Objective: This study was performed to investigate the prevalence of impaired fasting glucose (IFG) and its related characteristics among healthy adults in some Korean rural areas. Methods: We conducted a cross-sectional study using the data from 1352 adults who were over the age 40 and under the age 70 and who were free of diabetes mellitus (DM), cardiovascular diseases and other diseases and who participated in a survey conducted as part of the Korean Rural Genomic Cohort Study. IFG was defined as a serum fasting glucose level between 100 and 125 mg/dL. Results: The prevalence of IFG was 20.4% in men, 15.5% in women and 12.7% overall. Multivariate logistic regression analysis demonstrated that the independent risk factors for IFG were male gender, having a family history of DM, the quartiles of gamma glutamyltransferase and high sensitive C-reactive protein and the waist circumference. The homeostatis model assessment for insulin resistance was very strongly associated with IFG. The prevalence of metabolic syndrome (MS) and MS components was higher in the subjects with IFG then in those with normal fasting glucose (NFG). Conclusions: The result of study could supply evidence to find the high risk population and to determine a strategy for treating IFG. Further research is needed to explain the causal relationship and mechanisms of IFG.

BACKGROUND/OBJECTIVES: Fasting and postprandial hyperglycemia should be controlled to avoid complications of diabetes mellitus. This study investigated the effects of autumn olive (Elaeagnus umbellata Thunb.) berry (AOB) on fasting and postprandial hyperglycemia in mice. MATERIALS/METHODS: In vitro ${\alpha}$-glucosidase inhibitory effect of AOB was determined. Maltose solution (2 g/kg) with and without AOB extract at 500 mg/kg or acarbose at 50 mg/kg was orally administered to normal mice after overnight fasting and glucose levels were measured. To study the effects of chronic consumption of AOB, db/db mice received the basal diet or a diet containing AOB extract at 0.4% or 0.8%, or acarbose at 0.04% for 7 weeks. Blood glycated hemoglobin and serum glucose and insulin levels were measured. Expression of adiponectin protein in epididymal white adipose tissue was determined by Western blotting. RESULTS: In vitro inhibitory effect of AOB extract on ${\alpha}$-glucosidase was 92% as strong as that of acarbose. The AOB extract (500 mg/kg) or acarbose (50 mg/kg) significantly suppressed the postprandial rise of blood glucose after maltose challenge and the area under the glycemic response curve in normal mice. The AOB extract at 0.4% or 0.8% of diet or acarbose at 0.04% of diet significantly lowered levels of serum glucose and blood glycated hemoglobin and homeostasis model assessment for insulin resistance values in db/db mice. The expression of adiponectin protein in adipose tissue was significantly elevated by the consumption of AOB at 0.8% of diet. CONCLUSIONS: Autumn olive (E. umbellata Thunb.) berry may reduce postprandial hyperglycemia by inhibiting ${\alpha}$-glucosidase in normal mice. Chronic consumption of AOB may alleviate fasting hyperglycemia in db/db mice partly by inhibiting ${\alpha}$-glucosidase and upregulating adiponectin expression.
The purpose of this study was to measure the counseling effects for diabetes mellitus patients. To measure the effects of the nutrition counseling, 40(13 men and 27 women) outpatients with diabetes mellitus were selected and they were randomly assigned to either the counseling group or non counseling group. Twenty-one counseling group patients received nutrition counseling weekly for 6 weeks and 19 patients served as non counseling group(control group) did not received counseling over same study period. The results of this study can be summerized as follows : 1) The food attitude score of the counseling group significantly increased from 61.9$\pm$15.9 prior to counseling to 87.0$\pm$7.8 after counseling (p<0.001). 2) Fasting blood glucose level and postgrandial blood glucose level also showed significant difference between two groups(p<0.01). In the counseling group, the fasting blood glucose level significantly decreased from 163.5$\pm$48.6mg/dl to 142.3$\pm$40.6mg/dl(p<0.01), and the postgrandial blood glucose level significantly decreased from 281.3$\pm$105.1mg/dl(p<0.001). 3) There was a significant difference of glycosylated hemoglobin between the two groups(p<0.005), and the glycosylated hemoglobin level of counseling group significantly decreased from 11.2$\pm$2.9% to 9.7$\pm$3.6%(p<0.005). 4) There was a significant correlation between the food attitude score difference and the glycosylated hemoglobin level difference in counseling group(p<0.05).

