구안와사 환자의 12경맥 전위측정 연구 (Differences in Electric Potential of Meridian System -Comparing Electrical Potentials of Patients with Facial Hemiparalysis-)

  • Objective : Assuming that the characteristic of meridian system has been similar to this of electric potentials in human body and that measurements of electric potential at well(井穴) and sea (合穴) points in branches of the twelve meridians(WSBTM) will be representative of measurements of the twelve meridians, to measure the electric potentials of 13 patients with Facial Hemiparalysis (구안와사<口眼蝸斜>, FH), to find out the characteristic of meridian system in patients with AK. Methods : Electric potentials of well and sea points in the meridians in twenty one patients with the pain in the lion diagnosed as FH were repeatedly measured by physiograph(PowerLab). Measurements of those electrical potentials were analyzed by factor analysis. Results : The electric potentials of WSBTM at the left side were divided into five factors. On the other hand those at the right side Were divided into four factors. Conclusions : In conclusion, their electrical potentials at the left and right side were each other five and four factors. Thus electrical potentials of well and sea points might be the representative meridian to show their characteristics.

하악 과두 과형성증에 관한 방사선학적 연구 (A DIOGRAPHIC STUDY OF THE CONDYLAR HYPERPLASIA)

  • Condylar hyperplasia is a self-limiting condition characterized by a slowly progressing, enlargement of the mandible that results in facial asymmetry and a crossbite malocclusion. The facial asymmetry, open bite or crossbite, and radiographic evidence of an enlarged condyle confirm the diagnosis of condylar hyperplasia. The etiology of the condition is unknown. This condition usually first becomes apparent during the second decade of life, when one condyle continues to grow while the other is no longer active. Radiographically, the condyle may appear enlarged or the neck of the condyle maybe elongated or both may occur, Sometimes, however, no radiographically demonstrable condylar abnormality will be noted. Surgical correction with subcondylar osteotomy is the treatment of choice. We have observed two cases of condylar hyperplasia occurred in the left mandibular condyle of 24-year-old and 35-year-old women. We obtained that two cases were shown the followed results; 1. Clinically, both cases was unilaterally developed on C/C area, with temporomandibular disorders and pain, facial asymetry and malocclusion. 2. Radiographically, hyperplastic mass confined to the condyle. 3. Histopathologically, these cases shown increased hypertrophic region in parts, and lamellated bone with irregular trabeculae.

Subcutaneous emphysema after uncommon traumatic and iatrogenic events: a report of two cases

  • Cervicofacial subcutaneous emphysema is defined as the abnormal introduction of air into the subcutaneous tissues of the head and neck. It is mainly iatrogenic and traumatic in origin. Our two case reports are also due to the same cause, but the features of the trauma and the site of the dental treatment are different from the existing reports. A 29-year-old man visited our hospital with facial swelling and pain after experiencing facial trauma in a soccer game. Another 55-year-old woman visited with similar symptoms after replacement of her maxillary anterior fixed prosthesis. In the two cases presented, subcutaneous emphysema was gradually treated with no complications during antibiotic prophylaxis and supportive care. In this paper, we report two cases of traumatic and iatrogenic subcutaneous emphysema and their diagnoses, etiologies, complications, and treatments based on a literature review.

하악과두흡수 환자의 임상적 평가 및 악안면 골격형태에 대한 연구 (Clinical Assessment and Cephalometric Characteristics in Patients with Condylar Resorption)

  • 2006년 한 해 동안 경북대학교 구강내과에 측두하악장애를 주소로 내원한 전체 환자 중에 파노라마 방사선 사진 상에서 인정될만한 하악과두흡수가 관찰된 환자를 대상으로 측모두부방사선 사진을 촬영하였으며, 측두하악장애의 임상적 평가와 악안면 골격형태를 분석하여 다음과 같은 결론을 얻었다. 1. 임상적 평가 1) 측두하악장애를 주소로 내원한 총 환자 수는 2419명 이었고 그중 224명 약 9.3%가 과두흡수를 나타내었다. 과두흡수가 있는 환자군 중 여자는 183명(81.7%)이고 남자는 41명(18.3%)으로 여자에서 월등히 많이 유발되었다. 환자의 연령은 12세에서 70세에 걸쳐 분포되었으며 평균 연령은 30.6세였다. 연령분포는 10대가 26.3%(59명), 20대가 34.8%(78명), 30대가 13.8%(31명), 40대가 11.2%(25명), 50대가 7.1%(16명), 60대가 6.3%(14명), 그리고 70대가 0.4%(1명)로 나타났다. 2) 과두흡수 부위의 통증을 호소한 경우는 145명, 통증을 호소하지 않은 경우가 79명이었다. 3) 과두흡수 환자 중 이갈이 습관을 53명, 이깨물기 습관은 80명이 호소하였으며, 다른 기타 습관들을 보고한 경우가 204명이었다. 4) 과두흡수 부위의 통증을 호소한 환자의 치료기간은 1개월 미만이 81명으로 전체적으로 치료기간이 짧게 나타났다. 2. 악안면 골격 형태 평가 전체 과두흡수 환자 224명중 18세 이상의 186명(여자 155명, 남자 31명)만이 한국 성인 정상교합자의 측모두부방사선사진 계측연구 결과와 비교하였다. 1) SNA, SNB는 남녀 모두에서 유의한 작은 값을 나타냈고, ANB항목에서는 남자는 평균 3.57 여자는 5.05으로 여자 환자에서 큰 값을 보여 남자에 비해 하악이 더 후퇴되어 있음을 알 수 있다. 2) 남녀 모두 SN-GoMe, FMA의 각도가 과두흡수 환자에서 유의하게 상당히 커져 있음을 보였다. FMA는 남자는 평균 30.44, 여자는 31.69로 여자에서 수직적 성장이 더 크게 나타났다. 3) 총후안면고경(posterior facial height)이 과두흡수 군에서 모두 작은 것이 특징으로 보였으며, 총전안면고경(Anterior facial height)은 과두흡수 환자군에서 남녀 모두 정상군에 비해 차이가 없었다. 저자의 연구에서 얻어진 결과에 의해 하악과두흡수는 정형외과적으로 퇴행성관절염과는 달리 젊은 여성에서 유병률이 높았으며 이는 남성보다 여성에서 수직적 골격 성장 양상으로 인해 하악과두 전방부의 압축응력을 증가시키는 위험요인을 가진 환자에서 발현이 높은 것으로 생각되며 그리고 노년층보다는 젊은 사람에서 이상기능활동으로 인한 부하의 증가 때문이라 생각한다. 하악과두흡수를 보이는 환자들의 치료기간이 대체적으로 짧으며, 나이가 증가함에 따라 내원하는 환자수가 증가하지 않는다는 것은 이 질환이 자기한정적임을 알 수 있고, 생리적 내성 범위 이상의 부하가 가해져 발생한 하악과두의 재형성 과정으로 생각한다.

