This paper suggests a weibull time delay model to evaluate failure risks in FMEA(failure modes and effects analysis). Assuming three types of loss functions for delayed time in failure cause detection, the risk of each failure cause is evaluated as its occurring frequency and expected loss. Since the closed form solution of the risk metric cannot be obtained, a statistical computer software R program is used for numerical calculation. When the occurrence and detection times have a common shape parameter, though, some simple results of mathematical derivation are also available. As an enormous quantity of field data becomes available under recent progress of data acquisition system, the proposed risk metric will provide a more practical and reasonable tool for evaluating the risks of failure causes in FMEA.

In this study, the failure analysis of a decanter is carried out and the methods of performance improvement are presented. The decanter is a centrifugal separator that is used to separate water and solids from municipal and industrial sludge. Therefore, the decanter should be designed to improve the dewatering of sludge. Besides high performance, the decanter should guarantee its life time under a severe using condition. For theses reasons, the failure analysis and performance improvement of the decanter are studied. It is found from this study that the failure is caused by mass unbalance, wear, clogging or crack. If these failure causes are prevented, the life time as well as the performance is expected to be improved.
In spite of progress in tribological research, machine component failure due to friction and wear has been reported frequently. This failure may lead to secondary damage that can cause huge expense for maintenance and repair. To prevent economic loss, it is important to detect and predict the initial failure point. In this sense, various researchers have been tried to develop Condition Monitoring (CM) method using Acoustic Emission (AE) generated while the materials undergo failure. In this study, effect of particles on friction and wear was investigated using the pin-on-plate friction test and AE signal was recorded with a band-width type AE sensor. The experiments were performed in dry and lubricant conditions using steel and glass as specimens. After the experiment, 3D laser microscope image was captured to evaluate the wear behavior quantitatively. The AE signal was analyzed in time-domain and frequency-domain. The amplitude was compared with the frictional results. The results of this study showed that particle generation accelerate wear, generate high magnitude AE signal and change the frequency characteristics of the signal. Also, lubricant condition test results showed low coefficient of friction, low wear rate, and low magnitude of AE signal compared to the dry condition. It is expected that the results of this study will aid in better assessment of wear in CM technology
In this study, SiC powder was added to twill woven carbon fiber reinforced plastic (CFRP) composites to improve its mechanical properties. An acoustic emission (AE) frequency analysis method was suggested for the prediction of failure behaviors. Tensile tests were conducted and the fracture characteristics of each component of the SiC reinforced composite were evaluated using AE. The results showed that SiC powder improved the strength of twill woven CFRP composites and the fracture behavior of the SiC reinforced CFRP composite and its crack extension could be effectively evaluated on the basis of the specific AE frequency bands which are 100 to 228 kHz and 428 to 536 kHz upon the resin failure and 232 to 424 kHz due to addition of SiC powder and 576 to 864 kHz at the fiber breakage.

As the average usage period of transformers increases, it is becoming increasingly necessary to know the internal condition of transformers. It is therefore critically important to establish monitoring and diagnostic techniques that can perform transformer condition assessment. Frequency response analysis, generally known as FRA, is one of the technologies to diagnose transformers. Using case studies, this paper presents the effectiveness of FRA as measurements for detecting transformer failures. This paper introduces the fact that FRA waveforms have useful information about diagnosis of failure on core earths and winding shield, and that the condition outside transformers can affect frequency response characteristics.

This studies were established to find out the characteristics of frequency distributiom for the number of occurrence and magnitude, probable flood flows according to the return periods, design floods, and design frequency factors for the studying basins in relation to the risk levels which can be correlated with design return period and the life of structure in the non-annual exceedance series. Eight watersheds along Han, Geum, Nak Dong and Seom Jin river basin were selected as studying basins. The results were analyzed and summarized as follows. 1. Poisson distribution and Exponential distribution were tested as a good fitted distributions for the number of occurrence and magnitude for exceedance event, respectively,at selected watersheds along Han, Geum, Nak Dong and Seom Jin river basin. 2.Formulas for the probable flood flows and probable flood flows according to the return periods were derivated for the exponential distribution at the selected watersheds along Han, Geum, Nak Dong, and Seom Jin river basin. 3.Analysis for the risk of failure was connected return period with design life of structure in the non-annual exceedance series. 4.Empirical formulas for the design frequency factors were derivated from under the condition of the return periods identify with the life of structure in relation to the different risk levels in the non-annual exceedance series. 5.Design freguency factors were appeared to be increased in proportion to the return periods while those are in inverse proportion to the levels of the risk of failure. Numerical values for the design frequency factors for the non-annual exceedance series ware appeared generally higher than those of annual maximum series already published by the first report. 6. Design floods according to the different risk levels could be derivated by using of formulas of the design frequency factors for all studying watersheds in the nor-annual exceedance series.

In this paper, analyzes the type of failure and its effect on the hydrogen fueling nozzle used in hydrogen station. Failure of hydrogen fueling nozzle was analyzed using a qualitative risk assessment method, failure mode and effect analysis. The failure data of hydrogen fueling nozzles installed in domestic hydrogen stations are collected, and the failure types are classified, checked the main components causing the failure. Criticality analysis was derived based on frequency and severity depending on the failure mode performed. A quality function is developed by a performance test evaluation item of the hydrogen fueling nozzle, and the priority order of design characteristics is selected. Through the analysis results, the elements to improve the main components for enhancing the quality and maintenance of the hydrogen fueling nozzle were confirmed.

With the enforcement of environmental regulations by the International Maritime Organization, the market for eco-friendly ships is expanding, and ships using electric propulsion devices are emerging as a promising solution. Many studies have been conducted to predict the failure of ships, but most of them are mainly research on the main diesel engine of ships. As the ship's propulsion method changes, new data is needed to predict the failure of electric propulsion ships. In this paper aims to analyze the failure characteristics of the electric propulsion system in consideration of the difference in the type of failure between the internal diesel engine and the electric propulsion system. The ship's propulsion unit assumed a DC motor and a signal pattern for normal conditions and general failure modes, but the failure record of the electric propulsion device operated on the actual ship was not available, so it generated a failure signal for small electric motor equipment to identify the failure signal. Assuming unbalance, misalignment, and bearing failure, which are the primary failure modes of the ship's electric motor, a failure signal was generated using a "rotator vibration data generator," and the frequency band, size, and phase difference of the measured vibration signal were analyzed to analyze the characteristics of each failure condition. Finally, the characteristics of each failure condition were identified so that the signals according to the failure type could be classified.

In the field of nuclear reactor safety study, common cause failures (CCFs) became significant contributors to system failure probability and core damage frequency in most Probabilistic risk assessments. However, it is hard to estimate the reliability of such a system, because of the dependency of components caused by CCFs. In order to analyze the system, we propose an analytic method that can find the parameters with lack of raw data. This study adopts the shock model in which the failure probability increases as the shock is cumulated. We use two-step Expectation and Maximization (EM) algorithm to find the unknown parameters. In order to verify the analysis result, we perform the simulation under same environment. This approach might be helpful to build the defensive strategy for the CCFs.

In this paper we consider a Bayesian theoretic approach to periodic incomplete preventive maintenance with minimal repair at failure. We assume that the system failure rate is increasing as the frequency of PM increases and that the system is replaced at the K-th PM under this maintenance strategy. The optimal policies which minimize the expected cost rates are discussed. We seek the optimal periodic PM interval x and replacement time K under a Weibull failure intensity. Assuming suitable prior distribution for the Weibull parameters, we derive the posterior distribution incorporating failure data and obtain the updated optimal replacement strategies.
