Needs for Home Care Nursing in the Vulnerable Elderly

  • Purpose. The purpose of this study was to determine the subjects' health status according to the needs of visiting health and the function of the family in home care nursing. Sample and Method. The data collection period was from 07/01/04 to 10/31/04 and the subjects were 488 of those above 60 years of age staying at home or living alone who registered at a visiting health service of public health center at an urban area in Korea. This survey was carried out by visiting health nurses and participation was agreed on by the elderly people. Results. The extent of the subjects' total health status to the general characteristics had differences according to the age, sex, monthly income, perceived health status, known functional disorder, and yes-or-no for disease. At all health status domains, visiting health need care in the group I was very lower than one in II, III, or IV groups. Also the severe dysfunctional family was lower than lightly dysfunctional family and normal functional family in all health status domains. Conclusion. Nurses must provide their characteristics considered nursing intervention for the elderly who have high visiting health needs and severe dysfunctional family with vulnerable health care.

순환모델에 근거한 저소득층 중년여성의 가족기능과 정신건강 (Analysis of Family Function and Mental Health State for Low-income Middle-aged Women)

  • Purpose: The purpose of this study was to identify the types of family function and mental health in low-income middle aged women using the circumplex model. Method: A descriptive research design was used. A sample of 116 low income middle aged women participated in the study. The Family Adaptability and Cohesion Evaluation Scale III was used to measure the types of family function. Mental health was measured by the SCL. Results: The types of family function identified were balance family (59.4%), extreme family (25.9%), and mid-range family (14.7%). Significant differences were found in mental health by the family cohesion (F=3.44, p=.019) and family adaptability (F=3.31, p=.023). The mental health status of extreme family was better than mid-range family and balanced family, but such result was not statistically significant (F=0.25, p=.783). Conclusion: The Circumplex model's main hypothesis was not empirically supported that extreme family has more problematic than mid-range family and balance family. These findings emphasize the need for the development of a family system model for Korean family.

아토피피부염 환아의 중증도에 따른 가족관리방식의 비교 (A Comparative Study of Family Management Style According to Severity of Childhood Atopic Dermatitis)

  • Purpose: This study of secondary analysis aims to compare family management style according to severity in children with atopic dermatitis. Methods: A convenience sample of 109 Korean mothers caring for a child with atopic dermatitis, were recruited from the pediatric departments of two general hospitals in Seoul. Data were collected from November 1, 2015 to February 28, 2016. A structured self-report questionnaire was used. Data were analyzed using descriptive statistics, correlation analysis, and one-way ANOVA with IBM SPSS Version 22.0. Results: For family management style, condition management effort, family life difficulty, and view of condition impact were significantly different according to the severity of the atopic dermatitis in these children. Conclusion: The findings indicate that family management strategies to enhance care of children with atopic dermatitis as well as members of the family should include these significant variables in a family-centered approach.

지역사회 치매가족의 간호요구도 (Nursing Service Needs of Caregivers for Demented Elderly in the Community)

  • Purpose: to investigate nursing service needs of dementia caregivers in the community, and, also to provide basic data that is helpful in developing nursing intervention for them. Method: MMSE-K exam was administered on 15,216 men more than 65years old who live in Gu, Seoul city, and the subject of this study were caregivers of 792 demented elderly who were below 20 points in the MMSE-K exam. Result: Total score of nursing service needs of family appeared lower than average. By specific area, information nursing needs was the highest(M=3.29), and therapeutic nursing needs (M=3.31), welfare service nursing needs(M=3.32), emotional support nursing needs (M=3.34) followed in order. The factors which increased the total score of nursing service needs of dementia caregivers as general and physical characteristics were man (p=.04), lower ADL (p=.00) and IADL(p=.00), longer length of sick days(p=.01), having diseases(p=.03), decline of cognition(p=.00), existence of dementia symptom(p=.00). And. high economic status(p=.03), good caring attitude(p=.00) were family characteristics which increased the nursing service needs of caregivers. As predicting variables dementia symptom (10.4%), caring attitude(1.9%), existence of economic activity (1.5%), and length of sick days(1.2%) exhibited explanation power in order. Conclusion: In future, active intervention methods that correspond to family nursing service needs have to be developed, and it is also needed to develop nursing intervention strategy on specific problem with dementia problematic behavior, exercise etc.. Also, it is needed to do research on the caring attitude of dementia family because active caring attitude of family was significant influencing factor on nursing service needs of family.

