The factors associated with Vitamin D deficiency in community dwelling elderly in Korea

  • BACKGROUND/OBJECTIVE: Recent studies showed vitamin D deficiency is linked to chronic diseases in addition to skeletal metabolism which could threaten the elderly. We analyzed health conditions and socio-demographic factors associated with vitamin D deficiency in community dwelling people aged 65 years and older. SUBJECTS/METHOD: Data from the Korea National Health and Nutrition Examination Survey (KNHANES) 2010 to 2012 were obtained. A total of 2,687 subjects aged 65 years and older were participated. The cutoff value of the Vitamin D deficiency was considered as serum 25-hydroxyvitamin D [25(OH)D] ${\leq}20ng/mL$ (50 nmol/L). RESULTS: The overall prevalence rate of vitamin D deficiency in the elderly was 62.1%. The factors such as female, obesity, metabolic syndrome, current smoker, and skipping breakfast were positively associated with vitamin D deficiency, but high intensity of physical activity and more than 9 hours of sleep duration were negatively associated with vitamin D deficiency (all P < 0.05). CONCLUSIONS: It is important that health professions know that the factors proved in this study are connected to vitamin D deficiency thus provide information and intervention strategies of vitamin D deficiency to old aged people.

  • A taxonomic survey of the dinoflagellate family Prorocentraceae Stein was conducted on 17 locations off the coast of Korea. This monograph is the result of the microscopic analyses. A total of eight species have been identified and described, of which Prorocentrum concavum Fukuyo is new record for Korea.

  • A taxonomic survey of the dinoflagellate family, Triadiniaceae Dodge was conducted on 17 locations off the coast of Korea. Three species of Goniodoma collected from the coast of Korea: Goniodoma orientale Lindemann, G. polyedricum (Pouchet) Jorgensen, and G. sphaericum Murray et Whiting.

  • The revised ISO9000 family of standards represents a significant improvement on the 1994 version and, if implemented well within organizations, should facilitate the development of effective, process-oriented Quality Management Systems. This paper describes how the International Standardized Testing Organization (ISTO) tests the understanding of ISO9000 : 2000, and presents an analysis of the results of the first examinations held in the UK, USA, Mexico and Hong Kong, in February and March 2001.

A Study on Differences in the Caregiving Burden of Primary Caregivers by Type of Caregiving -Focused on Caregivers in Home Stay, Day Care Center, and Nursing Home Situations- (부양형태에 따른 주부양자의 부양부담의 차이 및 영향 변수에 관한 연구 -재가, 주간보호시설, 시설거주 노인의 주부양자를 대상으로-)

  • This study investigates differences by type of caregiving using data on three groups of caregivers, 177 from home stay situations, 189 from day care centers, and 138 from nursing homes. First, the result shows that characteristics of both caregivers and elders differ by type of caregiving. Second, caregivers in the home stay situation have the highest caregiving burden of the three groups. Third, from examination of the related variables, the caregiving burden of home stay caregivers is affected by family income, caregivers' health, type of job, and whether or not the elder has symptoms of dementia. Also, the study reveals that emotional services for elders reduce the aggravation of family relations and economic burden, but that instrumental services highly increase economic burden. It reveals that caregivers of elders in day care centers, especially those who are in bad health, are more likely to experience feelings of constriction, aggravation of family relations and economic burden. On the other hand, caregivers who receive more emotional services have better experiences in family relations, including relations with the elder. In case of the elders of nursing homes, if the main caregiver is a daughter-in-law, aggravation of family relations is higher than if the main caregiver is a spouse. Finally, the caregiver's burden is affected by their own health and income, and by whether the elder has symptoms of dementia or stroke.

A Study on Correlation of Cardiovascular Disease and Periodontal Disease among the Elderly Living Alone and the Elderly Living with Family (한국 노인의 심혈관질환과 치주질환의 관련성 연구 : 독거노인과 가족동거노인을 중심으로)

  • The purpose is to investigate correlation of periodontal disease and cardiovascular disease among those living alone and living with family by using 6th Korean National Health and Nutrition Examination Survey. According to general characteristics, cardiovascular diseases were found to be related to gender, education level of those living with family; and age and income level of elderly living alone. The oral health status, oral health patterns and cardiovascular disease distribution of both groups for the past year were examined. The relationship between cardiovascular disease and periodontal disease was related to hypertension only in the living with family. Cardiovascular disease and periodontal disease are the most common diseases so considering the two diseases together is necessary to check the health status in the future.

The Relationship of Socioeconomic and Behavioral Risk Factors With Trends of Overweight in Korea

  • Objectives: Previous studies have shown that overweight (including obesity) has increased significantly in Korea in recent decades. However, it remains unclear whether this change has been uniform among all Koreans and to what extent socioeconomic and behavioral factors have contributed to this increase. Methods: Changes in overweight were estimated using data from the 1998, 2001, 2005, 2007-2009, and 2010-2012 Korea National Health and Nutrition Examination Survey (n=55 761). Results: Overweight increased significantly among men but not among women between 1998 and 2012. Changes in socioeconomic and behavioral factors over the time period were not associated with overall trends for both men and women. However, we found significant differences in the prevalence of overweight relative to key risk factors. For men, overweight increased at a significantly greater rate among the non-exercising (predicted probability [PP] from 0.23 to 0.32] and high-calorie (PP from 0.18 to 0.37) groups compared to their active and lower-calorie counterparts, respectively. For women, overweight increased only among the non-exercising (PP from 0.27 to 0.28) and low-income (PP from 0.31 to 0.36) groups during this period. Conclusions: These findings suggest that programs aimed at reducing overweight should target Korean men and women in specific socioeconomic and behavioral risk groups differentially.

