A 17- Year Clinicostatistical Analysis of Cleft Lip and/or Palate Patients in Clinics for Maxillo-Oral Disorders, Tohoku University Dental Hospital (동북대학 치학부속병원 악구강기능치료부에 있어서 구순구개열 환자의 임상통계 -1987년부터 2002년까지-)

  • The frequency of abnormality at birth is average 1-1.5%, and of these, cleft lip & palate is known to be the most frequent congenital abnormality, Cleft is considered to be due to multi-factorial heredity correlated with genetic and environmental factors, Cleft patients require the collaborative treatment with several medical departments, Clinics for Maxillo-Oral Disorders of Tohoku University Dental Hospital performs the total managements related to such as occlusion and language for the patients with congenital maxilla-facial abnormality, This study examined the patients with cleft lip and/or palate who came to the Clinics for Maxilla-Oral Disorders of Tohoku University Dental Hospital for the past 17 years from Jan. 1987 to Dec, 2002, and had the results as follows, 1. Annual mean number of patients The annual mean number of the patients for 17 years from Jan, 1987 to Dec, 2002 was 91 patients, ranging from 63 minimum to 116 maximum, 2, Gender and types of cleft There were 747(51%) males and 709(49%) females, with a male to female ratio 1,05:1. CLP was the most frequent cleft type as shown in 616 patients, and other patients manifested different complaints such as CL, CP, SMCP and MC in order. 3. The laterality in cleft type The lip cleft was frequently expressed orderly on left, right and both sides of CL patients while orderly being shown on left, both and right sides of CLP patients. Accordingly, lip cleft was most commonly found on the left side. 4. Address at first visit Of 1,456 subjects, 850(58.4%) patients were residing in Miyagi Prefecture, where this hospital is located. 5. Age at first visit 615(42.2%) patients came to the hospital at their age younger than 1 year old, comprising 282(19.4%) patients age younger than 2 months old and 333(22.9%) patients age between 2 month old and 1 year old. 6. Mother's age at birth For the mother's age at birth, 526(39.9%) patients were at the age of 25 to 30 years old, and 17(1.3%) patients were over 40 years old. 7. Birth weight 34.3%(443 patients) had a birth weight of 2500-3000gm and 56.0%(724 patients) had a 3000-4000gm. It was also found that 7.9%(102 patients) had a birth weight of less than 2500gm. 8. Familial expression The frequency of familial expression was 6.5%(94 patients).

A Clinical Study on Salivary Gland Tumors (타액선 종양의 임상적 고찰)

  • We have reviewed a 9-year experience with 47 patients treated for salivary tumors which arose In the parotid gland (23 patients; 48.9%), submandibular gland (15 Patients, 31.9%), minor salivary gland (8 patients; 17.0%), and sublingual gland (1 patient; 2.1%). The age of patients ranged from 12 to 71 with a mean of 42.9 years. The male to female ratio showed a female preponderance, 19:28 (1:1.5). An asymptomatic mass (91.5%) was most common presentation. With fine-needle aspiration, the predictive value of a neoplasm was 88.9%. Histopathologically, the most common salivary gland tumors was Pleomorphic adenoma(57.4%). The pleomorphic adenoma was most common in benign tumor group, and the mucoepidermoid carcinoma was most common in malignant tumor group. Of the salivary gland tumors, the incidence of cervical metastasis was 22.2%. Major postoperative complications were facial nerve paralysis and wound infection.

  • Objective : The purpose of this study is to investigate nucleotide sequence and extract cytotoxicity of Scolopendrae corpus. The nature and taste of Scolopendrae corpus is hot, Warm and toxic, and the effect of this is dispelling wind, anti-spasmodic action and detoxication so it has been used for C.V.A, facial palsy, sensory disorder at extremities, wounds and arthritis. Methods : Scolopendrae corpus were collected by locality on the market. They were morphologically classified. Their nucleotide sequence was investigated and compared among them. In addition, the water-alcohol extract cytotoxicity of them was studied by MTT-based cytotoxicity assay. Results : It was shown that the each Scolopendrae corpus by locality is almost identical at genetic result and is identified as Scolopendra subspinipes mutilans L. Koch. Nucleotide sequence of Scolopendra subspinipes mutilans L. Koch in this study will help to discriminate other species of Scolopendrae corpus. The water-alcohol extract of Scolopendra subspinipes mutilans L. Koch did not induce cytotoxicity on Hep G2, L929 cell and peritoneal macrophages. Besides, it did not influence nitrite production of peritoneal macrophages. These results can be used as basic data for genetic discrimination with another species of scolopendrae corpus.