Development of a Family Nursing Model for Prevention of Cancer and Other Noncommunicable Diseases through an Appreciative Inquiry

  • Background: Cancer and non-communicable diseases are a major issue not only for the developed but also developing countries. Public health and primary care nursing offer great potential for primary and secondary prevention of these diseases through community and family-based approaches. Within Thailand there are related established educational curricula but less is known about how graduate practitioners enact ideas in practice and how these can influence policy at local levels. Aim: The aim of this inquiry was to develop family nursing practice in primary care settings in the Isaan region or Northeastern Thailand and to distill what worked well into a nursing model to guide practice. Materials and Methods: An appreciative inquiry approach involving analysis of written reports, focus group discussions and individual interviews was used to synthesize what worked well for fourteen family nurses involved in primary care delivery and to build the related model. Results: Three main strategies were seen to offer a basis for optimal care delivery, namely: enacting a participatory action approach mobilizing families' social capital; using family nursing process; and implementing action strategies within communities. These were distilled into a new conceptual model. Conclusions: The model has some features in common with related community partnership models and the World Health Organization Europe Family Health Nurse model, but highlights practical strategies for family nursing enactment. The model offers a basis not only for planning and implementing family care to help prevent cancer and other diseases but also for education of nurses and health care providers working in communities. This articulation of what works in this culture also offers possible transference to different contexts internationally, with related potential to inform health and social care policies, and international development of care models.

재가 치매가족의 휴식 돌봄: 개념명료화 (Respite Care for Family Caregivers of Elders with Dementia : Concept Clarification)

  • Purpose: Respite care is not a discrete intervention, but encompasses a range of services. This research was conducted to clarify the phenomenon of respite care for family caregivers of elders with dementia from a nursing perspective. Method: The Hybrid Model of concept development was applied to clarify the concept of respite care for family caregivers of elders with dementia. The study was conducted in the following three steps, theoretical phase, fieldwork phase, and final analytic phase. Results: The definition of respite care for family caregivers of elders with dementia was delineated through integration of data analyses in theoretical and fieldwork phase, and has three dimensions; tailored supports for caregivers, tailored supports based on physical and cognitive function of elders with dementia and community interventions related to family care function. Conclusion: Through this study, the concept of respite care for family caregivers of elders with dementia is clarified and reformulated as nursing practice phenomena in the Korean context, which indicates ways to develop caring practice forms for a family living with an elder with dementia in a community setting.

가정간호 환자의 가족기능도와 만족도 조사 (A Study of Family Functioning Level and Satisfaction Level of Home Health Care Patient)

  • The purpose of this study was to determine the level of family functioning and the degree of patient satisfaction for 102 patients who received home health care nursing service. The results are as follows ; 1. The score assessed for the family functioning was averaged at the score of $4.95{\pm}3.11$ (10 perfect score) and this was regarded as low, especially for the patients with chronic illness, and the reason was assumed due to poor psychological support from the family members and high expectations from the patients. 2. Patient satisfaction levels were toward the received services were high, according to the 13 items which consited of 3 point scales, with average score of $1.27{\pm}0.53$. Based on the above study results, the author suggests the followings : 1. A new nursing service program and model devolopment should be established and utilized because of low family functioning levels of those who receive family nursing service. 2. More standardized tool explorations for the study of family nursing service and patient satisfaction are required.