Alternative Treatment of Osteoma Using an Endoscopic Holmium-YAG Laser

  • Osteoma is one of the most common tumors of the cranial vault and the facial skeleton. For osteoma in the facial region, endoscopic resection is widely used to prevent surgical scarring. Tumors in a total of 14 patients were resected using an endoscopic holmium-doped yttrium aluminium garnet (Ho:YAG) laser with a long flexible fiber. Aside from having the advantage of not leaving a scar due to the use of endoscopy, this procedure allowed resection at any position, was minimally invasive, and caused less postoperative pain. This method yielded excellent cosmetic results, so the endoscopic Ho:YAG laser is expected to emerge as a good treatment option for osteoma.

An unusual complication during arthrocentesis: N. facialis paralysis, with N. lingualis and N. alveolaris inferior anesthesia

  • This case report aims to review complications that can occur during arthrocentesis and report an unusual complication observed in a 55-year-old man. The patient received arthrocentesis in an attempt to treat painful locking episodes of his right temporomandibular joint (TMJ). One hour after the operation, the patient experienced temporary facial paralysis in the area of the facial nerve and anesthesia of the lingual and alveolar inferior nerves. No persistent complications were detected during the postoperative follow-up. We suspected this complication occurred after anesthetic solution overflowed from a traumatic perforation in the joint capsule to the infratemporal area during the operation. To our knowledge, this complication has not been previously reported in the literature.

Sclerosing polycystic adenosis arising in the parotid gland with trismus: a case report and literature review

  • Sclerosing polycystic adenosis (SPA) is a rare, asymptomatic disease that occurs mainly in the salivary glands. We report the case of a 51-year-old man who presented with trismus and pain upon mouth opening. Magnetic resonance imaging revealed a 2-cm mass located in the anterior portion of the left parotid gland. SPA was diagnosed based on histopathological examination of the surgical specimen. In pathologic findings, there was a well-circumscribed multicystic nodule in the parenchyma. Dense fibrosis and chronic non-specific inflammatory cells were observed in the stroma. In 13 previous reports on SPA, the most preferred treatment was superficial or total parotidectomy. This report suggests that simple excision of SPA preserves facial nerve function and facial volume.

Pterygoid hamulus bursitis as a cause of craniofacial pain: a case report

  • Pain on the soft palate and pharynx can originate in several associated structures. Therefore, diagnosis of patients who complain of discomfort in these areas may be difficult and complicated. Pterygoid hamulus bursitis is a rare disease showing various symptoms in the palatal and pharyngeal regions. As such, it can be one of the reported causes of pain in these areas. Treatment of hamular bursitis is either conservative or surgical. If the etiologic factor of bursitis is osteophytic formation on the hamulus or hypertrophy of the bursa, resection of the hamulus is usually the preferred surgical treatment. We report on a case of bursitis that was managed successfully by surgical treatment and a review of the literature.

  • 우측 암면부, 특히 안구주위에 심한 통증을 호소하고 안검하수와 더불어 우측안구의 완전마비를 동반한 선천성 난장이 환자를 경험하였던 바 이는 신경방사선과학적소견, 검사실 검사소견, 전신적 corticosteroid치료에 대한 반응이 전형적인 Tolosa-Hunt Syndrome과는 다른 비전형적인 Tolosa-Hunt Syndrome이었기에 문헌적 고찰과 함께 보고하는 바이다.

A Case of Eagle's Syndrome Treated with Carbon Dioxide Laser

  • Eagle syndrome is relatively uncommon with an incidence of abnormal stylohyoid length being 4% to 7.3%. A vast majority of individuals with elongation of the styloid process are asymptomatic. It is a syndrome marked by the clinical signs and symptoms of facial pain, ear pain, throat pain, dysphagia and a globus sensation in the throat. The cause of Eagle syndrome is believed to be a congenital or hormonal change and reactive osseus hyperplasia of the styloid process in response to pharyngeal trauma or surgical intervention, such as tonsillectomy. We present here a case of a 37-year-old female with a twelve-month history of both sided oropharyngeal pain and globus sensation which has no trauma or surgical intervention. The patient presented with a long, slender, bony intraoral projection that was found to be an elongated styloid process. We removed this elongated styloid process with a CO2 laser, and her symptoms disappeared.
