Relationship between Pulmonary Function and Symptoms in Dental technicians (치과기공사의 호흡기 증상과 폐환기능 검사와의 관계)

  • This study was performed to determine respiratory manifestations of 105 male and 16 female dental technicians who were resident in seoul. The prevalence of respiratory symptoms was 19% on cough, 40% on sputum, 33% on coryza, and 36% on dyspnea without past cardiopulmonary disease. The number of subjects who was abnormal ventilatory function among % predicted value for FVC(<80%), FEV1(<80%) and $FEV_1$/FVC(<75%) was 18(22%). Indices FVC and $FEV_1$ among smokers were 3 to 6% lower than that of nonsmokers.

Prediction of Postoperative Pulmonary Function Following Thoracic Operations - Perfusion Lung Scanning Method - (폐관류스캔에 의한 폐절제술후 폐기능 예측)

  • The purpose of this study is to predict postoperative lung function by perfusion lung scanning method. 40 patients who underwent lobectomy or pneumonectomy between 1983-1985 were analyzed. Mean preoperative FEV1 was 2.36 L in lobectomy cases and 1.73 L in pneumonectomy cases. Preoperative and postoperative lung function were measured by routine spirometry in sitting position. Perfusion lung scanning was performed by 99mTc-MAA radioisotope. Postoperative FEV1 and VC were predicted by the formula; Postoperative FEV1 [VC]=Preoperative FEV1 [VC] x percent function of regions of lung not to be resected. In this study, I concluded that perfusion lung scanning is a simple and useful method to predict postoperative ventilatory function after pneumonectomy of lobectomy.

  • Pulmonary function test is a group of tests which are composed of measurement for lung function. Thy are spirometry, blood-gas analysis, lung volumes, exercise test, diffusion capacity, and bronchial challenge test. In this article, I will review the pulmonary function test and it's application in terms of clinical aspect and insurance medicine. The standard spirometric indicies are forced vital capacity(FVC), forced expiratory volume at 1 second(FEV1), and the ratio of FEV1 over FVC(FEV1/FVC). If the value of FEV1/FVC less than 70%, the examinee has obstructive ventilatory dysfunction.

  • The medical record of ECC and pulmonary function test of 297 cases who were dead at hospital or admitted as coal workers' pneumoconiosis were used for the study of the relationship between forced expiratory volume in one second($FEV_{1.0}$) and ECG sign of pulmonale. The incidence of T wave inversion in $V_1$ lead was significantly increased as $FEV_{1.0}$ decrease. The incidence of T wave inversion in $V_1$ lead was over the half in the group of $FEV_{1.0}$ less than 0.7l.

Change of FVC, $FEV_1$ after Discontinuance of Bronchodilator in Coal Workers' Pneumoconiosis Patients (탄광부진폐증 환자에서 기관지확장제 투여 중단 후의 노력성폐활량 및 일초폐활량의 변화)

  • For the evaluation of change of FVC and $FEV_1$ after discontinuance of bronchodilator in the coal workers' pneumoconiosis patients, 17 pairs of patients were selected. They were matched by the age(${\pm}5$ y.o.) and the type of ventilatory impairment. Pulmonary function was measured 2 times bimonthly before and after the drug discontinuance discontinued after measurement of PFT for 2 times. In case group the bronchodilator was discontinued after measurement of PFT for 2 times. In control group there was no interruption of medication. FVC, $FEV_1$ decreased in both group as measurement progress. Simple linear regression coefficients against the month of measurement were calculated in both group and tested for parallelism between two groups. The results of test revealed that both regression coefficients were parallel. So in conclusively, discontinuance of medication of bronchodilator for coal workers pneumoconiosis patients has no effect on the decreasing rate of FVC, $FEV_1$.