The prevalence of diabetes mellitus continues to rise alarmingly because of industrialization of society. The aim of this study was to investigate the antidiabetic effects of black ginseng extract (BGE) which was performed panax ginseng. First we examined the inhibitory effects of BGE on ${\alpha}$-glucosidase. But there was no practical effects in our observations. We, also, investigate the effects of BGE on glucose uptake of skeletal muscle using 2-NBDG in $C_2C_{12}$ myotube. BGE significantly improved the glucose uptake considered as a lowered blood glucose level. Effects of GBG05-FF on fasting blood glucose and glycated hemoglobin (HbA1c) were investigated in streptozotocin (STZ)-induced diabetic mouse. After injection of STZ, fasting blood glucose and glycated hemoglobin rapidly increased. But STZ-induced diabetic mouse treated with GBG05-FF significantly reduced the level of fasting blood glucose and HbA1c. This results showed that supplementation of BGE improve the diabetic parameters and BGE have a potentiality as a functional food for Diabetes mellitus.
This research was conducted to study the effects of the supplementation of multi-extracts of mori folium (MF) and of exercise on plasma insulin and glucose levels in streptozotocin (STZ)-induced diabetic rats. Eight male Sprague-Dawley rats, 4 weeks old, were assigned to each experimental group and were raised in the laboratory for 10 weeks. The animal groups consisted of a normal-control group, a STZ-control group, 3 STZ-induced diabetic groups supplemented ad libitum with various amounts of MF extracts (MF-720, MF-360, and MF-180 groups), and a STZ-induced diabetic group supplemented with MF-360 along with exercise. In the normal-control group, glucose tolerance tests resulted in the peak blood glucose level being achieved in 15 minutes and a fasting blood glucose level being achieved in 60 minutes. In the STZ-control group, the peak blood glucose level was reached after 60 minutes and, even after 90 minutes, blood glucose shown at a significantly higher level compared to the fasting levels. In the groups supplemented with MF extracts, the blood glucose level peaked after 30 minutes of glucose challenge, and returned to the fasting level after 90 minutes; the MF-360 and MF-360+exercise groups showed the best levels of glucose tolerance. Blood glucose levels in the STZ-induced diabetic groups were significantly higher compared to the normal-control group. However, after 7 weeks of supplementation with MF extracts, a significant lowering of blood glucose levels was observed in all groups supplemented with the MF extract. The best effect was observed in the group given MF extract combined with exercise. Compared to the normal-control group, blood insulin levels were significantly lower in all STZ-induced diabetic groups; however, a significantly higher level of insulin was observed in the groups given MF extracts compared to the STZ-control group. This study shows that the supplementation of MF extracts in STZ-induced diabetic rats resulted in increased blood insulin levels and lower blood glucose levels.
In this study, body weight levels of glucose, insulin and triglyceride in blood and glucosidase activity of the small intestine were investigated to determine the effect of Sangbaegpitang and Supungsungiwhan on the glucose metabolism of db/db mice. The GLUT4 mRNA of muscle tissue and the Acetyl CoA Carboxylase and the activation rate of GLUT2 mRNA of liver tissue were measured by the reverse transcription-polymerase chain reaction method and by the vitro transcription. The results were obtained as follows: 1. In the Sangbaegpitang administration group, (1) The level of triglyceride was decreased significantly and the glucosidase activity of the small intestine was inhibited remarkably, (2) The amounts of the GLUT4 mRNA in muscle tissue and Acetyl CoA Carboxylase mRNA in liver tissue were increased significantly. (3) Though glucose level in both fasting and non-fasting, were decreased and the insulin level in blood was increased, the results showed no statistical significance. 2. In the Supungsungiwhan administration group, (1) The levels of glucose and triglyceride were decreased significantly in the blood of non-fasting animals. (2) The glucosidase activity of small intestine was inhibited markedly and the amounts of GLUT4 mRNA of muscle tissue and GLUT2 mRNA of liver tissue were increased significantly. (3) The glucose levels in the fasting group were reduced, while insulin level was increased but showed no statistical significance, Based on the above results, our conclusions are as follows: Sangbaegpitang & Supungsungiwhan are thought to be capable of inhibiting the activity glucosidase, the enzyme which influences carbohydrate metabolism in the small intestine of db/db mice(the experimental diabetic model) and delaying the absorption of carbohydrate, thus proving effective on inhibiting the increase of non-fasting glucose level effectively. Futhermore Sangbaegpitang and Supungsungiwhan are though: to be capable of preventing the composition of free fatty acids by restoring the production of GLUT4 mRNA of muscle tissues and GLUT2 mRNA of liver tissues. Those results suggests that above prescriptions can be applied to non-insulin dependent diabetes mellitus in order to improve insulin resistance.

