병률은 공복혈당 100 mg/dl 미만은 38.6%, 100 mg/dl 이상은 57.9%로 나타나 100 mg/dl 이상에서 치주질환이 많았다(p<0.0001). 공복혈당장애의 위험은 지역사회치주지수검사에서 CPI가 0~ 2인 그룹을 기준으로 하였을 때 CPI가 3~4인 그룹이 2.18배 높게 나타났다.

Streptozotocin 유발 당뇨쥐에 있어서 발효콩을 주원료로 한 기능성 식품의 혈당강하 효과 (Hypoglycemic Effect of the Functional Food Manufactured by Fermented Soybean as Main Materials in Streptozotosin - Induced Diabetic Rats)

  • 본 연구에서는 발효콩을 주원료로 한 기능성 식품이 streptozotocin으로 유발된 당뇨쥐의 체중, 장기무게, 혈당, 혈중 중성 지방, 혈중 콜레스테롤, HDL-콜레스테롤에 미치는 영향에 대하여 조사하였다. 기능성 식품군에서는 당뇨군에 비하여 완만한 체중 감소를 나타내었으며 식이섭취량은 모든 당뇨군에서 현저한 증가를 나타내었다. 식이효율은 모든 당뇨군에서 낮은 수치를 나타내었으나 기능성 식품의 투여로 식이효율이 당뇨군에 비해 유의적인 증가를 나타내었다(p<0.05). 장기의 중량 비교에서 간과 신장의 경우, 모든 당뇨군에서 정상군에 비하여 증가를 나타내었으며 기능성 식품의 투여로 다소 감소를 나타내었다. 심장과 비장의 무게는 모든 실험군에서 유의적인 차이를 나타내지 않았다(p<0.05). 경구 당내성 검사에서는 당뇨군에서 50분대에 혈당치가 최고에 달하였다. 기능성 식품 투여군에서는 30분대에 최고치에 이르렀으며 120분 후에는 현저하게 감소되어 거의 공복전의 수준에 도달하였다. 혈중 포도당의 수치는 기능성 식품의 4주간 투여에 의해 당뇨군에 비하여 유의적인 감소를 나타내었다(p<0.05). 혈청 중 총콜레스테롤, 중성지방, AI의 수치는 묘든 당뇨군에서 정상군에 비해 유의적으로 높은 수치를 나타내었다(p<0.05). 기능성 식품 400 mg/kg을 투여한 투여군에서는 당뇨군에 비하여 각각 16.4%, 15.4% 그리고 48.3%의 감소를 나타내었으나 시료사이의 유의적인 차이는 인정되지 않았다(p<0.05). HDL-콜레스테롤은 기능성 식품 투여에 의해 당뇨군에 비해 다소 증가를 나타내었으며, 400 mg/kg을 투여한 군에서는 정상의 수치에 근접하여 정상군과 유의적 차이가 인정되지 않아(p<0.05) 발효콩을 주원료로 한 기능성 식품이 당뇨 흰쥐의 혈중 콜레스테롤 저하와 지질대사 개선효과가 있음을 알 수 있었다.

Inhibitory activity of Euonymus alatus against alpha-glucosidase in vitro and in vivo

  • The major goal in the treatment of diabetes mellitus is to achieve near-normal glycemic control. To optimize both fasting blood glucose and postprandial glucose levels is important in keeping blood glucose levels as close to normal as possible. ${\alpha}-Glucosidase$ is the enzyme that digests dietary carbohydrate, and inhibition of this enzyme could suppress postprandial hyperglycemia. The purpose of this study was to test the inhibitory activity of methanol extract of Euonymus alatus on ${\alpha}-glucosidase$ in vitro and in vivo to evaluate its possible use as an anti-diabetic agent. Yeast ${\alpha}-glucosidase$ inhibitory activities of methanol extract of E. alatus were measured at concentrations of 0.50, 0.25, 0.10, and 0.05 mg/ml. The ability of E. alatus to lower postprandial glucose was studied in streptozotocin-induced diabetic rats. A starch solution (1 g/kg) with and without E. alatus extract (500 mg/kg) was administered to diabetic rats by gastric intubation after an overnight fast. Plasma glucose levels were measured at 30, 60, 90, 120, 180, and 240 min. Plasma glucose levels were expressed in increments from baseline, and incremental areas under the response curve were calculated. Extract of E. alatus, which had an $IC_{50}$ value of 0.272 mg/ml, inhibited yeast ${\alpha}-glucosidase$ activity in a concentration-dependent manner. A single oral dose of E. alatus extract significantly inhibited increases in blood glucose levels at 60 and 90 min (p<0.05) and significantly decreased incremental response areas under the glycemic response curve (p<0.05). These results suggest that E. alatus has an antihyperglycemic effect by inhibiting ${\alpha}-glucosidase$ activity in this animal model of diabetes mellitus.