Face recognition using PCA and face direction information (PCA와 얼굴방향 정보를 이용한 얼굴인식)

  • In this paper, we propose an algorithm to obtain more stable and high recognition rate by using left and right rotation information of input image in order to obtain a stable recognition rate in face recognition. The proposed algorithm uses the facial image as the input information in the web camera environment to reduce the size of the image and normalize the information about the brightness and color to obtain the improved recognition rate. We apply Principal Component Analysis (PCA) to the detected candidate regions to obtain feature vectors and classify faces. Also, In order to reduce the error rate range of the recognition rate, a set of data with the left and right $45^{\circ}$ rotation information is constructed considering the directionality of the input face image, and each feature vector is obtained with PCA. In order to obtain a stable recognition rate with the obtained feature vector, it is after scattered in the eigenspace and the final face is recognized by comparing euclidean distant distances to each feature. The PCA-based feature vector is low-dimensional data, but there is no problem in expressing the face, and the recognition speed can be fast because of the small amount of calculation. The method proposed in this paper can improve the safety and accuracy of recognition and recognition rate faster than other algorithms, and can be used for real-time recognition system.

Anthropometric Analysis of Frontal Sinus Using 3D CT in Koreans (한국인 성인 남녀에서 3차원 전산화단층촬영술을 이용한 전두동의 형태학적 연구)

  • Purpose: The frontal sinuses are a pair of triangularly shaped, air-filled chambers lined by mucoperiosteum and located between the inner and outer tables of the frontal bone. Until recently, our understanding of gender variations in craniofacial anatomy has been chiefly built upon anthropometric studies, which typically employ facial surface measurements or plain film radiography. The aim of this study i to determine the sizes of the frontal sinus in both sexes in Koreans. Methods: 95 Korean subjects who underwent maxillofacial 3-Dimensional computed tomography (CT) between January 2009 and December 2009 were enrolled. Frontal sinus dimensions and forehead measurements were taken at midline and at 10, 20, and 30 mm to the left and right of midline using sagittal, coronal, and axial images. The data was analyzed for significant differences between measurements made at the selected points in the frontal sinus, for left to right variations, for gender variations, and for racial differences. Results: The mean thickness of the anterior table ranged from 2.31 to 3.23 mm. Mean anteroposterior depth of the frontal sinus ranged from 7.38 to 9.45 mm and did not vary significantly at any distance from midline. Frontal sinus height was greatest at midline (mean=29.24 mm) and progressively lessened at lateral distances. Mean total width at the level of the supraorbital ridge was 53.66 mm. For all measurements, no significant left to right variation was noted. Comparing the sexes, males were found to have greater dimensions in most frontal sinus measurements, though these differences were only found to be significant at or close to midline. The male forehead was marked by more acute nasofrontal angle ($133.3^{\circ}$ versus $141.6^{\circ}$) and a steeper posterior forehead inclination ($14.9^{\circ}$ versus $7.7^{\circ}$). Conclusion: Using CT imaging, forehead and frontal sinus dimensions have been described. Generally, males had larger overall frontal sinus dimensions. And Korean had similar sized frontal sinus to Caucasian in height and width. But in AP distance Korean had lesser measurement. The result of this study may be helpful in the comprehension of normal size of frontal sinus in Korean.

3D Face Recognition in the Multiple-Contour Line Area Using Fuzzy Integral (얼굴의 등고선 영역을 이용한 퍼지적분 기반의 3차원 얼굴 인식)

  • The surface curvatures extracted from the face contain the most important personal facial information. In particular, the face shape using the depth information represents personal features in detail. In this paper, we develop a method for recognizing the range face images by combining the multiple face regions using fuzzy integral. For the proposed approach, the first step tries to find the nose tip that has a protrusion shape on the face from the extracted face area and has to take into consideration of the orientated frontal posture to normalize. Multiple areas are extracted by the depth threshold values from reference point, nose tip. And then, we calculate the curvature features: principal curvature, gaussian curvature, and mean curvature for each region. The second step of approach concerns the application of eigenface and Linear Discriminant Analysis(LDA) method to reduce the dimension and classify. In the last step, the aggregation of the individual classifiers using the fuzzy integral is explained for each region. In the experimental results, using the depth threshold value 40 (DT40) show the highest recognition rate among the regions, and the maximum curvature achieves 98% recognition rate, incase of fuzzy integral.

Clinical Analysis of the Nasal Bone Fracture (비골골절에 대한 임상적 분석)

