The guarantee of transportation for elderly and handicapped people is increasingly necessary owing to their growing social and economic activity. In March of 2013, a partial amendment to the law for transportation of the vulnerable was made by the government, to make more convenient transport a legal requirement. The amendment describes standards for the installation of transport facilities, and its support available. However, the adjustment of low-floor bus routes is not included. Therefore, low-floor buses are operating without consideration for the handicapped. This study evaluates the current low-floor bus system user experience by considering the residential distribution of vulnerable customers and analyzing their reasons for using public transport. As a result of this work, adjustments to current bus routes are proposed. The residential distribution of vulnerable people has been assessed using the supporting materials of the urban renewal project in Busan City, and their reasons for using public transport have been collected by a survey of the target user groups. The results show that group A, which has good accessibility, was in high demand, but provided the most limited service. Whereas group C, which also has good accessibility but was in low demand, provided its service most frequently. The data show the supply and demand inconsistencies of low-floor buses, and the inefficiency of bus route allocation. Therefore, this study proposes improvements to current methods for the effective operation of low-floor buses.
This study was conducted to empirically verify the factors that affect work-life balance according to the age of social workers. The subjects of the study were social workers working at social welfare centers, welfare centers for the disabled, and welfare centers for the elderly located in Jeollabuk-do, and 313 copies of 42 facilities that participated in the survey were used for the final analysis. The research method used a multi-level model that calculated individual factors (Level 1) and institutional factors (Level 2). The main research results are as follows. First, 20.2% of the total variance in the work-life balance of social workers was found to be different for each local welfare center. Second, as a result of examining the factors affecting work-life balance of social workers by age, it was found that job demands had a negative effect on work-life balance in all age groups. In particular, in their 30s, family demands and women's work-life balance were more negative than men's, and economic status was found to be an important influencing factor on work-life balance. Based on these results, we discussed ways to improve work-life balance for social workers.
A coastal ferries, which serve as a mode between mainland and offshore islands, are frequently used by islanders and travelers visiting islands. Therefore, it is located in a place with good accessibility to the downtown so that users can use it conveniently. However, research on the built environment and walking environment of coastal ferry terminals has not been conducted to ensure that elderly islanders and general travelers can use comfortably the coastal ferry terminal. In this study, with a focus on the coastal ferry terminal space, the walking section was set as 1)the accessibility of the coastal ferry terminal, 2)the built environment for using the coastal ferry terminal, and 3)walking environment in the ferry boarding and disembarkation at the coastal ferry terminal, and 4, 5)walking environment in the ferry. An evaluation tool was developed to measure the walking environment for each walking section, and the importance and satisfaction of users were analyzed using the IPA for the Yeosu Coastal Ferry Terminal. As a result, in the process of approaching the ferry terminal, 'convenience of using public transportation by foot' and 'convenience of using signposts guiding the location and direction of the ferry terminal' were investigated as concentrated areas. On the other hand, the built environment inside the ferry terminal and the walking environment showed that the satisfaction of the users was high. Regarding the walking environment from the ferry terminal to the ferry, the 'surface walking environment of the walking path', 'convenience of walking with luggage', 'convenience of walking in the slope section', and 'convenience of walking in the ferry ramp section' showed low satisfaction. This study can be used as basic data for improving the facilities and walking environment of Yeosu Coastal Ferry Terminal and is expected to be used as data for comparative research with other coastal ferry terminals.

In keeping with the increasing demand for medical services in accordance with the economic development, the use of oriental medicine is on the rise year by year. In particular, it is a tendency that the elderly over age 65 prefer the oriental medicine to the western medicine in Korea. Under the circumstance, changing into an aging society, the importance of oriental medical services has been growing. In this regard, this study evaluated the quality of medical services by conducting the density analysis and the accessibility analysis of oriental medical clinic targeting Jinju, which is one of local small and medium-sized cities. The study results reveal that oriental medical hospitals accounted for 27% of the entire medical facilities in Jinju, and there was an imbalance in the quality of the oriental medical services according to districts(called dong) because oriental medical hospitals were mainly concentrated in the central commercial areas. In particular, in terms of the analysis on the accessibility of an aging population over age 65 to the oriental medical hospitals, over 50% of the whole districts (dongs) in Jinju were blind spots in medical services, showing a very vulnerable condition. Accordingly, it is judged that systematic measures for improvement in support of the oriental medical services in local small and medium-sized cities should be provided in Korea when we consider the characteristics of Korea where an aging population is rapidly increasing.