The Self Concept of Adolescent Consumers and the Evaluation and Experience of Adolescent Entry into Harmful Place (청소년 소비자의 자아개념과 유해업소 출입에 대한 평가 및 경험)

  • This study examined the subcategories of the self concept of adolescent consumers, and investigated the influence of this self concept and the demographic characteristics on the evaluation and experience of adolescent entry into harmful places. The adolescent group was divided into 4 subgroups: (group I with negative evaluation and without entry experience, group II with negative evaluation but with experience;, group III with positive evaluation but without experience, and group IV with positive evaluation and with experience). The differences of demographic characteristic and self concept between these. subgroups were inspected. The results of this study were as follows. First, the self concept of the adolescent consumer consists of body self, social self, achievement self, scholarship self and family self. Among these self concepts, scholarship self was the highest, and family self was the lowest. Second, from the result of regression analysis using the demographic element as an independent variable to inspect the factors influencing self concept, sex, school year, living with parents, and economic status were statistically significant. Third, in case of entry evaluation, singing room was evaluated most positively among 10 places with a score of 4.20 and pub/drunkenness was evaluated most negatively. In case of entry experience, singing room was the most frequently experienced place and discotheque was the least. Fourth, with respect to demographic characteristics and influences of self concept, the influencing factors on entry evaluation were sex, school year, social self, and family self, and the influencing factors on entry experience were sex, school year, living with parents, social self, and family self. Finally, examination of the differences of demographic characteristics and self concept according to entry evaluation and experience, revealed the distinctive variables among the 10 harmful places to be sex, school year, living with parents, body self, social self, achievement self, scholarship self, and family self.

Colon Cancer Prevention by Detection of APC Gene Mutation in a Family with Attenuated Familial Adenomatous Polyposis

  • Background: Genetic mutation is a significant factor in colon CA pathogenesis. Familial adenomatous polyposis (FAP) is an autosomal dominant hereditary disease characterized by multiple colorectal adenomatous polyps affecting a number of cases in the family. This report focuses on a family with attenuated familial adenomatous polyposis (AFAP) with exon 4 mutation, c.481C>T p.Q161X of the APC gene. Methods: We analyzed 20 members of a family with AFAP. Clinical and endoscopic data were collected for phenotype determination. Genetic analysis was also performed by direct sequencing of the APC gene. Result: Five patients with a phenotype of AFAP were found. Endoscopic polyposis was demonstrated among the second generation with genotype mutation of the disease (age > 50 years) consistent with delayed phenotypic adenomatous polyposis in AFAP. APC gene mutation was identified in exon 4 of the APC gene, with mutation points of c.481C>T p.Q161X. Laparoscopic subtotal colectomy was performed to prevent carcinogenesis. Conclusion: A family with attenuated familial adenomatous polyposis of APC related to exon 4 mutation, c.481C>T p.Q161X, was reported and the phenotypic finding was confirmed by endoscopic examination. Genetic mutation analysis might be advantageous in AFAP for long term colon cancer prevention and management due to subtle or asymptomatic phenotype presentation in early adulthood.

Caregiver Burden in Caring for Elders Before and After Long-term Care Service in Korea (장기요양서비스 전.후 가족의 수발 부담감에 영향을 미치는 요인)

  • Purpose: Purposes of this study were: evaluation of family burden of caring for elders who receive long term care services, and examination of differences in burden before and after the introduction of long term care service in Korea. Methods: Data were collected by questionnaires from 416 caregivers of elders who were registered with the Long Term Care Insurance Corporation in six cities. Data were collected in September, 2010 and analyzed using descriptive statistics, paired t-test, and ANOVA with the Scheffe test, and stepwise multiple regression. Results: Family burden decreased significantly after longterm care service was initiated. Subjective burden decreased from 2.93 to 2.69 (t=11.78, $p$<.001), and objective burden, from 3.40 to 3.10 (t=12.73, $p$<.001). Stepwise multiple regression analysis revealed that factors affecting subjective burden were family relations (F=13.60, $p$=.003), age (F=5.47, $p$=.019), job (F=6.98, $p$=.008), and education (F=4.59, $p$=.032), and that factors affecting objective burden were living together (F=17.66, $p$<.001), job (F=13.34, $p$=.003), monthly income (F=6.61, $p$=.010), and type of service (F=6.62, $p$=.010). Conclusion: The results of this first study to investigate caregiver burden after the Korean Long-term Care Insurance System was begun provide positive information for the development of strategies to decrease family burden in long term care.