The Relationship between the Cause of Death and Life Expectancy by $FEV_1$ in Coal Workers' Pneumoconiosis (탄광부진폐증으로 입원중 사망환자에서의 일초량을 이용한 직접사인과 여명과의 관계)

  • Sixty-two medical records of patients with coal workers' pneumoconiosis who died in hospital as coal workers' pneumoconiosis were analysed for study of the relationship between forced expiratory volume in one second ($FEV_1$) and life expectancy in coal workers'pneumoconiosis. In the group who died of asphyxia from hemoptysis, life expectancy were well fitted with $FEV_1$(p<0.05). But others were not well fitted to simple linear regression equation. The prevalence of ECG sign of col pulmonale was more in the group of cardiorespiratory failure than asphyxia group. So, in the case of far advanced cor pulmonale, it was difficult to predict life expectancy by simple linear regression equation

The Efficacy of Pulmonary Rehabilitation Using Air Stacking Exercise in Cervical Cord Injured Patients (경수 손상 환자에게 공기 누적 운동을 이용한 호흡 재활의 효과)

  • Purpose : This study investigated the effects of air stacking exercise on respiratory ability of patients with cervical cord injury. Methods : The subjects of this study were 30 patients with cervical cord injury were randomly placed in an experimental group(n=15) and a control group(n=15), respectively. Basic therapeutic exercise(ROM exercise, stretching exercise, strengthening exercise) were conducted twice a day for 30 minutes each time in all subjects and air stacking exercise was additionally conducted on the experimently group only. Air stacking exercise was conducted for 4 weeks, twice a day, 5 times a week and repeated 10 to 15 times each time. Lung capacity, MIC and, peak cough flow were measured and evaluated. Results : The results showed that FEV1, FVC, MIC, UPCF and APCF were significantly increased(p<.05), but FEV1/FVC didn't show the significant differences in an experimental group. In a control group, the findings showed that FEV1, FVC were increased significantly(p<.05) while FEV1/FVC, MIC, UPCF, and APCF didn't show the significant differences.There were significant differences in FEV1, FVC, MIC, and APCF between a experimental group and a control group in the results of Pulmonary Function Test after conducting the pulmonary rehabilitation. However, no significant differences were found in FEV1/FVC, and UPCF between a experimental and a control group(p>.05). Conclusion : air stacking exercise has positive effects on the improvements of cough functions and that of pulmonary functions such as lung volume, lung elasticity in patients with cervical cord injury.

Effects of High-intensity Intermittent Training and Moderate-intensity Training on Cardiopulmonary Capacity in Canoe and Kayak Paddlers during 8 Weeks

  • PURPOSE: The purpose of this study was to investigate the effects of high intensity intermittent training on cardiopulmonary capacity in canoe and kayak paddlers. METHODS: A total of 16 canoe and kayak paddlers were participated in this study. Experimental group(n=8) was performed high-intensity intermittent training and control group(n=8) was moderate intensity training. All subjects performed a treadmill test in order to compare the difference before and after the intervention. Finishing the test, all subjects were measured to their heart rate(HR), forced vital capacity(FVC), forced expiratory volume in one second (FEV1) and forced expiratory ratio(FEV1/FVC). Recovery of heart rate(RHR) was calculated using the HR. HR and pulmonary flow values was measured before and during the intervention period per 2, 4, 6 and 8 weeks. To compare the differences over time between experimental group and the control group, used(time${\times}$group) two-way repeated measures ANOVA. One-way repeated ANOVA was performed to determine where differences over time within-group. RESULTS: One-way repeated ANOVA revealed a significant difference in the experimental and control group. In experimental group, %RHR3min and FEV1 were significantly increased after 4 weeks(p<.05). Also, %RHR1min, FVC and FEV1/FVC were significantly increased after 6 weeks(p<.05). In control group, %RHR1min, %RHR3min, FVC, FEV1 and FEV1/FVC were significantly increased after 6 weeks(p<.05). CONCLUSION: Not only moderate training but also high-intensity intermittent training contributes to cardiopulmonary capacity in canoe and kayak paddlers. Although high-intensity intermittent training is very short time, the training has high degree of efficiency. Therefore, developed this training in the future, it will be better to improve the cardiopulmonary capacity for athletes and healthy people.