The problem of care of patients and families with Alzheimer's disease has become a conscious raising social policy issue in Korea. The government of the Republic of Korea has become cognizant of the situation and has begun searching for ways to remedy it. Thus, there is a need for a comprehensive under-standing of the situation in which patients and their families are struggling and the enormous problems of care. With a realization of the urgent need, this study was done to investigate the situation and the care needs of families with patients with Alzheimer's Disease, and to compare the effectiveness of services utilized by the families in terms of cost and effects on patient's conditions and on family live. The Subjects for the study were 29 families with hospitalized patients, 25 families utilizing hospital outpatient clinics, 14 families utilizing day care facilities, and 16 families with homebound patients. A total of 84 families were interviewed by four trained interviewers using structured and semistructured questionnaires. The data produced from these interviews included : the patient's stage of Alzheimer's disease, patient's bizarre behavior, hours spent on patient care per day, family burden and quality of life, direct and indirect costs encountered in the care of patients, and the families' evaluation of the effectiveness of the services received. The data were analyzed to determine the relationships between family charactersistics, patient's conditions and services utilization. The effectiveness of each of the service entities was assessed through families evaluation and hoped for service and comparisons were made between services in terms of the cost-effectiveness ratios. After initial comparison of cost-effectiveness ratios, further analysis was done to compare between groups for incremental effectiveness for each incremental unit of cost to determine the most cost-effective service entities. The findings of the study are as fellows : 1. The choice of living arrangement and the types of services are a function of the stage of Alzheimer's condition and the economic status of the family. 2. Comparision of the cost of care showed that most expenses were encountered in by families with hospitalization, families using outpatient services, and families using day care services in that order. The least expense was involved in the care of homebound patients. The economic burden felt by families was in the same order as expenses. 3. The average number of hours spent on daily patient care was 9.9 hours for the outpatient clinic users, 9.7 hours for homebound patients, and 5.4 hours for day care users. 4. There were significant differences in the patient's conditions (CDRL), bizarre behaviors and the families's burden by living arrangement and /or types of service. However, no significant difference was found between groups in the family's quality of life. 5. The families rated the services of day care center as most effective for the care of the patients and families, except for a few families who had experienced some improvement in the patient's conditions. The outpatient clinic users expressed psychological comforts mainly in that the patient was being taken care of. For those hospitalized patients, families expressed the comfort of being relieved of the burden of care and that the patient is being professionally cared for. Form the analysis of the costs, hours of patient care, patient's bizarre behaviors, family's quality of life and burdens, and family's evaluation of services, it is concluded that up to the mid stage of Alzheimer's condition, the utilization of day care center services is found to be the most cost-effective, and toward the end stage of the Alzheimer's disease, it is hoped that there will be a establishment of long term or short term in-patient facilities for the protection of patients and preservation of the integrity of families for less cost. Thus. it was concluded that the family centered system of care is the most effective for Korea with systematic support systems developed for the care of patients and their families according to the needs of families as the patient's condition deteriorates.
A descriptive correlation study was done to provide a basic data for comprehensive nursing care by analyzing the relationship between spiritual wellbeing, hope and perceived health status of the elderly. 195 respondents who lived at their homes and 148 respondents who lived at the facilities for elders such as nursing homes and elder's rehabilitation centers were selected and their age was over 60 years old. Paloutzian and Ellison(1982)'s spiritual wellbeing scale, Nowotny(1989)'s hope scale and Northern Illinois University's health self rating scale was used. From August 10th to August 25th, 1998, ready made questionnaires were handed out by researcher to those who can fill it out and for those who cannot fill out the questionnaires alone, the researcher read it and finished by interview. This study used Pearson's correlation coefficient for the hypothetic test and the average point and standard deviation of spiritual wellbeing, hope, and perceived health status were checked. To find out the difference between spiritual wellbeing, hope, and perceived health status by general characteristics ANOVA and Tukey test were used. The results are as follows : 1. The mean score for spiritual wellbeing of the elders was 42.27($SD{\pm}9.67$) in a possible range of 20-80. The average point of spiritual wellbeing was 2.11($SD{\pm}0.97$) point to 4 point full marks. The mean score of religious wellbeing was 21.37($SD{\pm}7.02$) and that of existential wellbeing was 20.90($SD{\pm}4.63$) in a possible range of 10 - 40. Background: Hospital-acquired pneumonia (HAP) and ventilator-associated pneumonia (VAP) are significant public health issues in the world, but the epidemiological data pertaining to HAP/VAP is limited in Korea. The objective of this study was to investigate the characteristics, management, and clinical outcomes of HAP/VAP in Korea. Methods: This study is a multicenter retrospective cohort study. In total, 206,372 adult patients, who were hospitalized at one of the 13 participating tertiary hospitals in Korea, were screened for eligibility during the six-month study period. Among them, we included patients who were diagnosed with HAP/VAP based on the Infectious Diseases Society of America (IDSA)/American Thoracic Society (ATS) definition for HAP/VAP. Results: Using the IDSA/ATS diagnostic criteria, 526 patients were identified as HAP/VAP patients. Among them, 27.9% were diagnosed at the intensive care unit (ICU). The cohort of patients had a median age of 71.0 (range from 62.0 to 79.0) years. Most of the patients had a high risk of aspiration (63.3%). The pathogen involved was identified in 211 patients (40.1%). Furthermore, multidrug resistant (MDR) pathogens were isolated in 138 patients; the most common MDR pathogen was Acinetobacter baumannii. During hospitalization, 107 patients with HAP (28.2%) had to be admitted to the ICU for additional care. The hospital mortality rate was 28.1% in the cohort of this study. Among the 378 patients who survived, 54.2% were discharged and sent back home, while 45.8% were transferred to other hospitals or facilities. Conclusion: This study found that the prevalence of HAP/VAP in adult hospitalized patients in Korea was 2.54/1,000 patients. In tertiary hospitals in Korea, patients with HAP/VAP were elderly and had a risk of aspiration, so they were often referred to step-down centers.