뇌졸중에 관한 국내 간호학 논문의 분석 (Analysis of Nursing Studies about Stroke Research in Korea)

  • This study was done to analyze the trends of research on stroke in Korea, to suggest future direction, for research on stroke. This article reviewed 75 nursing research papers on stroke done since 1990 to 2000 by examining them according to the period of publication or presentation, research design, type of subjects, measurement variables, the intervention outcome of experimental research, and theme of qualitative research. The research were as follows : 1. 26 of 75 studies were composed of master thesis and dissertation of graduate school. There were 53 nonexperimental research, 15 experimental research, and 7 qualitative studies design. 2. Stroke patients subject' Characteristic were 44 and Family and Spouses of caregiver support were 28 of total studies. 3. Most measurement instruments used for research were translated and redesigned it into Korean that developed by foreigners, and development measurement instruments used Kang's ADL. 4. Family support and Social support was shown the frequently among correlational research. 5. Methodological research were development of an evaluation tool for the quality of nursing care in stroke patients, caregiver support of development of nursing intervention list, and Home care Nursing Intervention protocol, and development of client selection criteria based on the needs of services to be offered. 6. The experimental research of intervention were mostly education nursing intervention, rehabilitation program on functional recovery, support nursing intervention, and applying a home care protocol on the nursing care intervention. 7. Theme of qualitative research were family experience, home care in family experience, soobal experience, illness experience, hope of Stroke patients and so on. Phenomenologic methodology and Granded Theory was designed of Qualitative research. On the basis of the above finding the following recommendations are made: 1. It's necessary to develop a reliable and variable measurement tool for stroke patients and family care of stroke patients. 2. It's necessary to study the comparison of Nursing Studies of stroke research abroad, the replication to establish the effect of nursing intervention stroke patients and family care of stroke patients.

입원아동 돌봄을 위한 가족중심 순회의 통합적 고찰 (An Integrative Review on Family-Centered Rounds for Hospitalized Children Caring)

  • Purpose: Involvement of families in rounds is one strategy to implement patient- and family-centered care to help families get clear information about their child, and be actively involved in decision making. The purpose of this paper was to identify the major concepts of family-centered rounds for hospitalized children. Methods: We searched five electronic databases for relevant articles and used Whittemore and Knafl's integrative review methods to synthesize the literature. Articles published between June 2003 and January 2016 were reviewed and through full text screening 24 peer-reviewed articles were found that met the selection criteria for this review. Results: Through in-depth discussion and investigation of the relevant literature, four overarching components emerged: (a) cognition of parents and medical staff, (b) effective communication, (c) collaboration of family and medical staff, (d) coaching of medical staff. Conclusion: For successful family-centered rounds positive cognition is important. Appropriate communication skills and consideration of multi-cultural family can lead to effective communication. Offering consistent and transparent information is important for collaboration between family and medical staff. Prior education on family-centered rounds is also important. Four major components have been identified as basic standards for implementing family-centered rounds for hospitalized children.

유방암 환자와 배우자의 갈등해결방식과 부부친밀도 및 가족기능 (Conflict Resolution Styles, Marital Intimacy and Family Functions of Breast Cancer Patients and Their Spouses)

  • Purpose: The purpose of this study was to explore conflict resolution styles, marital intimacy and family functions among breast cancer patients and their spouses. Methods: The subjects were total 126 participants. Breast cancer patients who completed chemotherapy and or radiation along with their spouses. Data were collected using questionnaires with questions about conflict resolution styles, marital intimacy and family functions. Results: There were no differences between breast cancer patients and their spouses in verbal aggression, avoidance of conflict resolution styles and family functions. As patients reported using positive conflict resolution styles the spouse-perceived marital intimacy and family functions were higher. Those patients who perceived marital intimacy as lower they also reported more verbal aggression and avoidance. As breast cancer patients perceived family functions increasing, their spouses perception of both intimacy and family function increased. Conclusion: As these results, it should be considered as basic data to develop family intervention programs such as positive communication and effective stress management and improving of conflict resolution, intimacy and family functions among breast cancer patients and their spouses.