Use of Quantitative CT to Predict Postoperative Lung Function (Comparison of Quantitative CT and Perfusion Lung Scan) (폐절제술후의 폐기능 예측에 대한 나선식 정량적 CT의 유용성 (나선식 정량적 CT와 폐관류스캔과의 비교))

  • Background : the prediction on changes in the lung function after lung surgery would be an important indicator in terms of the operability and postoperative complications. In order to predict the postoperative FEV1 - the commonly used method for measuring changes in lung function- a comparison between the quantitative CT and the perfusion lung scan was made and proved its usefulness. Material and Method : The subjects included 22 patients who received perfusion lung scan and quantitative CT preoperatively and with whom the follow-up of PFT were possibles out of the pool of patients who underwent right lobectomy or right pneumonectomy between June of 1997 and December of 1999. The FEV1 and FVC were calibrated by performing the PFT on each patient and then the predicted FEV1 and FVC were calculated after performing perfusion lung scan and quantitative CT postoperatively. The FEV1 and FVC were calibrated by performing the PFT after 1 week and after 3 momths following the surgery. Results : There was a significant mutual scan and the actual postoperative FEV1 and FVC at 1 week and 3 months. The predicted FEV1 and FVC(pneumonectomy group : r=0.962 and r=0.938 lobectomy group ; r=0.921 and r=913) using quantitative CT at 1 week postoperatively showed a higher mutual relationship than that predicted by perfusion lung scan(pneumonectomy group : r=0.927 and r=0.890 lobectomy group : r=0.910 and r=0.905) The result was likewise at 3 months postoperatively(CT -pneumonectomy group : r=0.799 and r=0.882 lobectomy group : r=0.934 and r=0.932) Conclusion ; In comparison to perfusion lung scan quantitative CT is more accurate in predicting lung function postoperatively and is cost-effective as well. Therefore it can be concluded that the quantitative CT is an effective method of replacing the perfusion lung scan in predicting lung function post-operatively. However it is noted that further comparative analysis using more data and follow-up studies of the patients is required.

Changes in Peak Expiratory Flow, Forced Expiratory Volume in 1 Second and Peak Cough Flow Related to Functional Level and Measurement Position in Patients With Duchenne Muscular Dystrophy (뒤시엔느 근 이영양증 환자에서 기능 수준과 측정 자세에 따른 최대호기유량, 1초간노력성호기량 및 최대기침유량의 변화)

  • It is important to find the effective position for cough and sputum clearance in respiratory physical therapy. The purpose of this study was to compare the changes in peak expiratory flow (PEF), forced expiratory volume in 1 second ($FEV_1$), and peak cough flow (PCF) related to functional level and measurement position in patients with Duchenne muscular dystrophy. Twenty one subjects were classified into three functional levels, and measurements was undertaken in three different measurement positions (upright sitting, $45^{\circ}$ reclining and supine). Vitalograph PEF/FEV DIARY was used to measure PEF and $FEV_1$, and Ferraris Pocket Peak was used to measure PCF. Mixed two-way analysis of variance and Bonferroni post-hoc test were used for statistical analysis. The results of the study were as follows: 1) Significant main effects for measurement position were found. 2) PEF was the highest in upright sitting, followed by $45^{\circ}$ reclining, and supine in order. 3) $FEV_1$ in upright sitting and $45^{\circ}$ reclining were significantly greater compared with that in supine. 4) PCF in upright sitting and $45^{\circ}$ reclining were significantly greater compared with that in supine. 5) No significant main effects for functional level were found in PEF, $FEV_1$, and PCF. 6) No significant functional level by measurement position interactions were found in PEF, $FEV_1$, and PCF. Therefore, it is concluded that upright sitting and $45^{\circ}$ degree reclining positions are recommended for effective cough and sputum clearance.
