We describe the framework of a data-fuelled, interdisciplinary team-led learning system. The idea is to build models using patients from one's own institution whose features are similar to an index patient as regards an outcome of interest, in order to predict the utility of diagnostic tests and interventions, as well as inform prognosis. The Laboratory of Computational Physiology at the Massachusetts Institute of Technology developed and maintains MIMIC-II, a public deidentified high- resolution database of patients admitted to Beth Israel Deaconess Medical Center. It hosts teams of clinicians (nurses, doctors, pharmacists) and scientists (database engineers, modelers, epidemiologists) who translate the day-to-day questions during rounds that have no clear answers in the current medical literature into study designs, perform the modeling and the analysis and publish their findings. The studies fall into the following broad categories: identification and interrogation of practice variation, predictive modeling of clinical outcomes within patient subsets and comparative effectiveness research on diagnostic tests and therapeutic interventions. Clinical databases such as MIMIC-II, where recorded health care transactions - clinical decisions linked with patient outcomes - are constantly uploaded, become the centerpiece of a learning system.
Purpose: A survey was conducted to investigate the role, priority, and awareness of physical therapists in the community care system for the elderly. Methods: Nine hundred ninety-three physical therapists were invited to an online survey from 1st to 30th June 2020. Results: While the awareness of physical therapists for the community care system for the elderly was found to be low, the role priority and performance ability were found to be high. Fall training scored the highest for the job duties of physical therapists, and functional training scored the highest for the performance ability of physical therapists. For the role priority by the education status regarding the community care system, the participants who received the education program showed higher scores in the performance ability than those who did not. For the role priority and performance ability according to the education levels, higher education levels resulted in higher scores in both outcomes. Regarding the clinical experience, low scores were found in the participants with less than five years of experience. Conclusion: Physical therapy establishes itself as an essential area of expertise in the community care system for the elderly. Therefore, these results can be utilized in developing a model for Korean community care for the elderly.
Purpose: The purpose of this study was to understand the meaning of the driving force of life experienced by the elderly and the realistic structure of their experience. Methods: The research question was "What is the essential meaning of the driving force experienced by the elderly?". Data were collected from individual in-depth interviews between March, 2017 and May, 2017 and analyzed using Van Manen's hermeneutic phenomenological methodology to identify the essential themes of their experience. Results: Through the analysis process, 22 themes and 14 essential themes were derived. The 14 essential themes of the driving force of the life of elderly were as follows: 'Memories from the past', 'Poverty to escape', 'Hand down family's heritage', 'A body bound by marriage', 'A body necessary for old age', 'A dignified body even if they grow older', 'A body that does not want to fall', 'A fruit of my life', 'Mature real love', 'Unchanging fraternity', 'Extended family', 'An old friend with whom share my heart', 'Satisfied with the situation', and 'Expanding the breadth of life'. Conclusion: Nursing programs that assess the driving force of life supporting the elderly and supports the driving force of such a life should be developed.
This study aims to analyze kinematic variables regarding lateral break-fall quantitatively that can protect the body, prevent the injury and minimize the impact. To this end, three Hapkido, judo athletes of H University with experience of over 5 years were selected. Test was conducted through three dimensional image analysis by checking the time and order the subjects reach a mat. In this study, lateral break-fall was repeated five times and among them, the best movement was selected. The picture shot with high-speed camera was analyzed by using KWON3D ver. 3.1 program through three dimensional coordinate calculation based on DLT method and smoothing process of data. Study results were as follows. 1. With respect to time variables shown in lateral break-fall of A, B, C athletes, there is small difference in temporal variables and in the order the body reaches a mat. With respect to average value, hand is ($0.94{\pm}0.20$), elbow ($0.97{\pm}0.17$), hip ($0.97{\pm}0.18$), back ($0.98{\pm}0.18$), and shoulder ($1.04{\pm}0.16$). Time variable the body reaches a mat in lateral break-fall is in hand, elbow, hip, back and shoulder. 2. With respect to moving distance variables shown in lateral break-fall of A, B, C athletes, hand is ($34.33{\pm}34.59$), elbow ($52.00{\pm}26.06$), hip ($70.00{\pm}15.72$), back ($153.67{\pm}17.93$), and should ($130.67{\pm}29.02$). The fact that this study contributed to improving security martial arts technique and protecting the body by understanding the principle of lateral break-fall movement is of significance. In addition, the fact that this study provided systematic basic data for improving security martial arts technique is significant.
Objective : The purpose of this study was to investigate the effects of occupational therapeutic intervention including functional task training on activities daily living of falls experienced elderly. Methods : This study used a single subject experimental design with alternating treatment and reversal design. One participant who have had falls experience among the community-dwelling elderly completed. This study a total of 18 sessions were conducted. Fall Efficiency Scale(FES), Berg Balance Scale(BBS) and task modification scale were used to assess the activities of daily living during each session, and Canadian Occupational Performance Measure(COPM) and Assessment of Home-based Activities(AHA) were used to evaluate the activities of daily living at pre-and post-test. Results : The participant showed significant improvements in the FES, BBS scores. However, the improvement was greater when progressive resistance exercise was practised along with functional task training. COPM and AHA scores improved at post-test. Conclusions : These findings provide evidence for occupational therapeutic intervention for falls-experienced elderly.