  • Purpose: Nasal bone fracture is the most common facial fracture. Although nasal bone fractures are considered to be minor injuries, the incidence of post-traumatic nasal deformity remains high. This study is designed to support management and patient satisfaction by classifying the simple nasal bone fracture, and survey the care method and result, which is compared with other studies. Methods: From May 2008 to April 2010, 334 patients with simple nasal bone fractures visited our hospital. The incidence, cause, types of nasal bone fracture, treatment, and complications are analyzed according to clinical examination, patient's record and radiographic images. Results: The mean age of patients was 30 years old, with 74% of the patients having been male, and 26% were female. The highest incidence of this fracture was between late teens to late twenties. The causes of nasal bone fracture were the following: having slipped or fallen down (39%), violence (16%), sports accident (14%), traffic accident (11%), industrial accident (6%), and others (16%). Patient's radiographic images were analyzed by Stranc and Robertson classification, frontal impact plane I was 38%, plane II was 16%, plane III was 1%, lateral impact plane I was 21%, plane II was 21%, and plane III was 3%. On average, surgical treatment was performed 7.2 days after trauma under general anesthesia. Closed reduction of nasal bone fracture was performed in 99% of patients. In patients with septal injuries, septal management was performed in 76.7% of cases. Aesthetic surgery was done on same time in 24% of patients. There were some complications, such as residual nasal deformity (7.2%), nasal obstruction (0.9%) and hyposmia (0.3%). Conclusion: According to this study, nasal bone fractures occurred commonly in physically active age groups (age 15~29 years), as a result of having slipped or fallen down, at afternoon and at night time. And it could be treated successfully by closed reduction and septoplasty by 7 days after trauma.

Implant treatment on anterior cross-bite of a patient who had orthognathic surgery 20 years ago (20년전 악교정수술을 받았던 환자의 심한 전치부 반대교합의 해결을 위한 임플란트 치료 증례)

  • Orthognathic surgery of skeletal Class III malocclusion improves oral function and facial appearance. The greater amount of skeletal discrepancy, the greater amount of teeth movement required for decompensation, and this often causes pathological changes in periodontal tissue especially in lower anterior dentition. We made a Top-Down treatment plan with personalized analysis using Face Hunter, Plane System and ARCUS Digma II, in order to resolve severe mobility and cross-bite of lower anterior teeth for 49-year-old female patient who had undergone orthognathic surgery 20 years ago due to skeletal Class III malocclusion and mandibular prognathism. Lower anterior teeth were extracted and alveoloplasty was done. After healing of the wound, immediate loading was conducted immediately after implant placement. Final restorations were fabricated Zirconia using CAD/CAM, and inserted intraorally screw-retained type. During 6-month follow-up, no abnormal episodes of restorations were observed, and obtained satisfactorily both of functional and esthetic outcomes.

The first Korean case with Floating-Harbor syndrome with a novel SRCAP mutation diagnosed by targeted exome sequencing

  • Floating-Harbor syndrome is a rare autosomal dominant genetic disorder associated with SRCAP mutation. To date, approximately 50 cases of Floating-Harbor syndrome have been reported, but none have been reported in Korea yet. Floating-Harbor syndrome is characterized by delayed bony maturation, unique facial features, and language impairment. Here, we present a 6-year-old boy with a triangular face, deep-set protruding eyes, low-set ears, wide nose with narrow nasal bridge, short philtrum, long thin lips, clinodactyly, and developmental delay that was transferred to our pediatric clinic for genetic evaluation. He showed progressive delay in the area of language and cognition-adaption as he grew. He had previously undergone chromosomal analysis at another hospital due to his language delay, but his karyotype was normal. We performed targeted exome sequencing, considering several syndromes with similar phenotypes. Library preparation was performed with the TruSight One sequencing panel, which enriches the sample for about 4,800 genes of clinical relevance. Massively parallel sequencing was conducted with NextSeq. An identified variant was confirmed by Sanger sequencing of the patient and his parents. Finally, the patient was confirmed as the first Korean case of Floating-Harbor syndrome with a novel SRCAP (Snf2 related CREBBP activator protein) mutation (c.7732dupT, p.Ser2578Phefs*6), resulting in early termination of the protein; it was not found in either of his healthy parents or a control population. To our knowledge, this is the first study to describe a boy with Floating-Harbor syndrome with a novel SRCAP mutation diagnosed by targeted exome sequencing in Korea.

A Study on Experience of Day Hospital and Day Care Service Use of The Mothers with Severely Disabled Children (중증장애아동 어머니의 낮병원 및 보육서비스 이용 경험에 관한 연구)

  • Objective: The study wanted to find out what changes the mother, who is the main parent of the severely disabled children, experienced in their lives through the use of day hospitals and child care services, and what difficulties they had, and discussed policy practical improvements that could satisfy both the medical and developmental needs of the severely disabled. Methods: The participants of the study were 9 mothers of a child with severe disabilities who had experience using hospitals and kindergarten. They were recruited by snowball sampling method. Focus group interview and individual interview were conducted. Results: As a result of the analysis, the mother of severely handicapped child gradually began to seek the balance of life as her child grew and became aware of the institution other than the day hospital. - (e.g) Although I was distracted and worried for the first time, I was gradually adjusting to the time away from my child thinking positively. Children with severe disabilities were also positively influenced by various experiences. They become to show som positive behavior or feeling such as brightening facial expressions or smile as they were using daycare centers or kindergartens in addition to daytime hospitals. Conclusion/Implications: Through the results of this study, comprehensive policy proposals on child care, welfare, and medical care were discussed.