The purpose of this study is to investigate tile effects of the Paecilomyces japonica on blood glucose in non-insulin dependent diabetes mellitus volunteers. During the 3 months of the experimental period, freeze-dried powder of Paecilomyces japonica (2 g/meal) was given to the subjects right before each meal. Paecilomyces japonica caused a significant decrease of $27.5\%\;and\;35.5\%$ in fasting and postprandial blood glucose levels, respectively. There were significant reductions of $28.5\%\;and\;33.5\%$ in fasting and postprandial blood glucose levels of the male subjects, respectively Paecilomyces japonica also significantly decreased in fasting and postprandial blood glucose levels of $26.1\%\;and\;37.3\%$ in female patients after treatment. This study shows that Paecilomyces japonica supplementation significantly lowered the fasting and postprandial glucose levels in NIDDM patients. These effects presumably contribute to the hypoglycemic activity.
Onion (Allium cepa Linn) has been reported to have hypoglycemic activity in human and several animal models. In the present study, we performed intraperitoneal glucose tolerance test (IPGTT) in young (1.5mo) and aged (5 mo) rats treated with onion in order to determine whether aging can influence on the anti-hy-perglycemic effect of onion. In addition, we investigated the hypoglycemic effect of onion in streptozotocin- induced diabetic rats treated with aqueous extracts of onion (500 mg/kg, i.p., daily) for 4 weeks. Blood glucose level was determined in fasted and fed rats by using a glucometer (Johnson & Johnson). In glucose tolerance test, blood glucose level was maximally increased 15 min after glucose load (2 g/kg, i.p.), and recovered to the basal level 3 hr after glucose challenge in young and old rats. The maximum blood glucose levels of young and aged rat were 184$\pm$7.49 and 225.2$\pm$ 12.55 mg/dl, respectively. A single i.p. injection of aqueous extract of onion (1 g/kg) 30 min before glucose challenge significantly decreased blood glucose levels at 15, 30, 60, 90 min after glucose load in aged rats, while the administration of onion did not show any significant effect in young rats. In onion-treated diabetic rats, significant hypoglycemic effect (p<0.05) was observed, and the effect was greater in fasted rats than in fed. In conclusion, these results suggest that anti-hyperlycemic effect of onion can be changed by age and fasting.
Caloric restriction is a popular approach to treat obesity and its associated chronic illnesses but is difficult to maintain for a long time. Intermittent fasting is an alternative and easily applicable dietary intervention for caloric restriction. Moreover, intermittent fasting has beneficial effects equivalent to those of caloric restriction in terms of body weight control, improvements in glucose homeostasis and lipid profiles, and anti-inflammatory effects. In this review, the beneficial effects of intermittent fasting are discussed.
Plasma glucose levels before and after oral glucose administration have been compared in a group of 76 thyrotoxic subjects and a group of 8 normal control subjects in order to study the effect of glucose loading in thyrotoxicosis. Following were the results: 1. The mean fasting plasma glucose level was elevated in the thyrotoxic group(95.5mg%) compared to normal control group (88mg%). 2. The peak of glucose tolerance curve is at 30 minutes after glucose administration in both groups, but its mean value was 44mg% higher in thyrotoxic group than in control group. 3. The plasma glucose levels returned towards the fasting level in the later stage of the test more rapidly in thyrotoxic group than in control group. 4. 69.6% of oral glucose tolerance tests were impaired in the thyrotoxic group, and the occurance of abnormal glucose tolerance could be related to the degree of thyrotoxicity, sex and age. 5. The mechanisms of the impaired glucose tolerance in thyrotoxicosis are thought to be related to an increased rate of glucose absorption from gastrointestinal tract, abnormal liver function with decreased hepatic glycogenesis, increased glucose oxidation, decreased pancreatic release of insulin, and genetic relationship between diabetes and thyrotoxicosis.