Fall is one of the most intimidating health conditions in elders. Comprehensive assessment is necessary to understand the individual and environmental aspects of the falls such as balancing abilities, depression, and quality of life. The purpose of this study was to compare the balancing ability, depression, and quality of life between elderly fallers and elderly non-fallers. Thirty-two community-dwelling elders (fifteen males and seventeen females between 65 and 83 years old), who have experienced fall on walking during last twelve months, were involved in the elderly fallers group. And twenty-four males and twenty-two females between 65 and 83 years old of community-dwelling elders, who have no experienced fall on walking during last twelve months, were involved in the elderly non-fallers group. Berg balance scale (BBS), timed up and go test (TUG), and functional reach test (FRT), were used to evaluate the ability of the physical balance. 'Beck depression scale in Korean' questionnaire was used to assess the depression. 'Korean version of World Health Organization Quality of Life Assessment Instrument-Bref' questionnaire was used to assess the quality of life. The results were as follows: 1) Balancing abilities measured by the BBS, TUG in the elderly fallers group were meaningfully lower than that of the elderly non-fallers group (p<.05), whereas no significant difference in the FRT was found (p>.05). 2) Depression level in the elderly fallers group was significantly higher than that of the elderly non-fallers group (p<.05). 3) Quality of life in the elderly fallers group was significantly lower than that of the elderly non-fallers group, excluding environment domain (p<.05). Therefore, in order for clinical evaluation of the community-dwelling elders those with reduced balancing ability, it is necessary to evaluate and understand the fall experience, depression, and quality of life.

The purpose of this study was to investigate the efforts of Knowledge, Attitude and Perceptions of Patient Safety Culture on Fall Prevention Activities in Mental Hospital Nurses. This study is a descriptive research study of 153 nurses working in Busan and Gyeongnam mental health hospitals, the data were collected from April 4 to December 31, 2018. Data were analyzed using IBM SPSS/win 24.0 program, which included t-test, ANOVA and multiple regression analysis. As a result, The attitude toward falls differed depending on subject's license or qualifications, the higher the education level. The higher the level of perceptions of patient safety culture, and the higher the work experience, the more prevention activities toward falls. The higher the perception of patient safety culture, the higher the attitude toward falls. The higher the prevention activities toward falls, the higher the attitude of falls and the perceptions of safety culture. As a result of multiple regression analysis of factors affecting Knowledge, Attitude and Perceptions of Patient Safety Culture on Fall Prevention Activities were 12.5%. Therefore, in order to promote fall prevention activities of mental hospital nurses, the knowledge needs to be expanded through continuous education. Education programs should be developed and provided to change attitudes toward falls. and At the hospital organization level, a wide range of support is required, including changes in the overall human and institutional environment for safety.
Purpose: The purpose of this study was to investigate the relationships between a experience of falling, fear of falling, depression, and perceived health status in urban areas. Methods: After obtaining Institutional Review Board (IRB) approval, a one-time, face-to-face, and private interview was conducted with each participant who was eligible and agreed to participate in this study from May 2007 to August 2007 by trained graduate-level nursing students. The questionnaires consisted of Fall Efficacy Scale, K-GDS, and SF-36. The collected data were analyzed with SPSS/PC 12.0 program, which was used for frequency, percentage, mean, standard deviation, t-test, and hierarchical regression. Results: The major findings of this study were as follows; 1) approximately 9% of participants had fallen within one year. Study participants reported moderate fear of falling(M=43.80); moderate physical health (M=42.31) and mental health(M=46.05); and low depression status(M=10.38). 2) there were significant differences in fear of falling according to gender, experience of falling, exercise, and depression. 3) significant factors influencing on fear of falling were experience of falling and physical health status. Conclusion: The results of this study indicate that nurses working closely with older adults should be taught the impact of previous falls and physical health status on fear of falling and need to reinforce exercise behavior for older adults who are vulnerable to fear of falling.
The purpose of this study was to investigate the experiences of accident and visiting emergency center, and the experiences of education about emergency care for parents. The subjects of this study were 117 mothers whose children are attending 9 different nursery schools or kindergartens. The instrument used in this study was developed by researchers and validated by nursing professor, first aid doctor and the president of nursery school, and consisted of 'most important thing in child rearing' ,'experiences of accident and visiting emergency center', 'type of trauma' and 'need for parent education program' the result of this study were as followed ; 1.There were 70.09% of the experience of visiting emergency center among the subjects. The cause of visiting emergency center were high fever and convulsion(42.86%), and trauma by accident(28.04%) 2. The most common type of the indoor accident were fall down and slipping(76.92%), the common type of trauma were contusion (76.92%), abrasion(47.01%), laceration(29.06%), dislocation or fracture(12.82%), burn(10.26%), piercing(8.55%). 3.47.01% of subjects had the experience of education about emergency care for parents. They were educated by child rearing journals (32.72%), small books which were published by hospitals or community health center (25.45%), mass media(21.8%), parent seminar (12.72%), nursery school or kindergarten (5.45%). In conclusion, the need of emergency care for children was strongly found. Therefore, on the basis of above results, the parent education program which fits their cognitive level and their health care needs So, it must be directed on the further study of parents' knowledge about emergency care for children. To enhance the effectiveness of program and accomplish the children's health promotion, advanced instructional media and demonstration must be included.

The purpose of this study is to identify contradictions in library internship, and address them. As a case study, students who were enrolled in the internship class opened in the 2016 fall semester in a university in Seoul were interviewed. Through applying Activity Theory, the process of internship and contradictions presented in its process were analyzed. The findings indicate that students were satisfied with their internship experience. The internship experience also makes positive impacts on students in terms of understanding the library and information science and deciding their future career as a librarian. Such contradictions as internship duration, operation of the internship process, level of participation or practice, and overlap of part-time experience in a library were identified. This study suggests to make the internship systematic in order to address the contradictions through that all parties in internship are actively participated in internship and that they share information related with the internship.
